SDJ is dedicated to publishing original research articles, review articles, and also case reports. The editors will carefully select articles to present the most recent findings in basic and clinical sciences in dental research. Abstract

Background: The aim of endodontic treatment is to eliminate bacteria from the root canal. Bacterial removal from the root canal can be achieved with a mechanical approach using an instrument and a disinfecting irrigation agent. Enterococcus faecalis is the most prevalent bacteria found in root canals. Research studies have also been conducted to examine irrigation systems using herbal products such as drumstick tree leaf (Moringa oleifera) instead of NaOCl. Active compounds in M. oleifera, such as flavonoids, tannins, saponins, alkaloids, phenolics, and triterpenoids, possess antibacterial effects. Objectives: The aim of this study is to assess the antibacterial effect of drumstick tree extract (M. oleifera) in several concentrations (25%, 50%, 75%, and 100%) as an irrigation solution against E. faecalis through in the root canal ex vivo. Methods: This study used 24 mandibular premolars, divided into six category groups. Group 1 received 5.25% NaOCl as a positive control, Group 2 received 25% M. oleifera extract, Group 3 received 50% M. oleifera extract, Group 4 received 75% M. oleifera extract, Group 5 received 100% M. oleifera extract, and Group 6 received irrigation using phosphate‑buffered saline as a negative control. Quantitative polymerase chain reaction methods used to analyze the E. faecalis number in the root canal after treatment with M. oleifera extract. Results: A one‑way ANOVA showed significant differences (P = 0.05) between the three types of irrigation solutions against E. faecalis. Among the six study groups, the most prominent efficacy was found in Groups 1, 4, and 5. Conclusion: M. oleifera extract solution at concentrations of 75% and 100% is as effective as 5.25% NaOCl. This extract may be used as an alternative irrigation agent for root canal treatment. However, further studies are warranted to examine the toxicity effect.

Abstract

Background: Porcelain veneer restoration is the primary choice for indirect restoration, especially for anterior teeth, given its high esthetic properties and lower failure rate than resin composites. Glass-based ceramics such as IPS e.max Press and Vintage LD Press are a choice for veneer due to its superior physical properties. Resin cement is used to attach the veneer restoration to the teeth. The polymerization of resin cement used in veneer restoration affects the stability, mechanical properties, and resistance of the restoration. The composition and thickness of the porcelain material affect the light‑curing unit to cured resin cement. Objectives: The aim of this study was to evaluate the influence of porcelain thickness, light intensity, and exposure time in the hardness of resin cement. Methods: Porcelain samples measuring 5 mm in diameter with three types of thicknesses of IPS e.max Press and Vintage LD Press were used in the study. Resin cement in a metal mold was placed under a porcelain sample before curing with a light-emitting diode (LED) intensity of 1300 or 1700 mW/cm2. The hardness test was then carried out on the bottom of the resin cement. Result: The highest hardness value was obtained from a Vintage LD Press with a thickness of 0.7 mm (cured at 1300 mW/cm2 for 20 s). A four‑way ANOVA test showed significant differences for brands, thicknesses, and times of exposure (P < 0.05) as well as insignificant difference for LED intensity. Conclusion: The study indicates that polymerization of resin cement with lower thickness presented higher hardness values. Irradiation time affected hardness, while LED intensity did not.

Abstract

Background: Maldistribution of health workers, including dentists, is a global issue faced by not only poor and developing countries but also developed countries due to the mobility of health workers, who prefer working in urban areas. The World Health Organization (2012) released global policy recommendations that cover education, regulation, incentives, and personal and professional support to overcome this issue. However, they have not been able to completely solve the issue, as is shown by a description of Indonesia’s Health Profile (2018), which highlights that 43.53% of public health center lack of dentists. The number of studies concerning the real condition experienced by dentists working in rural and remote areas of Indonesia is still limited. Objectives: To analyze job satisfaction and factors affecting dentists’ retention in rural and remote areas of Indonesia. Methods: This study used questionnaire to collect data which was designed based on seven major themes and inserted in Google Form. The link was distributed to dentists who work in rural and remote areas of Indonesia. The sample was selected based on random sampling techniques. Result: The study showed that 45.90% of respondents were not satisfied working in rural and remote areas. Most of the respondents claimed that working conditions, incentives, career and professional development, living conditions, and personal characteristics influenced their interest in working permanently in remote and rural areas. However, political and cultural factors did not give a high impact. Conclusion: The description of job satisfaction and factors affecting dentists’ retention in rural and remote areas of Indonesia is expected to have a positive impact on formulating policy regarding dentists’ maldistribution.

Abstract

Background: Worn dentition has become a common and crucial issue, especially in the older generation. Beyond esthetics, functional diminution can create a serious impact on the overall well-being of the individual. The etiology of the worn dentition can be multifactorial; therefore, the treatment approach should be systematic and must not only address the existing problem but also prevent any recurrence. Case Report: Rehabilitation of a patient with worn dentition requires proper clinical, radiographic examination and recording of the occlusal vertical dimension (OVD). In this case report, a 65-year-old male patient presented with worn out and missing teeth in his upper and lower jaws. An arbitrary face-bow transfer was conducted, and the diagnostic casts were articulated. A mock full contoured wax up with a raised OVD of approximately 4 mm was completed. After 3 months of interocclusal splint therapy with the raised OVD, temporary restoration was cemented. After a follow-up period of 3 months, when the patient was devoid of any signs and symptoms, permanent restorations fabricated using computer-aided design and computer-aided manufacturing were cemented. An interocclusal splint was provided to preserve the restorations. Conclusion: Rehabilitating patients with worn dentition involves careful decision-making during the diagnosis as it will direct the entire treatment sequence. This case report and the resulting steps involved might serve as guidelines for other clinicians while treating similar patients

Abstract

Aggressive periodontitis is a multifactorial disease; the body’s immune response is one of the factors that contributed to the disease development. One of the responses is in the form of the secretion of cytokines—that is, interleukin1β (IL-1β). The secretion of the said cytokine influences the development of periodontitis,particularly the occurrence of rapid bone loss. This study aimed to confirm whether there is a correlation between the levels of IL-1β and the severity of aggressive periodontitis from collected sources. The research performed is a type of literature review that employs the scoping review method to map the results of the analysis from among the topics discussed. Article searches were performedon four databases—PubMed, Cochrane Library, Google Scholar, and Science Direct—using keywords with the Boolean Operators method. In addition, a manual search wasalso performed. The articles were selected with reference to the Preferred Reporting Items for Systematic Reviews and MetaAnalyses Extension for Scoping Review (PRISMA-ScR) flow in accordance with the predetermined inclusion and exclusion criteria. This led to the collection of 10 relevant articles, which were reviewed by reviewing the levels of IL-1β in an individual with aggressive periodontitis. In addition, how much bone damage occurs is seen through the depth of the pocket. Eight of ten articles showed a correlation between levels of IL-1β and bone loss in aggressive periodontitis. Elevated IL-1β levels in patients with aggressive periodontitis reveal a correlation with the severity of the disease that is bone loss.

Abstract

Background: Root canal irrigants play an essential role in removing the smear layer, pulp tissue, and microorganisms in the root canal system. The irrigation solution can remove the smear layer, which can interfere with the bond between the sealers. As a result of sealer penetration into dentin tubules following smear layer removal, an apical seal was obtained. One of the most recent innovations in root canals consists of a combination of silver citrate (BioAKT, New Tech Solution, Brescia, Italy) and chlorhexidine-ethylenediaminetetraacetic acid (CHX-EDTA; SmearOFF, Vista Dental, Racine, USA). These irrigants were used as the final irrigant for root canal disinfection. Epoxy sealers (AH26, Dentsply Maillefer, Ballaigues, Switzerland) and bioceramic sealers (ceraseal, Meta Biomed Co., Cheongju, Korea) have excellent adhesive properties and sealing ability and are commonly used as obturation materials. Objective: The aim of this study was to compare the effectiveness of a root canal combination irrigant between silver citrate and CHX-EDTA on the apical sealing ability of epoxy sealer and bioceramic sealer. Materials and Methods: Forty extracted mandible single-root premolars were used in this study. The root canal preparation was done with a rotary instrument until no. 30.06. Irrigation was performed using 5.25% NaOCl between instrumentations. The final irrigant and sealers were Group I: BioAKT + ceraceal; Group II: BioAKT + AH26; Group III: SmearOFF + ceraceal; and Group IV: SmearOFF + AH26. Obturation was then performed using the warm vertical compaction technique. The sealing ability of endodontic sealers was evaluated using the dye penetration method under the magnification of the stereomicroscope. The data were statistically analyzed using a two-way analysis of variance (ANOVA) (SPSS, IBM, USA). Results: No significant difference (P > 0.05) was observed between the four experimental groups. Conclusion: Both combination irrigants and sealers showed comparable effectiveness in smear layer removal and a similar sealing ability.

Abstract

Background: Oral health is a vital component of overall health and well-being and is an important indicator of healthy living and quality of life. According to the Global Burden of Disease Study 2019, approximately 3.5 billion people worldwide have oral diseases. Objective: This study assessed the level of oral hygiene in a rural community in North India. Materials and Methods: This community-based cross-sectional study included randomly chosen men and women from a rural area of the Ludhiana district of Punjab from August 2021 to February 2022 using the World Health Organization (WHO) Oral Health Questionnaire. Results: This study included 405 participants, comprising 205 (50.6%) men and 200 (49.4%) women, with a mean age of 44.5 + 16.7 years. It was found that only 17.3% of the participants had good status of their gums and teeth, without caries, compared with 17.8% who had poor or extremely poor status. In addition, 54.2% of the participants reported experiencing pain and discomfort in their gums and teeth due to poor oral hygiene. Conclusion: Only a small percentage of our rural cohort had good oral hygiene, indicating a need for increased awareness initiatives.

Abstract

Background: Foods containing gluten and casein are widely consumed by children with autism spectrum disorder (ASD). Indeed, they are among their favorite foods. These foods contain carbohydrates, the primary substrate causing caries. Dental caries is one of the oral manifestations in children with ASD due to an imbalance in the demineralization and remineralization of tooth enamel. This study aimed to determine the relationship between food containing gluten and casein and the incidence of dental caries in children with ASD at the Biruku Indonesia Foundation (Bandung City, Java, Indonesia). Methods: This quantitative study used an analytical observational design with a cross-sectional approach. The research sample consisted of 17 subjects selected using the total sampling technique. Data were obtained after the Food Frequency Questionnaire (FFQ) and intraoral examination (DMFT/dmft score) were completed by the children’s parents. Correlation analysis was performed using Spearman’s rank test. Results: A total of 11 children, 64.7% had a low frequency of consuming food containing gluten and casein, whereas 6 children (35.3%) had a high frequency with a high dmft index (6.2) and a low DMFT index (1.5). Correlation analysis showed a negative correlation or inverse relationship between the two variables. Moreover, a significant relationship was found between consuming food containing gluten and casein and the dmft score (P value = 0.0498) and DMFT score (P value = 0.0383). Conclusion: Consuming food containing gluten and casein has a negative correlation or inverse relationship with dental caries in ASD children.

Abstract

Background: The major challenge associated with composite resin is microleakage, which leads to gaps and secondary caries. The prevalent organism responsible for secondary caries is Streptococcus mutans. Objective: The purpose of this study was to evaluate the antibacterial efficacy of an eighth-generation bonding agent (BA) supplemented with propolis against S. mutans. Materials and Methods: A microtiter plate consisting of 96 wells was utilized to assess the antibacterial activity of two experimental groups: Group I (n=16 wells), eighth-generation BA (G-premio bond), and Group II (n=16 wells), eighth-generation BA (G-premio bond) modified with propolis. Each group had four subgroups: “A” well (n= 4), with BA and bacterial suspension in brain heart infusion (BHI); “B” well (n=4), without BA, but with the bacterial suspension in BHI; “C” well (n=4), with BA and BHI, but without bacteria, as negative control; and “D” well (n = 4), only bacterial suspension as positive control. A clinical isolate of S. mutans was procured from the patient’s saliva and dental caries. A direct contact test was used to estimate the bacterial growth by monitoring the changes in optical density values of each well using a spectrophotometer at 600 nm at regular intervals of 30 min, 1, 2, 8, and 16 h. The mean and standard deviation were calculated for each variable. Analysis of the data between the groups was carried out by Tukey’s post hoc test, and a P < 0.05 was considered statistically significant. Results: The study demonstrated statistically significant antibacterial activity in both groups, but the growth of S. mutans was higher in Group I, which used an eighth-generation dentin BA, compared to Group II, which was modified with the addition of propolis. Conclusion: The incorporation of propolis in the eighth-generation dentin BA exerted higher antibacterial activity against S. mutans compared to the unmodified dentin BA.

Abstract

Background: Endodontically treated teeth with extensive loss structure may require a post. Fiber post is made of a resin polymer matrix held by epoxy resin, which has a high conversion rate and high cross-linking, which may not interact with the monomer of resin cement that can reduce the bond strength that may cause post-debonding, which is a common failure of fiber post. Post-surface treatment and post-length are factors that affect post-bond strength. Objective: This study aims to determine the effect of surface treatment on fiber post and post-length on the bond strength of fiber post. Materials and Methods: Thirty mandibular premolars were selected, decoronated 15 mm from the coronal to the apical region, and then subjected to root canal treatment, obturation, and post-preparation. The samples were initially divided into two groups; each was assigned to a specific post-length; then, each group was randomly divided into a negative control group, air abrasion group, and laser group, for a total of six groups (n = 5). Self-adhesive resin cement is used for post-cementation. Selfadhesive resin cement is used for post-cementation. The evaluation of fiber postbond strength in this study was evaluated with the pull-out test using a universal testing machine at a speed of 0.5 mm/min. Data were analyzed by two-way ANOVA and Tukey’s test. Results: The air abrasion group showed a higher bond strength (P < 0.05) compared with the control and Er:Cr:YSGG laser groups. No significant difference in bond strength was observed between the control and Er:Cr:YSGG laser groups with both post-lengths. Moreover, post-length had no effect on bond strength, and no significant interaction was found between surface treatment and post-length. Conclusion: In this study, the bond strength was higher with surface treatment by air abrasion. Post-length had no impact on bond strength.
