Abstract

Patient loyalty is a key indicator of a hospital's management success in maintaining its service ecosystem and competitive advantage. In the digital era, patient loyalty is influenced not only by clinical service quality but also by the patient experience, hospital reputation, and the hospital's ability to provide easy, fast, and responsive digital interactions. This study aims to analyze the factors influencing patient loyalty, focusing on the roles of service quality, patient experience, and hospital reputation through a Systematic Literature Review (SLR) approach. A literature search was conducted on scientific articles published between 2021 and 2026 using relevant databases and journal sources. Articles were selected based on inclusion and exclusion criteria and subsequently analyzed systematically to identify patterns of relationships between variables. The synthesis results indicate that service quality is a fundamental determinant of patient loyalty, as reliable, responsive, safe, and patient-centric care enhances satisfaction and the likelihood of repeat visits. Patient experience is a crucial factor, as loyalty is shaped by the overall experience throughout the patient journey. Hospital reputation also plays a role in building trust and reinforcing patient preferences. Furthermore, digitalization expands patient-hospital interactions through websites, applications, social media, online registration, and telemedicine. The synthesis reveals that satisfaction and trust potentially act as mediators in the relationships between service quality, patient experience, reputation, and loyalty. Consequently, enhancing patient loyalty requires an integrated strategy that combines service quality, patient experience, institutional reputation, digital transformation, satisfaction, and trust.

Abstract

The sustainability of Arabica coffee farming in Karo Regency is confronted with yield volatility arising from technical, biological, and environmental factors. This study aims to identify sources of production risk using a quantitative Z-Score approach and to analyze the level of relative risk using the Coefficient of Variation (CV) and standard deviation. This mixed-methods study was conducted in Simpang Empat District and involved 200 coffee farmers (100 PT Olam partner farmers and 100 traditional non-partner farmers). The results show that, at the aggregate level, the relative risk of Arabica coffee farming in Karo Regency is low and stable, with an average CV of 8.85% and an operational loss probability of only 14.12%. Based on the Z-Score analysis of 25 risk indicators, a deconstruction of the conventional risk paradigm was identified, whereby the main risk sources categorized as occurring very frequently were dominated by downstream market requirements. These included strict requirements for packaged products, with an occurrence probability of 85%, and the requirement for products to have trademarks, also with an occurrence probability of 85%. In contrast, upstream risks such as declines in selling prices (11%) and pest and disease attacks (11%–12%) had the lowest probabilities and were therefore categorized as very rare. The study concludes that although farming activities generally remain within a safe zone, farmers need to strengthen their adaptive capacity to modern market regulations in the downstream sector.

Abstract

The quality of dental and oral health services is a crucial component in enhancing hospital service quality, as it directly impacts patient experience, trust, and satisfaction. Effective quality management is essential to ensure that services are safe, effective, timely, patient-oriented, and aligned with professional standards. This study aims to analyze and synthesize scientific evidence regarding the implementation of quality management in dental and oral health services and its impact on increasing patient satisfaction in hospitals. A Systematic Literature Review (SLR) design utilizing the PRISMA approach was employed. Literature was obtained through searches of scientific databases using keywords related to dental and oral health services, quality management, service quality, patient satisfaction, and hospitals. Selected articles consisted of quantitative studies addressing dental and oral health service quality and patient satisfaction, published within the researcher-defined timeframe, and available in full text. The selection process was based on inclusion and exclusion criteria; subsequently, data were extracted, assessed for methodological quality, and narratively synthesized. The synthesis indicates that improvements in dental and oral health service quality management are associated with increased patient satisfaction. Consistently contributing factors include service reliability, healthcare worker responsiveness, professional assurance and competence, empathy, effective communication, facility availability, environmental cleanliness, waiting times, and ease of access. Key strategies for enhancing patient experience include implementing quality standards, conducting periodic satisfaction evaluations, strengthening healthcare worker competence, and pursuing continuous improvement. In conclusion, integrated and patient-oriented quality management plays a vital role in boosting patient satisfaction with hospital-based dental and oral health services. Strengthening quality evaluation systems and continuous quality improvement efforts is essential to ensure the delivery of safe, effective, responsive, and patient-centered care.

Abstract

This study evaluates the commercial feasibility of an integrated shredder and 3D printer (ECO-C3DP) machine for the MSME segment through the integration of eco-modular architecture and Techno-Economic Analysis (TEA) based on stochastic Monte Carlo simulation. The technical evaluation results indicate that the machine architecture is divided into four functional modules with a Modularity for End-of-Life (MEOL) score of 0.6269, placing the product in the ideal Region A quadrant characterized by ease of disassembly and maintenance efficiency. Deterministically, an initial investment of IDR 800,759,131 with a recommended selling price of IDR 65,818,471/unit is proven to be highly feasible, generating an NPV of IDR 908,669,032, an IRR of 45.76%, a PBP of 2.61 years, and a BCR of 1.40. Under stochastic market conditions across 1,000 iterations, the capital return certainty (NPV≥0) reaches 76.9% under the triangular distribution and 66.6% under the uniform distribution, with a minimum sales tipping point of ≥13"-" 14" units/year" . Based on Government Regulation (PP) No. 7 of 2021, this business unit operates in the Micro Enterprise category with a 27.2%–32.2% transition probability of upgrading to a Small Enterprise. The commercialization strategy is directed through a Business-to-Government-to-Community (B2G2C) partnership using an Estate Model scheme based on the LKPP E-Catalog listing and TKDN certification.
