Abstract

Background: Hepatitis C virus is a major cause of post-transfusion hepatitis infection and remains a significant global health problem. Patients with thalassemia major are at high risk of HCV due to frequent blood transfusions from infected donors. Objective: To evaluate the sero positivity of HCV infection among patients aged 3 to 18 years with β-thalassemia major. Methods: A retrospective study was conducted on 254 patients (131 females, 123 males) attending the Thalassemia Center at Karbala Children's Hospital from August 2014 to March 2015. Ages ranged from 3 to 18 years. Serological viral markers for anti-HCV antibody were tested using enzyme linked immune sorbent assay. Results: Out of 254 patients, 123 (48.4%) were male and 131 (51.6%) were female. Seventeen (6.7%) were seropositive for HCV. Among patients <6 years, 1 (2.3%) was positive; in ages 6-12 years, 7 (5.3%) were positive; and above 12 years, 9 (11.5%) were infected. Blood transfusion frequency showed that 1 (2.2%) of 45 patients receiving <100 transfusions was positive, 6 (5.4%) of 111 receiving 101-200 transfusions, 6 (9.8%) of 61 receiving 201-300 transfusions, and 4 (10.8%) of 37 receiving >300 transfusions were positive. None of the 10 patients who had undergone splenectomy were infected, while 17 (7%) without splenectomy were infected. Among 59,886 volunteer blood donors in Karbala from 2012 to 2014, 54 (0.1%) were anti-HCV positive. Conclusion: Higher rates of HCV infection in older thalassemia patients with more transfusions highlight the importance of accurate blood screening techniques like PCR to prevent infections in thalassemia patients. Highlights: HCV infection increases with patient age and number of transfusions. Older thalassemia patients show higher seropositivity rates. Emphasizes the need for PCR screening to ensure safer transfusions. Keywords: Hepatitis C, Thalassemia Major, Blood Transfusion, Seropositivity, Pediatric Infection

Abstract

General Background: Broadcasting systems function as central institutions for disseminating information, shaping public discourse, and reflecting societal values within political and technological contexts. Specific Background: Indonesia’s broadcasting landscape has undergone major regulatory transformation from the New Order’s centralized control to the Reform era’s democratization, decentralization, and expansion of private and local media institutions. Knowledge Gap: Despite these transitions, existing legal frameworks have not kept pace with rapid digitalization, particularly regarding internet-based distribution and over-the-top platforms, resulting in unequal governance between conventional broadcasters and digital services. Aims: This study analyzes the evolution of broadcasting regulations from the New Order to the Reform period and examines their alignment with technological developments. Results: Findings indicate that the New Order employed strict state control to manage ideology and public opinion, whereas the Reform era introduced press freedom, the Network Station System, and Local Public Broadcasting Institutions, although implementation encountered obstacles such as media conglomerate dominance and regional infrastructure disparities. The rise of internet-based media shifted audiences toward digital platforms, while regulatory adaptation remained limited, producing gaps in oversight and accountability. Novelty: The study offers a comprehensive legal trajectory linking political transition, decentralization policy, and digital disruption within Indonesia’s broadcasting governance. Implications: The findings highlight the urgency of revising regulatory frameworks to ensure equitable supervision, legal certainty, and democratic media development in the digital environment. Highlights: State monopoly during the earlier political period restricted content diversity and public access to alternative viewpoints. Post-1998 policy reforms introduced network-based structures and locally oriented public institutions but faced structural barriers. Rapid migration of audiences to internet services created governance gaps between legacy broadcasters and online providers. Abstract

Background: Double impaction of two teeth in the same region specifically tooth 22 and tooth 23 is a rare condition that requires a combined treatment approach involving fixed orthodontics and surgical exposure. Fixed orthodontic treatment is essential to prepare space, guide tooth movement after surgical exposure, prevent root resorption, and correct overall malocclusion. Surgical exposure is performed to expose the tooth crown, preserve the alveolar bone, avoid root exposure, and facilitate the attachment of buttons and ligatures. Impacted teeth can affect the stomatognathic system, facial growth, and aesthetics. The maxillary incisors and canines play a critical role in occlusion, masticatory function, and facial aesthetics, thus requiring appropriate management. Objective: To describe the management of a double impaction case in the anterior left maxillary region of a 19-year-old female patient using a combined approach of fixed orthodontic and surgical treatment. Methods: The patient underwent fixed orthodontic treatment and sequential extraction of premolars 14 and 24 to create space for the eruption of the impacted teeth and to correct the midline before surgical exposure. Post-surgical exposure, orthodontic treatment was continued to guide both teeth into proper alignment within the dental arch. Results: Teeth 22 and 23 successfully erupted with good occlusion. The patient's masticatory function and facial aesthetics improved over a 2-year period following surgical exposure. Conclusion: Fixed orthodontic treatment combined with surgical exposure is an effective strategy for managing double impaction of anterior teeth in the left maxillary region, considering the position of the crowns, tooth inclination, and root apex morphology of the impacted teeth. Highlights: Double impaction of teeth 22 and 23 is rare and affects function and aesthetics. Combined surgical and orthodontic treatment effectively guided tooth eruption. Treatment restored occlusion, midline, and facial aesthetics over 2 years. Keywords: Anterior tooth double impaction, surgical exposure, fixed orthodontic treatment
