Abstract

There are numerous factors that might affect social restrictions regulation on the reduction of the number of Covid-19 cases in 27 cities and districts in West Java Province. This study aims to identify the effect of social restrictions and economic variables on Covid-19 pandemic in West Java Province. The investigation uses several data sourced from the Covid-19 Information & Coordination Center of West Java Province, and the Central Bureau of Statistics in West Java Province. The dependent variable in this study is the average increase in daily Covid-19 cases. Meanwhile, the independent variable in this study are dummy of social restrictions, population density, per capita GRDP, education level, mobility level, unemployment rate, number of health workers, percentage of the poor population, and life expectancy. All the information gathered in the period between March 2, 2020 and March 2, 2021, which consists of 17 periods based on the Decree of the Governor of West Java. A panel data analysis with the Feasible Generalized Least Square (FGLS) approach is applied using Stata MP-64 software. The results show that social restrictions, population density, unemployment rate, and the number of health workers have a positive and significant effect against the reduction of the Covid-19 cases in West Java Province. Per capita GRDP shows a positive effect against the reduction of the Covid-19 cases, however it is not significant. Level of education, level of mobility, percentage of the poor population, and life expectancy have a negative and significant effect against the reduction of the Covid-19 cases in West Java Province. Therefore, social restrictions are the right policies to reduce the Covid-19 pandemic in West Java Province.

Abstract

Capitation is a payment method to primary health care providers. The application of capitation without considering risk adjustment could have a negative impact. There are several models of risk adjustment capitation payments. This study aims to review the development of the capitation payment system with risk adjustment for primary care in several countries. This study used the PRISMA method and got the articles from three online databases, namely Pubmed, EBSCOhost Medline, and Scopus. A total of 212 articles were collected, and 7 articles meet the criteria for further review. The review focuses on the risk-adjusted capitation payment system model. The results obtained that the risk-adjusted capitation payment system model is vary from simple to complex. The most frequently used risk factors were age and gender. The risk-adjusted capitation payment system model continues to develop, it is important to carry out periodic updates and adjustments. Various benefit from the implementation of the risk-adjusted capitation payment system model can be a reference for countries to get an appropriate risk-adjusted capitation policy formulation to be applied which proper to the conditions of each country

Abstract

The aims of this research are first, to explore the factors that are associated with health status. Second, to address the individual characteristics and household conditions that affect the health status of labor force, taking into account the urban-rural differences. This research utilized data from SUSENAS Cor 2018, and employed a cross sectional - multinomial logistic regression analysis method. The dependent variable is the health conditions of the labor force. The findings show that, age and number of household members were significant for both sick and very sick health status in all two type of areas analyzed. In urban and rural areas simultaneously gender, expenditure per capita, and drinking water were only significant for sick health group while marital status, education, employment status, sanitation, house area was only significant for very sick health status group. In urban areas, the results indicated that gender, employment status, and drinking water were only significant for sick health status, while marital status, education, sanitation, and house area significant for very sick health status. Moreover, in rural areas, marital status was only significant for sick health status whereas, sanitation and house area were significant for the very sick health status.

Abstract

Stroke is one of the catastrophic diseases which is the most cause of death in Indonesia. Probolinggo is the second highest prevalence city of stroke in East Java. The prevalence and mortality of stroke have increased in RSUD dr. Mohamad Saleh Probolinggo. Stroke is a high-cost and life-threatening disease, so that the propitious financial management is needed to ensure the effectiveness and efficiency of this services. Therefore, it is necessary to do a study of cost of treatment that measures the cost of stroke treatment. The purpose of this study was to describe cost of treatment of stroke patients as BPJS participants in RSUD dr. Mohamad Saleh Probolinggo. This study was partial economic evaluation that aim to describe a cost of an object without comparing with other intervention. It was carried out at RSUD dr. Mohamad Saleh Probolinggo. Sample of this study were stroke patients as BPJS participants in RSUD dr. Mohamad Saleh Probolinggo of 2018 who fulfilled the inclusion and exclusion criteria which is consisted of 40 respondents. The results show that average of direct cost is IDR. 8,278,584. Average of indirect costs is IDR. 974,383. While average of cost of treatment is IDR. 9,252,967. The recommendation is to increase the good recording of nursing actions and do corrective actions. Implementing the monitoring and evaluation to carry out the cost containment.

Abstract

The mechanism for procuring goods and services in handling emergencies is different from regular/ordinary conditions. The main difference is in the stage of the procurement implementation. Today's problems are the enormous price disparity, the increasing availability and demand, especially for personal protective equipment (PPE). The disparity has resulted in more significant expenses in hospital spending, this condition impact hospital finances. This research is an observational study. The method is qualitative with interviews and data collection. The data taken are primary data and secondary data. Informants consist of main informants and triangulation informants. The data collection instrument was an in-depth interview guide. From the research results obtained: before the COVID-19 pandemic, two methods of implementing procurement (E-Catalogue and Direct Procurement) while procurement during the pandemic used three methods (E-Catalogue, Direct Procurement and Direct Appointment). The procurement process is following the established rules. The procurement process that is carried out has complied with the procurement principles and ethics. There has been a change in the General Procurement Plan for 2020 due to the pandemic. The realization of revenue for Dr Kariadi Hospital in 2020 exceeded the predetermined target, so it can be said that the financial condition of Dr Kariadi Hospital during the COVID-19 pandemic was considered healthy. This study concludes that the procurement process in the COVID-19 pandemic condition follows the principles, ethics and regulations that have been set, as well as the financial condition of Dr Kariadi during the COVID-19 pandemic, was considered healthy.

Abstract

Hospital visit volumes decline to 50% during the COVID-19 pandemic, which has reduced hospital revenue. The burden of hospitals handling COVID-19 patients is getting higher due to the late payment of COVID-19 claims. This study aims to review COVID-19 payment regulation in hospitals, constraints of policy implementation, and hospital financial challenges in Indonesia. This is a descriptive-analytic study, data collection is carried out through literature searches from scientific journals, BPJS Kesehatan, the Ministry of Health, Hospital Associations, and scientific seminars in online mass media. BPJS Kesehatan data as of 28 January 2020 shows, number of COVID-19 claims was 433,077 and from verification result found 266,737 (61.59%) eligible claims equivalent to 17.3 trillion, 165,189 dispute claims (38.14%) at 9.7 trillion, and 1,151 claim were not suitable (0.27%). Up to December 2020, the total claim submitted was Rp. 22.9 trillion, payment delivered to the hospitals was Rp. 14.5 trillion (63,3%) and the remaining unpaid was Rp. 8.4 trillion (36,6%). The challenges on COVID-19 claims include; claim payment was not made on time, a high number of dispute claims due to perceptual differences on the claims regulation, incomplete claim documents requirement, unpreparedness of dispute instrument tools, and a limited number of verifiers from the Ministry of Health to re-verify. The decrease in cash flow resulted in hospital operational obstacles, including; inability to pay obligations to drug and medical device vendors, and also payment of salaries for health workers and hospital staff.

Abstract

The Governor of West Sumatra Province of Indonesia imposed a mandatory social distancing (PSBB) to slow down the Covid-19 outbreak from 22 April to 7 June 2020. The cons argued that PSBB was ineffective because of the impossibility to limit people’s movements. The pros, on the other hand, viewed PSBB as a training facility to increase people’s awareness about the pandemic. Our research aims at evaluating the effectiveness of PSBB by using the Kermack-McKendrick SIR pandemic model and the NLSUR non-linear least square estimation. Using daily data published by the Covid-19 task force, we found that PSBB had a positive impact on flattening the curve. Assuming that vulnerable people were around 30% of the population, PSBB has reduced the rate of spread of Covid-19 from around 2 persons to less than 1 person for every infected. We also found that the economic consequences of PSBB on commodities varied by their demand and supply characteristics. We, therefore, suggest that policy interventions related to price control and subsidy should consider these characteristics. For future research, as data more available, the effect of PSBB on broader economic variables such as poverty and growth in the province needs to be examined.

Abstract

Increasing the utilization of maternal health services is an important target for the Indonesian government to increase the achievement of the SDGs targets. Funds budgeted for the health sector are at least 5% of the APBN and 10% of APBD excluding salaries. This is smaller than countries in the low-income category, while WHO shows that an increase in the allocation of health funds has a positive relationship with the degree of public health. Therefore, this study aims to determine whether there is a relationship between the realization of health fund allocation and the utilization of maternal health services in Indonesia. Data source from the 2017 IDHS and provincial government financial statistics. The dependent variables used were the ANC visits, delivery in the public sector, and PNC within two months after delivery. The main independent variable is the realization of health funds and socio-economic characteristics as control variables. With binary logistic regression analysis, the results of this study indicate that the higher the realization of health funds allocation, the more likely the mother is to use maternal health services at the antenatal care and postnatal care stages but reduces the possibility of utilizing childbirth in the public sector.

Abstract

Program Keluarga Harapan (PKH) aims to improve the standard of living of the people as measured through the Human Development Index (IPM) and the level of inequality in expenditure of the Indonesian population (Gini Ratio) by accommodating the utilization of health services as one of the indicator. In 2018 there are still 16% of deliveries that were not performed by trained health workers at healthcare facilities. One health component that is required as a PKH Beneficiary Family (KPM) is that pregnant women are required to deliver in a health care facility. The purpose of this study is to look at the effect of PKH on the use of health facilities for childbirth in Indonesia.The Methods that used in this study is a quasi-experimental with cross sectional design using Susenas data in 2018 with a total sample of 28,785 mothers aged 15-49 years who had given birth to live births in the period of two years before the survey and economic status in deciles 1-3. The analysis uses the Propensity Score Matching (PSM) method with the Logit model that looks at the OR value. The Results is PKH increased maternity utilization in health facilities. PKH recipients have a 1.23 times higher chance than non-PKH recipients, after controlling for other variables. PKH implementation has quite good benefits, along with other dominant supporting factors, namely the area of residence, education, communication tools, transportation facilities and JKN-PBI in order to help the community, especially the poor and vulnerable, to utilize health care facilities for delivery.

Abstract

This paper contributes to the empirical evidence of ex ante moral hazard on the National Health Insurance System (NHIS) in Indonesia. We find that JKN participation has a negative relationship with physical activity and positive relationship with smoking spending. Furthermore, by calculating marginal effect we obtained that JKN participation reduce the probability of physical activity by 2.7% and also reduce the intensity of individual physical activity. For the smoker, JKN Participation is expected increase in their spending for smoking. The result indicates that there is an evidence of ex ante moral hazard since there is a positive relationship between JKN participation and risky health behaviors such as physical activity and smoking behavior.
