Abstract

The COVID-19 pandemic which started at the end of 2019 has had a major impact on various sectors, including the health sector in Indonesia. One group of health workers who are greatly affected are Village Midwives, who play an important role in Maternal and Child Health (KIA) services. During the pandemic, Village Midwives were faced with a double challenge, namely providing MCH services and playing a role in handling COVID-19 at the village level. With social restrictions and changes in health service policies, their duties and responsibilities have become more complex and difficult. This research aims to explore the duties and responsibilities of Village Midwives during the COVID-19 pandemic. This research uses a qualitative research method with a phenomenological approach. The subjects in this research were 5 village midwives who worked in the Cimareme Community Health Center working area. This research was analyzed manually using mixed inductive and deductive data analysis. The results of this research are that during the COVID-19 pandemic, Cimareme Community Health Center Village Midwives received additional duties in handling COVID-19 cases, namely tracing, testing, treatment, recording and reporting. The conclusion of this research is that during the COVID-19 pandemic, the Cimareme Community Health Center Village Midwife faced dual duties and responsibilities, namely carrying out MCH services and handling COVID-19 cases. The researcher's suggestion is that there is a need for further research using mixed methods and increasing the number of informants in order to provide a more comprehensive picture.

Abstract

Child’s development in the early of life will effect children wellbeing in the future. Early child development will be optimal if their nutritional need has been fullfiled. Breast Milk is the best food for baby in their early life. The purpose of this research was to know the association between duration of breast feeding with baby development age 6-12th month in Puskesmas Jambon work area district Ponorogo. This study was quantitative researches with cross sectional design which was done to 141 baby age 6-12th month in Puskesmas jambon work area. The result of this study was 47,5% baby who was not receiving exclusive breast feeding suspected had delayed developmental status, 26,2% baby who had been exclusively breast feeding for 6 month suspected had delayed developmental. Statistical analyze with logistic regression showed there are positif correlation between nutrional status with baby’s development p=0,020. Baby who was not exclusively breast feeding until 6-month 2,5 time higher to have delayed development (95% CI 1,158-5,308).

Abstract

Abstract Third trimester pregnant women will experience physiological changes. One of them is back pain. Untreated back pain will cause discomfort and interfere with daily activities. Interventions that can be given to third trimester pregnant women are pharmacological and non-pharmacological. Pharmacologically, the therapy given uses non-narcotic analgesic and narcotic analgesic drugs. While non-pharmacological therapies include massage, aromatherapy, acupuncture, reflexology, rest, and light exercise (walking, swimming, pilates, and pregnancy exercise. This research to determine the effect of pregnancy exercise on reducing lower back pain in pregnant women at the Cawas II Klaten Health Center. This type of research is a Quasi Experimental study with a One Group Pretest Posttest research design. The researcher measured lower back pain before (pre) pregnancy exercise treatment was carried out on the group of pregnant women, then the group of pregnant women was given pregnancy exercise treatment (treatment) and after being given pregnancy exercise, the group of pregnant women was measured for lower back pain (post) and so on until the lower back pain disappeared. The research design used a quasi-experimental one group pretest-posttest parallel. The scale of lower back pain before pregnancy from 28 respondents before prenatal exercise was on average the lowest on day-1, which was 3.43 with a minimum pain scale of 2 and a maximum of 6. While the scale of lower back pain before prenatal exercise was on average the lowest on day-6, which was 0.07 with a minimum pain scale of 0 and a maximum of 1. The scale of lower back pain from 28 pregnant women after prenatal exercise was the highest on day-1, which was 3.00 with a minimum scale of 1 and a maximum of 5. The scale of lower back pain after prenatal exercise was the lowest on day-6, which was 0.00 with a minimum and maximum scale of 0. It can be interpreted that from 28 pregnant women on day-6 the pain scale disappeared because the pain scale was known to be 0 (zero). Abstract

Postpartum blues is categorized as a mild mental disorder, therefore it is often ignored so that it is not diagnosed and not managed properly, finally it can become a difficult problem, and sometimes this disorder can develop into a more severe condition, namely depression and postpartum psychosis, which has a worse impact, especially in the problem of marital relations with husbands and the development of their children. This study aims to determine the Relationship between Parity Status and the Incidence of Postpartum Blues in the Juwiring Klaten Health Center Work Area. The design of this study is an analytical survey study. The approach used is cross-sectional. The population in this study were all postpartum mothers in the Juwiring Klaten Health Center work area as many as 44 people in March 2017. The sampling technique used purposive sampling. Data analysis used chi square. Respondent characteristics (age 20-35 years old as many as 40 people (97%), education is high school as many as 31 people (72.1%), occupation is housewife as many as 33 people (76.7%). Parity status in the work area of ​​​​Juwiring Klaten Health Center is mostly multiparous as many as 33 people (76.7%). The incidence of postpartum blues in postpartum mothers in the work area of ​​​​Juwiring Klaten Health Center is 17 people (39.5%) with moderate postpartum blues. There is a relationship between Parity Status and the Incidence of Post Partum Blues in the Work Area of ​​​​Juwiring Klaten Health Center with a p value = 0.035 (α> 0.05).

Abstract

Anxiety before childbirth is a common psychological response experienced by primigravida pregnant women. Factors such as self-acceptance and social support, particularly from the husband, play an important role in reducing this anxiety. This study aims to analyze the relationship between self-acceptance, husband’s support, and anxiety in primigravida pregnant women in the working area of Puskesmas Amahusu. The research design used is descriptive analytic with a cross-sectional approach. The results indicate that good self-acceptance and good husband support are significantly related to lower levels of anxiety. The P-value for self-acceptance (P = 0.002) and husband support (P = 0.002) suggest that these two factors have a significant influence on the anxiety levels of primigravida pregnant women. This study recommends the importance of enhancing emotional support from partners and strategies to improve self-acceptance in pregnant women in order to reduce anxiety before childbirth.

Abstract

Abstrak Kram perut bagian bawah adalah salah satu keluhan yang sering dialami oleh ibu hamil pada trimester III kehamilan, yang sering kali mengakibatkan ketidaknyamanan dan meningkatkan risiko stres. Salah satu pendekatan non-farmakologis yang efektif dalam mengurangi keluhan ini adalah terapi relaksasi. Penelitian ini bertujuan untuk mengevaluasi efektivitas terapi relaksasi dalam mengurangi intensitas kram perut pada ibu hamil trimester III. Metode yang digunakan adalah penelitian eksperimental dengan desain pra-eksperimen, di mana ibu hamil trimester III dengan keluhan kram perut bagian bawah diinstruksikan untuk melakukan beberapa teknik relaksasi, termasuk latihan pernapasan dalam dan visualisasi. Pengukuran intensitas kram dilakukan sebelum dan sesudah terapi menggunakan skala nyeri yang distandardisasi. Hasil penelitian menunjukkan adanya penurunan yang signifikan dalam intensitas kram perut setelah pelaksanaan terapi relaksasi. Terapi relaksasi terbukti aman, mudah diterapkan, dan memberikan efek positif dalam mengurangi keluhan kram perut bagian bawah pada ibu hamil trimester III. Diharapkan metode ini dapat diterapkan sebagai intervensi yang mendukung kesejahteraan ibu hamil pada fase akhir kehamilan. Abstract

Cervical cancer in Indonesia is a big problem in health services because most patients come at an advanced stage. This is thought to be due to inadequate screening programs. IVA examination is a simple way to detect cervical cancer as early as possible with the aim of reducing the morbidity of the disease by early treatment of cases found to identify abnormalities in the cervix. However, the examination can be an obstacle due to the mother's lack of compliance with this examination. The public interest in this examination is that most women are afraid to undergo the examination, apart from being embarrassed about their intimate organs, women are also afraid of the results they will obtain during the examination. This study aims to determine the influence of WUS attitudes towards compliance in participating in early detection of cervical cancer through the IVA test at the Hitu Community Health Center, Central Maluku Regency. The type of research used was analytical observational with a cross sectional research design, while the data analysis used was univariate and bivariate analysis with the Chi Square test. The research results found were that the majority expressed a positive attitude of 72%, the majority were willing to undergo an IVA examination, namely 30 people (60%). Based on statistical tests using the Chi-Square test, the p value was 0.000, where the p value < α (0.000 < 0, 05). The conclusion in this study is that there is an influence between WUS attitudes on compliance in participating in early detection of cervical cancer through VIA examinations at the Hitu Community Health Center, Central Maluku Regency. It is recommended that WUS want to carry out an IVA test and that health workers continue to provide health education about preventing cervical cancer through a healthy lifestyle and early detection through an IVA test. Abstract

This study aims to measure the motivation level of postpartum mothers in exclusive breastfeeding using the Motivation For Health Behavior Scale (MHBS), analyze the relationship between sociodemographic factors and motivation, and assess the relationship between motivation level and the sustainability of exclusive breastfeeding. A cross-sectional research design was used involving 228 postpartum mothers in Tangerang City and Regency selected through purposive sampling. Data collection utilized the MHBS instrument consisting of three dimensions: autonomous motivation, controlled motivation, and amotivation. The results showed that the majority of respondents (58.3%) had high motivation levels, with 65.8% having high autonomous motivation. Statistical analysis revealed significant relationships between education level and autonomous motivation (p=0.003), employment status and controlled motivation (p=0.021), and family support with all three motivation dimensions (p<0.05). Autonomous motivation had the strongest relationship with exclusive breastfeeding sustainability (OR=5.23; 95% CI: 3.12-8.75), followed by controlled motivation (OR=1.87; 95% CI: 1.09-3.21), while amotivation was negatively associated with exclusive breastfeeding (OR=0.31; 95% CI: 0.17-0.58). Mothers with high motivation were 4.18 times more likely to consistently provide exclusive breastfeeding. In conclusion, the motivation of postpartum mothers, especially autonomous motivation, plays an important role in the sustainability of exclusive breastfeeding. Interventions focusing on strengthening autonomous motivation through improved education and family support are needed to increase exclusive breastfeeding coverage. The use of MHBS as a motivation measurement instrument provides a more comprehensive approach in identifying determinants of exclusive breastfeeding.

Abstract

Masa nifas adalah periode penting bagi ibu setelah melahirkan, dengan tantangan utama pada keberhasilan menyusui. Dukungan dari suami berperan signifikan dalam keberhasilan ini, karena dapat memengaruhi fisik dan psikologis ibu. Penelitian ini bertujuan untuk menggali pengaruh dukungan suami terhadap keberhasilan menyusui pada ibu nifas, melalui studi kasus terhadap Ny. A di wilayah kerja Puskesmas Air Besar Kota Ambon. Ny. A, seorang ibu berusia 26 tahun, mengalami kesulitan menyusui pada minggu pertama pasca melahirkan, dengan keluhan ASI yang belum lancar, nyeri pada puting, serta kelelahan emosional akibat kurangnya dukungan suami. Penelitian ini mengobservasi perkembangan menyusui ibu selama 7 hari pascapersalinan, dengan intervensi berupa edukasi menyusui, teknik pelekatan, serta pendekatan komunikasi dengan suami. Hasil observasi menunjukkan bahwa keterlibatan suami dalam pekerjaan rumah tangga dan dukungan emosional berperan penting dalam memperbaiki kondisi ibu. Pada hari pertama, ibu masih kesulitan menyusui, namun setelah pendekatan kepada suami, terjadi peningkatan signifikan pada produksi ASI, kenyamanan ibu, serta kualitas tidur ibu pada hari kelima dan ketujuh. Penelitian ini menegaskan pentingnya dukungan emosional dan praktis dari suami dalam mendukung keberhasilan menyusui, yang tidak hanya meningkatkan kesehatan ibu dan bayi, tetapi juga memperkuat hubungan emosional mereka. Dukungan suami yang optimal sangat berperan dalam mendukung kelancaran proses menyusui, mengurangi stres, dan meningkatkan kualitas kehidupan ibu pasca persalinan. Pendekatan keluarga dalam asuhan kebidanan komprehensif sangat diperlukan untuk meningkatkan kualitas layanan kesehatan ibu dan anak

Abstract

In the world of obstetrics and gynecology, narrow pelvis or narrow pelvimetry often causes a mismatch between the size of the fetus and the birth canal called cephalopelvic disproportion (CPD). A narrow pelvis is one of the main causes of CPD, which is often an indication for cesarean section. In cases like this with a head presentation and narrow maternal pelvis, the risk of prolonged labor and complications increases significantly. Cesarean section is the main intervention of choice to save the life of the mother/baby so that it has an impact on reducing morbidity and mortality rates. However, if the implementation is not carried out on time or is repeated, it has significant physiological and psychosocial impacts. Various studies have shown that cesarean section intervention in cases of narrow pelvis significantly reduces the risk of obstetric complications in both the mother and the baby. Although cesarean section is effective in preventing morbidity and mortality, this procedure can also be at risk of causing postoperative complications and perinatal disorders if not carried out in a timely manner. This case study aims to describe the handling of cases of narrow pelvis with head presentation through cesarean section and the clinical impact on the mother and baby. Cesarean section in cases of narrow pelvis with cephalic presentation has been proven effective in preventing labor complications. Early diagnosis and appropriate action decisions are essential to ensure the safety of the mother and baby.
