Abstract

Psychological disorders during the postpartum period constitute a significant health issue that can affect maternal well-being, infant caregiving, and overall family quality of life. Several factors are presumed to be associated with the psychological condition of postpartum mothers, including spousal support and bonding attachment between mother and infant. This study aimed to analyze the relationship between spousal support and bonding attachment with the psychological condition of postpartum mothers at Royal Prima General Hospital Medan in 2025. This study employed a quantitative analytic design with a cross-sectional approach. The sample consisted of 40 postpartum mothers selected using an accidental sampling technique. Data were collected using questionnaires on spousal support, bonding attachment, and the Edinburgh Postnatal Depression Scale (EPDS) to assess the psychological condition of postpartum mothers. Data analysis was conducted using univariate and bivariate methods with the Chi-Square test at a 95% confidence level, and Odds Ratios (OR) along with 95% Confidence Intervals (95% CI) were calculated. The results showed a significant relationship between spousal support and the psychological condition of postpartum mothers (p=0.001). Mothers who received good spousal support were 11.66 times more likely to have a normal psychological condition compared to those who did not receive such support (OR=11.66; 95% CI=2.221–61.27). In addition, bonding attachment was also significantly associated with the psychological condition of postpartum mothers (p=0.001). Mothers with good bonding attachment were 116.0 times more likely to have a normal psychological condition compared to those with poor bonding attachment (OR=116.0; 95% CI=9.288–1448). This study concludes that spousal support and bonding attachment are significantly associated with the psychological condition of postpartum mothers. Therefore, the involvement of husbands in supporting mothers and the early strengthening of bonding attachment should be optimized as efforts to improve maternal mental health during the postpartum period.

Abstract

The pre-analytical phase is a critical stage in clinical laboratory testing because inappropriate specimen handling may affect the accuracy of laboratory results, including serum triglyceride measurements. This study aimed to determine the differences in serum triglyceride levels between blood samples that were immediately centrifuged and those allowed to clot before centrifugation. This analytical study employed a cross-sectional paired-comparison design involving 31 respondents selected using purposive sampling. Venous blood samples from each respondent were subjected to two different pre-centrifugation treatments: immediate centrifugation and clotting for approximately 20 minutes before centrifugation. Serum triglyceride levels were determined using the enzymatic colorimetric glycerol phosphate oxidase–phenol aminophenazone (GPO-PAP) method with a photometer at a wavelength of 546 nm. Data were analyzed using descriptive statistics and the Wilcoxon Signed Rank Test at a significance level of 0.05. The results showed that the mean serum triglyceride level in blood samples that were immediately centrifuged was 142 mg/dL, whereas the mean level in blood samples allowed to clot before centrifugation was 156 mg/dL. Statistical analysis revealed a significant difference between the two treatments (p < 0.001), indicating that pre-centrifugation sample handling significantly influenced serum triglyceride measurements. It can be concluded that blood samples allowed to clot before centrifugation produced significantly higher serum triglyceride levels than samples that were immediately centrifuged. Therefore, adherence to standardized pre-analytical procedures is essential to ensure the accuracy, reliability, and consistency of serum triglyceride measurements in clinical laboratory practice.

Abstract

Bayi prematur adalah bayi yang lahir sebelum usia kehamilan 37 minggu dan berisiko tinggi mengalami gangguan pertumbuhan, termasuk keterlambatan peningkatan berat badan. Perawatan Metode Kanguru (Kangaroo Mother Care/KMC) merupakan intervensi berbasis kontak kulit ke kulit yang terbukti membantu stabilisasi fisiologis dan mendukung pertumbuhan bayi prematur. Penelitian ini bertujuan untuk mengetahui hubungan antara perawatan metode kanguru dengan peningkatan berat badan bayi prematur di UPTD Puskesmas Mompang Kecamatan Panyabungan Utara Kabupaten Mandailing Natal Tahun 2026.Penelitian ini menggunakan desain kuantitatif dengan pendekatan analitik korelasional dan metode cross-sectional. Sampel berjumlah 30 bayi prematur yang dipilih dengan teknik purposive sampling. Data dikumpulkan melalui lembar observasi pelaksanaan KMC dan pengukuran berat badan menggunakan timbangan digital bayi. Analisis data dilakukan secara univariat dan bivariat menggunakan uji Chi-Square dengan tingkat signifikansi α = 0,05.Hasil penelitian menunjukkan bahwa 50% ibu melakukan KMC dengan frekuensi sedang (2–3 kali/hari) dan 50% bayi mengalami peningkatan berat badan kategori tinggi (>25 gram/hari). Hasil uji statistik menunjukkan p-value = 0,000 (p < 0,05), yang berarti terdapat hubungan signifikan antara frekuensi KMC dan peningkatan berat badan bayi prematur.

Abstract

Medical discharge summaries are essential documents in healthcare services that provide a comprehensive overview of a patient's clinical condition throughout hospitalization. The consistency of completing medical discharge summaries within an Electronic Medical Record (EMR) system plays a crucial role in ensuring documentation quality, patient safety, and the efficiency of administrative processes and health insurance claims. This study aimed to explore healthcare workers' experiences in maintaining consistent medical discharge summaries within the Electronic Medical Record system at Imelda Pekerja Indonesia General Hospital. A qualitative research design with a phenomenological approach was employed. The participants consisted of the Attending Physician (DPJP), the Head Nurse of the Emergency Department, medical record officers, Case Mix officers, and Hospital Information System (HIS) officers. Data were collected through in-depth interviews and analyzed using data reduction, data display, and conclusion drawing techniques. The findings revealed that the consistency of medical discharge summary documentation was influenced by three major factors: organizational support through training and socialization programs, healthcare workers' understanding of clinical documentation standards and Standard Operating Procedures (SOPs), and hospital policies supporting the implementation of Electronic Medical Records. In addition, workload, differences in perceptions regarding documentation completeness, and technical issues within the EMR system remained significant challenges affecting documentation consistency. The study concludes that strengthening healthcare workers' competencies, optimizing validation features within the Electronic Medical Record system, and implementing continuous monitoring and evaluation are necessary to improve the quality of medical documentation and hospital healthcare services.

Abstract

Type 2 diabetes mellitus is a chronic disease requiring comprehensive management, including proper dietary regulation. Poor adherence to meal schedules often results in uncontrolled blood glucose levels despite patients' awareness of recommended food choices. Diet menu schedule education is considered an important non-pharmacological intervention to improve patients' knowledge and glycemic control. This study aimed to determine the effectiveness of diet menu schedule education on patients' knowledge and blood glucose levels among individuals with diabetes mellitus at Sultan Iskandar Muda Hospital, Nagan Raya, Indonesia. A quantitative study employing a quasi-experimental one-group pretest-posttest design was conducted involving 30 patients with type 2 diabetes mellitus selected through purposive sampling. Participants received structured diet menu schedule education. Knowledge was assessed using a validated questionnaire, while random blood glucose levels were measured through clinical examination. Data were analyzed using the Paired Sample t-test with a significance level of 0.05. Before the intervention, most participants demonstrated poor knowledge (63.3%), with a mean blood glucose level of 236.4 mg/dL. Following the educational intervention, 76.6% of participants achieved good knowledge scores, while the mean blood glucose level significantly decreased to 149.1 mg/dL. Statistical analysis revealed significant improvements in both knowledge and blood glucose levels after the intervention (p < 0.001). Diet menu schedule education effectively improved patients' knowledge and significantly reduced blood glucose levels among individuals with type 2 diabetes mellitus. This educational strategy should be incorporated into routine diabetes management programs to enhance patients' self-management and glycemic control.

Abstract

Background: Early Breastfeeding Initiation (EBI) is a WHO/UNICEF global strategy to reduce neonatal mortality. However, its implementation still faces challenges, including the role of midwives as facilitators. Methods: A descriptive qualitative study using a case-study approach. Data were collected through semi-structured interviews and observation of 5 midwives/nurses in the Lactation Room of USM Hospital Kelantan. Data were analyzed using data reduction, data display, and verification. Results: (1) Midwives' support included education (benefits and technique of EBI), direct assistance, and promotion of rooming-in (100% of mothers); (2) EBI success was achieved in 80% of mothers, with infants finding the nipple within 1 hour (80%), skin-to-skin contact ≤2 hours (100%), and strengthened emotional bonding (100%); (3) There was a clear relationship between midwives' support and EBI success: mothers with high support (4 individuals) achieved optimal EBI, while limited support (1 individual) resulted in partial EBI. Conclusion: Holistic midwifery support (education, technical assistance, and emotional support) plays a crucial role in EBI success. Strengthening midwives' capacity through continuous training is recommended to optimize the program.

Abstract

Background: Premature birth is a significant global and national health issue causing substantial psychological distress in primiparous mothers. Their lack of experience and the challenging NICU environment exacerbate maternal anxiety and self-distrust. This study aims to explore the experiences of primiparous mothers in caring for premature infants at Imelda Pekerja Indonesia Hospital, Medan. Objective: To explore the experiences of primiparous mothers in caring for premature infants at Imelda Pekerja Indonesia Hospital, Medan, including their level of knowledge and readiness, pregnancy and delivery history, psychological status, and family and healthcare professional support. Methods: A descriptive qualitative study with a phenomenological approach was employed. A sample of 7 primiparous mothers was selected through purposive sampling. Data were collected via semi-structured interviews and analyzed thematically. Validity was maintained through member checking and source triangulation. Results: Five main themes and thirteen subthemes were identified: (1) Mother's Knowledge and Readiness Level (lack of knowledge, readiness to care, care practices, role transformation); (2) Pregnancy and Delivery History (ANC non-compliance, complications, signs of prematurity); (3) Psychological Status (psychological changes, coping mechanisms); (4) Support (family, husband, healthcare professionals); and (5) Mother's Hopes (healthy baby). Primiparous mothers faced significant challenges but demonstrated strong adaptation due to comprehensive support. Conclusion: Primiparous mothers experience emotional and knowledge complexities in caring for premature infants but show resilience and adaptation. Support from family, husbands, and healthcare professionals is crucial in this process, shaping hopes for the baby's optimal health. Recommendations include enhancing education, psychosocial support, and a more supportive care environment.

Abstract

Pregnancy-related hormonal and biomechanical changes may reduce pelvic and back stability and contribute to back pain, particularly in the third trimester. The Rebozo technique, which uses a shawl to provide gentle rocking or pressure around the pelvis, is a non-pharmacological approach intended to reduce discomfort and support maternal well-being. This study aimed to determine the relationship between the application of the Rebozo technique and reduced back pain levels to improve reproductive health in pregnant women. A quantitative analytical study with a cross-sectional approach was conducted at the Juliana Dalimunthe Clinic from April to June 2026. Thirty-five third-trimester pregnant women with back pain were selected using accidental sampling. Data were collected using a questionnaire and analyzed using the chi-square test at a 95% confidence level (alpha = 0.05). Thirty respondents (86%) applied the Rebozo technique. The association between Rebozo application and reduced back pain was statistically significant (p = 0.001). The Rebozo technique may be used as a complementary, non-pharmacological option to help pregnant women manage third-trimester back pain and support reproductive health.

Abstract

This study aims to develop and evaluate a Digital Quality Assurance (DQA) Framework to support continuous accreditation readiness at the Faculty of Medicine, Universitas Imelda Medan (FK UIM). The study employed a mixed-method approach, combining Design Science Research for framework prototyping with a descriptive correlational (cross-sectional) design using questionnaires administered to lecturers and administrative staff involved in quality assurance. The results show that the level of Digital Quality Assurance Framework implementation was predominantly in the moderate category (45.0%), while the level of continuous accreditation readiness was predominantly in the not-ready category (57.5%). The Chi-Square test indicated a significant relationship between framework implementation and accreditation readiness (p = 0.001), with Spearman correlation showing a strong positive relationship (rs = 0.612). The resulting framework prototype consists of five modules: digital document management, performance indicator dashboard, learning outcome monitoring, early-warning, and automated reporting.

Abstract

The release of medical record information is a critical component of the private health insurance claim process, as it is closely related to document completeness, patient data confidentiality, and the timely reimbursement of healthcare services. Incomplete medical record documentation may result in claim rejection or resubmission, leading to delays in reimbursement and reduced administrative efficiency within hospitals. This study aimed to analyze the barriers to the release of medical record information for private health insurance claims based on the 5M framework (Man, Material, Method, Machine, and Money) at Mitra Sejati General Hospital, Medan. A qualitative study with a case study approach was employed. Three informants were purposively selected, consisting of one Head of the Insurance Unit and two medical record information release officers directly involved in the insurance claim process. Data were collected through in-depth interviews, direct observation, and document review, and were analyzed using the interactive model of Miles and Huberman. The findings revealed that, of the 490 private insurance claim files submitted, 384 (78%) were complete, while 106 (22%) were incomplete. The primary cause of incomplete claims was incomplete medical discharge summaries (44%). The main barriers were associated with the Man factor, including an insufficient number of personnel, inadequate verification of document completeness, and delays in completing discharge summaries by the attending physicians. The Material and Method factors also contributed to claim delays due to incomplete supporting documents and the absence of detailed standard operating procedures specifically governing the release of medical record information for private insurance claims. In contrast, the Machine and Money factors were not identified as significant barriers because adequate facilities and financial support were available. The study concludes that improving the quality of medical record documentation, optimizing human resources, and developing more comprehensive standard operating procedures are essential strategies for enhancing the effectiveness of medical record information release in the private health insurance claim process.
