Abstract

UVB-induced photoaging accelerates extracellular matrix (ECM) degradation by increasing MMP-1 and suppressing TIMP-1 expression. Centella asiatica, a traditional medicinal herb with antioxidant and anti-inflammatory properties. However, its effect on the MMP-1/TIMP-1 balance under acute UVB exposure hasnot been widely studied. This study aimed to evaluate whether topical C. asiatica extract cream at 5% and 10% concentrations can reduce MMP-1 and increase TIMP-1 in UVB-irradiated Wistar rat. A total of 25 male Wistar rats were randomly assigned to five groups: healthy control, UVB + vehicle, UVB + 5% niacinamide (positive control), UVB + C. asiatica 5%, and UVB + C. asiatica 10%. UVB irradiation (160 mJ/cm²) was administered once daily for five days, with topical application performed 20 minutes before and 4 hours after irradiation. MMP-1 and TIMP-1 levels were measured from skin tissue. UVB exposure significantly increased MMP-1 levels compared to healthy controls (3.100 ± 0.5234 vs. 1.778 ± 0.4081 pg/mL, p < 0.05). The 10% C. asiatica group showed a significant MMP-1 reduction(2.238 ± 0.2705 pg/mL, p < 0.05 vs. UVB + vehicle), comparable to niacinamide. The 5% C. asiatica group showed a non-significant reduction (2.678 ± 0.3148 pg/mL, p > 0.05). TIMP-1 levels did not differ significantly among groups (p = 0.3432), although the 5% C. asiatica and niacinamide groups showed higher TIMP-1 trends. The 10% Centella group had the lowest TIMP-1 levels among treated animals. We conclude that topical C. asiatica extract, especially at 10%, efficiently reduces UVB-induced increase of MMP-1, while its effects on TIMP-1 are still unclear. These results validate C. asiatica’s promise as a topical anti-photoaging drug, necessitating additional research into the best dosage and ECM regulatory mechanisms.

Abstract

Oral squamous cell carcinoma (OSCC) accounts for approximately 90% of oral malignancies and most frequently involves the mobile tongue, where it is associated with substantial functional impairment and mortality. Although OSCC is classically associated with tobacco and alcohol exposure, it may also occur in patients without a history of active smoking or alcohol use. We report a case of HPV DNA-negative, well-differentiated invasive squamous cell carcinoma of the ventrolateral mobile tongue in a 72-year-old woman. The patient presented with a three-month history of a progressively enlarging tongue lesion accompanied by pain, difficulty eating, and unintentional weight loss. She denied active smoking, alcohol consumption, and betel quid use but reported passive exposure to tobacco smoke. Clinical examination revealed an exophytic erythroleukoplakic lesion involving the ventrolateral tongue, along with multiple missing teeth and untreated dental caries. Histopathological examination of a punch biopsy demonstrated invasive nests of malignant squamous epithelial cells with keratin pearl formation, prominent intercellular bridges, and invasion into the underlying connective tissue and muscle, consistent with well-differentiated HPV-negative squamous cell carcinoma.. Following multidisciplinary evaluation, the patient underwent induction TPF chemotherapy (docetaxel, cisplatin, and fluorouracil), followed by hemiglossectomy with modified radical neck dissection and adjuvant radiotherapy. This case highlights that persistent lesions of the mobile tongue in elderly patients warrant careful evaluation, even in the absence of active smoking and alcohol use history. Early biopsy, thorough histopathological assessment, and multidisciplinary management are essential for timely diagnosis and appropriate treatment planning.

Abstract

Retinopathy of prematurity (ROP) remains a leading cause of preventable childhood blindness. Although gestational age and birth weight are well-established risk factors, the contribution of other clinical factors may vary across settings, particularly in low-and-middle-income countries such as Indonesia. The study aimed to evaluate risk factors associated with the incidence of retinopathy of prematurity, including gestational age, birth weight, duration of oxygen therapy, and infection, in a tertiary-care hospital in Indonesia. This retrospective study reviewed medical records of premature infants with and without ROP at Prof Dr. RD Kandou General Hospital, Manado, Indonesia. Eligible infants underwent initial ophthalmologic examination at or before 1 years old. Data collected included sex, gestational age, birth weight, duration of oxygen therapy, presence of infection or sepsis, and ROP status. Associations were evaluated using Chi-square tests and bivariate logistic regression to estimate crude odds ratios (ORs). Multivariable logistic regression analysis was performed to adjust for potential confounding, with adjusted odds ratios (aORs) and 95% confidence intervals (CIs) reported. Multivariable results were interpreted as exploratory due to the limited number of ROP cases. A total of 51 infants were included, of whom 14 (27.5%) developed ROP. Lower gestational age (≤34 weeks), lower birth weight (≤1500 grams), and oxygen therapy duration ≥1 weeks were significantly associated with ROP in bivariate analyses. After multivariable adjustment, none of the variables remained statistically significant, although the direction of association persisted for gestational age, birth weight, and oxygen therapy. Infection or sepsis was not significantly associated with ROP. In conclusion, lower gestational age, lower birth weight, and prolonged oxygen therapy are associated with an increased incidence of ROP in this cohort. These findings emphasize the importance of targeted screening and careful oxygen management in premature infants. Larger prospective multicenter studies are needed to clarify independent risk factors for ROP in Indonesia.
