Abstract

PT. ABC is a company engaged in the manufacture of automotive spare parts and machinery. Based on the results of interviews and observations, it is known that there are problems regarding the large number of product defects produced when producing fuel filters of 3.84%. The purpose of this research is to improve the quality of producing fuel filter products using the six sigma method to identify, analyze and provide several suggestions for improvement. The most common types of product defects are broken filters, non-centered filters and reject settings. Based on the results of data processing, the Cp value was 0.96, the Cpk was 0.88, the DPMO was 12788 units and the sigma value was 3.73. Then implemented the proposed improvements in the form of making a checksheet, one point lesson and parameter standardization. The results of calculations performed at the control stage are the p control chart showing that the data is more stable, the process capability value increases to 1.17 and 1.11, the DPMO value decreases to 11652 and the sigma value increases to 3.76. This shows that the implementation has succeeded in improving the quality of producing fuel filters.

Abstract

This research is located in conventional car washes where there are still many who use human labor in carrying out car wash activities. The posture during the car wash is considered less ergonomic, so this study aims to help car wash workers reduce their fatigue and even help their work position become more ergonomic. This research began with observations at the car wash and also interviews with workers. Interviews with the workers were conducted using the Nordic Body Map (NBM). Based on the results of observations and interviews, it was determined that the working posture was considered less ergonomic where there were three working postures, namely the posture when taking the water hose, the posture of washing the side of the car, and the posture of washing the top of the car. The three postures that are considered problematic are analyzed using the Workplace Ergonomic Risk Assessment (WERA) to determine the value of their ergonomics. When he learned that the three postures were problematic, the research continued by designing tools to reduce workload, this design was carried out using the VDI 2222 method. Designing with the VDI 2222 method there are 4 stages, namely analysis, concept, design, and completion. By using the VDI 2222 method, the concept, size, and specifications needed for belts and seats are known as a car washing aid. The last stage of this study is to re-analyze with the WERA method on all three postures but with the help of tools.

Abstract

PT. XYZ is a company that produces industrial shelf. There were several complaints such as dented racks, peeled off paint racks, uneven heights, incomplete and untidy welds, or other things that caused customer dissatisfaction. This research aims to measure the level of customer satisfaction with the company's performance in the service aspect and also the quality of the products produced. With this research, it will provide recommendations for improvements to PT. XYZ. The service quality method is used to determine the distribution of attributes based on quadrants. After the results were obtained, further questionnaires were distributed regarding product quality. Finally, it is continued with the Quality Function Deployment method for making a House of Quality and the priority technical response is obtained, namely the need to improve product quality as a top priority, the need to select and use appropriate and reliable material suppliers as the second priority, the need to pay attention to checking goods so that they are delivered properly as the third priority, it is necessary to hold training for production workers as the fourth priority. Meanwhile, based on the normalized row weight, the attributes that affect quality are the design of the shelf model, the neatness of the product in the process, the superiority of the product compared to competitors, the product reaches the customer without any defects, the product has been tested in use.

Abstract

Material handling is one of the things that is often a problem for both large companies and MSMEs. One example of a manual material handling problem occurs in MSMEs Tirta 88 as a provider of drinking water in gallons. The average sales of gallon refillable water at MSMEs Tirta 88 over the past 6 months is 348 gallons / day. Therefore, workers at MSMEs Tirta 88 have many complaints. The complaint data collection uses a Nordic Body Map (NBM) questionnaire. Then the work posture assessment using the Rapid Entire Body (REBA) method and the Workplace Ergonomy Risk Assessment (WERA). The design method used is VDI 2221. The results of this study are in the form of a new 3D gallon basket design that is in accordance with the anthropometry of workers in MSMEs Tirta 88 and will be implemented. Before being implemented, a 3D design simulation analysis was carried out in order to determine the decrease in Rapid Entire Body (REBA) and Workplace Ergonomy Risk Assessment (WERA) scores. The simulation results showed a decrease in the Rapid Entire Body (REBA) score to four for all postures and the Workplace Ergonomy Risk Assessment (WERA) to 35 for two postures and 30 for one posture.

Abstract

Occupational accidents are unexpected events, because accidents that cause injuries, disabilities, up to the worst can result in death, or the cessation of the production process. PT. Indofood Fortuna Makmur is a manufacturing company engaged in the production of snacks. In production activities, potential work hazards can be found easily in work areas, in warehouses, production departments and packing departments. During 2021-2022 there were several work accidents that occurred due to worker negligence or potential hazards from the work environment. Based on the results of the HIRARC table for the warehouse, production and packing departments, it can be seen that the most high risks occur in the production department and the results of interviews with employees with the potential for work accidents are more common in the production department. part. One of the risks of work accidents that often occur is workers slipping because of slippery floors. To analyze and carry out implementation in reducing the risk of work accidents, in addition to using the HIRARC method to identify and analyze potential hazard risks, the HAZOP method to classify or classify potential hazards, and the FMEA method are used to determine priority sequences for handling work accidents. The results of this study are in the form of proposed improvements and implementation of the production process of potato chips and tempeh chips at PT. Indofood Fortuna Makmur. The implementation that has been carried out is the use of safety shoes for the production floor area, the use of food tongs in the sorting area, the installation of K3 displays, the socialization of K3. Based on the implementation results that have been carried out, there has been a change in potential risk.

Abstract

Good product quality or not depends on factors that influence it such as the quality of raw materials used, production processes, production tools, skilled human resources and can work effectively to produce a product in the company. The company under study was founded in 1992 in the Pulo Gadung area, East Jakarta. This company produces a product in the form of pump casting. This pump casting has several types of products, such as volute casing, impeller, bearing housing, and casing cover. Based on the results of observations and so on, the three biggest wastes are defects, overproduction and waiting. The research was conducted utilizing the lean six sigma method with the DMAIC (Define, Measure, Analyze, Improve, Control) stage to minimize waste and create continuous improvements. After analyzing and processing the data, it was noted that the three biggest wastes during the production process were defects of 26.35%, overproduction of 22.90%, and waiting of 12.60%. Regarding the sigma and DPMO values, the DPMO calculation results obtained are 14,972 with a value. sigma is at 3,671. After providing recommendations for improvements to the problem, the duration of non-value added activity in value stream mapping was successfully reduced from 671,700 seconds to 361,080 seconds which increased the PCE value from 25.32% to 38.67%, with improvements the DPMO value is 4289 units and the sigma value after repair is 4.128.

Abstract

In increasingly fierce business competition, companies must have a strategic plan that aims to focus on achieving long-term goals by linking organizational strategic plans. PT. IBE is a company in Indonesia operating in the recycling of metal resources. In carrying out its duties, the company currently only focuses on finances to be able to win the competition in the industrial world, so that other aspects are not considered. Thus, PT. IBE needs to measure company performance so that it can set targets and performance in company assessments. In measuring this performance, the Balanced Scorecard (BSC) approach is used by describing the company's vision and mission into 4 perspectives, namely Finance, Customers, Internal Business Processes, and Learning and Growth which have weights of 0.314, 0.247, 0.201 and 0.239 respectively. In this study sparked 12 Key Performance Indicators (KPI), Objective Matrix (OMAX) and the Traffic Light System assessment system and found that there were 2 KPI indicators that were already good, 5 KPI indicators had not been achieved but were close to the target, and 5 KPI indicators were still far away. under targets. After that, strategic planning is carried out using Hoshin Kanri using the Hoshin planning X-Matrix in order to achieve long-term goals and strengthen the vision and mission statement that the company expects to continue to grow.

Abstract

The high number of product defects in the manufacturing industry has a significant impact on the products quality and productivity of the enterprise. The research was conducted at manufacturing companies that produce drums, pail cans, and other can packaging. In the production process, they found a high defect rate in the drum production process, which resulted in a decrease in the quality and productivity of the factory. This research is aimed at assisting companies in reducing the defect rate of drum production; improving process efficiency; providing proposed process improvements; and minimizing existing waste. This study used the Lean Six Sigma approach, using SIPOC and CTQ to define problems, and measure process efficiency and product quality, obtaining a process cycle efficiency value of 8.06% and a sigma value of 4.6%. Based on the pareto diagram, the dominant defects are abrasions, paint top/body damaged, and screen printing top/body damaged, while based on WAM, the dominant defects are defects, inventory, and waiting. Based on this stage, analysis was carried out on all three defects using fault tree analysis, why-why analysis, and FMEA. In the future, improvements will be implemented to reduce the number of defects and increase productivity so that there is an increase in the value of PCE of 9.23% and a sigma value of 5.04 after improvements.

Abstract

The company is required to provide the best quality with zero defects in the products they produce. However, zero defects cannot be realized. Therefore, strategies were developed to improve product quality using the six sigma method (DMAIC) and the house of quality. The object of research in this study was crumble-shaped broiler feed. Based on the calculations performed, it can be seen that the company's DPMO value and sigma levels are 2,358.248 kg and 4.326. Based on the house of quality that has been arranged, it can be seen that the three technical responses (quality improvement strategies) with the highest ratings that can be prioritized by the company are counseling regarding the application of work instructions and SOPs to all employees, inspection of production machines or equipment before the production process is carried out, and implementation of safety stock and reorder points on fast-moving spare parts. Assuming that the quality improvement strategy is implemented, it can be seen that there was an increase in the Cp value to 1.297, an increase in the Cpk value to 0.7733, a decrease in the DPMO value to 1,257.65, and an increase in the sigma level to 4.521.

Abstract

Printing machine is a tool for the process of transferring images for the printing industry. The machines studied at this company are the GTO 52, MO 65, and GTO 4W engines. Production will be problematic when there are many failure factors that affect machine performance and result in reduced productivity results. As in March, the total downtime of printing machines reached 1,067 minutes, while the rejects were 13,559 pcs. This is because the company also does not have proper maintenance implementation so that operators must be alert. Therefore, this study aims to carry out a comparative analysis of the effectiveness and determine the failure potential of each machine owned by this company using the FMEA and OEE methods. Based on the implementation results, it can be seen that the potential for failure occurs due to machine handling which must be improved through scheduling or periodic checking in order to reduce the hazard from the impact of machine conditions, product quality and increase the efficiency of printing machines. Meanwhile, the OEE value for April 2022 of the three machines produced 30.87% on the GTO 52 and 39.78% on the MO 65, which means that the effectiveness value is lower than the GTO 4W engine of 200.85%. The results of other months are still experiencing irregular increases and decreases, which means that the three machines need the right handling to be able to produce high OEE values ​​so that their operations are more efficient.
