Abstract

Background: Invasive bladder cancer presents a substantial health challenge due to its aggressive nature and high incidence. Surgical management, primarily through open radical cystectomy (ORC), laparoscopic radical cystectomy (LRC), and robotic-assisted radical cystectomy (RARC), is essential in treating this malignancy. Understanding the comparative outcomes of these surgical approaches and identifying prognostic factors is crucial to optimize patient care. Methods: This systematic review synthesized data from studies meeting specific inclusion criteria focused on the outcomes of ORC, LRC, and RARC for invasive bladder cancer. Major databases, including Emerald, Science Direct, and Sagepub, were searched to collate relevant studies. Key aspects analyzed included perioperative outcomes, oncological results, and the influence of prognostic factors such as lymph node dissection and surgical margin status. Results: The review revealed that minimally invasive techniques, namely LRC and RARC, demonstrate comparable oncologic outcomes to ORC, with distinct advantages in perioperative recovery, including reduced blood loss and shorter hospital stays. Prognostic factors like extensive lymph node dissection and achieving negative surgical margins were associated with improved survival and recurrence rates. Conclusion: The findings support that the choice of surgical approach should be tailored based on patient-specific factors and institutional expertise to enhance outcomes. High-quality, individualized surgical care is paramount, emphasizing the role of centralizing bladder cancer surgeries to high-volume centers and establishing referral networks for optimal patient management.

Abstract

Background: Giant omphalocele (GO) is a severe form of omphalocele, characterized by a defect larger than 5 cm, often including the liver. Management strategies include staged surgical closure and non-operative delayed closure, with the latter focusing on neo-epithelialization and secondary operations. This study aims to explore diagnosis and management during neonatal care. Methods: This systematic review exclusively examined full-text articles published in English between 2014 and 2024, adhering to the PRISMA 2020 guidelines. Result: The study conducted a thorough review of more than 300 publications that were obtained from reputable databases, such as ScienceDirect, SagePub, and PubMed. Eight publications were identified as necessitating a more comprehensive examination subsequent to an initial screening. As a result, a comprehensive review of these selected studies was conducted to guarantee an exhaustive and rigorous assessment. Conclusion: Giant omphaloceles management involves a multifaceted approach, including primary closure, staged surgical closure, and nonoperative delayed closure. Synthetic materials and biological meshes promote granulation tissue formation and gradual closure, but further research is needed to reduce morbidity and mortality.

Abstract

Background: Laryngeal carcinoma is a malignancy arising from squamous cells of the epithelial lining of the larynx. It is one of the more common head and neck cancers, with an incidence of approximately 3.2 cases per 100,000 individuals annually worldwide. Risk factors include smoking, alcohol consumption, gastroesophageal reflux disease, exposure to chemical inhalants, and viral infections, particularly Human Papillomavirus (HPV). Objective: This study aimed to evaluate the demographic characteristics, histopathological features, and risk factors associated with laryngeal carcinoma among patients at Mardi Waluyo Hospital. Methods: This descriptive study utilized data from medical records and histopathological examinations of patients between 2018 and 2023, analyzed in January 2024 at Mardi Waluyo Hospital. Results: Among 39 patients, 34 were male (87%), and 5 were female (13%). Patients aged ≤60 years accounted for 36%, while those >60 years constituted 64%. Histopathological examination revealed squamous cell carcinoma in 34 cases (87%) and other pathologies in 5 cases (13%). Smoking history was significant: 75% had smoked for more than 10 years, while 25% smoked for less than 10 years or were non-smokers. Conclusion: Over the past five years, an average of 8 cases per year of laryngeal carcinoma were reported. Smoking was identified as the most significant risk factor. Squamous cell carcinoma was the predominant histopathological finding in patients at Mardi Waluyo Hospital.

Abstract

Background: The management of pneumothorax significantly across different clinical guidelines, reflecting ongoing uncertainty in the optimal approach to treatment. Recent research has highlighted the potential benefits of conservative management compared to surgical approach. This systematic review aims to compare the outcomes of conservative management and chest tube drainage in the treatment of pneumothorax by analyzing available studies of the last 10 years. Methods: The study adhered to PRISMA 2020 standards, examining English literature from 2014 to 2024. It excluded editorials, reviews from the same journal, and submissions without a DOI. PubMed, SagePub, SpringerLink, and Google Scholar were utilized as literature sources. Result: Initially retrieving 360 articles from online databases (PubMed, SagePub, SpringerLink and Google Scholar) eight relevant papers were selected after three rounds of screening for full-text analysis. Conclusion: Conservative management of pneumothorax, particularly in clinically stable patients, has shown effectiveness comparable to chest tube drainage, with benefits such as shorter hospital stays and fewer complications. However, careful patient selection and further trials are needed to confirm its long-term safety and efficacy.

Abstract

Background: Allergic conjunctivitis (AC) is a common ocular condition that significantly impacts the quality of life of individuals worldwide. This systematic review aims to provide a comprehensive overview of the prevalence, diagnostic approaches, and management strategies for allergic conjunctivitis. Methods: The study followed PRISMA 2020 guidelines, reviewing English-language publications from 2014 to 2024. Editorials, duplicate reviews from the same journal, and papers lacking a DOI were excluded. The literature search was conducted using PubMed, SagePub, SpringerLink, and Google Scholar. Result: A total of 2,172 articles were initially identified through online databases (PubMed, SagePub, SpringerLink, and Google Scholar). After three rounds of screening, eight relevant studies were selected for full-text analysis. Conclusion: Allergic conjunctivitis is common and impacts quality of life. Management includes pharmacologic and non-pharmacologic treatments, with dual-acting antihistamine-mast cell stabilizers being key. Many remain undiagnosed or inadequately treated, highlighting the need for improved awareness, diagnostics, and future research.

Abstract

Background: Cataracts, the leading cause of blindness and a major contributor to vision impairment worldwide, disproportionately affect older adults, particularly in low- and middle-income countries with limited access to eye care. While cataract surgery is a highly cost-effective intervention capable of restoring vision, the prevalence of cataracts continues to rise due to socio-economic and infrastructural challenges. To address these issues, the World Health Organization (WHO) has introduced initiatives such as the Package of Eye Care Interventions (PECI) and emphasized the need for updated, region-specific clinical practice guidelines to improve cataract management and enhance universal health coverage. Methods: This systematic review adhered to the PRISMA 2020 guidelines, focusing exclusively on full-text articles published in English between 2014 and 2024. Editorials and review articles without a DOI were excluded to ensure high-quality sources. An exhaustive literature search was conducted across reputable databases, including ScienceDirect, PubMed, and SagePub, to identify relevant studies. Result: An initial database search yielded 500 publications relevant to adult cataract management. After implementing a rigorous three-stage screening process, eight studies meeting the inclusion criteria were selected for comprehensive analysis. These studies were critically evaluated to provide an in-depth assessment of preferred practices in adult cataract management, ensuring a focused and high-quality synthesis of the available evidence. Conclusion: Management strategies for cataracts are crucial for addressing global blindness and visual impairment, focusing on early detection, early detection, and addressing global disparities to optimize surgical outcomes and patient quality of life.

Abstract

Background: Placenta Accreta Spectrum (PAS) refers to a group of abnormal placental invasion disorders involving the uterine wall. High-Intensity Focused Ultrasound (HIFU) has emerged as a promising non-invasive treatment modality for PAS. This systematic review aims to evaluate the clinical outcomes associated with HIFU in the management of PAS. Methods: The study followed PRISMA 2020 guidelines, reviewing English-language publications from 2014 to 2024. Editorials, duplicate reviews from the same journal, and papers lacking a DOI were excluded. The literature search was conducted using PubMed, SagePub, SpringerLink, and Google Scholar. Result: A total of 2,172 articles were initially identified through online databases (PubMed, SagePub, SpringerLink, and Google Scholar). After three rounds of screening, eight relevant studies were selected for full-text analysis. Conclusion: HIFU offers a promising, non-invasive alternative for managing Placenta Accreta Spectrum (PAS), preserving fertility, minimizing complications, and ensuring positive outcomes. While further research is needed, it could become a standard treatment option in obstetrics and gynecology.

Abstract

Background: Rehabilitation after ankle fracture surgery is critical for optimizing functional recovery. This systematic review explores various post-surgical rehabilitation strategies, including weight-bearing protocols, compression therapy, manual therapy, and active controlled motion, focusing on their efficacy in improving patient outcomes. Methods: A systematic review was conducted using seven randomized controlled trials and one retrospective analysis. Studies were assessed for interventions such as early versus delayed weight-bearing, compression stockings, manual therapy, active controlled motion, and ankle supports. Key outcomes included functional scores (OMAS, AOFAS), quality of life measures, range of motion, and return-to-activity timelines. Results: Early weight-bearing was associated with better functional recovery and earlier return to work, while active controlled motion improved range of motion and patient-reported outcomes. Compression therapy effectively reduced swelling and enhanced functional outcomes, but manual therapy showed limited benefits. Ankle supports, such as walker boots and stirrup braces, reduced pain and improved gait parameters compared to Tubigrip. Overall, no significant increase in complications was observed with early mobilization strategies. Conclusion: Individualized rehabilitation plans incorporating early mobilization, compression therapy, and supportive devices can optimize functional recovery after ankle fracture surgery. Future research should focus on long-term outcomes and cost-effectiveness of these strategies.

Abstract

Background: Indonesia ranks fourth globally in rheumatic heart disease (RHD) prevalence, with a mortality rate of 4.8 per 100,000—higher than malaria. Despite global progress in RHD management, Indonesia lacks a comprehensive national prevention program. Secondary prevention strategies could significantly improve outcomes in high-risk populations by targeting disease progression. Method: This systematic review and meta-analysis examined the impact of secondary prevention on echocardiographic outcomes in Indonesian RHD patients. The review included randomized controlled trials, observational studies, quasi-experimental designs, and case-control studies, emphasizing the role of secondary prevention in mitigating disease progression. Results: Six studies meeting inclusion criteria underwent full-text analysis, with four eligible for meta-analysis. These studies revealed that secondary prevention improved echocardiographic markers, demonstrating its potential to alter disease trajectories in resource-limited settings. Conclusion: Indonesia faces challenges in RHD management due to geographical and healthcare constraints. Echocardiographic screening, a sensitive and specific diagnostic tool, is vital for early detection and monitoring. With technological advancements, cost-effective screening is increasingly feasible, even in remote areas. Strengthening early detection, targeted screening, and secondary prevention strategies is critical for reducing the RHD burden in Indonesia.

Abstract

Background: Extirpation of the uterus during the puerperium, after vaginal delivery, or after a caesarean section is known as an emergency obstetric hysterectomy (EOH). It is typically carried out when obstetric haemorrhage is severe and life-threatening. A woman who almost died but survived a pregnancy-related or childbirth-related issue, or within 42 days of the pregnancy ending, is said to have experienced a near-miss episode. The aim: The aim of this study to show about incident, indications, risk factor, management and outcomes of emergency hysterectomy. Methods: By the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, this study was able to show that it met all of the requirements. This search approach, publications that came out between 2014 and 2024 were taken into account. Several different online reference sources, like Pubmed, SagePub, and Sciencedirect were used to do this. It was decided not to take into account review pieces, works that had already been published, or works that were only half done. Result: Eight publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: In obstetrics, emergency obstetric hysterectomy (EOH) is a necessary evil. In many instances, it saves the mother's life, even though it limits the woman's future ability to bear children. Rather than the treatment itself, the majority of its morbidity is caused by its indications and underlying illnesses.
