Abstract

Background: Polycystic Ovary Syndrome (PCOS) is the most common polygenic endocrine disorder in women of reproductive age. PCOS is characterized by symptoms of excess androgen hormones, ovulation disorders, and obesity. The polyphenol content in Camellia sinensis extract has been reported to be beneficial in lowering testosterone levels and stabilizing luteinizing hormone production, thereby improving the ovulation process in PCOS patients. Objective : Identify studies that use clinical trials and studies that evaluate Camellia sinensis extract in animal models of PCOS Research Methods/ Implementation Methods : A comprehensive search was conducted on five databases, including SCOPUS, Science Direct, PubMed, ProQuest, and EBSCO, published from 2015-2025 met inclusion criteria. Risk of bias was assessed using SYRCLE’s. Data were analyzed using RevMan 5.4 software Results: Three studies were included in the meta-analysis, which involved 74 female rats. Compared to the untreated PCOS rat group, managing of Camellia sinensis polyphenols to PCOS rats had a significant effect in reducing testosterone levels (MD = -7.17, 95% CI: -9.96 to -4.39, P < 0.00001, I² = 59%), luteinizing hormone (LH) levels (MD = -4.57, 95% CI: -4.67 to -4.46, P < 0.00001, I² = 15%), and body weight (MD = -40.31, 95% CI: -50.62 to -30.00, P < 0.00001, I² = 0%) indicating high consistency of treatment effects across all included studies. Conclusion/Lesson Learned : Camellia sinensis polyphenols significantly reduced testosterone, LH, and body weight levels in PCOS mouse models, demonstrating their potential as an effective alternative therapy for polycystic ovary syndrome management.

Abstract

Background : The use of polluted river water for household purposes is still frequently found in various regions of Indonesia. Objective : This narrative review aims to explore community perceptions regarding the meaning of health and sickness in relation to the practice of using river wate Research Methods/ Implementation Methods : The literature review indicates that communities develop unique perceptions of water quality based on sensory indicators (color, smell) and direct experience, rather than microbiological parameters. Results : The concept of 'sickness' tends to be defined functionally as a condition that disrupts daily activities, while mild illnesses like diarrhea and skin rashes are often normalized. Socio-cultural, economic factors, and limited knowledge shape these perceptions and create a cycle of dependency on river water. Conclusion/Lesson Learned : It is concluded that there is a gap between scientific health definitions and community perceptions, therefore public health interventions need to consider participatory approaches that understand the construction of health-sickness meanings at the community level.

Abstract

Background : Digital transformation is a key strategy in modernizing the health sector globally and nationally. In Indonesia, the implementation of health information systems such as E-Puskesmas aims to improve work efficiency, clinical data accuracy, service effectiveness at First-Level Health Facilities. Objective : This study analyzes the performance indicators of health services at UPTD Puskesmas Loa Bakung after the implementation of the E-Puskesmas system, focusing on service time efficiency, curative workload, and the achievement of promotive–preventive programs. Research Methods/ Implementation Methods : A descriptive approach was used based on secondary data from E-Puskesmas operational reports from January to September 2025. Data were obtained from individual service unit (UKP) reports, monthly BPJS Health reports, and public health program records. The analysis focused on operational efficiency, disease burden profiles, and priority program achievements. Results : Findings reveal a gap between digitalization goals and operational reality. Only 50.1% of patients were served within less than 10 minutes, indicating workflow bottlenecks. Essential Hypertension (2,985 cases), Acute Respiratory Infections (1,300 cases) dominated curative services, while screening achievements for Hypertension (16.43%) and Diabetes (4.10%) remained below targets. This reflects a reactive service orientation with limited promotive and preventive efforts. Conclusion/Lesson Learned : : E-Puskesmas has improved data recording accuracy but has not fully enhanced systemic performance. Optimization is needed through better network infrastructure, improved digital competence among health workers, and data-driven decision-making. Strategic focus should emphasize strengthening promotive and preventive programs as the core of primary health digital transformation.

Abstract

Background: Periodontitis is a chronic inflammatory disease affecting the supporting tissues of the teeth and is increasingly recognized for its systemic implications. Chronic periodontal inflammation contributes to persistent immune activation, endothelial dysfunction, insulin resistance, and dyslipidemic profiles, thereby elevating the risk of cardiovascular and metabolic disorders. Objective : This narrative review aims to summarize current evidence regarding risk factors of periodontitis among patients with systemic comorbidities, particularly those enrolled in chronic disease management programs. Research Methods/ Implementation Methods : A literature search was conducted across PubMed, Science Direct, and BMC Oral Health databases for articles published between 2020 and 2025 using keywords such as “periodontitis,” “systemic inflammation,” “diabetes mellitus,” “hypertension,” and “cardiovascular disease.” Inclusion criteria consisted of full-text research articles in Indonesian or English focusing on associations between periodontitis and systemic diseases. A total of 33 relevant articles were identified and narratively synthesized. The review consistently demonstrates that systemic comorbidities significantly influence periodontal status. Results : Patients with diabetes, hypertension, and dyslipidemia experience more severe periodontitis due to poor metabolic control and inflammation. Inadequate oral hygiene worsens this condition, while non-surgical periodontal therapy can improve HbA1c, lipid levels, and blood pressure. Conclusion/Lesson Learned : Systemic factors synergistically worsen periodontitis through chronic inflammation and endothelial impairment. Integrating oral health services into chronic disease management may improve metabolic outcomes and prevent systemic complications.

Abstract

Background : Chronic kidney disease (CKD) is a major complication of type 2 diabetes and contributes significantly to morbidity and healthcare costs in Malaysia. Early recognition of individuals at risk is essential, yet current clinical prediction practices remain inconsistent and are not systematically informed by combined clinical and biochemical parameters. A structured prognostic score offers a systematic approach to support early risk stratification and timely intervention. Objective : This review aims to synthesize evidence on the development and validation of the i-CKD prognostic scoring tool designed to predict chronic kidney disease risk among patients with type 2 diabetes in Malaysia. Research Methods/ Implementation Methods : This review will explore the staged development process of the i-CKD score, including the identification of key predictive factors, development of the scoring model, and subsequent internal and external validation. The methodological evaluation will emphasize the selection of predictors and statistical assessment of model discrimination and reliability throughout each phase. Results : A prognostic scoring tool (i-CKD score) will be developed and undergo internal and external validation to determine its predictive performance. Conclusion/Lesson Learned : This review underscores the value of structured risk stratification in the early detection and management of chronic kidney disease. While various prognostic tools have been proposed, limitations in standardization, validation, and clinical integration persist. The i-CKD score has the potential to strengthen clinical decision-making by supporting earlier identification of high- risk individuals.

Abstract

Background : Type 2 diabetes mellitus (T2DM) is a chronic metabolic disease whose prevalence continues to increase globally, including in Indonesia. Health education plays an important role in increasing knowledge and preventive behaviors for T2DM. Media videos and live presentations are considered capable of strengthening public understanding, particularly among young people who are familiar with digital technology. Objective : To determine the effect of education using a combination of videos and presentations on increasing public knowledge about type 2 diabetes mellitus. Research Methods/ Implementation Methods : This research uses a pre-experimental one-group pretest-posttest design. A total of 100 Generation Z respondents (born 1997–2013) with non-health backgrounds in Samarinda were selected through purposive sampling. The intervention consisted of a combined educational approach using videos and live presentations about type 2 diabetes. The research instrument was a validated and reliable knowledge questionnaire. Normality testing using Kolmogorov–Smirnov showed the data were not normally distributed (p < 0.05), so the analysis of knowledge score differences was performed using the Wilcoxon Signed-Rank Test. Results : The results showed p < 0.05, which means there was a significant difference between the pretest and posttest scores. These results indicate that a combination of video and presentation education is effective in increasing respondents' knowledge about DMT2. Conclusion/Lesson Learned : Health education using a combination of video media and presentations has been proven to significantly increase public knowledge about type 2 diabetes mellitus. This multimodal approach can be an effective strategy in digital health promotion, especially for young people.

Abstract

Labor pain is a subjective experience caused by uterine muscle ischemia, stretching and traction of the uterine ligaments, traction of the ovaries, fallopian tubes, and lower uterine distension, pelvic floor muscles, and perineum. The labor pain that occurs can lead to anxiety and stress in the mother, excessive worry, prolonged first stage of labor, and traumatic experiences during childbirth. Endorphin massage is a non-pharmacological therapy, a modification of Indonesia's cultural heritage, used to reduce the intensity of labor pain in the first stage. Objective: To determine the effectiveness of endorphin massage on pain levels during labor, in order to provide scientific evidence for the management of labor pain during the first stage, and to support safe, effective, and evidence-based midwifery practices. Methodology: A literature review with a study design of Randomized Controlled Trials (RCT) and Quasi-Experimental studies. Literature searches were conducted using Google Scholar, PubMed (Medline), ProQuest, ScienceDirect, and Scopus, covering publications from 2018 to 2025 in both Indonesian and English. Study selection was based on inclusion criteria derived from titles, abstracts, and full texts, and on the studies' feasibility assessment, which were then analyzed based on their findings. Results: Ten journals met the criteria, and after analysis, eight studies were deemed suitable. Pain measurement was conducted using the Numeric Rating Scale (NRS) or Visual Analog Scale (VAS). Endorphin massage was performed for 10 minutes on the back and waist of laboring mothers during the first stage, three times. The results showed that the pain intensity before the endorphin massage was 7.27, and after the massage, it decreased to 4.93. This demonstrates that endorphin massage significantly reduces the intensity of labor pain. Conclusion: Endorphin massage is a complementary therapy that can be used as part of the management of labor pain during the first stage.

Abstract

As hospital competition and service demands rise, patient loyalty becomes a critical asset. Therefore, understanding how Customer Relationship Management (CRM) influences Relationship Quality (RQ) is essential. CRM is no longer just an administrative tool; it is a core strategy to strengthen RQ. By improving understanding, interaction, and personalization, CRM directly builds the trust, satisfaction, and commitment that form a strong RQ, which is the foundation for patient loyalty. This study analyzes the influence of Customer Relationship Management on Relationship Quality in inpatient services at Abdul Wahab Sjahranie Hospital. Method Using a quantitative analytical survey design, data were collected from 390 purposively selected inpatients who had visited the hospital at least twice in the past six months. A closed-ended Likert scale questionnaire was used, and data were analyzed with the Chi-Square tests and Logistic Regression were included. The results show that Customer Knowledge, Customer Knowledge Management Capability, and Customization have a significant effect on RQ, while Customer Interaction has no significant effect. Overall, RQ plays a central role as a mediator, where the application of the Customer Relationship Management strategy is significantly able to strengthen the loyalty of inpatients at Abdul Wahab Sjahranie Samarinda Hospital through the formation of quality relationships. These findings highlight that understanding patients comprehensively, effectively managing patient knowledge, and tailoring services to patient needs are strategic in strengthening hospital–patient relationships. Practical implications suggest prioritizing CRM strategies focused on patient needs and experiences to foster long-term, mutually beneficial relationships.
