Jurnal ARSI (Administrasi Rumah Sakit Indonesia) was initiated by the Center for Health Administration and Policy Studies (CHAMPS) Faculty of Public Health, University of Indonesia (FKM UI) and is currently managed by the Department of Health Administration and Policy, Faculty of Public Health, University Indonesia . Jurnal ARSI (Administrasi Rumah Sakit Indonesia) was published with the Indonesian Hospital Association (PERSI) and the FKM UI Hospital Management Association (IKAMARS). Jurnal ARSI (Administrasi Rumah Sakit Indonesia) is a peer-reviewed journal that focuses on service administration and management in hospitals in Indonesia. Abstract

Linen management is an important process in hospitals that is directly related to service quality and Infection Prevention and Control (IPC) efforts. This study aimed to analyze the implementation of Lean Management in linen management at the laundry unit of RSU X and to evaluate its contribution to improving process efficiency and strengthening IPC practices. The study employed an operational research design with a qualitative approach through observation, in-depth interviews, and document review. Data analysis was conducted using Value Stream Mapping, waste identification based on the eight waste concept, and root cause analysis using the 5 Whys method. The results showed that the average lead time of linen management before improvement was 534.41 minutes, with non-value-added (NVA) activities accounting for 61.11% of the total process time. The most dominant type of waste identified was Defects in Products and Information, characterized by non-compliance with work procedures, inconsistent use of personal protective equipment (PPE), and suboptimal separation of clean and dirty linen flows. As a follow-up action, Lean Kaizen interventions were implemented through the Plan-Do-Check-Act (PDCA) cycle, including the development and dissemination of standard operating procedures (SOPs), work process standardization, reinforcement of PPE compliance, and improvement of supporting facilities. After the implementation, the lead time decreased to 343.24 minutes and NVA activities were reduced to 134.47 minutes. The study concludes that Lean Management is effective in reducing waste, improving the efficiency of linen management processes, and strengthening IPC implementation in hospitals.

Abstract

Medical record administration is an essential component in supporting healthcare service quality. During the transition toward Electronic Medical Records (EMR), many hospitals still use semi-digital systems that combine conventional and electronic processes. This study aimed to describe semi-digital medical record administration at RSU X, Depok City, based on the Man, Money, Material, Method, and Machine (5M) management elements and the Planning, Organizing, Actuating, and Controlling (POAC) management functions. This study employed a descriptive qualitative approach through in-depth interviews and observations involving five informants. The results showed that the semi-digital system operated through the parallel use of physical medical records and the EMR system. In terms of Man, only four staff members had a medical record educational background, and training had not been equally provided. Regarding Money, budgeting was managed by the central management. For Material, storage facilities did not meet standards, particularly storage racks, tracers, and color-coding systems. In terms of Machine, technical problems with computers and printers were still encountered. Regarding Method, implementation complied with the Indonesian Ministry of Health Regulation No. 24 of 2022 through 15 Standard Operating Procedures covering manual and digital processes. The implementation of POAC functions was not optimal because coordination and supervision remained limited to the unit coordinator level without active involvement from the department head. This study indicates that the semi-digital system at RSU X Depok City has not been fully integrated and requires improvements in human resources, infrastructure, and policy support to ensure an effective transition toward full EMR implementation.

Abstract

Dengue Hemorrhagic Fever (DHF) is an acute infectious disease that often requires hospitalization with a variable length of stay (LOS). Several factors have been shown to influence LOS; however, findings from previous studies conducted in different hospitals remain inconsistent. This study aimed to analyze the association between age, gender, platelet count, hematocrit level, and leukocyte count with LOS among patients with DHF at Nahdlatul Ulama Islamic Hospital (RSI NU) Demak. This study employed a cross-sectional design using medical record data from 64 hospitalized patients with DHF admitted between January and May 2025, selected through purposive sampling. The sample eligibility criteria included inclusion and exclusion criteria. The inclusion criteria were patients diagnosed with DHF (ICD-10: A90) who were hospitalized for ≥24 hours and had complete medical records. The exclusion criteria were patients with severe comorbidities or incomplete medical records. Data were analyzed using Spearman’s correlation test, which demonstrated a significant association between platelet count and LOS (rₛ = -0.322; 95% CI [-0.522; -0.080]; p-value = 0.009). The negative correlation coefficient indicates that an increase in platelet count was associated with a shorter LOS, although the strength of the association was weak. These findings indicate that platelet count plays an important role in determining the length of hospital stay among patients with DHF. Early monitoring of platelet count is important for supporting prognosis assessment and guiding therapeutic decision-making.
