Abstract

Background: Self-directed learning (SDL) as part of student-centered learning, has been applied in medical education curriculum in Indonesia since the Competency-Based Curriculum was introduced. Students’ perception towards SDL concept in relation to clinical stage and how it is applied are important to identify problems from students’ point of view. This study aimed to see SDL readiness and its correlation to academic achievement; and to obtain student perceptions towards SDL.Methods: A cross-sectional quantitative study was conducted in the Faculty of Medicine Universitas Indonesia (FMUI) from April to June 2013, involving 209 of year 4 students to see SDL readiness and its correlation to academic achievement. Qualitative methods with phenomenological approaches were used to obtain student perceptions of SDL.Results: 71.3% of students were in the category of ready for SDL with a mean score of 57.03 (SD 7.416). Among students’ characteristics, significant mean difference was found in type of education program. The international class group had a higher mean score than the regular group (p 0.014). SDLRS scores did not correlate with student academic grades. Students’ perceptions towards SDL were in accordance with known concepts, and students preferred SDL rather than teacher-centered learning in the clinical stage with suggestions of improvement in some areas. Conclusion: Clinical stage students were ready to conduct SDL. International class students seemed more ready for SDL. Students at year 4 clinical stage had good perception of SDL. This positive perception would provide sufficient readiness for the implementation of SDL in medical education.

Abstract

Open globe injury is an injury that requires prompt treatment which could predict the final outcome. This study evaluates the characteristics and outcome of corneal rupture management in dr. Cipto Mangunkusumo National Hospital’s emergency room. We conducted a retrospective study based on the patient’s medical record between January 2016 – December 2017. Data regarding patients’ demographic, clinical characteristics according to Birmingham Eye Trauma Terminology Score (BETTS) criteria, duration of trauma to surgery, use of early topical steroid, one month visual function result, and risk of endophthalmitis were collected. A repeated ANOVA test with posthoc Bonferroni was used to compare the mean of a paired numeric variable. A total of 163 patients with corneal rupture were recorded, consisted of mostly male (92%) and median age of 27 years old. We found 36.8% cases occurred in the household, 71% cases in the zone I, 62.6% penetrating cases, and mostly came with 4/200-light perception visual acuity (VA) for all zones, mostly caused by inorganic materials and 50.9% cases involved lens. Endophthalmitis was higher in inorganic causes (69.2% vs 30.8%, p=0.33). A total of 95.2% of cases performed surgical treatment in < 24 hours. In post-operative follow up, 53.3% of cases did not need re-operation or proceeding surgery. Several risk factors might affect the treatment outcome of eye injury. Most rupture occurred at zone I. Administration of topical steroids directly post-surgery was not associated with the incidence of endophthalmitis.

Abstract

Introduction: To highlight the therapeutic dilemma in managing complications in high myopia patient with complicated cataract after vitrectomy who previously had refractive surgery. Case Report: A 37-years old man, presented with half the right eye view was being blocked by a black curtain for one week before admission. He had a history of high myopia on both eyes and underwent phakic intraocular lens (PIOL) implantation 12 years ago. Presenting visual acuity (VA) of the right eye was hand motion (HM). Retinal detachment (RD) on inferior side, and clear lens with anterior chamber (AC) PIOL were found in the examination. Patient underwent vitrectomy, endolaser, and silicone oil in the next four days. A sudden severe pain with red-eye was found one month after surgery, intraocular pressure (IOP) was increased to 43 mmHg, the lens was white opaque with lens material seen in AC. Discussion: The diagnosis of traumatic cataract and lens-particle-induced glaucoma was made. Lens mass evacuation surgery was performed after initial antiglaucoma medication. Posterior capsule rupture was found intraoperatively with only a small amount of lens mass remaining. Visual acuity result after the second surgery is still dissatisfied with the presence of rubeosis iridis, with the possibility of retinal re-detachment was still present. Conclusion: High myopia still poses a greater risk of ocular complications. It can be corrected by refractive surgery, with expensive cost and it will not prevent potential future ocular problems. Repeated assessment before and after refractive surgery procedure is mandatory in order to prevent sight-threatening complications of high myopia which might lead to other blinding complications.

Abstract

Background: To evaluate the effect of prophylactic nepafenac eye drops on macular thickness changes after phacoemulsification surgery in mild to moderate NPDR patients. Method: This study is an open label randomized clinical trial. Thirty-six subjects who met the inclusion criteria underwent phacoemulsification. One group (18 subjects) were given nepafenac 0.1% eye drops and the rest were given placebo. Foveal thickness was measured by SD-OCT before surgery and the fourth week after phacoemulsification. Best corrected visual acuity (BCVA) and degree of inflammation in the anterior chamber were also being assessed. Result: There was a statistically significant increase foveal thickness in the placebo group 4 weeks after phacoemulsification (p=0.022). Clinically, percentage degree of inflammation in anterior chamber in placebo group was higher than nepafenac group (38.9% : 5.6%) but not significantly different between 2 groups (p=0.27). Nepafenac group achieved clinically better BCVA than the placebo group 4 weeks after phacoemulsification, although statistically there was no significant difference between 2 groups (p=0.991). Conclusion: Nepafenac 0.1% eye drops could prevent foveal thickening 4 weeks after phacoemulsification in mild to moderate NPDR patients. Clinically, nepafenac 0.1% eye drops could decrease the risk of inflammation in the anterior chamber, risk of CME, and vision deterioration although did not reach statistically significant. Abstract

Introduction : To evaluate visual outcome and complication rate of phacoemulsification in post vitrectomy eyes. Methods : retrospective descriptive study Result : Data from seventy-five eyes was analyzed. Mean BCVA improves from 1.8 LogMAR (0.2- 2.5) to 1.3 LogMAR (0-2.5) postoperatively, with biggest improvement of 0.57 LogMAR shows in patients with preoperative BCVA of <1.8 LogMAR. Only 1.3% patient has intraoperative complication, and 52% patient has postoperative complication, of which 27.9% is PCO. Patients with silicon oil who underwent combination surgery of phacoemulsification and silicon oil extraction has higher redetachment rate (33%) compare to patient who only underwent phacoemulsification (20%). Conclusion : Phacoemulsification improves BCVA in most patients, and intraoperative complications rate was low. There was evidence of correlation between higher redetachment rate and combination of phacoemulsification with silicon oil extraction.

Abstract

Abstract Introduction & Objectives : Age-related macular degeneration (AMD) is a common ocular disease that affects millions of people worldwide. Cataract surgery is one of the most performed surgical procedures in the world and is effective to improve visual function and quality of life (QoL) in individuals with cataracts. Both cataract and AMD are conditions that occur in late age. However, the presence of AMD may complicate the postoperative outcomes and QoL of these patients. This review aims to examine the impact of cataract surgery on the QoL of patients with AMD. Methods : A search of the literature was conducted using electronic databases. Studies were included if they evaluated QoL outcomes in patients with AMD who underwent cataract surgery. Results : The results of the review suggest that cataract surgery can improve visual acuity and QoL in patients with AMD. However, the degree of improvement is influenced by the severity of AMD. Patients with mild to moderate AMD tended to have greater improvements in QoL and visual acuity compared to those with advanced AMD. Incidentally, it is still cost effective to perform cataract surgery in patients with advanced AMD. Conclusion : Overall, patients with AMD who underwent cataract surgery experienced improvements in their QoL. Nevertheless, individual characteristics including severity of AMD must be considered to optimize the procedure outcomes. Further research, particularly with longer follow up times, is needed to determine the long-term effects of cataract surgery on QoL in patients with AMD.

Abstract

Abstract Introduction & Objectives : Modern cataract surgery technique entails replacing the crystalline lens with an artificial intraocular lens (IOL). While the standard monofocal IOL implantation with monovision technique has been cost- effective and provided patients with good visual acuity, multifocal IOLs present the prospect of spectacle-free vision despite various adverse outcomes. It remained contentious whether multifocal IOLs were superior to monovision. Our review provide an up-to-date assessment of monovision vs. multifocal IOL implantation's visual acuity and spectacle independence following cataract surgery. Methods : A systematic search was performed in April 2023 on six databases (Medline, SCOPUS, Proquest, EBSCO, Embase, and PubMed). Studies were extracted for the following outcome of interest: monovision, multifocal intraocular lens, spectacle independence, and visual acuity. All studies published up to April 2023 were reviewed. Cochrane risk of bias tool (RoB 2) was used to screen studies for risk of bias where appropriate. A meta-analysis was done to quantify any reported quantitative data. Results : Ten studies were identified. Pooling analysis for uncorrected binocular distance visual acuity and binocular uncorrected near visual acuity showed no difference between monovision vs. multifocal IOL (SMD 0,31; 95% CI -0,21 to 0,82 and SMD 0,41; 95% CI -0,58 to 1,40, respectively). Spectacle independence showed monovision was inferior to multifocal IOL (OR 0,22; 95% CI 0,08 to 0,61; heterogeneity I2=75%). Conclusion : Current evidence showed that multifocal IOL had comparable visual acuity to monovision with slightly higher likelihood of being spectacle-free. Despite that, monovision can be an efficacious and affordable option to be implemented in countries with high cataract burden.

Abstract

Abstract Introduction & Objectives : Unresolved cataract primarily has caused patients to have disturbance in carrying out daily activities, thus lowering their quality of life (QoL). We aim to use the Visual Function Index-14 (VF-14) questionnaire, one of the most frequently used tools, to discover the extent of daily activities disturbance in patients with cataract. Methods : In this observational study, we conducted eye screening to 47 subjects in Jonggol subdistrict, Bogor, Indonesia. Patients were asked several questions based on the translated VF-14 questionnaire into Bahasa and also tested to find out their best-corrected visual acuities. Lens examination was carried on using handheld slit lamp. Results : Out of all 47 subjects, 34 (72,3%) patients had cataract that was marked by lens opacities in either one or both eyes. According to distant vision impairment categories by International Classification of Diseases 11, 8,8% of cataract patients had normal vision from the better eye, 20,6% had mild, 64,7% had moderate, 0% had severe impairment, and 5,9% had blindness. From the assessment using VF-14 translated questionnaire, 14,7% subjects had no disruption in doing daily activities, 82,3% had disruption, and 2,9% could not do the mentioned activities at all. From the Spearman correlation test, there was a significant association with moderate correlation between the degree of vision impairment and QoL (p = 0.006, rs = 0.459). Conclusion : The majority of cataract patients in Jonggol subdistrict had moderate vision impairment and had disruption in doing daily activities, therefore lowering their QoL.

Abstract

Abstract Introduction & Objectives : Presbyopia after cataract surgery is still a problem despite recent surgical techniques and intraocular lens technology development. Mini-monovision techniques and pure extended depth of focus (EDoF) lenses are alternatives to achieve good distance and intermediate visual acuity at relatively inexpensive. This study aims to compare the clinical outcome after cataract surgery with binocular mini-monovision and cataract surgery using a pure monocular EDoF lens. Methods : Subjects were randomized into the mini-monovision group (standard monofocal lenses with -1.25 D of anisometropia) and the pure EDoF group. The blinding was achieved by masking the clinical outcome examiners. Results : Interim analysis of 16 patients revealed the outcome of uncorrected and corrected distance visual acuity (UCDVA and BCDVA) were not significantly different between groups. Median of uncorrected intermediate visual acuity (UIVA) and mean of uncorrected near visual acuity (UNVA) of binocular mini-monovision were 0.10 (0-0.10) LogMar and 0.26 + 0.12 LogMar. Median UIVA & mean UNVA monocular EDoF were 0.19 (0.14-0.50) LogMar and 0.54 + 0.11 LogMar, respectively. The Mann- Whitney U test of UIVA between groups revealed p=0.001, and the unpaired T-test of UNVA revealed p=0.000. Mean defocus curve of -2.50 D lens were 0.31 LogMar in binocular mini- monovision group and 0.51 LogMar monocular in EDoF group (p=0.019). Conclusion : UIVA, UNVA and defocus curve of -2.50 D lens in the mini-monovision group were better than the EDoF group.
