Abstract

Introduction: The relationship of family support is closely related to the level of adherence to the diet of people with diabetes mellitus. Family support is the key to successful dietary compliance for people with diabetes mellitus because the family maintains and controls the nutritional intake that must be limited and carried out by sufferers. Purpose: The purpose of this study was to determine the relationship between family support and dietary adherence in adult diabetics at hosfital Imelda Medan. Method: This research method is descriptive with a quantitative approach and a cross sectional research design. Primary data collection was obtained directly through interviews consisting of the identity of the respondent, the characteristics of the respondent (age, gender, education level, type of work, and marital status), as well as the Perceived Dietary Adherence Questionnaire (PDAQ) and Hensarling’s Diabetes Family Support Scale (HDFSS). Data processing uses a computer system and data analysis uses SPSS with a chi-square test. Results: The results showed that there was a relationship between family support and dietary compliance in adults with diabetes mellitus at Hosfital Imelda Medan, which indicated a p-value of 0.001, where the number <α = 0.05. Conclusion: Family support is related to dietary adherence in adults with diabetes mellitus at Hosfital Imelda Medan

Abstract

This study aims to determine the efficiency of the patient registration process using the e-Puskesmas application at the Terjun Community Health Center (UPT). The research method used was qualitative with a descriptive approach, while data collection techniques were carried out through observation and interviews with two registration officers. The study was conducted from April to July at the Terjun Community Health Center. The results showed that the use of e-Puskesmas increased service efficiency, including faster registration times, easier data and patient history searches, and reduced queues due to the online queue feature through JKN Mobile. However, there were several obstacles such as occasional system errors and unstable networks, which affected the smoothness of the registration process. The conclusion is that e-Puskesmas has proven efficient in accelerating patient administration services, although IT infrastructure improvements are needed to overcome technical obstacles. This study recommends improving system stability and staff training to optimize the application.

Abstract

Chronic Kidney Disease (CKD), particularly End-Stage Renal Disease (ESRD), is a progressive and irreversible condition characterized by a gradual decline in renal function, resulting in the inability of the body to maintain metabolic balance and fluid-electrolyte homeostasis. According to data from the Indonesia Renal Registry (IRR), approximately 98% of patients with Chronic Kidney Disease undergo hemodialysis therapy, while only 2% receive peritoneal dialysis (PD). This study aimed to analyze the relationship between self-efficacy and dietary adherence among patients with chronic kidney disease undergoing hemodialysis at Haji General Hospital, Medan, in 2024. This study employed a quantitative approach using a cross-sectional design. The study population consisted of all patients undergoing hemodialysis twice a week. Samples were selected using an accidental sampling technique, resulting in 17 respondents who underwent hemodialysis during the three-day data collection period. Data were collected using structured questionnaires and analyzed using the Chi-square test. The results showed that among the 17 respondents, most had a moderate level of self-efficacy (10 respondents; 58.8%). Regarding dietary adherence, the majority of patients were non-adherent to the prescribed diet (11 respondents; 64.7%). Statistical analysis revealed a significant association between self-efficacy and dietary adherence among patients with chronic kidney disease undergoing hemodialysis at Haji General Hospital, Medan, with a p-value of 0.007 (p < 0.05). In conclusion, self-efficacy was significantly associated with dietary adherence among patients with chronic kidney disease undergoing hemodialysis at Haji General Hospital, Medan, in 2024. These findings suggest that nutritionists and healthcare professionals should provide continuous nutrition education and counseling to improve patients' dietary adherence.

Abstract

Hypertension is one of the leading non-communicable diseases worldwide and a major contributor to cardiovascular morbidity and mortality. Inadequate physical activity is a modifiable risk factor associated with poor blood pressure control among older adults with hypertension. Active Range of Motion (ROM) exercises have been proposed as a simple, safe, and low-cost non-pharmacological intervention to improve cardiovascular function and reduce blood pressure. This study aimed to determine the effect of Active ROM exercises on blood pressure among older adults with hypertension. A quasi-experimental study with a one-group pretest–posttest design was conducted at the Tanjung Rejo Community Health Center, Percut Sei Tuan, Medan, Indonesia. Twenty-five older adults with Stage I hypertension were recruited using purposive sampling. Blood pressure was measured before and after participants performed Active ROM exercises for three consecutive days. Descriptive statistics were used to summarize participant characteristics and blood pressure measurements, while inferential analysis was performed to evaluate changes in blood pressure before and after the intervention. Statistical significance was established at p < 0.05. The mean blood pressure decreased from 154/93 mmHg before the intervention to 137/90 mmHg after the Active ROM exercise program. The proportion of participants with Stage II hypertension decreased substantially, while the number of participants with normal blood pressure increased following the intervention. Statistical analysis demonstrated a significant reduction in blood pressure after Active ROM exercises (p = 0.000, p < 0.05). Active Range of Motion (ROM) exercises significantly reduced blood pressure among older adults with hypertension. These findings suggest that Active ROM exercises represent a practical, safe, and effective non-pharmacological intervention that can be incorporated into routine hypertension management programs in primary healthcare settings.

Abstract

Social stigma towards people living with HIV/AIDS (PLWHA) remains a major challenge in HIV/AIDS prevention and management efforts in Indonesia. This study aims to evaluate social stigma in Pulo Brayan Darat I Village, Medan Timur District, and identify the factors that influence it. The study used a qualitative approach with a descriptive phenomenological design. Data were collected through in-depth interviews with eight informants selected using purposive sampling, supplemented by observation and documentation. The results of the study found that public understanding of HIV/AIDS is still low, especially regarding its transmission, which causes stigma and fear towards PLWHA. The community considers HIV to be a deadly and easily transmitted disease, so they tend to keep their distance and discriminate, although some have begun to show an inclusive attitude towards the rights of PLHIV to work and socialize. The conclusion of this study shows that low levels of knowledge are a major factor in the emergence of stigma and discrimination, so continuous education is needed to improve social acceptance and improve the quality of life of PLHIV.

Abstract

Background: Nurses' knowledge of EWS is one of the factors influencing patient detection in hospitals. Objective: This study aims to determine the description of nurses' knowledge of EWS based on characteristics at Imelda Regional Hospital, Indonesian Workers in 2024. Method: This study used descriptive research. The population was 137 people with a sample of 58 people. Sampling in this study was carried out using accidental sampling technique. Results: The study showed the results of the characteristics of respondents according to age that the most were in the 25-29 year age group with 23 respondents (39.7%). Based on gender, the majority of nurses were female with 42 respondents (72.4%). The majority of respondents were at the D3 Nursing education level with 30 respondents (51.7%). The majority of respondents were in the work period >3 years (long) with 38 respondents (65.5%). Meanwhile, the majority of respondents who participated in training were 55 respondents (94.8%). The frequency distribution of nurses' knowledge about EWS had sufficient knowledge with 46 respondents (79.3%). The most knowledgeable based on age 25-29 years as many as 17 respondents (73.3%), the most knowledgeable based on gender as many as 33 respondents (78.6%), the most knowledgeable based on D3 Nursing education as many as 26 respondents (86.7%), the most knowledgeable based on length of service category (>3 years) as many as 32 respondents (84.2%), the most knowledgeable based on training attended as many as 45 respondents (81.8%). Conclusion: Nurses' knowledge affects their ability to identify patients with worsening patient conditions. Suggestion: It is recommended that hospitals provide EWS training for those who have never attended training and those who have received training for new nurses, and for experienced nurses to be refreshed so that nurses' knowledge can continue to be improved.

Abstract

Hemodialysis is the process of filtering or separating blood from dissolved substances, which in this case are waste products or toxins circulating in the body through the blood, with the assistance of a machine outside the body called a dialysis machine. Chronic Kidney Disease (CKD) is a disorder that occurs in the kidneys and can be caused by various factors. The early onset of this disease generally does not show specific symptoms, making early detection difficult, which often results in patients with kidney failure receiving treatment late. The purpose of this study was to determine the description of knowledge among patients undergoing hemodialysis, to identify Continuous Ambulatory Peritoneal Dialysis (CAPD) in patients undergoing hemodialysis, and to determine the description of knowledge of patients undergoing hemodialysis at Imelda Indonesian Workers General Hospital regarding Continuous Ambulatory Peritoneal Dialysis (CAPD). This study used a quantitative descriptive research design. The study involved two variables: the independent variable, which was knowledge, and the dependent variable, which was Continuous Ambulatory Peritoneal Dialysis (CAPD). The sampling technique used in this study was purposive sampling, where samples were selected based on predetermined criteria. The total sample consisted of 60 respondents. The results of the Chi-Square test presented in the table showed a p-value of 0.004. This means that the probability value is less than the level of significance (α = 0.05), so Ha is accepted and H0 is rejected. Therefore, it can be concluded that there is a significant relationship between the knowledge of patients undergoing hemodialysis and CAPD.

Abstract

Multidrug-Resistant Tuberculosis (MDR-TB) is a form of tuberculosis resistance to at least two first-line anti-tuberculosis drugs, namely Isoniazid (INH) and Rifampicin (RMP), with or without resistance to other anti-tuberculosis drugs. MDR-TB can be influenced by various factors, including age, gender, education level, income, occupation, medication adherence, knowledge, attitude, stigma, smoking habits, the role of Treatment Supervisors (PMO), and drug side effects. This study aimed to identify risk factors associated with drug-resistant tuberculosis at RSU IPI Medan. This study employed a descriptive research design with a purposive sampling technique. The total number of respondents was 27 patients. The results showed that most respondents were aged 36–45 years (29.6%), male (59.3%), had a senior high school education (40.7%), worked as self-employed individuals (33.3%), and had low income levels (62.96%). Respondents who smoked accounted for 59.3%. All respondents had good PMO support (100%). Most respondents experienced drug side effects (92.6%), were adherent to medication (74.7%), had good knowledge (74.7%), and demonstrated positive attitudes (81.48%). Community stigma toward patients was categorized as low (59.26%), while patient self-stigma was considered high (55.56%). It is expected that the findings of this study can serve as a source of information and educational material to support the prevention of drug-resistant tuberculosis.

Abstract

Perineal wounds are among the most common complications experienced by postpartum mothers and may increase the risk of infection when not managed appropriately. One potential non-pharmacological therapy is guava leaf (Psidium guajava) water spray, which contains flavonoids, tannins, and saponins that contribute to wound healing. This study aimed to analyze the effect of guava leaf water spray on perineal wound healing among postpartum mothers at Pulo Bandring Public Health Center, Asahan Regency, in 2026. A pre-experimental one-group pretest-posttest design was employed. Twenty-five postpartum mothers were recruited using purposive sampling. Wound healing was assessed using the REEDA scale before and after the intervention. Data were analyzed using descriptive statistics and the Wilcoxon Signed Rank Test. The mean REEDA score decreased from 7.80 ± 1.98 before intervention to 2.76 ± 1.20 after intervention. The Wilcoxon test demonstrated a statistically significant difference (Z = −4.451; p = 0.000). Guava leaf water spray was effective in accelerating perineal wound healing among postpartum mothers. This intervention may serve as a safe, practical, and affordable complementary therapy in postpartum care.
