Abstract

The COVID-19 pandemic has revealed significant weaknesses in the global health system,  including the lack of preparedness in the pharmaceutical sector to handle large-scale crises.  The pharmaceutical industry and pharmacists play a strategic role in ensuring drug  availability, vaccine production, distribution of medical supplies, and public education. This  article aims to systematically review the critical roles of the pharmacy sector during  pandemic responses and to evaluate necessary preparedness strategies for future health  emergencies. A literature review approach was used, focusing on international publications from the last decade. The findings highlight that cross-sector collaboration, digitalization of  pharmacy services, strengthening of supply chains, and emergency preparedness training for  pharmacists are key strategies in building global pandemic resilience. Additionally, inclusive  and evidence-based policies are essential to optimize the contribution of pharmacists and the  pharmaceutical industry in sustaining global health security. These insights form a strong  foundation for developing more resilient pharmacy systems in the future.

Abstract

In many developing countries, antimicrobial resistance (AMR) is clearly not just a medical issue; it is a complex issue that intersects social, cultural, and economic factors. Practices in the field show that doctors often face pressure to prescribe antibiotics based solely on assumptions, because the availability of adequate diagnostic tools is very limited. On the other hand, patients often come with requests for antibiotics for minor complaints, assuming that it is the most effective and quickest solution. Another phenomenon that is no less concerning is the free circulation of antibiotics in the market, even without a prescription. Regulations do exist, but without consistent law enforcement, these policies often only become formalities on paper and have no real impact. In addition, the use of antibiotics in the agricultural and livestock sectors also exacerbates the cycle of resistance at the ecosystem level. Therefore, efforts to combat AMR in developing countries cannot simply adopt models from developed countries. Every intervention must consider local conditions, including lifestyles, norms, and specific challenges of local communities. Without a deep understanding of the root of the problem, the interventions carried out will only be superficial and unable to significantly change the direction of the resistance rate.

Abstract

This study is a literature review that aims to comprehensively examine health and drug regulation from three perspectives: the implementation of Real-World Evidence (RWE), the application of Good Regulatory Practices for Medicines (CROB), and a comparison of health regulations across countries. Using a literature review method, the findings indicate that Indonesia faces significant challenges in integrating RWE into its drug approval system, particularly due to limited data infrastructure, regulatory gaps, and data ethics issues. On the other hand, the CROB guidelines issued by the National Agency of Drug and Food Control (BPOM) represent a crucial effort to build a regulatory system that is transparent, accountable, and evidence-based. Furthermore, cross-country comparisons affirm that strict regulations positively impact the quality of healthcare services, enhancing patient safety and the competence of medical personnel. This study concludes that adaptive, integrated, and consistent regulation is essential to ensure the quality of medicines and healthcare services, and that harmonizing policies across sectors and countries is necessary to address global health challenges.

Abstract

Access to essential medicines is a critical component in ensuring the right to  health, yet it remains a major challenge in developing countries. Economic disparities,  patent monopolies, fragile supply chains, and inadequate distribution infrastructure have left large populations without access to safe, effective, and affordable treatment. This  study aims to analyze the strategic roles of international organizations such as WHO,  UNICEF, and Gavi, in expanding access to essential medicines, while also identifying  associated opportunities and challenges. A systematic literature review was conducted  using peer-reviewed journals, official reports, and current policy documents. The findings  indicate that WHO contributes through global standard setting and drug prequalification;  UNICEF focuses on vaccine procurement and distribution; and Gavi drives innovative  financing and global partnerships. Nevertheless, significant challenges persist, including  intellectual property constraints, dependence on imported raw materials, circulation of  falsified drugs, and sustainability issues in post-donor settings. Strategic opportunities  include cross-agency collaboration, digital technology adoption, and strengthening of  local production capacity. This study underscores the importance of aligning national  policies with international support to develop an equitable and sustainable pharmaceutical  system. Access to essential medicines must be recognized as a shared global  responsibility within the framework of human rights and global solidarity.

Abstract

The resilience of the global Active Pharmaceutical Ingredients (API) supply chain has become a strategic issue in public health systems, especially due to the high dependence on two major suppliers: China and India. This reliance poses significant risks in the event of production disruptions, geopolitical conflicts, or restrictive export policies. This study aims to analyze the core challenges in the global API supply chain and identify mitigation strategies that can be applied by developing countries such as Indonesia. A descriptive qualitative method was used through a literature review of secondary data, including national journals, government policy documents, and reports from global health organizations such as WHO and WTO. The findings show that dependence on China and India significantly affects the availability and pricing of medicines and disrupts the continuity of national pharmaceutical production. Three main mitigation strategies are identified: diversification of API sources, strengthening of domestic API manufacturing through incentives and industrial zones, and adoption of digital technologies to improve supply chain visibility. In conclusion, building API supply chain resilience requires an integrated approach involving government support, industry collaboration, and international cooperation. This study emphasizes the urgency for pharmaceutical system transformation to become more self-reliant and responsive to global challenges.

Abstract

The World Health Organization (WHO) established an international organisation  known as the Member State Mechanism, aimed at combating the circulation of falsified  and substandard medicines to ensure that the global population has access to high-quality,  safe, and affordable medicines. Indonesia, as one of the member countries, has  consistently taken an active role in this forum. However, in reality, both Indonesia and  other countries around the world have not yet succeeded in overcoming this issue. The  widespread circulation of falsified and substandard medicines poses significant risks to  global public health. The objective of this study is to examine the impact of falsified and  substandard medicines on global public health and to analyze the efforts undertaken by  Indonesia and other countries to handle the proliferation of these medicines. This article  employs a qualitative research method with an explanatory approach. The concepts of  international cooperation and international regimes are used to explain the outcomes of  collaboration within the Member State Mechanism and the role of WHO as an  international organization in supporting efforts to combat counterfeit medicines. The  result of this study is to illustrate the negative impacts caused by the circulation of  falsified and substandard medicines, as well as the challenges faced by Indonesia and  other countries in addressing the issue. These challenges stem from two main sources:  external factors related to international cooperation, and internal factors stemming from  domestic conditions.

Abstract

The rapid development of digital technology has transformed various health and pharmaceutical sectors. Digital pharmacy refers to the use of information and communication technology to provide online pharmaceutical services such as consultations, therapy monitoring, and drug purchases through digital platforms, so as to improve access and efficiency of pharmaceutical services, especially in hard-to-reach areas. Cross-border telemedicine and e-commerce-based pharmaceutical services offer great opportunities, such as increasing access to pharmaceutical services in remote areas and accelerating drug distribution and can make it easier for patients to get drug information, consultations, and drug delivery without having to come directly to the pharmacy. However, the main challenges include regulatory and legal aspects, such as differences in regulations between countries that can hinder the operation of cross-border services. Limited access to technology and lack of public understanding of the use of digital platforms are also obstacles.

Abstract

Traditional medicine has been an integral part of the community medicine system and has been passed down from generation to generation, and is now even more relevant in the midst of globalization. The increasing interest in herbal medicine cannot be separated from the public's concerns regarding the side effects of chemical drugs and the need for a more holistic approach to health. However, the development of traditional medicine faces quite complex challenges, ranging from the lack of standardization, limited scientific validation, to the threat of biopiracy against genetic resources and traditional knowledge. This paper seeks to analyze the literature that discusses pharmaceutical standardization efforts, scientific validation, and protection strategies against biopiracy in the development of traditional medicines.

Abstract

Pharmacogenomics is a field that explores how genetic variations influence individual responses to drug therapy. One of the major challenges in its clinical application is the genetic diversity across ethnic groups, which contributes to differences in drug metabolism, efficacy, and toxicity. Genetic polymorphisms in enzymes such as CYP2D6, CYP2C19, CYP2C9, and VKORC1 show varied distributions among ethnic populations, leading to significant variation in optimal dosing and adverse drug reactions. This narrative literature review aims to examine the relationship between ethnic-specific genetic variation and drug response, while also assessing the potential of personalized pharmacogenomic therapy in clinical practice. Findings suggest that pharmacogenetic approaches that account for ethnic background can improve therapeutic outcomes, reduce adverse effects, and support the implementation of precision medicine across diverse healthcare systems, including in developing countries.

Abstract

Neglected Tropical Diseases (NTDs) burden billions of individuals in developing countries, significantly impacting health and economy. While vaccination is a vital intervention, NTD vaccine development and access face multidimensional challenges related to investment, research, and distribution. This article analyzes funding gaps due to low market attractiveness and reliance on philanthropy. In research, the biological complexity of pathogens, limited infrastructure, and inadequate preclinical models pose crucial barriers. Distribution challenges include cold chain constraints, access to remote areas, and weak healthcare system capacity. Through a systematic literature review from 2004 to 2015, it was identified that these issues are interconnected, delaying vaccine availability. Dependence on public and philanthropic funding, alongside scientific and logistical complexities, hinders progress. This analysis highlights the need for stronger market incentives, increased research collaboration, development of more stable vaccine formulations, and strengthening healthcare systems to ensure equitable access. Implications for Indonesia are emphasized, highlighting the importance of research independence and strengthening distribution infrastructure.
