Abstract

The symptoms of liver diseases are very diverging, from the mild one till the severe one. Sometimes we may find that severe heart disorders but the symptoms are too less. We need some tools to make a good diagnosis. We can not only use a good anamnesis, but also have to use good physical examination and the other support test. Transaminase also called aminotransferase. This aminotransferase catalyzes the transfer of the amino group (?NH2) of an amino acid to a carbonyl compound. The liver contains specific transaminases for the transfer of an amino group from glutamic acid to ?-keto acids that correspond to most of the other amino acids. Other transaminases catalyze reactions in which an amino group is transferred from glutamic acid to other compounds. Transamination is one of the principal mechanisms for the formation of necessary amino acids in the metabolism of proteins. Transaminase as a sign to cell damage may divided into Serum Glutamic Oxalocetic Transaminase (SGOT), Serum Glutamic Pyruvic Transaminase (SGPT), and Lactic Dehydrogenase (LDH). Gamma GT and alkali fosfatase correlate with cholestasis. Cholinestrase correlate with liver synthesis capacity.

Abstract

Type 2 diabetes mellitus (T2DM) is group with high cardiovascular disease (CVD) risk. Inflammation which is reflected by increasing leukocyte number, CRP, and lipid profile. Despite that, infection as one of diabetic complication commonly occurs in patient with T2DM. The dyslipidemia status reflected by lipid profile composition. So here, we aim to evaluate the relationship between leukocyte count with lipid profile to know the association between traditional and non-traditional risk factor of CVD in T2DM patient. The type of this research is cross sectional analytic research with 94 samples was enrolled into this study. The data is obtained initially by collecting from the quistionare and from laboratorium examination. Mean of samples’ age is 55 years and large number of patients had leukocytosis as reflected by high value of its mean (15.02±8.78 x 103/?L). We found that 56.4% of the subjects fall into worst category of glycemic control (HbA1C). Only 24.5% of the subjects had good glycemic control. Further evaluation showed that there were significant strong negative correlations between leukocyte count with HDL (r= -0.555; p=0.000). Inflammation which mainly related to infectious disease is common among T2DM patients which increase CVD risk by lowering HDL and worsening glycemic control. Keywords: Type 2 Diabetes Mellitus, lipid profile, leukocyte count

Abstract

Reluctance to get treatment is one of the reasons why cancer patients come at the advance stage to health care provider. Numerous information of alternative medicine seems to influence the patients to try alternative medicine and delay their treatment in hospital. A descriptive study is conducted in 2 hospitals in Denpasar to obtain the description of alternative medicine history in cancer patients, with breast cancer and colorectal cancer patients as samples. Total samples were 38 patients, with 23 of them (60.5%) have tried the alternative medicine, whereas 13 patients (56.52%) after visiting the health care provider. Among those who tried the alternative medicine, 14 patients (60.87%) seek this medicine because of their fear to the conventional medicine. The most popular method of alternative medicines is herbal medicine. Most of the patients got information about alternative medicines from people whom close to them such as their family, relatives, and neighbors. However, most of these patients, who have tried the alternative medicine, in the end came back to the health care provider/ hospital after they realized that the result of the alternative treatment did not improve their health condition. Abstract

Obsessive compulsive disorder (OCD) is an anxiety disorder characterized by intrusive thoughts that produce uneasiness, apprehension, fear, or worry, by repetitive behaviors aimed at reducing the associated anxiety, or by a combination of such obsessions and compulsions. OCD sufferers generally recognize their obsessions and compulsions as irrational, and may become further distressed by this realization. An Epidemiological Catchment Area (ECA) study showed a lifetime prevalence of OCD of 2.5%. Biological, psychological and psychosocial all play a role in causing the disorder. A diagnosis of OCD is drawn from its clinical presentations. The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV- TR) is used to aid the diagnosis of OCD. Therapies for OCD are in the form of psychotherapy and psychopharmacology. OCD symptoms persist at moderate levels even following adequate treatment course and a completely symptom-free period is uncommon.

Abstract

Type 2 Diabetes Mellitus (T2DM) is chronic progressive disease which mainly marked by impairment of glucose utilization. Diabetic patients have high risk of anemia which is mainly caused by nephropathy with subsequent decreasing secretion of erythropoietin from renal medullar cells. A cross sectional analytic research was carried out to evaluate the prevalence of anemia among patients with T2DM and its correlation with CVD risk factors measured in medical record at Diabetes Centre of Sanglah General Hospital. The hemoglobin level, blood glucose concentration, HbA1c and lipid profile were collected and analyzed from 157 medical records as research sample which consist of 78 men and 79 women. The frequency of anemia is found to be quite high, reaching 39.7% from all samples (cut off value, Hb=10g/dl). Anemia significantly increased the risk of vascular complication by 8.143 times compare to non-anemic subjects with T2DM (p=0.004). From this research is known that anemia in T2DM may have significant impact on evaluation, treatment adjustment, mortality, and morbidity as well as precipitating CVD and accelerate progression of nephropati to ESRD which are major cause of mortality and morbidity in T2DM. Keywords: type 2 diabetes mellitus, anemia, hemoglobin, cardiovascular disease.

Abstract

Alopecia or hair loss is a quite disturbing case. It can occur in both women and men. In women, the most familiar reason for hair loss is telogen effluvium (TE)succeed by female pattern hair loss (FPHL) and chronic telogen effluvium (CTE). Telogen effluvium generally occurs at any age, it is caused by prematurely transition of anagen hairs to telogen hairs. It can be induced by several events such as physiologic or emotional stress or certain drugs. Meanwhile, female pattern hair loss is caused by increasing of 5?-reductase enzymes and androgen receptors in effected scalp. Chronic telogen effluvium is the shedding of telogen hairs that occur over 6 month with idiopathic cause. There are also other causes of hair loss in women such as alopecia areata (AA), trichotillomania, and traction alopecia.TE and CTE are treated by removing the triggering factors. Patients need to be informed and educated that it is self limited and usually resolve with time. For FPHL, the treatment includes minoxidil 2% solution, antiandrogen therapies and hair transplantation. Meanwhile, AA is treated by intralesional corticosteroid injection, psoralen and ultraviolet A (PUVA). In the other hand, women with trichotillomania and traction alopecia need counselling and education to change their behaviours. Abstract

Occupational contact dermatitis is one of the most common work-related illnesses in many developed countries, accounting for up to one third of all occupational diseases. A high prevalence has been documented in specific occupational groups, such as construction workers, nurses, hairdressers, food processing workers and metal workers. Irritant contact dermatitis and allergic contact dermatitis are known as the two type of occupational contact dermatitis. Reddening of skin, blister, itchiness are symptoms of these dermatitis. Knowledge of the factors which predispose to occupational contact dermatitis is useful in occupational health practice for the application of preventive measures and for career guidance for workers in high risk occupations. Thus to maintain an effective work flow and good health status of the workers both the company and the workers should consider manipulating the risk factors and try to decrease or prevent the occupational contact dermatitis from getting worst.
