The journal will cover technical and clinical studies related to health, ethical and social issues in field dentistry. The major focus of SDJ is clinical dentistry with emphases in conservative dentistry, periodontology, oral maxillofacial research and community dentistry. Manuscripts pertaining to the basic science dental research including molecular microbiology, regenerative dentistry and dental materials are also within the scope of the journal. SDJ is dedicated to publishing original research articles, review articles, and also case reports. The editors will carefully select articles to present the most recent findings in basic and clinical sciences in dental research. Abstract

Background: Tooth loss leads to functional disability, compromises aesthetics, and reduces quality of life. Tooth mortality reflects untreated dental disease and is considered a crude but useful measure of the dental status of a community. There are many contributing reasons for tooth mortality, and a patient’s individual request for the extraction of teeth is one of them. Objective: The objective of this study was to assess patients’ decisions regarding extraction, the various factors that influence extraction, and its ethical implications. Methods: A cross-sectional survey was conducted among 120 randomly selected subjects visiting the outpatient Department of Oral and Maxillofacial Surgery, Tamil Nadu Government Dental College, Chennai, Tamil Nadu. Data were collected using a self-administered recording form. Descriptive data collected were measured in proportion and percentage. Results: Among the participants, 40 were male, and 80 were female. Based on the dentist’s decision, 78.90% of extractions were carried out, and 21.1% were based on a patient request, despite the choice of alternative treatment being given to them. Pain, problems with mastication, long waiting periods for dental appointments, poor understanding of explanations given by the dentist, and fear of alternative modes of treatment were found to be the reasons for patients’ requests. Pain was found to be the most common reason for the request, followed by problems with mastication and an unaesthetic appearance. Conclusion: Socioeconomic status and education level strongly influence a patient’s request for extraction. Even though alternate strategies instead of extraction were available, considering the patients’ autonomy, treatment was carried out. Educating, motivating, and providing care at the preventive level can reduce irrational requests for tooth extraction.

Abstract

Periodontitis, a complex oral inflammation, poses health risks, especially in advanced stages. Traditional treatments like nonsurgical periodontal therapy (NSPT) have limitations, prompting exploration of adjunctive approaches like probiotics. This systematic review focuses on randomized controlled trials (RCTs) assessing probiotics' impact when used alongside NSPT for advanced periodontitis. Periodontitis, a chronic oral condition, requires effective interventions, especially in advanced stages marked by severe tissue damage. Probiotics, particularly Lactobacillus and Bifidobacterium, have gained attention for their potential in supporting oral health. This review aims to analyse RCTs exploring probiotics as adjuncts to NSPT in advanced periodontitis. From 59 identified studies, 5 met inclusion criteria, published between 2019 and 2021. These studies examined probiotics' effects on probing pocket depth (PPD) and clinical attachment level (CAL) in stage III/IV Grade C periodontitis. Variations in probiotic strains, formulations, and therapy durations were observed. Risk of bias assessment indicated generally low risks across selected studies. Findings on PPD reduction were inconclusive, with two studies favouring probiotics, while others showed minimal differences. CAL gain was observed in all three studies assessing it, but no significant differences between probiotics and control groups were found. The benefits of adjunctive probiotics in advanced periodontitis remain unclear, contributing to the lack of firm conclusions. While probiotics show promise in influencing periodontal parameters, the current evidence does not firmly establish their efficacy as adjuncts to NSPT in advanced periodontitis. Homogeneous RCTs with standardized protocols are crucial for a more comprehensive evaluation of probiotics' potential benefits in this context

Abstract

Primary Sjogren syndrome (pSS) is a chronic autoimmune disorder affecting the exocrine function of the eyes and mouth. Dryness of the mouth and the underlying inflammatory condition may cause alteration of the oral microbiota and connective tissue integrity which may ultimately lead to periodontal disease, such as periodontitis. Studies have shown varying result regarding the association of periodontitis in patients with pSS. This study aims to assess the risk of developing periodontitis in patients with pSS. Literature search was conducted on PubMed, ProQuest, EBSCOHost, and Cochrane databases, with studies evaluating the risk of developing periodontitis in patients with pSS from inception to December 2023 being selected. The Newcastle-Ottawa scale was used in this study to assess the perceived risk of bias of the included studies. This review included one retrospective study and two cross-sectional studies involving 773 patients with pSS. The prevalence of periodontitis in pSS patients is 479 patients (61.9%). Inconsistency of statistical significance is found in regards of relative risk of developing periodontitis. Patients with pSS has an increased relative risk of developing periodontitis. Inconsistency of statistical significance was found between studies.

Abstract

Moringa oleifera is one of the typical plant in Southeast Asia, especially Indonesia. Moringa leaves are known as plants containing bioactive compounds with multiple activities. Polyphenol, quercetin, tannin, and saponin compounds in Moringa leaves have anti-inflammatory, antidiabetic, and antibacterial activities. The potential of Moringa leaf extracts as an antibacterial need to be tested in silico and in vitro to determine the suitability of specific bioactive compounds. The aim is to know the potential of Moringa oleifera leaves as antibacterial, antibiofilm in root canal disease and tooth surfaces. This research method is an empirical study, In vitro and In silico.

Abstract

Background: Teeth rehabilitation after root canal treatment with extensive crown loss is a challenge for clinicians. Endocrown restorations are indicated for cases of extensive loss of crown structure, in conditions where the use of posts does not possible. The use of fiber resin coating acts as a stress breaker and absorption on the dentin thereby increasing fracture resistance. Aim: To determine the effect of fiber resin coating as core build up material and occlusal preparations thickness on fracture resistance of endocrown restorations. Methods: An experimental study was conducted on 20 extracted molars and root canal treatment was performed. The samples were divided into four groups, which consist of with and without fiber resin coating, and with a different composite resin endocrown thickness. Fracture resistances were tested on the samples using a universal testing machine. Results: One-way ANOVA test showed a significant difference in fracture resistance value (p>0.05) between the tested groups. Tukey’s post hoc test results showed significant differences in all groups except for the thickness of 3.5 mm and 2 mm groups with FRC. Conclusion: There was a significant difference in fracture resistance between composite resin endocrown restorations with different thickness, either without fiber reinforced composite or using fiber reinforced composite as the core build up material.

Abstract

Periodontitis is a persistent inflammatory condition caused by a buildup of oral bacteria that damages the tissues around the teeth. It is defined by the deterioration of these tissues, which can result in tooth loss if not treated. The extent of periodontitis is influenced by various factors, including the host's immune response to bacteria that cause periodontal disease. Periodontal infections induce the differentiation of CD4+ T cells into various subsets, including Th1, Th2, Th17, Tregs, and other subsets. This review aimed to determine if there is an association between different subsets of CD4+ T cells and the severity of periodontitis. The research conducted a scoping review, which utilized the scoping review method to systematically map and analyze the literature results on the subject issue. The article search method employed Scopus, PubMed, and ScienceDirect databases, and a manual search was also conducted using keywords and the Boolean Operators technique. The publications were chosen based on the Preferred Reporting Items for Systematic Review and Meta-Analysis Extension for Scoping Review (PRISMA-ScR) flow, following the established criteria for inclusion and exclusion. As a result, 11 relevant publications were gathered and subsequently examined, focusing on the CD4+ T cell subsets in individuals with periodontitis. The publications show that individuals with periodontitis display a more prominent expression of subgroups of T cells. Moreover, the severity of periodontitis is strongly influenced by the host's immune response, particularly the balance between proinflammatory and anti-inflammatory CD4+ T cell subsets.

Abstract

Background: Negative pressure rooms in dental hospitals are built to mitigate the risks of COVID-19, viruses, and other infectious diseases, ensuring the safety of patients, dentists, nurses, and the overall hospital environment. Patient satisfaction is a priority, measured through the Service Quality (Servqual) framework, which evaluates five dimensions: Tangibles, Responsiveness, Reliability, Assurance, and Empathy. Additionally, the Importance-Performance Analysis (IPA) method can identify key questions and dimensions prioritized in quadrant I. Objective: To assess patient satisfaction in the negative pressure room at the RSGM-P FKG USAKTI academic clinic. Methods: A descriptive observational study was conducted using a cross-sectional research design involving 109 participants. Data collection utilized a questionnaire that included 30 questions focused on importance and 30 questions focused on performance. Results: Patients expressed satisfaction with the services provided in the negative pressure room at KIH RSGM-P Trisakti University, achieving an average performance score of 3.61 and an importance score of 3.7. Conclusion: Patients are satisfied overall, but two dimensions that require improvement are responsiveness and assurance. Key areas for enhancement include the speed of response to patient complaints, operators' knowledge, communication skills, and nursing services.

Abstract

Background: Early Childhood Caries is a major health issue in Indonesia, with a prevalence of 67.3% according to the 2018 RISKESDAS, particularly in Banten, where the prevalence is 39.2%. The role of breastfeeding mothers is crucial in the prevention of ECC, especially in avoiding practices that can increase the risk of dental caries in children, such as the discouraged practice of giving milk through a bottle before bedtime. ECC is also influenced by the socioeconomic status of the family, including the type of occupation, income, and level of education, all of which have a considerable impact on the prevalence of ECC. Objective: To determine the knowledge of breastfeeding mothers regarding the prevention of Early Childhood Caries (ECC) based on socieoeconomic status. Methods: The type of research used is descriptive quantitative, with a sampling technique of quota sampling. A questionnaire was given to the respondents, consisting of sociodemographic questions and 8 questions regarding breastfeeding mothers' knowledge about the prevention of ECC. Results: The average age of breastfeeding mothers is 27.81±4.5 years, with the majority having a high school education, an income equivalent to the regional minimum wage (UMR), and working as employees. The average knowledge score of breastfeeding mothers regarding ECC prevention is 4.5±1.4, with 47 mothers having low knowledge and 50 mothers having high knowledge. Breastfeeding mothers with higher education, income at or above the UMR, and those who work as employees tend to have higher knowledge regarding the prevention of ECC. Conclusion: The level of knowledge of breastfeeding mothers regarding ECC prevention shows almost the same proportion between high and low levels of knowledge. Breastfeeding mothers who work as employees, have a higher level of education and income, have higher knowledge about preventing ECC.

Abstract

Background: Obturation is an important step in root canal treatment. Obturation has to result in a tight root canal closure and eliminate all apical and coronal connection pathways. Calcium hydroxide intracanal medicament residue left behind may interfere with the endodontic sealer penetration, and cause an apical leakage. Chitosan acetate 0,2% has been shown to have antibacterial effects and can remove the smear layer. Objective: To determine the effectiveness of different types of final irrigation on the apical seal of bioceramic sealer obturation after calcium hydroxide intracanal medicament cleaning. Method: The study is performed on a sample of lower first premolar teeth that have been prepared and filled with calcium hydroxide intracanal medicament paste. One week later, calcium hydroxide removal is performed with NaOCl. The samples were divided into 3 groups based on the final irrigation material prior to obturation: group 1 was irrigated with 2.5% NaOCl, group 2 with 17% EDTA, and group 3 with 0.2% chitosan acetate. All samples were obturated using the single cone technique and bioceramic sealer and then immersed in Indiana ink. Apical leakage was measured based on the deepest penetration of the dye into the root canal. Results: The smallest apical leakage value was the final irrigation group with 0.2% Chitosan Acetate, and the apical leakage value of the 2.5% NaOCl group was the same as 17% EDTA, but there was no statistically significant difference between the three study groups. Conclusion: There is no difference in the effectiveness of final irrigation between EDTA 17% and chitosan-acetate 0.2% on the apical seal of bioceramic sealer obturation after calcium hydroxide removal. Chitosan-acetate 0.2% can be used as an alternative as a final irrigation material for root canals after calcium hydroxide intracanal medicament, in obturation using a bioceramic sealer.
