Abstract

This study aims to review the management of physiotherapy in cases of fracture 1/3 of the distal radius. A study witnesses that this case is an enormous incident of hand fracture, accounted 17-25% of the total. A 24-years-old woman (as a student) experienced a motorcycle accident on 29th of July 2021. After being observed, she was diagnosed 1/3 distal radius fracture, ulna dislocation, and mild cerebral oedema. The doctor conducting open reduction internal fixation (ORIF) surgery to the patient on 30th of July 2021. Moreover, there were pain, oedema, and limitation of wrist in her incise area felt by the patient after that surgery. The visited physiotherapy clinic 8 times (2 times a week) with 4 evaluations every week. The patient was provided some treatments such us infrared (IR), TENS, static contraction, ROM exercise, strengthening, and release. The results of the evaluation for 8 treatments showed that there were changes in aspects of pain, edema, MMT and ROM in the wrist & elbow region. The recovery process for fractures accompanied by fixation of the bone, both internal and external, can lead to potential problems that can occur in patients with postoperative fractures of the distal 1/3 of the radius. It is very important to start physiotherapy as early as possible to improve circulation, prevent postoperative problems or complications and speed up the patient's recovery.

Abstract

The thenar pain and paresthesias experienced by people with CTS greatly interfere with their daily activities. Long-lasting repetitive strain injuries, thickening of the yellow ligament and pregnancy are suspected to be the cause. Objective: To determine the difference in effect of iontophoresis and phonophoresis in reducing pain in CTS cases. The research subjects were CTS patients who visited the physiotherapy center at UNS Hospital in Surakarta. Method: experimental research with two pre and post test design groups. 21 subjects aged 26 to 61 were randomly assigned to 2 groups, 10 were assigned to the iontophoresis group and 11 to the phonophoresis group. The cream used contains methyl salicylate. Pain assessment was performed before and after the intervention in both groups. Analysis: Test of normality of the VAS pain data before treatment of the two groups with the Shapiro Wilk test obtained p 0.05. Pre-test VAS homogeneity test in the two groups with the results of the Mann Whitney test obtained p 0.05. The pre and post difference test for each group used Wilcoxon's test with results for all groups showing p≤ 0.05. The difference test between the pre and post VAS differences using the Mann Whitney test was obtained p 0.05. Results: The difference between the pre and post VAS tests between the two groups showed that the two interventions did not show a different effect, but the pre and post VAS tests for each group showed significant results. Conclusion: The two interventions have the same effect on pain reduction in CTS cases.

Abstract

The stability of the knee joint during rotational movement so that excessive translation does not occur in the tibia and femur is the main function of the anterior cruciate ligament (ACL). This ligament is frequently injured and must be reconstructed to restore stability to the knee, reducing the risk of re-injury in order to return to sports safely. The purpose of this case report is to provide information regarding the appropriate physiotherapy exercise program in phase III post ACLR. A 38-year-old man who has a hobby of playing soccer has injured his ACL and has undergone ACL reconstruction in June 2021. When the post-ACLR examination was carried out, it had entered its 9th week and therapy was carried out for 4 weeks. The results showed that after carrying out 6 treatments in 4 weeks there was a significant change in passive knee dextra ROM. But there is no change in edema and there is no hypertrophy of the quadriceps and gastrocnemius muscles. The increase in functional activity as measured using the lower extremity functional scale (LEFS) showed an increase in activity standing for 1 hour and walking as far as 1 mile. Strength deficits and post-ACL reconstruction muscle atrophy, especially in the quadriceps muscles, can be overcome by incorporating a program of closed kinetic and open kinetic chain exercises as well as isometric exercises. Despite a rehabilitation program, the mechanical strength of the ACL does not fully return to normal

Abstract

Bell's palsy Bell's Palsy is weakness or paralysis of the peripheral facial nerves, acute in nature, and the cause is unknown (idiopathic). Bell's Palsy often occurs in some people due to exposure to wind, cooling or air conditioning and also due to viruses. The annual incidence of bell's palsy is between 11.5 and 40.2 cases for 100,000 people. A 45-year-old male patient who works as an office worker came to the Mulyosari Physiotherapy Clinic in Surabaya on February 18, 2021 from a neurologist's referral with a diagnosis of right facial weakness. Patients work in the office every day using air conditioning. The incident occurred when waking up, the patient felt pain behind the ear then the patient found it difficult to blink and stood in front of the mirror seeing that his side face was not symmetrical, his lips and eyebrows were not symmetrical and his eyes were difficult to blink. The evaluation results for 6 times of therapy by receiving therapy three times a week, it was found that there was an increase with the administration of Infrared (IR) modalities, Electrical Stimulation (ES), and Facial Massage. Measurements with the MMT scale and the Ugo Fisch scale were used to measure fascial strength and functional ability, as well as the final evaluation. The recovery process takes a fairly fast time if handled directly and routinely. It is very important to do physiotherapy as early as possible to improve circulation, increase facial muscle strength and facial functional abilities so as to prevent more chronic problems and increase patient confidence as before.

Abstract

Osteoarthritis is a degenerative disease whose cases are the most common globally. Based on radiological findings, it was found that 70% of patients aged over 65 years suffer from osteoarthritis globally. This problem develops progressively which is characterized by degeneration of joint cartilage so that it can cause limitations in functional activities. A 79-year-old woman complained of motion pain in both knees. This has been felt for 13 years especially when bending the knees and standing for long periods of time. When walking there is a sound in both knees. The result of the doctor's diagnosis was that the patient had bilateral genu OA. The patient has received knee fluid injections several times and has been taking pain medication frequently to date. In addition, the patient also underwent surgery on the spine on November 13, 2020. Physiotherapy in this case was only carried out for 3 meetings. Patients were given programs such as ROM exercise, strengthening, stretching, myofascial release, and gait training. The results of the evaluation for 3 times of therapy showed that there was a decrease in pain intensity, and the risk of falling as well as an increase in ROM, muscle strength, and functional index. Based on the results of supporting examinations, including severe OA due to high pain intensity, stiffness and crepitus sounds when performing flexion-extension movements coupled with predisposing factors, namely obesity that has been experienced for years without any medical action. Based on this, of course, the potential problems that occur have greatly affected not only the knee aspect but have affected other regions such as back and spinal aspects, reduce the risk of falling and can increase ROM, muscle strength, and functional index.
