Abstract

INTRODUCTION Nasopharyngeal carcinoma (NPC) is a common type of cancer in Southeast Asia, and its treatment often involves a cisplatin chemotherapy regimen. Ototoxicity is a side effect that often occurs in patients receiving this regimen. Antioxidant therapy, both synthetic and natural, such as curcumin, has been studied as having the potential to reduce ototoxicity due to cisplatin. This study was conducted to evaluate the effect of administering mecobalamin and curcumin as antioxidants in patients with nasopharyngeal carcinoma after cisplatin chemotherapy regimen with hearing loss. METHOD This study was an observational study with a cross sectional design, using consecutive sampling with inclusion criteria, namely patients diagnosed with NPC receiving the cisplastin regimen and willing to take part in the study. The data obtained was then analyzed using the Rank-Biserial Correlation's Test. Research approval was obtained from the Medical Research Ethics Commission, Faculty of Medicine, Sebelas Maret University/RSUD Dr. Moewardi Surakarta. RESULTS Of the 30 NPC patients who received cisplatin from the ENT-KL installation section of Dr Moewardi Hospital in the period March - September 2023, 30 data were obtained for each right ear and left ear. The average hearing threshold before intervention was 33.79 ± 10.40 dB and 34.10 ± 9.75 for the right and left ears. There was a difference in the average increase in hearing threshold for all parameters (Group I: Mecobalamin, Group II: Curcumin, Group III: Curcumin + Mecobalamin), with a value of p=0.000 (p<0.05). From these data it was concluded that the combined therapy of both was able to increase the hearing threshold along with the chemotherapy series. CONCLUSION Based on the research results, administration of mecobalamin and curcumin as antioxidants has a significant effect on patients with nasopharyngeal carcinoma after cisplatin chemotherapy regimen with hearing loss.

Abstract

ABSTRAK Autoimmune Inflammatory Rheumatic Disease (AIIRD) is a group of different disorders, which have similar clinical, laboratory and immunological manifestations. In East Asia, the top 3 diseases included in AIIRD are Systemic Lupus Erythematosus, Rheumatoid Arthritis, and Sjogren's Syndrome. Glucocorticoids are still one of the therapeutic modalities for AIIIRD with their antiinflammatory and immunosuppressive effects. However, glucocorticoids have some side effects. One side effect of concern is decreased bone density. The condition of glucocorticoid induced osteoporosis (GIOP) is reported with increasing prevalence. The main effect of the use of glucocorticoids on bones is the presence of impaired bone formation regardless of the role of inflammation.  Secretome stem cells contain bioactives that exhibit diverse physiological functions such as immunomodulation, anti-inflammatory, angiogenesis, anti-apoptotic and anti-oxidation. Secretome can modulate cell differentiation by promoting osteogenesis and inhibiting adipogenesis. In this case report, it showed improvements in markers of bone damage, inflamation, and vitamin D levels after stem cell secretome administration in AIIRD patients with long-term corticosteroid use.

Abstract

INTRODUCTION Chronic Rhinosinusitis (CRS) is a chronic inflammatory condition of the sinuses and nasal mucosa that affects quality of life. This study aims to analyze the correlation between Peak Nasal Inspiratory Flow (PNIF) and Sino-Nasal Outcome Test-22 (SNOT-22) in assessing nasal patency in CRS patients. METHODS This study used a cross-sectional design with CRS patient subjects who underwent PNIF examination and were interviewed using SNOT-22. Statistical analysis involved normality test, log transformation, and Mann Whitney test for non-normally distributed data. RESULTS Results showed that there was a significant correlation between increased PNIF scores and decreased SNOT-22 scores (p=0.021), indicating improved nasal patency and decreased CRS symptoms. The data showed a significant difference between the groups with normal and low PNIF in terms of SNOT-22 scores. In normal PNIF there were 6 people with Mild SNOT-22 and 1 person with M-S SNOT-22. In low PNIF there were 2 people with Mild SNOT-22 and 44 people with M-S SNOT-22. CONCLUSION PNIF and SNOT-22 are effective as measurement tools to assess nasal patency in CRS patients. The correlation between these two tools shows that increased nasal patency is associated with decreased CRS symptoms.

Abstract

Background: Onychomycosis is a fungal infection of the nails. Onychomycosis is caused by dermatophytes, non-dermatophyte molds, and non-dermatophyte fungi. Dermoscopy examination has gradually been used as a modern diagnostic method to assess non-invasive nail abnormalities that are easy and inexpensive to visualize abnormal microscopic features of the nail. However, it is still uncommon for medical personnel to diagnose onychomycosis using dermoscopy. Purpose: To provide information on the benefits of the nail dermoscopy technique that can diagnose onychomycosis and describe observable dermoscopic findings. Results: Dermoscopy findings on onychomycosis showed a diverse picture depending on the type. Distal and lateral subungual onychomycosis shows the proximal margin of the onycholytic area with spikes leading to proximal folds and longitudinal striae. White superficial onychomycosis shows large, brittle, irregularly spreading white-yellow patches on the nail's surface. Proximal subungual onychomycosis has one or more transverse white bands on the inner nail plate, while total dystrophic onychomycosis shows longitudinal striae and spikes and irregular distal terminations. Conclusion: Nail dermoscopy improves quality and simplifies examination to establish the diagnosis of onychomycosis because it can guide clinicians in conducting screening, choosing the best time for mycological sampling, and making therapeutic decisions.

Abstract

Nutrition is one of the factors that contribute to male infertility. Vitamin D3 are known to affect spermatozoa and usually served in fertility supplementation in doses of 400 and 1000 IU. Low levels of vitamin D3 have been linked to decreased sperm quality and quantity. This research would like to know the effect of Vitamin D3 to the DNA Fragmentation Index (DFI) and Testosterone blood level in men undergoing intrauterine insemination (IUI). This was an analytical observational study with two groups and a pretest-posttest design. Seventeen men aged 20 to 45 who were undergoing the IUI program and did not have diabetes or hypertension are participate in this study. Sperm DFI was determined by using an ELISA method with a DNA Kit. Blood testosterone levels in ng/mL units was measure using ELISA. IBM-SPSS version 22 was used to perform univariate and bivariate analysis on the collected data. The blood level of Vitamin D3 was significantly increased after the intervention of six week Vitamin D3. Meanwhile, this study shows there are no differences in the level of blood testosterone before and after treatment. The DFI was significantly increased in the 400 IU groups.
