Abstract

Health is a fundamental right guaranteed by the constitution and a key pillar in the development of a welfare state. Health law serves as a normative instrument that regulates the provision of services, protects patient rights, regulates the obligations of medical personnel, and ensures the accountability of healthcare institutions. However, the reality in Indonesia demonstrates a significant gap between normative construction and empirical implementation. Key challenges include disparities in access to services, a deficit in National Health Insurance funding, an unequal distribution of healthcare workers, and regulatory fragmentation. The COVID-19 pandemic further emphasizes the importance of health law not only as an administrative tool but also as a mechanism for state resilience in the face of public health crises. This study aims to analyze health law in three dimensions: theoretical, practical, and prospective. Theoretically, health law affirms the state's responsibility to guarantee the right to health as part of a welfare state. Practically, it reflects the effectiveness of public policy and healthcare system governance. Prospectively, health law must adapt to the digitalization of services, the protection of patient data, and the global commitment to universal health coverage. Thus, health law serves not only as a regulatory framework but also as a strategic foundation that unites human rights protection, policy effectiveness, and social justice in building a sustainable welfare state.

Abstract

Study This aim analyze phenomenon discrimination to patient Guarantee National Health Insurance (JKN) and limitations knowledge patient about his rights . Space scope study service - focused House sick in Indonesia with review aspect legal , social , and access health . The method used is study normative with approach studies literature , through secondary data review from the Indonesian Ombudsman, BPJS Watch, Riskesdas 2018, and various publication academic . Data includes surveys and reports national which shows more of 21% of JKN patients experienced treatment discriminatory , time wait an average of 3–5 hours, and low level knowledge rights (47%). Findings confirm existence discrimination systemic which has implications for injustice structural , reinforced by gaps information patient . Analysis test literature support the need reform service and socialization right patient . This study underscores the importance of improving both the dissemination of information about patients' rights and the responsiveness of healthcare services, to ensure equitable and just treatment for all individuals under the JKN program.

Abstract

Diabetes Mellitus is a chronic metabolic disorder with a rapidly increasing prevalence worldwide, particularly among the elderly population. Older adults with diabetes frequently experience multiple comorbidities that may complicate disease management, increase treatment burden, and reduce quality of life. Consequently, effective self-management is essential to maintain glycemic control and prevent complications. This study aimed to identify and analyze self-management among elderly individuals with diabetes mellitus and multiple comorbidities through a literature review. Methods: This study employed a literature review design using the PRISMA framework, including identification, screening, eligibility assessment, and synthesis of selected studies. A systematic search was conducted through PubMed, Scopus, and EBSCO. Articles published within the last five years were included. A total of 5,934 articles were identified from the three databases. Following the removal of duplicates, abstract screening, and evaluation based on inclusion and exclusion criteria, seven studies were selected for analysis. The review indicated that self-management among older adults with diabetes mellitus remains suboptimal. Major challenges included dietary regulation, physical activity, blood glucose monitoring, and medication adherence. Physical and psychological conditions were also found to significantly influence self-management abilities. Self-management among elderly individuals with diabetes and multiple comorbidities requires further improvement through continuous health education, enhancement of self-efficacy, and strong social and family support to reduce complications and improve quality of life.

Abstract

A study was conducted to evaluate the effect of the combination of bay leaf extract (Syzygium polyanthum (Wight) Walp.) and lemongrass leaf extract (Cymbopogon nardus L. Rendle) on total cholesterol levels in male white rats induced with 80% quail egg yolk and 2% propylthiouracil for 15 days. The combination extracts were administered orally at various doses as follows:Test I: bay leaf extract 350 mg/kg BW and lemongrass extract 200 mg/kg BW; Test II: bay leaf extract 350 mg/kg BW and lemongrass extract 400 mg/kg BW; Test III: bay leaf extract 700 mg/kg BW and lemongrass extract 200 mg/kg BW; Test IV: bay leaf extract 700 mg/kg BW and lemongrass extract 400 mg/kg BW. A 0.5% CMC suspension was used as the negative control, while simvastatin suspension at a concentration of .025% was used as the positive control. The Kruskal–Wallis test showed a significant result (p = 0.02), indicating that the combination of bay leaf and lemongrass extracts significantly reduced total cholesterol levels in hyperlipidemic white rats. The results demonstrated that the combination dose of bay leaf extract 700 mg/kg BW and lemongrass extract 400 mg/kg BW produced cholesterol-lowering effects comparable to simvastatin at a dose of 25 mg/200 g BW (p > 0.05). In conclusion, the combination of bay leaf and lemongrass extracts effectively reduced cholesterol levels in hyperlipidemic white rats.

Abstract

Psychological distress in the elderly with diabetes mellitus and multiple comorbidities includes emotional responses such as stress, depression, and anxiety that arise due to the complex and prolonged demands of disease management. This study aims to analyze psychological distress in the elderly with diabetes mellitus and multiple comorbidities through a systematic review design according to PRISMA guidelines. Articles were obtained from four databases, namely Scopus, PubMed, EBSCO, and ScienceDirect, for the period 2020–2026. The selection was made based on consensus and assessed using the criteria of the Joanna Briggs Institute (JBI). Out of a total of 1,486 articles, 7 articles were obtained. The results of the study showed that psychological distress in diabetic patients was related to clinical factors, number of comorbidities, length of illness, and self-management ability. Multiple comorbidities have been shown to be the main determinants that exacerbate psychological distress in the elderly with diabetes. In addition, psychological distress has significant implications for self-management behaviors, which are an important component in diabetes control. The conclusion of the study confirms that psychological distress can no longer be viewed as a secondary condition, but rather as a primary component in the course of chronic disease that requires clinical attention in parallel with the physiological aspects in diabetic patients with multiple comorbidities.
