Abstract

Background: Low back pain is a frequently occurring symptom closely associated with society, both in Indonesia and globally. It can be influenced by both unmodified and modified factors, such as excessive physical activity, obesity, body posture, and psychological factors. Considering these risk factors, it is known that individuals in the productive age range (<45 years) have a significant risk of experiencing low back pain. Objective: This study examines the correlation between low back pain subjective complaints and sleep quality in students at the Faculty of Medicine Diponegoro University. Methods: Analytical observational study with a cross-sectional design. Research subjects are students from the Faculty of Medicine, Diponegoro University, Semarang, which were selected using consecutive sampling. The low back pain subjective complaint is determined using the NRS and sleep quality data are determined by PSQI questionnaire. The relationship between the low back pain subjective complaint and sleep quality is analyzed using the Chi-Square correlation test and Fisher’s Exact alternative test. Results: From a total of 51 research subjects, the analysis of subjective low back pain complaint scores in relation to sleep quality yielded a p-value of 0.362. Therefore, it can be concluded that there is no significant relationship between subjective low back pain complaint scores and sleep quality. Conclusion: There is no relationship between the subjective complaints of low back pain and sleep quality among students at the Faculty of Medicine, Diponegoro University.

Abstract

Introduction: Numerous studies indicate that fatigue is among the most frequently reported side effects in hemodialysis, and if left unaddressed it can negatively affect their quality of life. Conversely, family support may significantly enhance the quality of life for hemodialysis patients. This study explore the association between family support and fatigue in patients undergoing regular hemodialysis. Methods: This study used a cross-sectional survey design involving patients who regularly received hemodialysis at Haji Adam Malik Hospital. Participants were selected through consecutive sampling. Fatigue levels were assessed using the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) Questionnaire, which was translated into Bahasa. Family support was assessed using the Family Support Questionnaire developed by Rosyidah. The data collected from the sampling were analyzed using SPSS version 27. Results: Among the 100 hemodialysis patients, 62% reported recieving good family support, 26% reported recieving enough family support, and 12% reported recieving low family support. A survey indicated that 48% of respondents felt mild fatigue, while 36% reported moderate fatigue, and 16% felt severe fatigue. Spearman Ranks test showed a significant association between family support and fatigue (r=0,378 ; p=<0,01). Conclusion: Family support is significantly associated with fatigue in regular hemodialysis patients.

Abstract

Background: Sodium-glucose cotransporter 2 (SGLT2) inhibitors are widely used in type 2 diabetes therapy, but selectivity over SGLT1 is critical to minimize gastrointestinal side effects. Objectives: To evaluate and compare the selectivity of SGLT inhibitors, especially Dapagliflozin and Empagliflozin, toward SGLT1 and SGLT2 through molecular docking to inform future inhibitor design. Methods: This study used molecular docking to compare the binding profiles of five SGLT inhibitors, Dapagliflozin, Empagliflozin, Canagliflozin, Sotagliflozin, and LX2761, against SGLT1 and SGLT2 transporters Results: Dapagliflozin was the only compound to show statistically significant selectivity for SGLT2 over SGLT1 (p = 0.036), while Empagliflozin and others lacked significant differences in isoform affinity. Although Empagliflozin exhibited slightly stronger nominal binding energy to SGLT2 than Dapagliflozin, ligand efficiency (LE) analysis indicated that this was primarily due to its larger molecular size, affirming Dapagliflozin’s more efficient binding. Interaction analysis further revealed that Dapagliflozin formed a clean, stabilizing pose within the SGLT2 pocket, free of repulsive interactions, unlike Empagliflozin, which displayed an unfavorable donor-donor contact in the same site. Conclusion: These findings support the structural basis for Dapagliflozin’s higher selectivity and reinforce its profile as a SGLT2-specific agent.

Abstract

Background: Pain is an unpleasant sensation that can range from mild to severe, causing discomfort and potentially disturbing daily activities. Therefore, pain management and treatment are necessary. Kumis kucing (Orthosiphon stamineus) is a traditional medicinal plant known to contain flavonoids with anti‑inflammatory and analgesic properties that can help alleviate pain. Objective: This study aimed to investigate the analgesic activity and determine the effective dose of Orthosiphon stamineus extract. Methods: The design of this study was a true experimental post‑test‑only randomized controlled group. Pain was induced using a 1% acetic acid intraperitoneal injection to evoke the writhing reflex in mice. The writhing reflex was observed every 10 minutes for 1 hour, and the data were analyzed using one‑way ANOVA followed by a post‑hoc test. Results: The results revealed significant differences in analgesic activity between the P1 and P3 groups, as well as between the P2 and P3 groups (p < 0.05), indicating that the kumis kucing extract at a dose of 0.24 mg/gBW effectively reduced pain, as demonstrated by a decreased total number of writhing reflexes. The pain protection percentage at the 0.24 mg/gBW dose was 48.75%. Conclusion: Kumis kucing extract has significant analgesic activity, with the most effective dose being 0.24 mg/gBW.

Abstract

Background: Pelvic organ prolapse in older women remains a major reproductive health issue in gynecology. Decreased pelvic floor compartment strength, which can affect multiple structures, presents new challenges beyond the scope of gynecology. Understanding the pathophysiological cascade from one compartment to another is essential to help determine appropriate initial therapy for individuals suffering from pelvic organ prolapse. Case presentation: In this case report, a patient with severe uterine prolapse for one month developed rectal prolapse and severe hemorrhoids, leading to anemia. Definitive management should be postponed to allow initial management to improve the patient's general condition. Conclusion: Asymptomatic pelvic organ prolapse should not be ignored, as it can lead to the development of other diseases and reduce the individual's quality of life. Chronic increased intra-abdominal pressure can lead to pelvic organ prolapse in more than one compartment and ultimately impair venous return. Pessary ring placement is the first option if the prolapsed mass can fully retract into the vaginal introitus. However, regular monitoring is mandatory to avoid complications after pessary ring installation.
