Abstract

ABSTRACTGeneral Background: Maternal mortality is associated with obstetric complications, including postpartum hemorrhage caused by birth canal trauma. Specific Background: Perineal rupture occurs in approximately 75% of vaginal deliveries in Indonesia and is associated with parity, maternal age, fetal weight, and perineal elasticity. Perineal massage using virgin coconut oil (VCO) is considered a nonpharmacological approach to maintain tissue flexibility. Knowledge Gap: Empirical evidence examining VCO-based perineal massage for preventing perineal rupture remains limited. Aims: This study evaluated perineal massage with VCO for preventing perineal rupture among pregnant women. Results: A quasi-experimental post-test only non-equivalent control group design involving 56 pregnant women at 34–38 weeks of gestation was conducted. The intervention group received weekly 10-minute massage. The intervention group showed 64.3% without rupture, while 70.4% of the control group experienced rupture, with a significant association (p = 0.021; OR = 0.234). Novelty: This study provides clinical evidence supporting VCO-based perineal massage as an antenatal preventive method. Implications: Integration of this technique into antenatal care may support safer vaginal delivery and reduce birth canal trauma. Highlights: Weekly Intervention Beginning at 34 Weeks Shows Lower Incidence of Birth Canal Tearing. Participants Receiving the Procedure Demonstrated Higher Rates of Intact Tissue During Delivery. Abstract

General Background: Preeclampsia remains a major contributor to maternal mortality and morbidity worldwide, particularly in developing countries, and continues to challenge antenatal health services. Specific Background: In Indonesia, the incidence of preeclampsia is substantial, and clinical observations indicate that maternal characteristics such as parity, body mass index (BMI), and antenatal care (ANC) attendance are frequently associated with its occurrence. Knowledge Gap: Previous findings have shown inconsistencies regarding the magnitude and pattern of associations between these maternal factors and preeclampsia, necessitating further contextual evidence from local healthcare settings. Aims: This study aimed to examine the relationship between parity, BMI, and ANC history with preeclampsia among third trimester pregnant women at a hospital in Sidoarjo. Results: Using a cross-sectional analytic design with 250 respondents selected through simple random sampling, chi-square analysis demonstrated statistically significant associations for parity (p=0.022), BMI (p=0.000), and ANC history (p=0.000). Primigravida mothers had 2.157 times higher odds of preeclampsia compared to multigravida, while obesity and incomplete ANC were also significantly associated.  Novelty: This study provides updated hospital-based evidence quantifying comparative odds across three maternal factors within a single analytic model. Implications: Strengthening antenatal surveillance, nutritional monitoring, and targeted counseling for first pregnancies may support maternal health strategies aimed at reducing preeclampsia-related complications. Highlights: First pregnancy status showed the highest odds ratio among examined maternal characteristics. Obesity category demonstrated a statistically significant association in chi-square testing. Abstract

In this study, we aimed to evaluate the efficacy of ephedrine infusion in preventing hypotension and minimizing nausea in healthy pregnant women undergoing elective cesarean sections with spinal anesthesia. Forty-four participants were included, with 20 receiving ephedrine infusion and 24 serving as controls receiving bolus injections. Results revealed a significant reduction in the incidence of hypotension (p > 0.1) and nausea (p < 0.001) in the infusion group compared to the controls. No instances of reactive hypertension were observed in the infusion group. Other variables including Apgar scores, fetal blood gas tensions, and onset of respiration remained comparable between the groups. These findings suggest that ephedrine infusion is not only safe but also preferable in mitigating adverse events during cesarean sections under spinal anesthesia in normal pregnancies. Highlights: Ephedrine infusion effectively prevents hypotension during cesarean sections under spinal anesthesia. Administration of ephedrine infusion significantly reduces the incidence of nausea compared to bolus injections. No instances of reactive hypertension were observed in patients receiving ephedrine infusion, highlighting its safety profile in this context. Abstract

This study aims to assess the knowledge, attitudes, and practices of nursing students regarding insulin therapy. Conducted at the College of Nursing - University of Basrah, the research utilized a questionnaire to gather data from 200 participants over a six-month period. Results indicate a good overall level of knowledge about insulin therapy, a fair attitude towards it, and a medium level of suggested practice. Interestingly, while there was a significant correlation between participants' knowledge and gender, no discernible relationships were found between knowledge and other demographic variables, nor between attitudes and any demographic factors. This suggests the need for targeted educational interventions to improve attitudes and practices towards insulin therapy among nursing students. Highlights: Varied knowledge levels among nursing students. Positive attitudes towards insulin therapy. Need for tailored educational interventions. Abstract

The impact of Information Technology (IT) on the management of Islamic boarding schools (ponpes) is profound, yet administrative tasks often remain handled through traditional means, leading to inefficiencies. This research aims to address this gap by developing a basic prototype of an Islamic boarding school enterprise system, focusing on optimizing information management across various administrative domains. Employing the System Development Life Cycle (SDLC) prototype method, the study progresses through needs analysis, design, prototype construction, evaluation, implementation, and maintenance phases. The prototype system, tested at Darul Atsar Islamic boarding school, demonstrates significant improvements in academic, secretarial, student, financial management, and reporting processes. This study underscores the potential of IT integration to streamline routine tasks and enhance decision-making in Islamic educational institutions, contributing to broader discussions on educational technology adoption and management optimization. Abstract

This study presents a structured sedation program for pediatric patients undergoing MRI, aiming to optimize safety and resource utilization. A total of 1857 children were included, with 48 admitted to the RCU, leaving 1809 for analysis. Children under 5 received oral sedation, while those 6 and older were assessed by referring physicians for sedation eligibility. The protocol involved chloral hydrate and paraldehyde, with a backup plan for intravenous sedation or general anesthesia overseen by an anesthetist consultant. Among 1039 scans, 597 were unsedated, and 442 were sedated successfully. Oral sedation failed in 50 cases (6.9%), predominantly in younger children (87% under 5 years). Notably, 2 out of 5 subjects using chloral hydrate experienced respiratory issues. This systematic approach led to efficient resource use and safe outcomes, highlighting the importance of tailored sedation protocols in pediatric imaging. Highlights: Reduced oral sedation failure rate to 6.9% in pediatric MRI. Anesthetist consultation enhances safety in MRI sedation procedures. Abstract

Despite the critical role of pacemakers in managing symptomatic atrioventricular and ventricular conduction abnormalities, there remains a significant gap in the knowledge and attitudes of nurses towards these devices. This study assesses the understanding and perceptions of pacemakers among nurses in Al-Basrah Governorate hospitals, exploring associations with demographic factors such as age, gender, educational level, years of experience, and training. Employing a descriptive methodology, a non-probability sample of 200 nurses from five hospitals (Abi Al-Khasib General Hospital, Umm Qasr General Hospital, Al-Zubair General Hospital, Al-Shifaa Hospital, and Al-Mawani Teaching Hospital) was surveyed from October 15, 2022, to August 15, 2023. Content validity was ensured by engaging eleven professionals, while reliability was assessed using Cronbach's Alpha. Results indicate a concerning deficiency in pacemaker-related knowledge, with 79.5% of nurses demonstrating poor knowledge, 18% showing intermediate knowledge, and only 2.5% exhibiting high knowledge. Similarly, attitudes were predominantly negative, with 81.5% of nurses expressing low enthusiasm towards pacemakers, while 11% and 7.5% displayed intermediate and high attitudes, respectively. These findings highlight the need for targeted educational interventions aimed at enhancing nurses' competence and attitudes concerning pacemakers to improve postoperative care and management outcomes for patients with these devices. Highlights: Knowledge Gap: A significant 79.5% of nurses show insufficient knowledge of pacemakers, indicating a critical need for improved medical education. Attitudinal Barriers: Over 81% of nurses have negative attitudes towards pacemakers, which may affect patient care quality. Abstract

This study assesses the knowledge and practices of emergency department nurses regarding the care of open fractures, highlighting significant disparities in competence. Conducted across five hospitals, the descriptive, cross-sectional survey included 103 nurses, analyzed using SPSS with results expressed through frequencies, percentages, and Chi-squared tests. Findings reveal that 42.7% of nurses, predominantly females, lack adequate knowledge and care skills, whereas 57.3%, mostly males, are proficient. The study suggests an urgent need for focused training programs to standardize and elevate emergency care for open fractures, aiming to improve patient outcomes through enhanced nursing education. Highlights: Discrepancy in Knowledge: Significant variations in the knowledge and care practices for open fractures among emergency nurses, with nearly 43% lacking sufficient skills. Gender Differences: A notable gender-based disparity in knowledge, with a higher proficiency observed among male nurses compared to female nurses.
