Abstract

Latar belakang : Mengetahui karakteristik demografis pasien, hasil terapi yang diberikan dan menemukan faktor prediktor keberhasilan terapi pasien NOT di RSCM. Metode : Penelitian deskriptif menggunakan data sekunder dari telusur rekam medis pasien yang terdiagnosis NOT dari poliklinik Divisi Neuro-Oftalmologi (NO) Departemen Medik Mata FKUI/RSCM pada periode Januari 2014 hingga Desember 2015. Hasil : Karakteristik demografik pasien NOT di RSCM ialah 82,4% laki-laki, dan rerata usia adalah 22,47 ± 10,68 tahun. Sebanyak 50% datang dalam rentang kurang dari 24 jam pasca trauma. Penurunan tajam penglihatan berat (58,8% NLP), 67,6% dengan RAPD positif, dan papil nervus optik yang bervariatif. Sebanyak 67,6% ditatalaksana dengan metilprednisolon intravena (1 g/hari selama 3 hari), 26,5% dengan metilprednisolon oral (0,8mg/kgBB/hari). Perbaikan tajam penglihatan dijumpai pada 32,4% pasien yang mendapatkan pengobatan selama satu bulan. Pasien close globe injury yang mendapat terapi < 24 jam secara statistik terjadi perbaikan tajam penglihatan (p = < 0.05). Kesimpulan : Faktor prediktor seperti awitan trauma, awitan terapi inisial dan hasil pencitraan pada penelitian ini belum kuat untuk menjadi acuan dalam memprediksi keberhasilan terapi. Akan tetapi, pasien dengan closed globe injury yang diobati kurang dari 24 jam menunjukkan perbaikan tajam penglihatan yang signifikan. Dosis metilprednisolon IV 1 gram/hari selama 3 hari yang dilanjutkan dengan penirusan dosis per inggu maupun pemberian preparat oral dengan dosis 0.8 mg/KgBB/hari dapat digunakan. Pengobatan dilakukan setidaknya sampai dengan 1 bulan pasca pengobatan karena kurang baiknya kepatuhan pasien neuropati optik traumatic.

Abstract

Background : The study aims to describe clinical profile of corneal ulcer patients in Saiful Anwar Hospital Malang. Method : Retrospective study was used to evaluate the medical record of corneal ulcer patients that were diagnosed by Infection and Immunology subdivision in Saiful Anwar Hospital, Malang, from January 2013 to December 2014. Data included patient’s gender, age, occupation, chief complaint, predisposing factors, visual acuity, corneal ulcer grading, microbiology examination, pharmacological therapy, surgical intervention and the clinical evaluation of treatment with slitlamp. Result : As many as 81 males and 26 females with corneal ulcer which showed the highest frequency was at the age of 41 to 50 years old (33.64%) and 69.16% cases had severe ulcers. Farmers were the most affected (46.73%). Trauma was the leading predisposing factors (78.50%) and plant material as the most cause (42.85%). Red eye was the most chief complaint in 40.19% patients. Visual acuity was 2/60-LP+ in 73.83% patients. Gram positive cocci were obtained in 72.90% patients, whereas fungi was obtained in 29.97% patients from Gram and KOH staining. Coagulase-negative staphylococcus was the most frequently detected organism in 22.43% patients amongst positive isolates. Quinolones are given in 39.25% patients, aminoglycosides in 32.71% patients, and itraconazole in 28.04% patients. As 21.50% patients needed amnion membrane transplantation. We found improvements upon the condition in 47.66% patient after the treatment evaluation. Conclusion : corneal ulcer diagnosis relies upon the complaint, predisposing factor, clinical and laboratory findings. Early and adequate treatment provides clinical improvement.

Abstract

Introduction and Aims. Refractive errors such as miopia, hyperopia and astigmatism in teenagers are common in Asian country nowadays. Gadget usage also influence the prevalence and incidence of refractive errors specially for miopia and astigmatism. Outdoor activities have positive impact to prevent miopia. The aim of this study is to report the role of outdoor activities as unique school program that could prevent refractive errors in Junior High School in rural area of Bali Method. This is analytic descriptive study with cross sectional approach of 231 Pekutatan II Junior High School students taken in December 2016. This school located in rural area; 35,3 km from Jembarana and 60,5 km from Denpasar, the capital city of Bali. The area of the school is about 1500 m2, where 70% of the land are still vacant to natural habitat and this school had their own garden area that being organized by the students themselves after school hours. This school do not have computer extra curriculum as well. Result. From 231 students there were only 14 students that have refractive errors consist of 5 boys and 9 girls, mostly aged 15 years. Six students (2%) were diagnosed as myopia and 8 students (3.4%) were diagnosed as compound astigmatism myopia. The length of gadget usage in this school were 1.66 hours and outdoor activities time were 4.78 hours per day. The hereditary factors of refractive errors were noted in 12 students (5 %). Outdoor activities associated with refractive errors in this study (p = 0.03). While time using gadget does not have an association with the refractive errors (p = 0.38). Conclusion. Rural area where the outdoor activities are still common have protective effect to prevalence of refractive errors in teenager.

Abstract

Introduction : To evaluate visual outcome and complication rate of phacoemulsification in post vitrectomy eyes. Methods : retrospective descriptive study Result : Data from seventy-five eyes was analyzed. Mean BCVA improves from 1.8 LogMAR (0.2- 2.5) to 1.3 LogMAR (0-2.5) postoperatively, with biggest improvement of 0.57 LogMAR shows in patients with preoperative BCVA of <1.8 LogMAR. Only 1.3% patient has intraoperative complication, and 52% patient has postoperative complication, of which 27.9% is PCO. Patients with silicon oil who underwent combination surgery of phacoemulsification and silicon oil extraction has higher redetachment rate (33%) compare to patient who only underwent phacoemulsification (20%). Conclusion : Phacoemulsification improves BCVA in most patients, and intraoperative complications rate was low. There was evidence of correlation between higher redetachment rate and combination of phacoemulsification with silicon oil extraction.

Abstract

Background : Orbital cellulitis is one of complication from rhinosinusitis. Although the prevalence in adult is less common than in children, the morbidity is higher. An appropriate treatment for this case should be given to avoid morbidity. Method : Serial cases of orbital cellulitis with unilateral sinusitis in Adult were reported in Dr.CiptoMangunkusumo Hospital. All cases occurred from December 2017-May 2018. Result : There were 4 cases reported. Range 26-57 years old. All cases had eyelid swelling, two cases had limitation eye movement, and two cases had visual deterioration. Ethmoid sinus was the most common sinus affected. All cases were treated with broad-spectrum antibiotic and metronidazole. Orbital decompression was performed in two cases. Conclusion: Appropriate treatments such as the use of systemic antibiotic and surgical is needed to decrease the morbidity.

Abstract

Latar belakang : Keratokonus adalah suatu keadaan pada kornea yang ditandai dengan adanya ektasia pada sisi posterior kornea, ketebalan kornea yang tidak normal serta disebabkan faktor non inflamasi. Kondisi ini terjadi secara bilateral, walaupun tingkatan keparahannya sering kali tidak sama. Apakah tujuan tindakan untuk mengurangi iregularitas astigmatisma, memperbaiki kelengkungan kornea, memperkuat kornea, untuk menghentikan progresivitas keratokonus ataukan untuk meningkatkan tajam penglihatan. Tinjauan kepustakaan ini akan membahas pilihan penatalaksanaan keratokonus terutama dalam upaya menghentikan progresifitas keratokonus. Indikasi dan penatalaksanaan keratokonus progresif menggunakan Corneal cross-linking, protokol Dresden, protokol akselerasi, manajemen komplikasi dan studi terbaru terkait efikasi dan keamanan tindakan termasuk dalam pembahasan ini. Metode : Penelusuran literatur dilakukan dari Wiley online Library serta MEDLINE menggunakan Pubmed, hanya artikel berbahasa Inggris yang disertakan. Daftar pustaka termasuk di dalamnya buku teks yang relevan terhadap artikel. Hasil : Terdapat sembilan belas artikel terkait pembahasan penatalaksanaan terkait keratokonus sesuai dengan kriteria pembahasan. Kesimpulan : Pilihan penatalaksanaan keratokonus pada pasien secara umum bergantung kepada hal yang ingin dicapai. Mengurangi iregularitas astigmatisma, memperbaiki kelengkungan kornea, memperkuat kornea ataupun untuk menghentikan progresifitas keratokonus. Pilihan tindakan dapat berupa pemberian kacamata, intracorneal ring segment, Corneal cross-linking, lensa fakik intraokular, tindakan laser dan keratoplasti. Corenal cross-linking adalah pilihan utama tindakan untuk menghentikan progesifitas keratokonus.

Abstract

Introduction : Methanol-induced toxic optic neuropathy (TON) is defined as a visual impairment due to optic nerve damage by methanol poisoning. Not only is this disease entity underdiagnosed at times, this sudden blindness is also often diagnosed at a stage where recovery of vision is no longer possible. Materials and Methods : A literature search was conducted using PubMed, ClinicalKey, Google Scholar and ScienceDirect by combining the keywords ‘methanol’ or ‘methyl alcohol’, ‘intoxication’ or ‘poisoning’, ‘toxic optic neuropathy’, and ‘visual acuity’ with ‘high-dose steroid’. Results : The total amount of subjects in each article varied from 2 to 37, with mean age distribution of 26.34 to 55 years old, where most patients were male. The follow up duration varied from 1 week to 1 year. Four articles do not mention high-dose steroids treatment as therapy while the other four mention use of 1000 mg of intravenous methylprednisolone per day with divided doses of either 2x500 mg or 4x250 mg. Improvement percentages show 100% improvement in all studies that used high-dose steroids, while in the non-high-dose steroids studies the improvement percentages range from 33.33% to 90%. Summary : High-dose steroids are showing efficacy in improving visual acuity and reducing the inflammation in methanol-induced TON. The period of how fast the therapy takes effect is inconclusive, as the mean follow-up time differs widely per study. However, because most of the reviewed studies here are retrospective case series, a larger, more comprehensive study is required to acknowledge more of the efficacy profile.

Abstract

Introduction : The eye care service in The Dr. Sardjito Hospital has one of the highest number of patients amongst other health care services provided. Thus evaluation of the patients' satisfaction upon the service provision is needed to improve the quality of the eye care service. Methods : A cross-sectional study was performed in the ophthalmology outpatient clinic at the teaching hospital in Daerah Istimewa Yogyakarta, Indonesia. Interviews were conducted on 77 patients using Patient Satisfaction Questionnaire Short Form (PSQ-18) in 2017. Results : Overall, patients were mostly satisfied with the interpersonal manner of the doctors (mean : 4.12  0.42) and least satisfied with the time spent with doctors (mean : 3.310.95). There was a statistically significant difference among each subscale (p=0.000). Gender, income, occupation, and education had no significant statistical difference to the aspects of quality. However, significant differences were found in the financial aspects of marital status, education level, and co-diagnoses group with p-value of 0.009, 0.043, and 0.048 respectively. Conclusions : Patient satisfaction could be improved by increasing the time spent at each visit with the doctor. Meanwhile, financial aspect is mostly affected by the patients’ background. This study needs to be continued to get wider perspectives on the larger population sample of eye patients. Therefore, the quality of eye care services may be improved.
Autologous Blood Versus Suture Conjunctival Limbal Graft as Pterygium Surgery Treatment Putu Budhiastra

Abstract

Introduction and Objective: A Pterygium is an elevated, superficial, external ocular mass that usually forms over the perilimbal conjungtiva and extends onto the corneal surface. A Pterygium can cause a significant alteration in visual function in advanced cases. It become inflamed, resulting in redness and ocular irritation. Pterygium excision surgery has been using many methods. There are three choices of method to attach the graft: by suturing, fibrin glue or autologous blood. The purpose of this study is to learn about the efficacy of autologous blood versus suturing conjungtival limbal graft (CLG) on pterygium excision surgery Methods: Retrospective study of 18 patients underwent Pterygium excision surgery with autologous blood gluing compared to suturing the graft. Six patients underwent CLG surgery with suturing and 12 patients were treated with autologous blood gluing. Suturing process was done with nylon 10-0 and the suture was released 7 days after surgery, whereas the graft on the patient with autologous blood CLG was placed and left there for about 10 minutes. Both groups were evaluated after day 1, 1st week, 2nd week and 1st month. Any report of irritating sensation, graft loss, and healing process were recorded thoroughly. Result: Patients with sutured CLG were complaining of uncomfortable sensation on their cornea until one week before suture was released, all grafts were attached until a month. Out of 12 patients with autologous blood CLG method, there were no complain of irritability on cornea until a week, 4 patients lost the graft on the first day, two patients grew granuloma on the second week, 10 patients start healing on the second week. Conclusion: There are still limitations to pterygium excision with autologous blood CLG compared to pterygium excision with CLG suture technique.

Abstract

Introduction : The longterm use of topical antiglaucoma might cause ocular surface instability due to active substance or preservative used. Impression cytology examination may reveal superficial epithelial cells on conjunctiva and cornea, including goblet cells. Goblet cell density decrease is the most important parameter on evaluation of ocular surface disorder. Objective : This study was to understand ocular surface remodeling due to active substance of topical antiglaucoma with impression cytology examination among the patient prior and 3 months after therapy. Methods : This was a randomized controlled trial study with single blind masking. A total of 45 eyes from 31 patients were used as subject and distributed onto three groups treatment, which were timolol maleat 0.5%, latanoprost 0.005%, and latanoprost-timolol maleat fixed combination. All topical antiglaucoma in this study were preservative free. Result : There were differences between 3 groups in goblet cells density after 3 months therapy (p=0,030). Goblet cell density in timolol group was lower than latanoprost (p=0,041) and fixed combination (p=0,045). There was no significantly difference between 3 groups in conjunctival epithelial metaplasia degree (p=0,706) and cell to cell contact degree in corneal epithelial cells (p=0.66) after 3 months therapy. Conjunctival epithelial metaplasia degree were increased among group of timolol (p=0,008) and fixed combination (p=0,046). Conclusion : Timolol maleat 0,5% caused lower goblet cell density after 3 months therapy compare with latanoprost and fixed combination. There was no significantly difference in conjunctival epithelial metaplasia and cell to cell contact degree in corneal epithelial cells among these glaucoma treatment groups.
