Abstract

Background: Mumps or epidemic parotitis is an acute disease that often occurs in the salivary glands caused by infection from paramyxovirus . This virus spreads easily through airborne droplets and saliva. In the world, the incidence of mumps is 500,000 cases each year. Incomplete basic vaccination is one of the risk factors for mumps. Case Description: An 11-year-old male patient came with complaints of swelling in the right and left parotid glands. Initially the swelling was on the left. The patient had never been like this before. The patient's basic immunization history was incomplete. From physical examination, enlargement of both salivary glands was found. 2 weeks earlier at the patient's school, there were several students who experienced the same thing. The patient was referred to dr. Ramelan Central Naval Hospital after 5 days of hospitalization at the previous hospital and experienced swelling in the left scrotum. Discussion: Mumps often occurs in children aged 2-12 years. The occurrence of mumps can be caused by infection with the paramyxovirus virus which spreads from airborne droplets and salivary glands. One of the complications of mumps that often occurs in children is orchitis . Providing basic vaccines to children is one effort to prevent mumps. Bed rest, adequate fluid intake, and intravenous administration of NSAIDs and antibiotics can be options for mumps therapy. Conclusion: In this case, Mumps with Orchitis complications were diagnosed based on anamnesis, physical examination, and laboratory examination. Management of mumps is symptomatic. Incomplete basic vaccination is one of the biggest factors in the occurrence of mumps.

Abstract

Introduction : Transcranial magnetic stimulation (TMS) is a non-invasive technique that offers potential for enhancing motor function recovery after stroke by modulating brain activity. Among the different forms of TMS, repetitive TMS (rTMS) has been identified as a promising tool for stimulating brain regions associated with lower limb movement and improving motor function. This review aimed at assessing the effect of in rTMS in post-stroke lower limb motor recovery Methodology : This systematic review followed PRISMA guidelines and included randomized controlled trials examining the effects of rTMS on lower limb motor recovery in post-stroke patients. Eligible studies involved adult stroke patients with motor dysfunction, and interventions included rTMS alone or combined with rehabilitation or medication therapy. The primary outcome assessed was motor function, with various measurement tools used. A comprehensive literature search was conducted across multiple databases, and data extraction and quality assessment were done independently by the authors using the Modified Jadad Scale. Results : A total of 14 randomized controlled studies were included in this review, involving 277 participants with varying stroke types and severities. The studies assessed the effects of low- frequency (1 Hz) and high-frequency (5-20 Hz) rTMS, combined with rehabilitation therapies, on lower limb motor recovery. The results showed significant improvements in lower limb motor function, balance, and spasticity, particularly with both high or low-frequency rTMS combined with other rehabilitation therapies. High-frequency rTMS also yielded positive effects, particularly in walking speed and gait symmetry. Conclusion : The rTMS procedure consistently showed improvements in lower motor function, balance, and gait in post-stroke patients compared to sham and control interventions.

Abstract

Background : According to WHO, breast cancer sufferers will increase by 20% every year and 84 million people will die worldwide. One of the treatments for breast cancer is chemotherapy. However, chemotherapy has physical and psychological side effects that will cause anxiety in patients, which will actually complicate the healing process, so family support is needed so that patients will not think that the chemotherapy they are undergoing is something threatening. Research objective: to determine the relationship between family support and anxiety in breast cancer patients undergoing chemotherapy at Abdul Moeloek Hospital, Bandar Lampung. Type of research : analytical survey, with a cross-sectional design . The population is all breast cancer patients undergoing chemotherapy at RSUD. Abdul Moeloek Bandar Lampung in 2018, totaling 120 people, and the sample uses accidental-sampling . This study uses univariate and bivariate analysis with statistical tests, namely chi square . Results: found from 120 breast cancer patients, the majority had good family support of 84.2%, and patients who were not anxious were 58.3%. The results of statistical tests with Chi Square on Pearson Chi Square p value (0.000) <0.05. Conclusion : There is a relationship between family support and anxiety in breast cancer patients undergoing chemotherapy at Abdul Moeloek Hospital, Bandar Lampung in 2018.

Abstract

Introduction: The literature surrounding congenital diaphragmatic hernia (CDH) in neonates has evolved significantly over the past decade, reflecting advances in both diagnostic and surgical approaches. Literature Review : Chaudhary et al. (2019) further contribute to understanding prognostic factors associated with CDH, identifying critical indicators that correlate with survival outcomes. Their retrospective audit highlights the variability in survival rates among healthcare centers, underscoring the need for standardized protocols in managing CDH. Similarly, Raitio et al. (2021) systematically review the impact of hernia sac presence on survival outcomes, revealing that the presence of a hernia sac may confer physiological advantages, thereby improving prognosis. Long-term outcomes for CDH survivors remain a critical concern, as highlighted by Yamoto et al. (2022), who report that while survival rates have improved, these patients are at risk for various long-term complications. Cimbak and Buchmiller (2024) reinforce this by advocating for long-term follow-up clinics to manage the complex health issues that arise in CDH survivors. Their work emphasizes the necessity of a structured approach to longitudinal care that adapts to the evolving needs of this patient population. Conclusion: In conclusion, the literature reflects a multifaceted understanding of the diagnostic and surgical outcomes for congenital diaphragmatic hernia in neonates. Continued research, collaboration among healthcare professionals, and comprehensive care strategies are essential for optimizing outcomes and addressing the long-term challenges faced by CDH survivors.

Abstract

Introduction: The literature on pediatric hernias and their surgical management has seen significant advancements, particularly in the comparison between laparoscopic and traditional open repair techniques. Literature Review : Research has highlighted various advantages of laparoscopic repair, such as shorter recovery times and reduced postoperative pain (Namgoong et al., 2014). However, concerns regarding recurrence rates and operational costs persist, prompting further investigation into the nuances of different surgical approaches (Min Lim et al., 2020). A retrospective study reinforced the notion that while laparoscopic techniques offer enhanced visibility and potential cosmetic benefits, open repair remains a reliable standard due to its simplicity and established success rates (N. Al-Taher et al., 2021). The growing body of literature underscores the importance of minimally invasive surgery (MIS) in pediatric settings, with bibliometric analyses indicating a trend towards laparoscopic techniques, particularly in cases of bilateral hernias and recurrences (Shu et al., 2022). Recent contributions have advocated for personalized surgical approaches that prioritize patient recovery and safety, reflecting an evolution in hernia repair methodologies.  Conclusion: Collectively, the literature illustrates a robust discourse surrounding pediatric hernia repair, where the ongoing evaluation of laparoscopic techniques against traditional methods is paramount. The findings indicate that while laparoscopic approaches are gaining traction, a balanced perspective that considers both the potential benefits and the established efficacy of open repair is essential for informed surgical decision-making in pediatric patients.

Abstract

Background : Congenital anomalies (CA) require early treatment and impair pediatric care. Knowing demographic group incidence, classification, and distribution of inherent anomalies helps treatment and resource allocation. Aims : This study examines forms and demographics of CA in children treated at dr. Doris Sylvanus Regional Hospital for a year. Methods : A retrospective study was carried out by analyzing 320 patient files between July 2023-June 2024 at dr. Doris Sylvanus General Regional Hospital. The classification system ICD-10 was used for the evaluation of frequency, gender, and geographical distribution of CA. Results : The obtained analysis revealed a higher incidence of CA in males than in females: 208 (65%) and 112 (35%), respectively. There were 72 (22.5%) cases of CA in the genital organs, the highest number found, followed by the circulatory system with 67 (20.9%) and digestive system with 44 (13.75%) cases. Most patients are coming from the city of Palangka Raya, where 197 cases were reported (61.56%), followed by Gunung Mas with 28 cases (8.75%) and East Kotawaringin with 20 cases (6.25%). Several diagnoses are often made, such as Down syndrome, Hirschsprung disease, and congenital heart malformation. Discussion : The findings provide evidence of the need for specific health interventions in regions that seem to have higher rates of CA, including the city of Palangka Raya. The gender bias found in this research is supported, and its presence is to be further deeply probed. Timely detection of focused therapeutic management is critical in getting better outcomes among children born with CA. Conclusion : The analysis indicated a significant prevalence of abnormalities in the genital and circulatory systems, highlighting the urgent requirement for specialized health services to address these issues at an earlier stage in life. The prevalence of congenital abnormalities is greater in males and in urban regions like Palangka Raya, where access to health services is more prevalent.  This disparity in access to health services between urban and rural areas is prevalent globally and significantly hinders the attainment of universal health coverage across all regions of Indonesia.  This study's findings align with a broader investigation in the area, indicating that disparities in access to health services and health outcomes are influenced by socioeconomic, geographic, and demographic factors.

Abstract

Introduction: Male circumcision is a longstanding procedure performed worldwide for cultural, religious, and medical reasons. In Indonesia, it is commonly carried out on children aged 5–12 years, typically using the dorsal slit technique. Objective: This report aims to present a case of a 10-year-old male circumcised for religious reasons and to compare conventional and modern circumcision techniques. Case Report: A 10-year-old boy, accompanied by his parents, presented for circumcision due to religious reasons. The patient was asymptomatic, with a fully retractable foreskin and no anatomical abnormalities. A dorsal slit circumcision was performed under local anesthesia, resulting in an uneventful recovery and proper wound healing. Discussion: Evidence suggests that circumcision can reduce the risk of STIs, including HIV, HPV, and HSV, by limiting pathogen entry and altering the local immune environment. Modern techniques, such as clamp-assisted and adhesive methods, have shown advantages over conventional methods, including shorter operative times, fewer complications, and improved cosmetic outcomes. Conclusion: Male circumcision remains a valuable public health measure with significant cultural and medical benefits. Adoption of modern techniques may further enhance patient outcomes while preserving the traditional advantages of the procedure.

Abstract

Background: Home-based rehabilitation offers a convenient and effective alternative for stroke patients, utilizing telerehabilitation and virtual reality therapy. These therapeutic interventions have shown promising results in improving motor function and daily living activities. However, integrating these programs into standard stroke care requires a comprehensive understanding of their efficacy, safety, and implementation challenges. Methods: This systematic review followed PRISMA 2020 guidelines and concentrated solely on full-text articles published in English from 2015 to 2025.  Editorials and review articles lacking a DOI were excluded to maintain the integrity of high-quality sources.  A literature review was performed using reputable databases such as ScienceDirect, PubMed, and SagePub to identify pertinent studies. Result: The initial database search produced more than 600 pertinent publications on the subject. After a thorough three-stage screening process, eight studies fulfilled the established inclusion criteria and were chosen for detailed analysis. Each study underwent a detailed critical evaluation, facilitating an in-depth analysis of the Home-Based Rehabilitation Program for Stroke Patients. This systematic approach ensured that the analysis was based on high-quality evidence, aligned with the study's objectives, and capable of yielding significant insights into this complex association. Conclusion: Stroke rehabilitation programs, incorporating telerehabilitation and virtual reality (VR), offer a viable alternative to traditional settings. These technologies improve functional outcomes, but addressing implementation challenges and ensuring equitable access is crucial for maximizing benefits. Future research should focus on user-centered designs and long-term outcomes.

Abstract

Background: Anesthesia is a cornerstone of modern surgery, providing the necessary sedation, pain control, and muscle relaxation that make complex procedures possible. However, its effects extend far beyond simply putting patients to sleep, particularly in those with preexisting cardiovascular disease. This systematic review aims to synthesize current evidence regarding the impact of anesthesia on patients with cardiovascular disease. Methods: The study followed PRISMA 2020 guidelines, reviewing English-language publications from 2015 to 2025. Editorials, duplicate reviews from the same journal, and papers lacking a DOI were excluded. The literature search was conducted using PubMed, SagePub, SpringerLink, and Google Scholar. Result: A total of 2,172 articles were initially identified through online databases (PubMed, SagePub, SpringerLink, and Google Scholar). After three rounds of screening, eight relevant studies were selected for full-text analysis. Conclusion: Anesthesia significantly impacts patients with CVD, requiring careful selection of agents and techniques to minimize perioperative risks. Volatile anesthetics improve outcomes in cardiac surgery, while conscious sedation offers advantages in TAVR procedures. α2-Adrenoceptor agonists, EEG-guided anesthesia, and multidisciplinary perioperative management are essential for optimizing patient outcomes. Further research is needed to enhance perioperative care for this high-risk population.

Abstract

Introduction: This review examines the global challenge of antimicrobial resistance (AMR), focusing on drug-resistant tuberculosis (DR-TB) as a major contributor to the global AMR burden. It provides an overview of the increasing threat posed by DR-TB and the need for advanced detection and management strategies. Methods: The review synthesizes current literature on DR-TB, particularly on resistance mechanisms, diagnostic advancements, and treatment strategies. It analyzes data from recent studies on gene mutations in Mycobacterium tuberculosis (M.tb) and the role of the bacterial cell envelope in resistance. Results: Key findings include the identification of genetic mutations linked to resistance to first- and second-line anti-TB drugs. The role of M.tb's cell envelope in drug resistance and host-pathogen interactions is also highlighted. Additionally, the review presents recent advancements in diagnostic tools, emphasizing the need for universal drug susceptibility testing and shorter, all-oral treatment regimens. Conclusion: The review underscores the challenges in diagnosing and treating DR-TB, including the need for improved diagnostic accuracy, accessibility, and treatment adherence. It emphasizes the importance of a multi-pronged approach, incorporating healthcare system strengthening, investment in new drug research, and global collaboration to achieve TB elimination by 2030.
