Abstract

Background: Hybrid coronary revascularisation (HCR) integrates minimally invasive coronary artery bypass grafting (CABG) with percutaneous coronary intervention (PCI), presenting a viable alternative for managing severe triple-vessel disease (TVD). Although conventional CABG is the benchmark for multivessel disease, HCR has arisen as a method intended to diminish surgical morbidity while preserving long-term revascularisation advantages. This systematic study assesses the long-term clinical results of HCR in comparison to CABG. Methods: A computerised literature search was conducted across three search engines (Google Scholar, PUBMED, Crossref) for the years 2014-2025, with the keywords: CABG, Hybrid, TVD, and Triple Vessel. Quality of studies assessed using the Mixed Methods Appraisal Tool (MMAT) version 2018. Results: Initially, 1339 papers were retrieved; utilizing the PRISM algorithm, 2 articles were ultimately reviewed. Basman et al. (2020) discovered that the 8-year survival rates were comparable between HCR (5.0%) and CABG (4.0%), with no significant disparity in Major Adverse Cardiovascular Events (MACE) rates. Esteves et al. (2020) observed a greater incidence of adverse events in HCR (19.3%) compared to CABG (5.9%) at two years, primarily due to increased unexpected revascularisation; however, HCR demonstrated feasibility with acceptable safety. Both studies indicated that HCR is most appropriate for specific patients in whom PCI can successfully enhance minimally invasive bypass methods. Conclusions: HCR has benefits like reduced hospital durations and diminished surgical morbidity; however its long-term results do not surpass those of CABG. Increased revascularisation rates in HCR prompt apprehensions over its longevity in comparison to normal CABG. Future extensive randomised trials are necessary to formulate definitive clinical recommendations for patient selection.

Abstract

Background: Pericardial effusion is a common manifestation of Systemic Lupus Erythematosus (SLE), a chronic autoimmune disease affecting young women. This rare symptom requires high suspicion and prompt diagnosis for appropriate treatment, emphasizing the importance of early detection and management. Case Description: A patient with pericardial effusion presented to the emergency department with symptoms such as weakness, mouth ulcers, and nausea. Despite no known medical conditions, a chest X-ray revealed cardiomegaly, suggesting an underlying cardiovascular condition. An echocardiographic evaluation revealed normal diastolic function and right ventricular contractility, but a moderate to severe pericardial effusion was identified. The patient was diagnosed with massive complex pericardial effusion, hypoalbuminemia, inflammatory anemia, and SLE. A multidisciplinary approach was implemented, including intravenous fluid therapy, corticosteroid therapy, gastroprotective therapy, anti-inflammatory therapy, and Video-assisted thoracoscopic surgery (VATS) pericardiotomy. Discussion: Pericardial effusion, a cardiac manifestation in SLE patients, is often caused by autoimmune-mediated inflammation of the pericardium. This inflammatory process, driven by autoantibodies and immune complexes, increases vascular permeability and facilitates fluid exudation, potentially leading to cardiac tamponade, requiring a multidisciplinary approach. Conclusion: Pericardial effusion in young SLE patients requires early recognition and multidisciplinary management, including VATS pericardiotomy. Treatment includes corticosteroids, immunosuppressive agents, and targeted antibiotics. Serial echocardiographic assessments and ongoing immunosuppressive therapy are crucial for long-term disease management.

Abstract

Background: The study aims to evaluate current evidence on diagnostic approaches, treatment strategies, and complications associated with Tuberculosis (TB), highlighting the need for further research and understanding due to nonspecific clinical presentation and limited diagnostic tools, and the potential for complications from both disease process and treatment. Methods: This systematic review complied with PRISMA 2020 guidelines and concentrated solely on full-text articles published in English from 2015 to 2025.    Editorials and review articles lacking a DOI were excluded to maintain the integrity of high-quality sources.    A literature study was performed using reputable databases such as ScienceDirect, PubMed, and SagePub to identify pertinent studies. Result: The initial database search produced more than 8000 pertinent publications on the subject.  After a thorough three-stage screening process, eight papers fulfilled the inclusion criteria and were chosen for detailed study.  Each study underwent a meticulous critical evaluation, facilitating a comprehensive grasp of the diagnosis, management, and complications of tuberculosis.  This systematic approach ensured that the analysis was based on high-quality evidence, aligned with the study's objectives, and was capable of yielding significant insights into this complex relationship. Conclusion: Treatment for tuberculosis requires accurate diagnosis and effective management to prevent complications. Advances in molecular diagnostics and emerging therapies offer hope for treatment, but barriers like limited access and late diagnosis hinder control efforts. Strengthening diagnostic capabilities and addressing socioeconomic determinants are crucial for improving patient outcomes.

Abstract

Pendahuluan: Morbus Hansen (kusta) tipe Multibasiler (MB) sering disertai dengan reaksi imunologis Erythema Nodosum Leprosum (ENL) yang dapat berulang dan menimbulkan komplikasi serius. Salah satu komplikasi neuropatik yang jarang namun berat adalah Charcot Neuropathic Osteoarthropathy (CNO), yang menyebabkan kerusakan sendi progresif. Laporan Kasus: Dilaporkan seorang pasien MH tipe MB dengan riwayat lebih dari 15 kali kekambuhan ENL. Pasien mengalami gangguan fungsi motorik dan sensorik serta manifestasi CNO pada ankle kiri dengan destruksi tulang dan malalignment sendi berdasarkan evaluasi radiologis. Diskusi: Reaksi ENL berulang memicu peradangan sistemik yang memperburuk kerusakan saraf perifer sehingga meningkatkan risiko terjadinya CNO. Penatalaksanaan meliputi terapi kortikosteroid dan methotrexate untuk mengendalikan reaksi imun serta imobilisasi ekstremitas bawah guna mencegah deformitas sendi lebih lanjut. Pendekatan multidisipliner sangat penting untuk mengoptimalkan hasil terapi. Kesimpulan: Deteksi dini dan penanganan komprehensif terhadap reaksi ENL berulang serta komplikasi neuropatik seperti CNO sangat krusial dalam mencegah kecacatan permanen pada penderita kusta MB. Kolaborasi antara spesialis kulit, saraf, dan ortopedi diperlukan untuk meningkatkan kualitas hidup pasien.

Abstract

Introduction: Kidney stones are a prevalent urological condition requiring accurate imaging for diagnosis and management. This study compares the sensitivity and specificity of three primary imaging modalities: computed tomography (CT), ultrasound, and X-ray. Methods: The study followed PRISMA 2020 guidelines, reviewing English-language publications from 2015 to 2025. Editorials, duplicate reviews from the same journal, and papers lacking a DOI were excluded. The literature search was conducted using PubMed, SagePub, Semanthic Scholar, and Google Scholar., focusing on studies that evaluated the diagnostic performance of CT, ultrasound, and X-ray in detecting kidney stones. Sensitivity and specificity data were extracted and analyzed. Results: Non-contrast CT emerged as the gold standard, demonstrating sensitivity rates between 86% and 98% and specificity approaching 100%. Ultrasound, while radiation-free, showed variable sensitivity (72%-85%) influenced by operator skill. X-ray exhibited moderate sensitivity (~78%) but was limited to detecting radiopaque stones. Combined imaging strategies, such as ultrasound followed by low-dose CT, improved diagnostic accuracy while minimizing radiation exposure. Discussion: The findings underscore the importance of selecting appropriate imaging modalities based on patient characteristics and clinical context. While CT offers superior diagnostic performance, ultrasound remains a valuable first-line tool, particularly in vulnerable populations. The integration of emerging technologies and combined imaging approaches may further enhance diagnostic capabilities. Conclusion: This analysis highlights the need for evidence-based imaging strategies in managing kidney stones, emphasizing the balance between diagnostic accuracy and patient safety. Future research should focus on optimizing imaging protocols and validating new technologies.

Abstract

Background: Yoga practices, focusing on mindfulness and meditation, have been found to positively impact cognitive function in older adults. These practices improve physical health, balance, flexibility, and cardiovascular function, while also promoting brain plasticity, neurogenesis, and stress-induced damage. The social aspect of yoga classes may also reduce isolation, thereby reducing cognitive decline. This study aims to provide a comprehensive analysis of existing research on the effects of yoga on cognitive function in older adults. Method: This systematic review was conducted in accordance with the PRISMA 2020 guidelines, focusing solely on full-text articles published in English between 2014 and 2024. To ensure the inclusion of high-quality, peer-reviewed sources, editorial pieces and review articles lacking a DOI were excluded. A comprehensive literature search was carried out across several reputable databases, including ScienceDirect, SpringerLink, PubMed, and SagePub, to identify pertinent studies that met the inclusion criteria. Result: The initial database search yielded over 1,000 relevant publications on the topic. Following a rigorous three-stage screening process, eight studies met the predefined inclusion criteria and were selected for in-depth analysis. Each study underwent a comprehensive critical evaluation, enabling a thorough examination of the relationship between psoriasis and hypertension. This systematic approach ensured that the analysis was grounded in high-quality evidence, closely aligned with the study's objectives, and capable of providing meaningful insights into this complex association. Conclusion: Yoga has been found to have beneficial effects on cognitive function in older adults, although methodological limitations and inconsistencies in intervention design need further investigation. Further research is needed to determine the true efficacy of yoga in promoting cognitive health among this age group.

Abstract

Carcinoma mammae stadium lanjut merupakan tantangan besar bagi pasien dan pasangan dalam menghadapi dampak emosional dan psikososial. Penelitian ini bertujuan untuk menganalisis peran pasangan dalam mendukung pasien dengan carcinoma mammae stadium lanjut di Rumah Sakit Ibnu Sina Makassar. Metode penelitian menggunakan desain deskriptif korelasional dengan pendekatan kuantitatif, dimana data dikumpulkan melalui kuesioner skala Likert. Sampel penelitian berjumlah 40 pasangan pasien yang memenuhi kriteria inklusi. Hasil penelitian menunjukkan adanya perbedaan signifikan antara pasien dengan carcinoma mammae stadium awal dan stadium lanjut dalam hal kesadaran pasangan, komunikasi, pengungkapan perasaan, ketanggapan pasangan, dan tingkat keintiman hubungan. Pasangan dengan pasien stadium awal menunjukkan dukungan emosional yang lebih baik dibandingkan dengan pasangan pasien stadium lanjut (p<0.05). Kesadaran pasangan dan komunikasi yang efektif berperan penting dalam menjaga kualitas hidup pasien, terutama pada tahap awal penyakit. Penelitian ini menyarankan perlunya peningkatan dukungan emosional dan edukasi untuk pasangan pasien carcinoma mammae guna mengurangi dampak psikososial yang lebih berat pada stadium lanjut.

Abstract

Introduction: Twin-Twin Transfusion Syndrome (TTTS) is a serious complication in monochorionic twin pregnancies caused by unbalanced blood flow through placental vascular anastomoses. Fetoscopic laser surgery has become the primary intervention to improve survival and reduce neurological morbidity. Methods: This review synthesizes data from randomized controlled trials, prospective and retrospective cohort studies conducted over the last decade. Studies included patients undergoing fetoscopic laser surgery with follow-up periods ranging from weeks to years, assessing survival, neurological, and developmental outcomes. Results: Survival rates after fetal laser surgery range from mid-70% to mid-80%, with one trial reporting 67-68% survival without neurodevelopmental impairment at 2 years. Cerebral palsy incidence is low (1-2%), and cognitive or motor impairments occur in 2-8% of survivors. Comparisons between surgical techniques (Solomon vs. standard laser, sequential vs. selective coagulation) show similar survival and neurodevelopmental outcomes. Immediate surgery versus expectant management in early-stage TTTS showed no significant difference in 6-month survival without severe neurologic morbidity (78% vs. 77%). Laser surgery reduces severe cerebral injury compared to no treatment (5% vs. 18%). Discussion: While fetal surgery improves survival and neurological outcomes, challenges such as preterm birth and growth discordance remain. Adjunctive therapies and long-term neurodevelopmental monitoring are essential to optimize care. Conclusion: Fetoscopic laser surgery is effective in managing TTTS, offering improved survival and reduced neurological morbidity. Continued research is needed to refine techniques and adjunctive treatments

Abstract

Introduction: Uterine cancer is a significant health concern, especially among women aged 35-65 years. Intrauterine devices (IUDs), including hormonal levonorgestrel-releasing intrauterine systems (LNG-IUS) and copper IUDs (Cu-IUD), are widely used for contraception and have been investigated for their potential role in reducing uterine cancer risk. Methods: A comprehensive review of studies published in the last decade was conducted, focusing on the association between long-term IUD use and uterine cancer risk in women aged 35-65. Studies included randomized controlled trials, prospective and retrospective cohorts, with follow-up durations ranging from six months to over ten years. Results: Evidence indicates that long-term use of both LNG-IUS and Cu-IUD is associated with a reduced risk of uterine cancer and its precursor lesions. LNG-IUS demonstrated superior efficacy compared to oral progestogens in treating atypical endometrial hyperplasia (AEH) and reducing cancer recurrence. Cu-IUD use was also linked to lower risks of endometrial and cervical cancers. Adverse events were generally mild, with weight gain being the most common side effect, and device-related complications such as expulsion and perforation were rare. Discussion: The protective effects of LNG-IUS are attributed to localized progestin release causing endometrial atrophy, while Cu-IUDs may exert effects through local inflammatory mechanisms. Despite promising findings, age-stratified data remain limited, and further long-term studies are needed. Conclusion: Long-term use of LNG-IUS and Cu-IUD offers effective contraception and a favorable reduction in uterine cancer risk with a good safety profile, supporting their role in preventive strategies for women aged 35-65.

Abstract

Introduction: Appendectomy is a common surgical procedure performed to treat acute appendicitis. This study aims to compare the clinical outcomes of laparoscopic appendectomy (LA) versus open appendectomy (OA) based on multiple studies. Methods: A comprehensive review of 40 high-quality studies, including randomized controlled trials and observational studies, was conducted. Primary outcomes analyzed were postoperative pain, hospital length of stay, complication rates, wound infection rates, and return to work time. Data on operative time and surgical techniques were also extracted. Results: The analysis showed that LA generally resulted in lower postoperative pain scores compared to OA. Hospital stay was significantly shorter in LA patients, with many studies reporting stays of 1-2 days versus 3-4 days for OA. Complication and wound infection rates were consistently lower in the LA group. Operative time varied, with some studies showing longer durations for LA, while others reported comparable times. Return to work and normal activities occurred earlier in LA patients. Discussion: The minimally invasive nature of LA contributes to reduced pain, faster recovery, and fewer infections. Although operative time may be longer in some cases, the overall benefits favor LA. Variations in study design and patient demographics were noted but did not significantly affect the overall trend. Conclusion: Laparoscopic appendectomy offers superior clinical outcomes compared to open appendectomy, including reduced pain, shorter hospital stay, and lower complication rates. It should be considered the preferred surgical approach for acute appendicitis when feasible.
