Abstract

Background: Root canal irrigation is a step needed in endodontic treatment, after extirpation of dental nerves, to clean the debris in the root canal in infected tooth pulp. The cleaned root canal was then filled with synthetic nerves and thus allowed the tooth crown to be preserved and function normally in the oral cavity.Objective: This study was aimed to compare the effectiveness of piper betel solution with the combination of sodium hypochlorite and hydrogen peroxide as irrigation material in dental root canal treatment. Material and Method: The material of study consisted of 6 upper incisors that were sectioned longitudinally into 12 parts. This study used in vitro method, by applying standard root canal treatment: preparation, extirpation, and irrigation of the root canal of incisor teeth. After irrigation (one group of teeth used piper betel solution; and the other group used combination of sodium hypochlorite and hydrogen peroxide), the tooth was longitudinally cut becoming mesial and distal parts. These parts were examined under scanning electron microscope with 500 magnification, and photographed using electron micrograph on the apical, middle, and coronal sections. The amount of debris was calculated (pm2) in each of those sections.Results: Irrigation using piper betel solution showed less amount of debris (n = 12, mean = 565 pm2) in the root canal, compared to combination of sodium hypochlorite and hydrogen peroxide (n = 12, mean =703,um2). However, the t test statistical analysis showed no significant difference between the two.Conclusion: Because of the toxicity of combination of sodium hypochlorite and hydrogen peroxide, no significant difference in irrigation material for root canal treatment, and the cleaner effect of piper betel solution, I suggest piper betel solution to be used widely as irrigant in endodontic treatment.Key words: root canal, piper betel, sodium hypochlorite, hydrogen peroxide, incisor teeth

Abstract

Two problems arising in determining an appropriate glaucoma therapy are when to treat and how to treat. Open-angle glaucoma is treated when damage to the optic nerve has been demonstrated in the form of progressive pathologic cupping and or characteristic visual field defects, or when pressure is elevated to an extent that is likely to cause damage to the optic nerve. On the other hand, the interrelationship between medical and surgical therapy (argon laser trabeculoplasty & drainage surgery) is complex. Initial treatment of primary open-angle glaucoma has commonly been medical, with surgery undertaken only if medical treatment fails or is not well tolerated. However, this assumption is currently under study; in some cases initial surgery may prove to be more beneficial. Hence, review article was made to prove how beneficial medical or surgical treatment is.Key words: Primary open-angle glaucoma - medical therapy - argon laser trabeculoplasty - drainage surgery

Abstract

Multiple sclerosis merupakan penyakit yang bersifat kronis progresif, yang dalam perjalanan kliniknya dicirikan oleh sifat yang khas, yaitu remisi dan eksaserbasi. Sampai dengan saat ini multiple sclerosis masih dianggap sebagai suatu penyakit auto-imun atau suatu penyaldtyang disebabkan oleh infeksi virus.Prevalensi multiple sclerosis di Indonesia belum diketahui. Pada umumnya prevalensi multiple sclerosis di negara-negara Asia sangatrendah, kurang dari 5 per 100 000; bahkan ada anggapan bahwa multiple sclerosis tidak diremukan di negara tropic.Dilaporkan empat kasus multiple sclerosis yang ditemukan di Daerah Istimewa Yogyakarta dalam kurun waktu 9 tahun terakhir. Di Indonesia laporan kasus ini merupakan laporan yang kedua. Pada tahun 1987 untuk pertama kali telah dilaporkan 3 kasus multiple sclerosis yang dirawat di RSUP Dr. Soetomo, Surabaya.Key Words: multiple sclerosis - atuoimmune disease - echovirus type II - latitude and disease -Schumacher criteria

Abstract

Diagnosis sebagian besar limtoma gastrointestinal hanya dapat ditegakkan dengan laparotomi. Duapuluh empat penderita limtoma gastrointestinal primer diteliti. Penderita dikelompokkan menurut umur, jenis kelamin, keluhan, adanya simtom B (demam, penurunan berat badan, dan keringat malam), lokasi tumor, tindakan bedah, diagnosis histopatologis menurut International Working Formulation, dan terapi.Dari 16 orang penderita laki-laki, umur antara 6,5 - 76 tahun dengan rerata 44,7 tahun, lama keluhan pada 10 penderita tidak Iebih dari 6 bulan, hanya terdapat 8 penderita dengan simtom B.Lima betas penderita dengan keluhan massa pada abdomen, 5 dengan gangguan pola pencernaan, 4 dengan obstruksi, dan sakit perut pada 10 penderita. Biopsi laparotomi dikerjakan pada 16 penderita dan reseksi hanya dapat dikerjakan pada 6 penderita. Dua penderita dilakukan apendektomi karena gejala yang mirip dengan apendisitis. Menurut International Working Formulation, 17 penderita dengan derajat keganasan sedang, 5 dengan derajal keganasan tinggi dan 2 dengan derajat keganasan rendah. Hanya 9 penderita mendapat kemoterapi dengan sikiotostamid, vinkristin dan prednisolon dan hanya 3 penderita mendapatkan 3 siklus atau Iebih kemoterapi.Key Words : gastrointestinal lymphoma -- laparotomy -- chemotherapy -- working formulation.

Abstract

The aim of parenteral fluid therapy in the child is to maintain body fluid and electrolyte balance and nutrition. Fluid and electrolyte balance in the child is more labile than in the adult.In the Department of Child Health, parenteral fluid therapy is used in:I. Gastroenteritis with severe dehydration.2. Dengue hemorrhagic fever and dengue shock syndrome.3. Premature infants, peritonitis, protein calorie malnutrition etc. for parenteral nutrition.The fluids frequently used are:1. Ringer lactate and 5% dextrose in half saline or a quarter saline according to age in gastroenteritis with severe dehydration.2. Ringer lactate and/or plasma, sometimes plasma expander, in dengue hemorrhagic fever and dengue shock syndrome.3. Aminofusin for parenteral nutrition.The dose and the composition of fluid administered are based on the severity of dehydration or shock, the aim of therapy and the composition of fluid loss.Key Words: parenteral fluid - rehydration -- shock - nutrition - low birth weight infant

Abstract

Dental caries is a major problem of dental health in Indonesia. Streptococcus mutans is a bacteria that can causecaries. In this recent time, the agents used to eliminate the caries is less effective due to S. mutans resistance tothose agents. Moreover, the agents may produce side effects after 10 years of consumption.Therefore, effort tofind alternative agents against dental caries is needed. Red betel (Piper crocatum Ruiz & Pav) is a promising naturalmaterial to treat dental caries. The P. crocatum Ruiz & Pav leaves contain secondary metabolites such as essentialoil, flavonoid, alkaloid, and phenolic compounds that active against S. mutans involved in caries formation. Thisstudy aimed to evaluate the activity of secondary metabolites isolated from red betel leaves against S. mutans andits inhibition effect on the glucosyltransferase (GTF) activity. The growth inhibition test of S. mutans was carriedout with liquid dilution test and diffusion test. Cultures of S. mutans were treated with essential oil, alkaloid,flavonoid, phenolic compounds isolated from P. crocatum Ruiz & Pav in various concentration and incubated at 37OCin anaerob condition. Povidone iodine was used as positive control and aquadest as negative control. The inhibitiontest of GTF activity was performed by measuring the levels of formed fructose that was analyzed by Somogyinelsonmethod. The data was analyzed using one-way ANOVA with a 95% confidence interval.The results showedthat essential oil and alkaloid had lowest MIC (Minimal Inhibitory Concentration) compared with the other activecompounds (5000 μg/mL) but higher than povione iodine as positive control (2500 μg/mL). The essential oil exhibitedsimilar inhibition zone diameter compared with povidone iodine, either at low or high concentration (p>0.05). Theessential oil had similar inhibition activity to the glucosyltransferase compared with povidone iodine (p>0.05). Inconclusion, the essential oil of red betel leaves had the strongest activity against the S. mutans growth through itsinhibition effect on GTF activity.Key words : red betel (P. crocatum Ruiz & Pav) – antibacterial - S. mutans – essential oil - glucosyltransferase

Abstract

Infertility is a problem among child bearing age couples. TORCH infection as one of causative factors infertility must be investigated of its role. The objective of this study is to compare TORCH antibodies seropositivity of infertile cases to pregnant women. Sera from 67 infertile women and 21 pregnant women were used for the study. The results are as follows: the mean of IgG anti-CMV and IgG anti-HSV2 titre of Infertile group are significantly higher than titre of pregnant group. The individual who has been exposed to HSV-2 will have a higher risk to Infertility than who has not. Coincidence infections of TORCH are similar between two groups. It can be concluded that problems of TORCH in infertility must be inferred to any other related factors.Key Word: TORCH - infertility - seroimmunological test - IgG anti-CMV - IgG anti-HSV2

Abstract

This article reiterated the peace warsituation during the first 40 years of the Atomic Age. The nuclear arms race poses not only an unprecedented threat to man's existence, but has taken millions of pre-detonation victims of the nuclear war, in the forms of morbidity and mortality due to famine, infectious diseases, poverty, ignorance, and peripheral wars in the Third World. The resources of out planet are diverted from welfare efforts to the unlimited production of genocidal weapons. World scientists, immediately after the atomic bombing of Hiroshima and Nagasaki, had warned mankind of the danger of a nuclear disaster, and physicians, true to their professional oath and ethics, have joined efforts to prevent nuclear war, since medicine will then be unable to cope with its innumerable, immeasurable and compound casualties, as they had done in previous war. The Third World countries, which mostly obtained their independence after World War suffered severely from the escalating arms race and could not escape the global consequences of a nuclear war anywhere, but still feel reluctant to voice their displeasure, consider it an irrelevant prob. km amidst their many-sided struggles towards progress, or are simply ignorant of the last threat that has confronted their existence and growth. It is, therefore, the duty of Third World physicians to also inform their patients about the imminent danger against their health, welfare and survival,. as they have been doing to their individual patients since the days of Hippocrates.
