Journal of Indonesian Health Policy and Administration is a journal that presents scientific articles mainly in the field of health policy, programs, and administration. This journal is intended to disseminate research from students, lecturers, or researchers in general who are concerned about improving health efforts through program evaluation and preparing policy recommendations. The articles or manuscripts contained in the Journal of Indonesian Health Policy and Administration include the realm of research, case studies, or conceptual, and limited to literature review. It is an online open-access, blindly peer-reviewed journal that is published every 4 (four) months or 3 (three) times a year, usually in January, May, and September. This journal is published by the Department of Health Administration and Policy, Faculty of Public Health, University Indonesia . Journal of Indonesian Health Policy and Administration is also supported by the Indonesian Public Health Association / Ikatan Ahli Kesehatan Masyarakat Indonesia (IAKMI).

Abstract

By 2015, 68% of deaths in Indonesia are due to non-communicable diseases (NCD) and are forecast to increase to 74% by 2030. Riskesdas data of 2013 show that 69.6% of cases of diabetes mellitus and 63.2% of cases of hypertension have not been diagnosed. The government has been trying to proactively make efforts to prevent NCD through the implementation of Posbindu (Integrated Health Post) NCD, but visit Posbindu NCD in the work area of Pasir Mulya Puskesmas, Bogor City is very diverse. The purpose of this study is to analyze the factors of management, communication, partnership, and innovation in the implementation of Posbindu NCD and determinants of the implementation of Posbindu NCD. We conducted in-depth interviews, focus group discussions, document review and observation at two Posbindu NCD with the highest number of visits and the lowest in Gunung Batu Village, Bogor City in 2017. We interviewed 15 informants, consisting of cadres, in charge of NCD Puskesmas, in charge of Puskesmas Pembantu and supervisor of Posbindu NCD. This research found 4 factors, namely management (human resources, fund, and facilities), innovation, communication and partnership that influence the result of Posbindu implementation. Therefore, to run effectively the Posbindu NCD program, then 4 of those factors need to be optimized

Abstract

In order to control Antibacterial Resistance at the hospital in Indonesia, the Government of Indonesia established an Antibiotics Resistance Control Committee and issued a Regulation from the Ministry of Health, regulation no 8/2015 on an antibiotics resistance control program at the hospital. This is an investigation on the Antibacterial Resistance Control Program (ARCP) at the Hospital, which compares its implementation at a government hospital and a private hospital, Persahabatan Central General Hospital (CGH) and Bogor Indonesian Red Cross Hospital (BIRCH). This was a descriptive study with a qualitative analysis obtained through indepth interviews and documentary research. We discovered that the ARCP was implemented better at Persahabatan CGH compared to BIRCH. Problems include ineffective communication, limited resources, lack of an antibiotic guideline (at BIRCH), lack of coordination between organizations, lack of supervision from the government, and the lack of motivation to implement the policy. These issues can be solved by intensifying the socialization of the policy, conducting more workshops and trainings for private and government hospitals that has not received any, providing an incentive for them to implement the program, such as including ARC into hospital accreditation indicators, and clarifying the benefits of the program to the stakeholders.

Abstract

In managed care, primary healthcarecare facilities acts as the gatekeepers, and its success is judged from the number of visits. In the year 2016, the Healthcare Centers in Tangerang was unable to meet the targets set by BPJS healthcare, ideally the visit rate should reach >150‰ and the referral ratio <10%. However, the visit rate only reachd 41‰ and the referral ratio was 29%. In addition, most healthcare centers the nonspecialised referral ratio was above 5%. The purpose of this research is to analyze the healthcare service system as the gatekeepers in managed care. This research is a descriptive analytical study with a qualitative approach. The data was collected through in-depth interviews, examination of documents and observations. Research conducted at healthcare centers and clinics Pedurenan Pabuaran Tumpeng from May-June 2018. From this research we discovered that the low contact lrate was caused be the low first contact levels, continuity, and comprehensiveness. The registering process was also complicated. We also discovered that the lack of human resources also influences the low contact rate.

Abstract

Law No. 36 of 2014 is a comprehensive law for health professionals. In that regulation, all health graduates that wish to practice their trade must have a letter of registration (STR). That policy also separated environmental health from public health. This was the reason why we decided to evaluate the policy that is related to public health professionals. This evaluation was a retrospective analysis that used in-depth interviews and literary research, respectively as primary and secondary data. From this research we discovered that public health graduates is has their own profession. And to obtain the right to practice their trade, they must pass the competence standard test. However, according to other regulations, only vocational or professional education graduates can participate in the test. Since there currently are no professional education institutions available for public health professionals, Minister of Health Regulation No. 41/2013 takes over. According to this law, all public health graduates will be provided an STR until there is a professional education institution. We also discovered that environmental health should NOT be separated from public health, since it is a large part of it. And therefore, the law must be amended.

Abstract

The accreditation of health centers is an endeavor by the Ministry of Health to improve the quality and performance of health centers regulated by Permenkes No. 4/2015. Of the 38 health centers in Brebes, only 10 has been accredited. We wish to discover how prepared are the health centers to be accredited based on the resources available and pre-accreditation survey. Data was obtained through in-depth interviews and documentary research. Results indicated that there was sufficient funds, facilities and equipment for the accreditation. However, the assessment scores for human resources sufficiency and documentary completeness was low.

Abstract

The Health Social Security Agency (BPJS kesehatan) is one of the government’s efforts to provide quality healthcare for the people. To guarantee its quality and maintain efficiency, a tiered referral system is used. However, there were many problems in its implementation. One of the ways to investigate is to evaluate the non-specialist outpatient referrals (RRNS). Therefore, in March-July 2018, this research was performed in order to analyze whether the system was implemented at Beji and Depok Jaya Health Center. The method used was the qualitative research method, with primary data from in-depth interviews and secondary data from documents. It was discovered that both Centers had optimal RRNS ratios (0%). This was because the doctors at both Centers has provided referrals as indicated and there was feedback on the RRNS total. The civil servant (PNS) doctors at both centers were experienced and skillful, but the number of doctors was insufficient compared to their workload. The non-PNS doctors also were not sufficiently trained. Other problems include insufficient equipment and medication provision.

Abstract

Soldier is a death-risk profession; thus, identification to have their exact identity is a fundamental right. The Presidential Regulation Number 107 of 2013 mandates Ladokgi RE. Martadinata as the investigator of victim identification activities for Indonesian Navy Soldiers. This study analyzes the implementation of the policy with a qualitative descriptive approach using the Edward III model analysis. Retrieval of primary informant data through in depth interviews and secondary data, including observation and review of the literature, documents, and related laws and regulations. Data validation is done by triangulating data sources, theories, and methodologies. The results of the study indicate that there are several supporting factors. First, the Ladokgi bureaucratic structure factor has a Military Dentistry Department in charge of managing dental antemortem data and had gathered 9000 soldiers data by 2019. The second is the resource factor. Ladokgi has regular budget sources from the state budget, personnel, health materials, and routine training programs. Third, the Ladokgi leadership's disposition factor has made the Navy Forensic Odontology into a functional organization under the control of the Ladokgi Chief. The fourth is communication factors, the existence of a tiered reporting system to the top command.However, there are several inhibiting factors. From the bureaucratic structure and resource factor is the absence of a root organization with various supporting professions for the identification. There is still the absence of standard information flow procedures in the reconciliation phase in the communication factor. In the disposition factor, the micro policy is absent in the form of guidelines for identifying victims in the Indonesian Navy. Conclusion. The implementation of the policy of identifying TNI AL casualties in Ladokgi RE. Martadinata has been implemented but is still partial, such as dental-based antemortem data management. We recommend that the commitment from the leader is needed in this case the Head of the Navy's Health Service regarding the involvement of required cross professional personnel in the organization of victim identification, through the revision of Presidential Regulation 107 of 2013 and the preparation of several technical policies needed in victim identification activities within the Navy.

Abstract

Schoolchildren are assets for the development of the nation. Therefore, healthy meals at schools are very important, because for them, it covers about 25-36% of the daily energy requirements. Unhealthy eating habits or consumption of nutrition deficient meals may cause stunting, cardiovascular diseases, cancer, diabetes, and osteoporosis. In the short term it may also cause dental caries, anemia, overweight children and obesity. Around 40-44% of meals at schools do not satisfy the food safety requirements. To increase the quality of the meals at the schools, the community should be empowered to perform independent monitoring of school meals. This is inscribed in the national action policy for safe, good-quality, and nutritious meals for schoolchildren. The aim of this research was to assess the effectiveness of the implementation of the national action policy for the snacks of schoolchildren and the factors that influences it. This was a qualitative research. The data was gathered from in-depth interviews and documentary studies and then analyzed using the policy implementation analysis of Mazmanian and Sabatier. From this research we discovered that the policy did not perform effectively in Batam City. There were no Procedure Standard Norms NSPK on the roles, tasks, and responsibilities of the implementers, there was no implementation structure, and no support from the Government and the Parliament of Batam City for the monitoring.

Abstract

The process to establish a planning and budgeting policy in the Directorate for Occupational and Sports Health is very dynamic and it fluctuates according to the goals and priorities of the policy. This research aims to ascertain how far the political aspects influences the establishment process. This research was a qualitative research accomllished through in-depth interviews with related parties. The framework used the planning and budgeting systems model and process/cycle. Results of this research indicated that politics played a major role in the policy establishment process in conflicts, cooperation, and negotiations when setting the size and allocation of the budget and the targets aimed. The main points in the process are the priorities of the program, the fiscal capacity of the nation, and the results of the previous year’s evaluation. We recommend that the human resources of the directorate are equipped abilities to advocate the importance of the occupational and sports health program.

Abstract

One of the functions of hospital bylaws are a means to provide legal protection for all all parties that are related to the hospital. Legal protection is essential for prosecuted hospital staff members. One of the reasons for indictments is adverse events. The goal of this research is to discover how the hospital by-laws protects the hospital staff in the event of an indictment based on an adverse event and how the by-laws adhere to the guidelines in a comprehensive way. The research design used the qualitative method. The results of this research indicates that the hospital by-laws of RSKO Jakarta, although it has served as a protective method, it still needs further technical-operational policies as a supplement, and there are still discrepancies compared to the guidelines. We suggest that the by-laws of RSKO Jakarta are amended to adhere to the governmental guidelines and to establish supplementing technical-operational policies.
