Abstract

Background: Differences in body composition across adulthood are associated with increased metabolic risk and functional decline. Although body mass index (BMI) is widely used to assess nutritional status, it has limitations in distinguishing adiposity from skeletal muscle mass. Objective: This study aimed to analyze differences in body composition according to age and sex, and to examine the associations between age and BMI, total body fat, visceral fat, and skeletal muscle mass among Indonesian adults. Methods: A cross-sectional study was conducted among 282 adults aged ≥18 years using purposive sampling. Body composition was assessed using bioelectrical impedance analysis. Statistical analyses included Spearman’s rho correlation, Kruskal–Wallis test, and ordinal logistic regression. Results: A total of 51.1% of participants were classified as obese, while 77.3% exhibited low skeletal muscle mass. Age was positively correlated with total body fat (ρ = 0.259; p < 0.001) and visceral fat (ρ = 0.267; p < 0.001), and negatively correlated with skeletal muscle mass (ρ = −0.294; p < 0.001). Middle-aged adults demonstrated the highest adiposity levels, whereas younger adults exhibited higher skeletal muscle mass. Conclusion: Older age was associated with increased adiposity and reduced skeletal muscle mass. Comprehensive body composition assessment is important to support early identification of metabolic risk and sarcopenic obesity.

Abstract

Background: Benign Prostatic Hyperplasia (BPH) is a degenerative prostate disease that causes Lower Urinary Tract Symptoms (LUTS) in older men. Demographic characteristics and clinical symptoms of LUTS  based on IPSS score, are thought to be associated with prostate volume, which is influenced by individual factors and clinical manifestations. Ultrasound is an important test for assessing prostate enlargement in BPH. Objective: This study aimed to analyze the relationship between demographic characteristics and clinical symptoms and the degree of prostate volume as determined by transabdominal ultrasound in patients with BPH. Methods: This study was an analytical observational study with a cross-sectional design, using total sampling on 71 respondents. Using both primary and secondary data. Data analysis was performed using univariate tests and bivariate tests (Fisher’s Exact test). Results: The study revealed significant associations between demographic characteristics and clinical symptoms and prostate volume. There is an association between educational level and prostate volume (p<0.001); occupation is associated with prostate volume (p<0.002); there is an association between comorbidities and prostate volume (p<0.028); and there is an association between the severity of LUTS symptoms as measured by the IPSS and prostate volume (p<0.001). Conclusion: This study found a significant association between demographic characteristics and clinical symptoms and the degree of prostate volume as determined by ultrasound. As age increases and the severity of LUTS symptoms worsens, prostate volume tends to be larger. Educational level, occupation, and comorbidities are also associated with larger prostate volume.

Abstract

Background: Dengue Haemorrhagic Fever is a health problem transmitted by the Aedes aegypti mosquito. Dengue fever control currently still uses chemical larvicides (ABATE) that can pose health and environmental risks, thus requiring natural biolarvicides that are easier to use and effective in controlling mosquito larvae. Objective: To determine the effectiveness of basil leaf tea as a larvicide in killing instar III Aedes aegypti larvae. Methods: This study used a true experimental design with Aedes aegypti instar III larvae as subjects. The treatments consisted of coarsely chopped basil tea and powdered basil tea, both at a 1% concentration. Larvae (20 per replicate) were immersed in 100 mL of the medium for 24 hours, with six replicates. The positive control used ABATE, and the negative control used clean water. Mortality data were analyzed using a one-way ANOVA test. Two treatments of 1% basil tea met the LD50 criteria (mortality≥ 50%) within 24 hours. Results: Coarsely chopped tea produced an average mortality of 60%, while powdered tea produced 55%. The positive control (ABATE) achieved 100% mortality, and the negative control (water) achieved 0%. The One Way ANOVA Post Hoc test showed no statistically significant difference between coarse chopped tea and powdered tea (Sig.= 0.673>0.05). Conclusion: Aromatic basil leaf tea (both coarse and powdered) is effective as a bioinsecticide for controlling DHF, as it can kill and damage the morphology of Aedes aegypti instar III larvae. This tea formulation has the potential to become a safer and more readily available natural larvicide alternative.

Abstract

Background: The increasing incidence of drug-resistant Mycobacterium tuberculosis (Mtb) necessitates the identification of novel molecular targets and therapeutic candidates. Decaprenylphosphoryl-β-D-ribose 2′-epimerase subunit 1 (DprE1) is a key enzyme in Mtb cell wall biosynthesis and represents a promising therapeutic target. Brazilin, a natural compound derived from Caesalpinia sappan L., has been reported to exhibit diverse biological activities; however, its interaction with DprE1 remains insufficiently explored, highlighting a gap for computational-based investigation. Objective: To evaluate the binding affinity and molecular interaction profile of brazilin against DprE1 through an in silico approach. Methods: Molecular docking was performed using the crystal structure of DprE1 (PDB ID: 5OEP) with Molegro Virtual Docker. Key parameters included an active-site grid radius of 15 Å, grid resolution of 0.30 Å, and 10 independent docking runs per ligand to ensure reproducibility. Method validation was performed via redocking of the native ligand, yielding an RMSD <2.0 Å. Binding affinity and interaction patterns of brazilin were compared with rifampicin, isoniazid, and the native ligand. Results: Brazilin exhibited a rerank score of −71.287, indicating moderate binding affinity, stronger than isoniazid but lower than rifampicin and the native ligand. Notably, brazilin consistently occupied the catalytic binding pocket and established hydrogen bonding and hydrophobic interactions with key residues, suggesting a stable and functionally relevant binding orientation. This represents the first target-specific computational evidence linking brazilin to DprE1 inhibition. Conclusion: Brazilin from Caesalpinia sappan L. shows potential as a natural DprE1 of Mycobacterium tuberculosis inhibitor, although further experimental validation is required to confirm its biological efficacy.

Abstract

Background: Continuity of care (CoC) is a key indicator that emphasizes the therapeutic relationship, service coordination, and continuity of information. Implementing EMR is expected to improve CoC, and the implementation of CoC can be influenced by aligning people, organizations, technology, and digital workloads. Objective: to analyze the impact of EMR type on continuity of care and assess the role of HOT-Fit factors and digital workload in community health centers. Methods: This study used an analytical, observational, cross-sectional design, involving 198 health workers from eight community health centers in Tasikmalaya City who use the ePus Klaster and Medisy systems. Respondents from nine professions were selected through multistage sampling. Data were collected using structured, reliable, and validated questionnaires based on the concepts of continuity of care, HOT-Fit, and cognitive workload (Cronbach’s alpha ≥0.70). The data were analyzed using bivariate and multivariate tests, including linear and logistic regression analyses. Results: Multivariate analysis shows that technology is the strongest predictor with the highest odds of increasing continuity of care across all dimensions, with results regarding informational continuity (β=0.51; p<0.001); (aOR=143.3; 95% CI=10.51–1954.2), relational continuity (β=0.53; p<0.001); (aOR=162.5; 95% CI=12.9–2035.3), and management continuity (β=0.48; p<0.001); (aOR=333.1; 95% CI=15.1–7343.8). Conclusion: Continuity of care is more influenced by technological and organizational factors than by the type of electronic medical record system itself. Therefore, strengthening technological capacity, organizational readiness, digital literacy, and the alignment of EMR systems with clinical workflows, rather than focusing solely on EMR adoption.

Abstract

Background: TB-HIV comorbidity represents a significant public health challenge due to biological interactions that elevate mortality. In Medan City, transmission is exacerbated by high population density, yet its distribution patterns remain poorly delineated. Objective: This study aimed to analyze the epidemiological data based on person, place, and time variables of TB-HIV cases in Medan City (2022–2024). Methods: This descriptive quantitative observational study utilized secondary data from all reporting healthcare facilities (Primary Health Centers, hospitals, and lung clinics) in Medan City. Data were obtained via the SITB 03 SO form (the Tuberculosis Information System’s Drug-Sensitive Tuberculosis Patient Register) from the North Sumatra Provincial Health Office, using a total sampling technique (820 cases). Microsoft Excel's pivot table features were employed for data cleaning and variable categorization, followed by descriptive analysis, calculating frequency, proportions, and annual trends. Results: Treated cases increased (from 177 cases in 2022 to 322 cases in 2023) and stabilized in 2024 (321 cases). Comorbidity was predominantly found in males and the productive age group of 18–59 years. Geographically, Medan Marelan District recorded the highest accumulation of cases. Conclusion: Case dynamics reflect strengthened SITB (Tuberculosis Information System) surveillance. These findings serve as a foundation for mapping intervention areas. The Medan City Health Office is advised to expand bidirectional screening into private facilities to capture unrecorded cases, and to optimize digital treatment monitoring alongside cadreled assistance to mitigate loss to follow-up among productive age male patients.
