Abstract

Acute Respiratory Infections, post-disaster epidemics, and infrastructure damage following hydrometeorological and seismic disasters in Aceh during 2025-2026 have placed enormous pressure on health facilities. Hospitals in disaster-prone regions such as Bireuen District must be assessed for their preparedness using validated instruments. This study aims to analyze the disaster preparedness level of Telaga Bunda Hospital Bireuen in facing flood and earthquake disasters using the Hospital Safety Index (HSI) approach developed by WHO/PAHO. The research employed a descriptive evaluative design using mixed methods, combining HSI self-assessment of 145 items across three modules (structural safety, non-structural safety, and emergency and disaster management) with structured observation, document review, and focus group discussions involving the hospital disaster team. Results indicated that Telaga Bunda Hospital Bireuen achieved a Safety Index of 0.78, categorized as Class A (the hospital is likely to function during and after a disaster). Module 2 (structural safety) scored 0.81, Module 3 (non-structural safety) scored 0.79, and Module 4 (emergency management) scored 0.74. The majority of indicators were rated as high safety level, particularly for structural integrity, electrical backup systems, and clinical staff readiness. Recommendations focus on strengthening disaster simulation drills, improving evacuation signage, and enhancing inter-agency coordination with BPBD Bireuen and Health Service.

Abstract

Breast milk is the most comprehensive biological nutrition for infants, yet the coverage of exclusive breastfeeding in postpartum mothers has not yet reached global targets. Delays in breast milk production, maternal stress, and lack of lactation support are factors that influence breastfeeding success. This study aims to analyze the effectiveness of oxytocin massage and breast care on smooth breast milk production in postpartum mothers. The study used a qualitative research method with a systematic literature review approach. The study population was scientific articles discussing oxytocin massage, breast care, and breast milk production in postpartum mothers in 2020–2025. The study sample consisted of 20 articles selected purposively based on inclusion and exclusion criteria. The research instruments included an article search sheet, inclusion and exclusion criteria, and a data extraction matrix. Data analysis was conducted using content analysis and narrative synthesis. The results showed that oxytocin massage and breast care significantly increased breast milk production, facilitated milk letdown, reduced maternal stress, and improved breastfeeding comfort. The combination of the two interventions provided more optimal results than either intervention alone. The conclusion of this study is that oxytocin massage and breast care are non-pharmacological interventions that are effective, safe, and easy to implement to support the success of exclusive breastfeeding in postpartum mothers

Abstract

Cancer is one of the leading causes of death worldwide, necessitating the development of more effective and safe anticancer therapies. Phytochemical compounds from natural sources hold great potential due to their diverse biological activities. This study aims to assess the potential of phytochemical compounds as anticancer agents through a systematic literature review of fifteen scientific articles selected based on specific inclusion criteria and analyzed qualitatively. The results indicate that compounds such as flavonoids including quercetin, polyphenols, alkaloids, and terpenoids have anticancer activity through inhibition of cell proliferation, induction of apoptosis, and inhibition of angiogenesis and metastasis, mediated by molecular pathways such as p53, nuclear factor kappa B, and caspases. In addition, approaches such as nanoformulation and combination therapy can improve the efficacy and bioavailability of the compounds. These findings indicate that phytochemical compounds have the potential to be safer and more effective anticancer agents, and require further research for clinical development

Abstract

Irreversible skin aging begins in early adulthood due to intrinsic factors such as genetics and cellular metabolism and extrinsic factors such as ultraviolet exposure, smoking, pollution, and lifestyle, which trigger reactive oxygen species (ROS) and collagen damage, leading to wrinkles and reduced skin elasticity. This study aimed to formulate an optimal anti-aging cream from ethanol extract of porang tubers (Amorphophallus muelleri Blume) using variations in the HLB value of Span 60-Tween 80 emulsifiers. This quantitative experimental study used three cream formulas (F1: HLB 4.95; F2: HLB 5.7; F3: HLB 6.8) prepared from 3 kg of porang tubers obtained from Sirapit, Langkat. Evaluations included organoleptic properties, homogeneity, pH, viscosity, spreadability, adhesiveness, cycling-test stability, irritation, preference, and antioxidant activity using the DPPH method. The results showed that F2 was the optimal formula, with medium viscosity, spreadability of 6.2 cm², pH 5.8, stability for six cycles, and an IC₅₀ value of approximately 114 μg/mL, indicating moderate antioxidant activity. Formula F2 therefore has potential as a topical antioxidant preparation for preventing premature skin aging based on locally sourced tropical material.

Abstract

Acute respiratory infections including pneumonia remain a major cause of morbidity and mortality in children under five and case detection in primary care is often low despite the availability of guidelines; Putri Ayu Community Health Center recorded 53 cases of pneumonia in children under five in 2025 with a detection coverage of 26.49% of the target of 90%, so an implementation evaluation is needed. Objective: To evaluate the implementation of the ARI Control Program in the management of pneumonia in children under five using a logic model (input → activity → output). Method: Descriptive qualitative case study at Putri Ayu Community Health Center (Dec 2024–Apr 2026) with snowball sampling (n=9 informants: manager, clinician, pharmacist, emergency department, nutritionist, and caregiver); data collection through in-depth interviews, participant observation, and document review; thematic analysis using NVivo 12 and the Miles-Huberman framework with triangulation. Results: Input was partially adequate but training was uneven and there was a shortage of nurses during peak periods; some drugs, injection preparations, forms, and ARI sound timers were unavailable or damaged; Funding is relatively sufficient but early year disbursement is hampered; SOPs exist but require cluster adjustments; program activities are inconsistent (dominant passive findings, uneven breath counts, variable antibiotic duration, pre-referral is not yet uniform, use of KIA Handbooks is lacking, monitoring does not comply with guidelines); output is low (26.49%). Conclusion: Implementation is partially in accordance with the 2018 FKTP guidelines but is not yet consistent; it is recommended to strengthen active surveillance and reporting networks, standardize clinical references, uniform training for supporting units, improve logistics and critical equipment, and improve communication and monitoring to improve case detection and management compliance
