Abstract

Background: High blood pressure, obesity, and elevated cholesterol are significant risk factors for cardiovascular diseases, making their interrelationship an important area of research. Purpose: To investigate the correlation between blood pressure, body mass index (BMI), and total cholesterol levels in adults residing. Method: Using a cross-sectional approach, data were collected from a sample of adults aged 25–60 years. Blood pressure was measured using a digital sphygmomanometer, BMI was calculated based on height and weight measurements, and total cholesterol levels were assessed through laboratory testing. Results: The findings indicate a positive correlation between BMI and blood pressure, suggesting that individuals with higher BMI are more likely to experience elevated blood pressure. Similarly, a significant relationship was observed between total cholesterol levels and both systolic and diastolic blood pressure. These results underscore the need for integrated health interventions targeting weight management and cholesterol control to reduce the risk of hypertension and associated health complications in the community. Conclusion: The study highlights the importance of routine health monitoring and lifestyle modifications in promoting cardiovascular health among adults.

Abstract

Background: A lot of research has been done on the treatment of palliative patients who come to the emergency department. Although there are several models, it is still difficult to integrate palliative care into the ED. Purpose: To determine how emergency care for palliative patients is implemented in the emergency department.   Method: The PCC framework (Population: palliative patients, Concept: emergency care, Context: emergency department) was used to generate the article questions for this literature review. A search for articles was conducted during November and December, 2024. Pubmed, the ScientDirect database and other resources were used as data sources. “Palliative care" OR "palliative medicine" OR "hospice and palliative care nursing" AND "emergency treatment" OR "emergency medical services" AND "emergency room" OR "ER" were used as keywords. Results: Emergency care for palliative patients in emergency room has been implemented with a focus on symptom management and reducing the patient's suffering, maintaining patient comfort and involving families in decision making, with effective communication being essential. However, there are still serious barriers and challenges such as lack of staff education, problems with current emergency department systems and processes. Conclusion: The knowledge and skills of nurses and other health workers need to be improved and more in-depth studies need to be carried out so the implementation of palliative patient care in emergency cases can be optimized.

Abstract

Background: Post-inflammatory hyperpigmentation (PIH) poses a considerable dermatological concern, affecting individuals of various ethnic backgrounds, with a notably higher prevalence among those with darker skin tones. The intricate pathophysiology of PIH, which involves inflammatory mediators and melanogenic pathways, necessitates a multifaceted treatment approach to achieve the best possible clinical results. Purpose: This systematic review evaluates the effectiveness, safety, and clinical outcomes of various combination therapies for managing post-inflammatory hyperpigmentation (PIH), with a focus on their synergistic effects and suitability for diverse skin types. Method: A systematic review with article selection guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The systematic review process begins with the formulation of clinical questions relevant to the topic. Before this, the authors establish PICOS criteria, which stand for: P (problem, patient, or population), I (intervention, prognostic factor, or exposure), C (comparison or control), O (outcome), and S (study design). For this article, P: Individuals of any age and skin type diagnosed with post-inflammatory hyperpigmentation (PIH), I: Any combination therapy involving two or more active ingredients or treatment methods, C: Single therapy, alternative combination therapies, or placebo, O: Clinical improvement in PIH, patient satisfaction, adverse effects, and treatment duration, S: Randomized controlled trials. The article search was conducted systematically using the keywords: “post-inflammatory hyperpigmentation” AND “combination therapy” AND “hyperpigmentation treatment” AND “topical therapy” AND “skin pigmentation” AND “melanin” AND “tyrosinase inhibitors” AND “retinoids” AND “kojic acid” AND “hydroquinone” AND “tranexamic acid” AND “niacinamide” AND “Boolean operators”. Results: The review of eight studies found that combination therapies were more effective than monotherapy for treating post-inflammatory hyperpigmentation (PIH). Dual treatments, such as glycolic acid cream with peeling and TCA with PRP, showed notable improvements. Combining chemical peels with topical agents was especially effective for stubborn cases. Success depended on proper patient selection, personalized plans, and timing. Overall, targeting multiple pigmentation pathways improved outcomes and was safe, particularly for darker skin types (Fitzpatrick IV–VI). Conclusion: Combination therapy is the most effective approach for treating PIH, as it utilizes synergistic treatment methods tailored to individual patient factors like skin type and PIH severity.

Abstract

Background: Individualized care in intensive care units (ICUs) refers to holistic nursing care provided to critically ill patients, which takes into account their clinical conditions, personal life circumstances, and encourages their active involvement in the decision-making process.  Purpose: To synthesize the impact of individualized care on the rehabilitation outcomes of critically ill patients. Method: A scoping review approach was employed. The literature search was conducted using PubMed, ScienceDirect, and EBSCO databases, utilizing a keyword strategy with Boolean operators "OR" and "AND" combining the terms: Individualized care and Rehabilitation. Results: The review revealed cross-country differences in nurses’ perceptions of individualized care. Nurses from Nordic countries and the United States generally rated the Individualized Care Scale–Nurse (ICS-Nurse) variables more highly, whereas nurses from Mediterranean and Southern European countries tended to provide lower ratings. Conclusion: Overall, nurses are committed to supporting patient individuality through comprehensive nursing practices, although certain barriers persist. It is recommended that nurses more consistently integrate patients’ personal life contexts into their care to enhance individualized care delivery.

Abstract

Background: Diabetic foot ulcers are a serious complication that increases morbidity and amputation risk in patients with diabetes mellitus. Self-care is an important aspect of diabetes management that includes various health behaviours, such as diet, exercise, blood sugar monitoring, medication adherence and foot care. Purpose: To measures the relationship between the level of self-care of patients with diabetes mellitus (DM) and the risk of diabetic foot ulcers. Method: This study uses Analytical research methods using a Crossectional study approach. The sample size in this study used total sampling, from January to early March 2025, totalling 66 respondents. Data analysis to find the relationship between variables using non-parametric correlation test analysis, namely Spearman Correlation with a meaning level of 95% (0.05) because the data is ordinal scale. Results: This study found that based on statistical tests there was a correlation between the level of self-care and the risk of diabetic foot ulcers with a p value of 0.000 <0.05. Spearman's correlation value of 0.521 shows a positive correlation with a fairly strong correlation strength. Conclusion: The better the level of self-care of type 2 diabetes mellitus patients, the lower their risk of developing diabetic foot ulcers.

Abstract

Background: One of the main efforts in promoting sustainability is the implementation of ISO 14001, an international standard for Environmental Management Systems (EMS). In addition to helping companies comply with environmental regulations and reduce the risk of legal sanctions, ISO 14001 also contributes to improving operational efficiency, reducing costs, and achieving business sustainability. Effective implementation not only supports environmental preservation but also creates healthier and more productive working conditions. Purpose: To analyze the implementation of the ISO 14001 environmental management system in a concrete production plant. Method: Descriptive observational research with a qualitative approach using a cross-sectional design. This research was conducted at a concrete production company in Demak City, namely PT Varia Usaha Beton Plant Sayung, which has been operating since 1991 and employs 59 workers. The researcher determined the main informant as the subject of this research, namely the manager who is directly responsible for the implementation of ISO 14001 and workers from the quality control section, while for the triangulation informant the researcher chose the Factory Head. ISO 14001 contains 10 clauses, but the researcher only used six clauses with a checklist to assess the implementation of ISO 14001, including organizational context, leadership, planning, support, operations, performance evaluation, and continuous improvement. Results: The implementation of ISO 14001 is in accordance with the standard of 87.2%. Around 71.1% of respondents have a smoking habit which is a major concern in the context of occupational health. This habit can increase the prevalence of health problems such as shortness of breath and flu/prolonged cough observed in some respondents. As many as 20.3% of respondents experienced shortness of breath while working, and 76.2% experienced flu or cough for more than a week. These figures are quite high and may indicate exposure to dust, air pollution, or poor ventilation in the workplace. Most respondents had no history of illness (83.1%), but the high prevalence of respiratory symptoms indicates the need for an environmental evaluation in the workplace. The results of dust measurements also showed that there were areas where the threshold value was exceeded. Conclusion: Although the company has taken positive steps in implementing an environmental management system, controlling dust and enhancing protection for workers remains a critical focus to prevent occupational diseases that are still occurring. Suggestion: To improve the effectiveness of the Environmental Management System (EMS), organizations need to strengthen operational control and communication with external suppliers to ensure that all processes and business partners meet the established environmental standards. In addition, emergency preparedness should be improved through regular evaluation of emergency response procedures, so that responses to critical situations can be faster and more effective.

Abstract

Background: Hypertension is one of the non-communicable diseases that is a concern throughout the world, including Indonesia. Hypertension occurs frequently in menopausal women, because at menopause there is a decrease in estrogen hormones, so there is a risk of suffering from hypertension. One of the non-pharmacological therapies to control blood pressure in menopausal women is aerobic exercise. Sejati intervention is aerobic exercise that can cause a decrease in heart rate, which will reduce cardiac output, which ultimately causes a decrease in blood pressure. Purpose: To see the effectiveness of Sejati intervention on blood pressure in menopausal women. Method: The research design used was a quasy experiment, namely with a pre-test and post-test research design with a control group. The number of samples in this study was 36 respondents with 18 respondents in the intervention group and 18 respondents in the control group. The inclusion criteria in this study were menopausal women with hypertension who have blood pressure ≥140/90 mmHg and the exclusion criteria of this study were respondents with comorbidities. The statistical analysis used in this study was a linear regression test. Results: There was the effect of sejati intervention on blood pressure in menopausal women with hypertension at the Bojonggambir Health Center, Tasikmalaya Regency with a p value of 0.000 < α 0.05. Conclusion: The sejati intervention has an effect on reducing blood pressure in menopausal women. Suggestion: This research can be used as a reference material for further research.

Abstract

Background: Standard precautions practice is the main action in preventing and controlling infections in hospitals. Nurse compliance with standard precautions is influenced by various factors, including knowledge, perception, and motivation. Purpose: To analyze the relationship between the level of knowledge, perception, and motivation of nurses with the practice of standard precautions. Method: A quantitative approach, specially focusing on analytical survey and cross-sectional method. The study took place from December 2024-Febuary 2025 in Islamic Hospital of Muhammadiyah Medical Care Center, Pekajangan. The independent variable was knowledge, risk perception, and motivation of nurses, dependent variable is standard precautionary practices in hospitals. Using systematic random sampling and slovin formula, 173 respondents were selected, who met specific inclusion and exclusion criteria. The analysis proceeded with a univariate test to determine the frequency distribution of respondent’s characteristics, followed by bivariate analysis using spearman rank test Results: The study revealed significant factors influencing the practice of standard precautions. It showed a gender-based difference in adherence (p = 0.008), a positive relationship between the level of perception and practice (p = 0.033), and a significant association with motivation level (p = 0.01). Conclusion: Nurses generally had good to fair levels of knowledge, perception and motivation regarding standard precautions, indicating effective implementation of patient safety. Perception and motivation significantly influenced compliance, while knowledge without significant influence.

Abstract

Background: A common and complicated side effect of diabetes is diabetic foot ulcers (DFUs), which frequently lead to chronic wounds that are challenging to heal. Another technique that has gained interest is low-level laser therapy (LLLT), which speeds up the healing of diabetic foot ulcers. The purpose of this systematic review is to evaluate how well LLLT aids in the healing of diabetic foot ulcers that are chronic. Purpose: To assess the effects of low-level laser therapy on the healing of diabetic foot ulcers, particularly with regard to ulcer size reduction, pain relief, and overall wound healing. Method: A comprehensive search for English-language full-text studies published between 2015 and 2024 was conducted across a number of databases, including PubMed, ScienceDirect, and Google Scholar. Randomized controlled trials that assessed the impact of LLLT on diabetic foot ulcers are required for inclusion. Measurements of wound size, ulcer healing progress, and pain reduction were used to analyze the results. Results: The analysis includes seven studies that consistently showed benefits from using LLLT. Significant decreases in ulcer size, better tissue repair, and when comparing the treated groups to the control groups, pain relief was seen. Although different laser parameters, such as wavelengths and dosages, were reported in the research, all of them shown positive outcomes in terms of speeding up wound healing and lowering pain levels related to diabetic foot ulcers. Conclusion: Low-level laser therapy is a useful supplementary technique for promoting the healing of diabetic foot ulcers. Patients with chronic diabetic foot ulcers benefit from LLLT by experiencing less pain, faster wound healing, and an overall higher quality of life. To ascertain the optimal laser parameters and assess the long-term efficacy of LLLT in the management of diabetic wounds, more extensive research is necessary.

Abstract

Background: Patients in intensive care units (ICUs) with tracheostomies are particularly susceptible to nosocomial infections, notably ventilator-associated pneumonia (VAP) and lower respiratory tract infections. A key contributing factor is the presence of self-care deficits, as these patients are generally unable to maintain airway hygiene or perform tracheostomy wound care independently. These deficits are compounded by healthcare worker workload, constrained ICU resources, and the absence of standardized care protocols. A comprehensive synthesis of existing evidence is therefore necessary to clarify the relationship between self-care deficits and infection risk. Purpose: To explore and analyze the association between self-care deficits and infection incidence in ICU patients with tracheostomy, with a focus on identifying key risk factors, implementation challenges, and effective infection prevention strategies. Method: A systematic literature search was conducted using databases including ProQuest, JSTOR, BMC, and Google Scholar, covering studies published between January 2015 and December 2024. Of 1,157 articles initially identified, 35 were selected for full-text review based on predefined inclusion and exclusion criteria. Quality appraisal was conducted using the Joanna Briggs Institute (JBI) framework, yielding 11 articles for final inclusion. Results: Self-care deficits substantially contribute to nosocomial infection risk in ICU patients with tracheostomies. Identified risk factors include ineffective secretion management, poor oral hygiene, and improper suctioning techniques. Inadequate education for both patients and caregivers following ICU discharge further exacerbates this risk. Additional barriers include limited medical equipment availability, insufficient training in tracheostomy wound care, and healthcare provider burden. Furthermore, there is a lack of research addressing the long-term consequences of self-care deficits and the effectiveness of patient-centered educational interventions. Conclusion: Self-care deficits significantly elevate the risk of infection among ICU patients with tracheostomies, primarily due to impaired hygiene and secretion management. Contributing factors include insufficient resources, absence of standardized protocols, and limited educational support for patients and caregivers. Although nursing interventions have demonstrated potential in mitigating these risks, their implementation remains inconsistent. Further research is warranted to assess the long-term effectiveness of targeted, patient-centered self-care education strategies.
