Abstract

Background: Indonesia continues to face challenges related to the circulation of substandard and counterfeit medicines. Perception plays an important role in shaping medicine-use behavior among healthcare professionals and patients. Objective: To identify factors associated with and dominant determinants of perceptions of substandard and counterfeit medicines among physicians, pharmacists, and patients in healthcare facilities in DKI Jakarta Province. Methods: A mixed-methods study with an explanatory design was conducted among 300 respondents, including 100 patients, 100 pharmacists, and 100 physicians. Quantitative samples were selected using cluster random sampling, while qualitative participants were recruited through purposive sampling. Data were collected through questionnaires and interviews and analyzed using chi-square tests and logistic regression (α<0.10). Results: The factors most strongly associated with perceptions of substandard medicines were knowledge among patients (95%), information among pharmacists (77%), and age among physicians (5%). For counterfeit medicines, the strongest associated factors were knowledge among patients (94%), gender among pharmacists (29%), and education among physicians (29%). Multivariate analysis showed that information was the dominant factor influencing perceptions of substandard medicines among patients (OR=2.000), pharmacists (OR=2.164), and physicians (OR=1.382). Perceptions of counterfeit medicines were predominantly influenced by knowledge, information, and education, respectively. Conclusion: Factors influencing perceptions of substandard and counterfeit medicines vary across respondent groups. Strengthening knowledge, education, and access to accurate information may improve awareness of substandard and counterfeit medicines.

Abstract

Background: Tumor size is critical prognostic factor in breast cancer, but evidence regarding its correlation with histological grading is still limited in the Indonesian population. Objective: The aimed of this study was to determine the correlation between tumor size and Nottingham histological grading in breast cancer patients. This study provides the first regional evidence from Lampung, Indonesia, examining the correlation between tumor size and grading using the Nottingham Grading System over 15-year retrospective period (2010–2025). Methods: A descriptive correlational study with a retrospective approach was conducted at RSUDAM, Bandar Lampung, from February to August 2025. Total of 152 breast cancer patients were selected through purposive sampling.Tumor size was categorized as <2 cm, 2–5 cm, and >5 cm based on histopathological examination. Grading was performed using the Nottingham Grading System (grades 1–3). Data were collected from medical records and analyzed using descriptive statistics and Spearman's Rank-Test (α=0.05). Results:The largest tumor size (>5 cm) was found in 44.3% of patients, and grade 3 was the most common (45%). Spearman correlation analysis revealed a statistically significant association between tumor size and histological grading (p=0.046). Among grade 3 tumors, 35.9% were >5 cm, while 75% of grade 1 tumors were 2–5 cm. Overall, a statistically significant negative correlation was observed between tumor grading and tumor size (rₛ = −0.199, p = 0.046). Conclusion: Tumor size correlated significantly with Nottingham histological grading, with larger tumors associated with higher grades, highlighting the need for early detection programs to identify breast cancer at smaller stages and lower grades.

Abstract

Background: Hospitals are vital facilities that must remain operational during disasters. However, many hospitals in Indonesia continue to face challenges in ensuring adequate preparedness in structural, non-structural, and emergency management. The Hospital Safety Index (HSI) is an important tool for assessing these preparedness levels, yet empirical evaluations of regional hospitals remain limited. Objective: To analyze the disaster preparedness level of Rajawali Citra General Hospital using the HSI framework to evaluate the resilience of healthcare facilities. Methods: A quantitative, descriptive–analytical design was employed, involving 28 respondents selected through total sampling from staff members involved in the hospital’s safety system. Data were collected using a Google Form-based questionnaire measuring the three main HSI dimensions: structural safety, non-structural safety, and emergency management. Statistical analysis was conducted using SmartPLS, including validity and reliability testing and structural model analysis to assess the contribution of each dimension to the hospital safety index. Results: Structural safety (coefficient 0.792) is the most dominant component influencing the HSI at Rajawali Citra General Hospital. In contrast, non-structural safety (coefficient–0.058) and emergency management (coefficient 0.100) demonstrate weaker effects, suggesting the need for improvements in facility conditions, operational systems, and emergency response procedures. Conclusion: Overall, the study highlights that hospital preparedness remains suboptimal. Therefore, a more holistic development of the HSI requires balancing physical, organizational, and human resource factors. These findings can serve as a reference for policymakers and hospital administrators in designing mitigation strategies to strengthen healthcare system resilience and ensure optimal hospital functionality during disasters.

Abstract

Background: Environmental factors such as household cleanliness and the availability of nutritious food, along with social factors such as positive peer interactions and parental support, play an important role in preventing feeding and swallowing disorders among kindergarten children. Objective: This study aimed to analyze the influence of environmental and social factors on the occurrence of feeding and swallowing disorders in children in Surakarta City. Methods: A cross-sectional design was employed from April to August 2025, involving 81 children selected through purposive sampling. The independent variables were environmental and social factors, while the dependent variable was feeding and swallowing disorders. Data were collected through structured interviews, observations, questionnaires, and the Dysphagia Disorder Survey (DDS) instrument. Data analysis was conducted using logistic regression with Stata 13 software. Results: The results showed that children with good environmental factors (OR = 15.15; 95% CI = 0.10–0.38; p = 0.015), positive social factors (OR = 14.57; 95% CI = 0.13 – 0.45; p = 0.016), appropriate age of the child (OR = 13.76; 95% CI = 0.11-0.41; p = 0.019), and high maternal education (OR = 8.43; 95% CI = 0.09 – 0.41; p = 0.042) showed an influence on children's ability to eat and swallow. Conclusion: Environmental and social factors play a significant role in reducing the risk of feeding and swallowing disorders in kindergarten children through family support and a healthy environment.

Abstract

Background: The processing of used delivery instruments is critical in preventing infections in both mothers and newborns. High-level disinfection (HLD) that fails to meet standards can increase infection risks, including those caused by Staphylococcus aureus, which potentially leads to serious complications such as sepsis and maternal mortality. Furthermore, the storage conditions of instruments following HLD can affect their safety for clinical use. Objective: This study aimed to determine the effectiveness of the boiling HLD method and autoclave sterilization on delivery instruments based on storage duration Methods: This comparative quantitative study utilized 42 instrument samples divided into two groups: boiling HLD (100°C for 20 minutes) and autoclave sterilization (121°C at 1 atm for 15 minutes). The instruments were stored in closed sterile containers, then swab-tested on Blood Agar Plate (BAP) media and incubated for 24 hours at 37°C. Data were analyzed using the independent t-test.. Results: Both methods showed 0 CFU on Day 1. On Day 3, microbial growth began to appear (HLD: 16.43 CFU; Autoclave: 13.14 CFU); however, there was no statistically significant difference (p>0.05). By Day 7, the boiling HLD group showed a significant surge in colonies (430.71 CFU) compared to the autoclave group (108.86 CFU), with a highly significant statistical difference (p<0.05). Conclusion: Boiling HLD is effective for sterilizing delivery instruments only if used within a storage duration of ≤3 days. For storage exceeding three days, the autoclave method is significantly more effective, and re-sterilization is recommended to ensure patient safety.

Abstract

Background: Adolescence is an important period in the formation of eating habits, yet many junior high school students still have a limited understanding of balanced nutrition and tend to adopt unhealthy eating patterns. This condition indicates the need for learning media that are more active, contextual, and suited to adolescents’ digital characteristics. Objective: To develop and evaluate an interactive health education e-module based on Problem-Based Learning to improve knowledge, attitudes, and healthy behavioral intentions related to balanced nutrition. Methods: This study employed a Research and Development approach using the ADDIE model. The participants were 60 seventh- and eighth-grade students at SMPN 2 Bajawa. The product was evaluated through expert validation, practicality testing, and effectiveness testing using a quasi-experimental non-equivalent control group design, in which the experimental group used the PBL-based e-module while the control group received conventional instruction. Data were analyzed using Aiken’s V, descriptive statistics, N-Gain, and significance tests. Results: The PBL-based interactive e-module demonstrated high validity (Aiken’s V = 0.89) and practicality (89.6%). It was also effective in improving knowledge, attitudes, and healthy behavioral intentions, with a higher average N-Gain in the experimental group (0.66) than in the control group (0.24). Significant differences in post-test scores were found between groups (p < 0.001). Conclusion: The developed interactive PBL-based e-module was proven to be valid, practical, and effective in supporting balanced nutrition education through active, contextual, and reflective learning among Indonesian junior high school students.

Abstract

Background: Patients with chronic kidney disease (CKD) undergoing hemodialysis often experience physical and psychological problems, including anxiety and depression, due to disease progression and its complications. Intradialytic virtual reality (VR) exercise is a non-pharmacological intervention that uses immersive three-dimensional technology to distract patients and potentially reduce psychological distress. Objective: To examine the effect of intradialytic VR on anxiety and depression levels in CKD patients undergoing hemodialysis. Methods: This quantitative study used a quasi-experimental pretest–posttest control group design and was conducted at PMI Hospital, Bogor, in 2025. A total of 76 respondents were recruited using purposive sampling and divided equally into intervention and control groups (38 each). Inclusion criteria were adult hemodialysis patients who were cooperative, hemodynamically stable, and willing to participate. Patients with complications or unstable conditions were excluded. The intervention group received intradialytic VR twice weekly for 4 weeks. Anxiety and depression were assessed using the Depression Anxiety Stress Scale (DASS). Data were analyzed using dependent and independent t-tests. Results: Intradialytic VR significantly reduced anxiety and depression levels in the intervention group compared with the control group (p=0.0001). The mean post-intervention difference between groups was 10.77 for anxiety and 10.03 for depression. Conclusion: Intradialytic VR is a promising nursing intervention for reducing anxiety and depression in hemodialysis patients with psychosocial problems.

Abstract

Background: Coronary artery disease (CAD) is one of the leading causes of death worldwide, with mortality rates continuing to increase each year. The severity of this condition, including the number of coronary artery occlusions, requires early identification to determine appropriate management strategies. The CHADBS2 score is a risk stratification tool commonly used in cardiovascular patients and may have potential to predict the severity of coronary artery disease. Objective: This study aimed to determine the relationship between the CHADBS2 score and the number of coronary artery occlusions Methods: This study employed a cross-sectional design with a sample of 105 respondents selected using consecutive sampling. Inclusion criteria included patients aged ≥ 18 years, post cardiac catheterization (angiography), and hemodynamically stable. Data were collected using CHADBS2 scoring sheet and analyzed using the Chi-square test. This study was conducted at Abdoel Wahab Sjahranie General Hospital, East Kalimantan Province. Results: The results showed a significant relationship between CHADBS2 score and the number of coronary artery occlusions (p=0.001)/ Respondents with high CHADBS2 scores had a 4.5 fold higher risk of multivessel occlusion compared to the low-risk group. Conclusion: CHADBS2 score can be used as a predictor of the number of coronary artery conclusions. The implication for nursing is that this score can be utilized for risk stratification, monitoring, patient education, complication prevention, and supporting interprofessional collaboration in patient management.

Abstract

Background: Diabetes Mellitus (DM) is a non-communicable disease with an increasing prevalence in Indonesia. Various management and prevention efforts, including DM exercise programs, have been implemented. To support independent exercise practice through audiovisual tutorials, appropriate exercise video media are needed. Objective: To examine the display quality and ease of following movements in DM exercise videos. Methods: This study employed a Sequential Exploratory Mixed Methods design. The qualitative phase involved five experts, consisting of one internal medicine specialist and Chair of the Indonesian Diabetes Association (PERSADIA), two exercise instructors, one sports health expert, and one media expert. The quantitative phase involved 10 participants in the PERSADIA Cimahi exercise. Data were collected through purposive sampling and guided interviews. Qualitative data were analysed using thematic analysis, while quantitative data were used to assess instrument validity, reliability, and video feasibility. Results: Thematic analysis identified two main themes: DM exercise material content and exercise video production aspects. The qualitative findings were used to develop a video assessment instrument. Instrument validation using Pearson product-moment correlation showed all items were valid (r > 0.361), while reliability testing yielded a Cronbach’s alpha of 0.919. The feasibility assessment produced a total score of 392 out of 450 (87.11%), indicating that the video was categorised as “very worthy”. Conclusion: DM exercise videos are considered very worthy for use.

Abstract

ABSTRACT Background: Pathological changes in the body due to diabetes mellitus interfere with the wound healing process. Syzygium polyanthum leaves contain abundant phenolic compounds and act as anti-inflammatory, antioxidant and antibacterial enabling them to play a crucial role in the healing of diabetic wounds. Patches are an effective drug delivery method because they are easy to control the release, easy to obtain and affordable for the general public. Objective: The purpose of study was to identify the best formulation and optimization of bay leaf patches as a potential for healing diabetic wounds. Methods: Syzygium polyanthum leaves were extracted using maceration techniques and compound identification using Gas Chromatography-Mass Spectrometry (GC-MS). The procedure for figuring out the ideal patch involved utilizing the Box-Behnken Design technique, which looked at three different things: bay leaf extract (BLE), polyvinyl pyrrolidone (PVP), and ethyl cellulose. The observed responses included air content (%), weight, and thickness. Results: The results of the GC-MS test obtained Tetradecanoic acid, 12-methyl - methyl ester, Estra-1,3,5 (10)-trien-17 ß-ol, n-Hexadecanoic acid, 9,10-Secocholesta-5,7,10(19) -triene-3,24, 25 triol, (3ß, 5Z,7E), Stigmasta-3,5-diene, Ergosta-5,22-dien-3-ol, acetate, (3ß,22E) . ANOVA analysis showed that the quadratic model was appropriate because it produced high R² values ​​and low PRESS values ​​for weight and thickness responses (p-value <0.0500). Conclusion: The optimal formulation for bay leaf patch was 117.271 mg of BLE, 396.274 mg of ethyl cellulose, and 227.237 mg of PVP. This formulation achieved the desired response of 2.683% water content, 0.462 mg weight, and 0.369 mm thickness.
