Abstract

Background: Steven-Johnson syndrome is one of life-threatening skin abnormality. SJS complication involve multiorgan disfunction such as mucosa, ocular, respiration, and digestive tract. Around 30- 50% of SJS patient experience chronic ocular sequel, which concludes progressive symblepharon, margo palpebral keratinization, trichiasis, entropion, corneal pannus, dry eye syndrome, and persistent epithelial cornea defect. Scleral contact lenses has beneficial in managing SJS patient, on both acute and chronic phases. Methods: A 36-year-old female patient with SJS, presented for eye examination due to unpleasant sensation on both of their eyes and unsatisfactory quality of her vision. She presented high myopia, margo palpebral keratinization with trichiasis and distichiasis, symblepharon and hyperemic conjunctiva, wide corneal scar, and ocular neovascularization. A diagnostic trial set was used in the fitting process, and she was assessed according to standardized fitting methodology. Visual acuity, corneal topography, biometry, and ocular aberrations were evaluated. The follow-up period was 12 months. Results: The best corrected visual acuity was 20/60 with correction lens S – 1.00 D for the left eye (LE) and 20/25 cannot be corrected with correction lens for the right eye (RE). After corneal contact lens fitting, visual acuity was improved to 20/20 and 20/25 for the RE and LE, respectively. The patient wore these contact lenses an average 8 hours a day. The patient’s dry eye complaint was significantly decrease with more comfortable ocular sensation Conclusion: The present case report describes how the patient had scleral contact lens fitted successfully for management of SJS ocular complication. They provided optimal visual quality and restore the patient’s comfort.

Abstract

Introduction: Cataract is the major cause of reversible blindness and visual impairment in developing countries. It has been assumed that there is poor access to information related to the cause, prevention, and treatment of cataracts in developing countries. There is also a paucity of data on knowledge, attitude, and behavior toward cataracts in Semarang. Objective: To determine the Knowledge, Attitudes, and Behavior Regarding Cataracts of The Elderly around the Primary Care Center Semarang Method: A community-based cross-sectional study was conducted among 40 participants in November 2019. The study participants were selected by a simple random sampling method. Data were collected with a structured questionnaire by using face-to-face interviews. The data were entered into Microsoft Excel 2016 and analyzed using Statistical Package for Social Sciences (SPSS) version 25. The level of statistical significance was set at a p-value less than 0.05. The Socio-demographic factor as Gender, Age, Education, Occupation, and Income set as independent variable in this study. Result: There were 40 respondents, based on questionnaire data 52,50% of respondents had good knowledge, 85% had a good attitude, and 60% had poor behavior. The Education and Knowledge (p=0,001), Education and Behavior (p=0,004), Knowledge and Behavior (0.57749p>0,5, Gender, Education and Income (p>0,05), Education and Income (p>0,05)had positive significant on the variables. Conclusion: A Good approach strategy is needed to achieve to increase public Knowledge, Attitude, and Behavior like doing health promotion periodically and improving health care performance, and facilitate their availability in primary care.

Abstract

This study aims to assess 1000 mg citicoline each day given for 60 days on chronic phase NAION (non–arteritic anterior ischemic optic neuropathy) patients. Double masked randomized clinical trial divided the patients into 2 groups: citicoline group (C–NAION) and placebo group (P–NAION). Analysis was done in 17 patients in each group. There were increament of ? amplitude P50 30 days in C–NAION group (0,775±2,6 µV) and ? amplitude N95 60 days in C–NAION group (–0,356±2,992 µV) but statistically insignificant compared to P–NAION. Age has a good correlation with ? amplitude P50 (r = –0.517; p = 0.002). Subjects with 1 risk factor experienced a higher ? amplitude of P50 than the subjects with more than 1 risk factor (1.856 ± 2.121 ?V vs –0.26 ± 2.45; p = 0.025). Citicoline did not show any changes in Retinal Ganglion Cell Thickness. There were improvement of ? mean deviation 60 days in C–NAION group (3,13±6,467 dB), but statistically insignificant compared to P–NAION. Citicoline tends to increase the ? amplitude of P50, N95 and mean deviation in chronic phase NAION. Age and the number of risk factors affects the increase of ? amplitude P50.

Abstract

Objectives: The objective of this study is to have an expanded evaluation on the effect of phacotrabeculectomy on ocular biometric (ACD/anterior chamber depth, lens thickness), gonioscopic and intraocular pressure (IOP) in primary angle closure glaucoma (PACG). Methods: A cohort study of PACG patient who underwent phacotrabeculectomy from September 2018- March 2019 at Dr.Kariadi hospital. Preoperative secondary data of baseline IOP, ACD, lens thickness, and gonioscopy were obtained from medical record. Postoperative primary data of IOP, ACD, and gonioscopy were obtained 2 weeks after surgery. Results: Thirty eyes from 23 patients had preoperative and postoperative mean IOP (26,20±2,24 and 18,35±2,49), mean ACD (2,46±0,37 and 3,40±0,40), mean gonioscopic score (4,97±0,96 and 8,33±1,63), mean preoperative lens thickness (4,58±0,39). There were significant differences between IOP, ACD and gonioscopic score (<0,001) before and after phacotrabeculectomy. The greater lens thickness was associated with IOP reduction ( <0,001 r=0,756), increased ACD ( 0,005 r=0,498), and increased gonioscopic score ( <0,001, r=0,802). The positive correlation can be seen between gonioscopic score and ACD ( <0,001 r=0,666) and the negative correlation between IOP and ACD ( <0,001 r=0.683), IOP and gonioscopic score ( <0,001 r=0,876). Conclusion: Phacotrabeculectomy may be effective in reducing IOP, increasing ACD, and gonioscopic scores significantly in PACG patients. Lens thickness may be positively

Abstract

Aim: To evaluate dry eye syndrome patients based on meibomian gland evaluation, tear break up time test, Schirmer test, and ocular surface disease index form the diabetic mellitus patients. Materials And Methods: A Cross Sectional Study used to determine the profil of dry eye Syndrome on 15 diabetic mellitus patients in Eye center Hasanuddin University from December 2019 until February 2020  Results: 15 participants have included in this study. From the meibomian gland evaluation, there are 10 participants with mild dry eye. From the tear break up time test, there are 10 participants with dry eye. From the Schirmer test, there are 8 participants having dry eye on Schirmer 1 test and 8 participants with dry eye on Schirmer 2 test. And, from the ocular surface disease index, 11 participants with severe dry eye disease were identified. Conclusion: Patients with diabetes mellitus have a high potential for dry eye. Dry eye screening examinations such as tear break up time and Ocular Surface Disease Index (OSDI) are recommended to use in patients with diabetic retinopathy.

Abstract

Objectives: Proliferative Diabetic Retinopathy (PDR) is one of the leading causes of blindness and visual loss. Panretinal Photocoagulation (PRP) reduces the risk of severe vision loss in PDR. Good visual acuity results can improve vision-related quality of life. The objective of this study is to evaluate the effect of laser PRP on patients with PDR on vision-related quality of life. Methods: In this prospective study, 30 PDR patients (12 men and 18 women) treated with PRP were included (mean age: 51 years, SD: 5,08 years). On average, patients received 1.264 laser spots. The National Eye Institute 7-Item Visual Function Index (VF-7) was used to evaluate patient's vision-related quality of life. The VF-7 was filled in by interview twice, at the beginning before laser PRP and at least 1 month after the completion of PRP. Comparison of VF-7 before and after laser PRP was performed. The correlation between alteration in visual acuity and VF-7 will be evaluated. Results: Mean VF-7 before laser PRP was 7.00 ± 0.00 and after laser PRP it was 3.43 ± 3.16 (p<0.001, Wilcoxon). VF-7 scores had a statistically significant difference between before and after laser PRP (p<0.05). There was a strong correlation between VF-7 score alteration with visual acuity (p <0.001, r 0.748). Conclusion: There was a significant improvement in vision-related quality of life following panretinal photocoagulation of patients with proliferative diabetic retinopathy.

Abstract

Introduction: Diabetic retinopathy (DR) is a major diabetic microvascular complication, affects 1 in 3 person with Diabetes Mellitus (DM). DR remains a leading cause of severe, irreversible visual impairment, and blindness in working aged population worldwide. Screening, early detection and prompt treatment of Vision-Threatening Diabetic Retinopathy (VTDR) allow prevention of diabetes-related visual impairment. Poor access to eye health care in rural area may become a factor of increasing number of underdiagnosed of DR. Objective: To identify characteristic of DR and its associated risk factor in primary health care in Tempuran, Karawang, West Java. Methods: A cross-sectional study was conducted on participants who met the inclusion criteria and the fundus photograph was done to evaluate the retinal status. Demographic data, visual acuity, and other ocular comorbidities was also documented. Results: A total of 57 participants with diabetes were examined for fundus photograph. Any DR, diabetic macular edema (DME), and VTDR in this study were found 54,4%, 14%, and 15,8% respectively. DR were found higher in participants with older age, women, shorter duration of diabetes, hyperglycemia, and hypertension. Conclusion: This study identified the characteristic of DR in primary health care in Tempuran, Karawang, West Java. The results of this study may become a recommendation for periodic diabetes screening of DR in the region.

Abstract

Introduction: Intraoperative floppy iris syndrome (IFIS) remains a challenge that increases the risk of complications in patients undergoing cataract surgery who use ?1-adrenergic receptor antagonists. To date, no definite consensus on a preventive strategy for IFIS is available. The aim of this review is to assess various pharmacological managements to prevent IFIS in high-risk patients. Methods: This review was based on Preferred Reporting Items for Systematic Reviews and Meta- Analyses guidelines. A systematic search using PubMed, Science Direct, Cochrane Library, and WorldCat database was performed. Quality of each study was evaluated using Cochrane Risk of Bias 2.0 (RoB 2.0), Risk of Bias in Non-randomized Studies of Interventions (ROBINS-I), or Newcastle- Ottawa Scale (NOS). Results: The search identified 1589 articles of which 7 met the eligibility criteria. Experimental and observational studies between 2010 and 2018 were included. Pharmacological managements included in this review are administered in varying routes. Phenylephrine, lidocaine, a combination of lidocaine and epinephrine are given intracamerally. Other pharmacological managements included are sub-tenon injection of lidocaine, topical atropine, a combination of topical atropine with intracameral epinephrine, combined irrigation solution of phenylephrine and ketorolac, and mydriatic cocktail- soaked wick sponges. Conclusion: Various pharmacological managements for IFIS prophylaxis have shown promising potential. However, studies that evaluate the efficacy of each agent and comparison between these strategies are still limited. Further research is needed to determine the best prophylaxis strategy to reduce the incidence of IFIS.

Abstract

Introduction: Axial length elongation contributes a challenge in myopia eyes due to morphological and visual field abnormalities. OCT instruments do not embed a normative database from high myopia. The study is aimed to compare Cirrus OCT and HFA parameters on high myopia and emetropia with/without glaucoma. Methods: This cross-sectional, prospective study was conducted at Sardjito General Hospital from 1st April 2021 to 11th May 2021. Patients with high myopia was enrolled with Spherical Equivalent (SE) ? -6D, 20 – 55 years of age. All participants underwent a complete ophthalmologic examination. Inclusion criteria: BCVA 20/40 or better, reliable visual field results. Result: From 22 eyes, there were no significant difference of age between high myopia with glaucoma/HMG (n=5), high myopia/HM (n=6), emetropia with glaucoma/EG (n=5), emetropia/E (n=6) with mean age was 36±3.2 y.o and 60% were male with SE -8.5±1.03D. Among ONH parameters, there were no differences between groups. In HMG, HM, EG, E group, median avgRNFL was 85um, 96.5um, 105um, 110um respectively. Thus, median vertical CD is 0.48, 0.40, 0.58, 0.55 in HMG, HM, EG, E group respectively. Whilst median GCIPL and visual index were 75um, 78.5um, 85um, 89.5um in HMG, HM, EG, E group respectively and had significantly different 0.012 (p<0.05) between groups. Median visual field index was 92%, 97%, 98% in HMG, HM, EG group respectively with significantly different 0.04 (p<0.05). Conclusion: The GCIPL and visual field index are significantly different between high myopia and emetropia with/without glaucoma
