Abstract

Kami beranggapan bahwa sangatlah riskan untuk bergerak dalam bidang psychosomatik tanpa persiapan yang matang dalam klinik. Untuk mencapai sukses dalam bidang psycho-analyse dan psycho-therapi, menurut kami tidak ada jalan lain dari pada selaiu mawas diri dalam menghadapi segala macam masaalah kehidupan, tanpa meninggalkan petunjuk-petunjuk yang dilatih untuk dihayatinya.Di bidang psychosomatik sang aku yang selalu mengikuti alam fikiran yang logis pada saat menghadapi masaalah-masaalah yang ternyata bertitik tolak pada sumber hidup, harus menyerahkan kedaulatannya pada Zat yang akhirnya memegang kekuasaan, kebijaksanaan, keadilan dan kebenaran.Psychosomatik yang mendasari jalan fikirannya pada petunjuk-petunjuk yang digali dari ilmu Ketuhanan merupakan jembatan yang kokoh antara ilmu kedokteran dan religi. Mudah dimengerti, kalau dengan demikian "geneeskunst" akan meningkat.Akhirnya kami berpendapat, bahwa Candra Jiwa Indonesia yang ternyata mampu memecahkan masaalah-masaalah yang rumit di bidang psychosomatik, perlu diselidiki lebih lanjut, untuk mengetahui sampai berapa jauh Candra Jiwa Indonesia mampu memecahkan masaalah-masaalah di bidang psychologi, pedagogi din ilmu sosial lainnya, yang hingga kini masih merupakan teka-teki.Kami mernpunyai keyakinan, bahwa masaalah-masaalah pendidikan yang masih merupakan duri yang tajam bagi pembangunan, akan terselesaikan dengan cepat, jika Candra Jiwa Indonesia diselidiki, kemudian dimengerti dan akhirnya dilaksanakan tanpa purbasangka.

Abstract

The paper presented a case of hypothyroidism in patient with Graves disease after have long been treated with antithyroid agent. The cause of hypothyroidism in this case might be (1) the effect of anti-thyroid agent, (2) natural history of Graves disease as a result of autoimmune mechanism, and (3) coincidental presentation of Graves disease and condition causes hypothyroidism (hashitoxicosis).A 39 year old woman, household , admitted to Sardjito Hospital because of general fatigue and facial edema. She was previously treated as Graves disease with PTU (propylthiouracil) 100 mg 3 times 100 mg/day for 2 years. Because of allergic reaction against PTU, the drug was replaced by Neomercazol®. One year later the patient complaining body weight (BW) gain, with the laboratory investigation showed T3 = 0.78 µg/d1 (N: 0.6-1.9); T4 = 2436 µg/d1 (N: 45-100). Six months before admission she has not taken any drugs. On admission, BW 60 kg, other vital signs were in normal limits, palpebral edema, T3 = 0.3 itg/d1; Ta = 2.0 jig/d1; Total cholesterol 329 mg/dl; LDL-cholesterol 238 mg/dl; triglycerides 298 mg/d1; Anti-thyroglobulin 1/1280 (N: negative); anti-microsomal negative; other laboratory results revealed normal values. She was diagnosed as hypothyroidism post treatment of Graves disease. She was treated with Thyrax® 1 tablet daily, and one year later she was in a normal condition.All patients with Graves disease should be followed up regularly eventhough the clinical condition already subsided, since other autoimmune disease may coincidentally present.Key Words: hypothyroidism Graves disease hashitoxicosis antithyroid agents autoimmune disease

Abstract

Immunity against malarial infection is a very complex molecular and cellular interaction and represents a combination of both humoral and cell mediated mechanism. However, which mechanism contributes to the protection effect is still not clear. Immunization against Plasmodium berghei infection represents a suitable malarial model to study the host immune responses against malarial infection. The present study was aimed at evaluating the effect of immunization of Swiss mice challenged with P. berghei on the blast transformation, prepatent period, parasitemia and mortality of the host. The result showed that P. berghei infection in .Swiss mice was acute and fatal. All non-immunized mice died following challenge with 1 x 108 parasite on day 8 -10 post infection. Blast transformation of splenic lymphocyte was higher in immunized mice than in non-immunized mice. Immunization with P. berghei and adjuvant in Swiss mice could evoke partial immunity to homolog parasite. Longer prepatent period, reduced parasitemia and decreased mortality were observed in this group. Eighty percent of immunized mice survived from P. berghei infection.Key Words : malarial immunity - immunization - blast transformation - prepatent period - parasitemia -mortality

Abstract

Background: Post-operative pain is one of the main problems in the surgical services so that analgesia is very much needed, even intravenously given. The concept of preemptive analgesia in surgical intervention is the provision of analgesia before the occurence of nociceptive stimulus. One of the agents for providing preemptive analgesia is local injection is local injection with local anasthetic, single or in combination. Objective: This study was aimed to evaluate the efficacy of combination of lidocain and bupivacain as preemptive analgesia in vitrectomy with or without sclera buckle under general anesthesia. Methods: Twenty eight patients who were scheduled for vitrectomy were included in this study. The patients were randomly classified into two equal groups. The surgery was conducted under general anesthesia for both groups, but in Group 1, subtenon anesthesia with local anesthetic was given as preemptive analgesia after induction anesthesia and before the start of surgery. Pain intensity was the primary outcome of this study. The pain intensity was measured in 3 hours and 24 hours after surgical procedure. Results: The baseline characteristics between the two groups were similar. The pain intensity was lower in the treatment group, but the difference was not statistically significant (1.00:t 1.71 vs 1.50:t 2.16, p = 0.109). The proportion of pain-free patients were similar between the two groups in 3 hours after procedure (65% vs 65%, p = 1.00). The proportion of pain-free patients were higher in the treatment group in 24 hours after procedure (100% vs 73%, p = 0.222, Fisher's Exact Test}. The subtenon injection procedure was safe. There was insignificant difference in the incidence of side effects between the 2 groups. Conclusion: This study did not show any additional benefit of subtenon injection for vitrectomy. The pain intensity was similar between two groups. There were no increases on side effects after the procedure. Key words: vitrectomy - local anesthetic - post-operative pain - preemptive analgesia

Abstract

The aim of the diabetic treatment is to restore glucose intolerance to normal. There are abundant evidence that chronic hyperglycemia, from whatever cause, leads to chronic complications, such as angiopa thy, retinopathy, nephropathy and neuropathy. Prophylactic treatment should include good diabetic control and avoidance of other factors involving pathogenesis of atherosclerosis, e. obesity, smoking and hypertension. To achieve this aim, all diabetics, regardless of their age or the severity of their diabetes, must take an active role in managing their disease, and they must have adequate information about the importance of dietary management and must be instructed in suitable diet planning. Diet remains the cornerstone of the treatment of diabetes in adults. Meal-schedule, caloric intake and the composition of the diet must be a relative constant, against which is computed the necessary medication and the individual's exercise habits.Education of diabetic patients and their families should begin at the time of diagnosis. In long term diabetes education, the basic concepts of management should be reviewed together with patients during clinic visits at stated interval.Ketonuria usually means that insulin will be necessary.Key Word: diabetes mellitus — home glucose monitoring — dietary management — glocuse intolerance — diabetic education.

Abstract

Background: An excessive use of drugs in primary health care is commonly unavoidable. This in part due to the limited knowledge of prescribers who are mostly paramedics and whose pharmacotherapeutic knowldges are limited. More than 90% patients with acute respiratory infection (ARI) and diarrhea received antibiotics and similar percentage of patients with muscle ache received injection unecessarily. Objective: To improve the availability of drugs at district level and to improve the quality of prescribing at primary health centers (PHCs).Design: Quasi experimental with pre and post test analysis and using a control group for comparison. An interactive, problem-based training intervention was carried out within 2 days period involving all PHCs and district's warehouses in three provinces, i.e West Kalimantan, West Sumater, and West Nusa Tenggara. Monitoring, supervision and feedback were carried out regularly upon completion of the inter- vention. This study involves 18 districts and 118 PHCs.Results: 11 .995 prescriptions were collected at baseline, 6 months and 1 year after intervention. An average number of drugs per prescription decreased significantly 1 year after intervention (p<0.05), while no improvement was found in the control group. The use of antibiotics for ARI decreased consis- tently and significantly 6 months and 1 year after intervention (p<0.05). The use of injection for muscle ache also significantly decreased 1 year after intervention (p<0.05). Drug availability at district level could be maintained at an ideal level (1 2-18 months) and no stock out was detected 1 year after the intervention.Conclusions: Interactive, problem-based training on rational use of drugs and training on managing drug supply could improve the quality of prescribing and drug availability at district level.Key words: drug availability - stock out - problem-based training - acute respiratory infection - diarrhea

Abstract

Background: The patient's compliance is very important for the successful of acne management. Two percent of salicylic acid lotion can be used for treatment of non comedogenic acne, and as moisturizer. Objective: This study was aimed to evaluate the effect of 2% salicylic acid on inflammatory acne vulgaris patients.Material and Method: A randomized clinical trial was done in two groups of inflammatory acne vulgaris patients or grade I, II, and III papulopustular acne vulgaris according to Plewig Kligman's classification. The first group was treated with doxycycline 50 mg once daily and 2% salicylic lotion, and the other group as a control was treated with doxycycline 50 mg once daily plus vehicle lotion. Evaluation was done by counting the non-inflammatory and inflammatory lesions, measuring skin lightness of the skin using chromameter, and the subject's opinion of their skin conditions about smoothness and moistness of the skin using visual analog scale. In the beginning of this study, there were 78 subjects participated. However, only 62 subject completed this study with 12 weeks. The differences of non-inflammatory lesions, inflammatory lesions, and the subject's opinion of their skin conditions were analyzed using Mann-Whitney Test. The difference of lightness of the skin was analyzed using independent sample t-test. Result: The result of this study showed that there were significant differences (p value< 0.05) in the reduction of non-inflammatory lesions, inflammatory lesions, and the increasing of lightness of the skin between both groups. No significant differences were found (p value>0.05) in the smoothness, and moistness of the skin.Conclusion: The effectivity of 2% salicylic acid on the inflammatory acne patients were the reduction of non-inflammatory and inflammatory lesions, and the increasing of lightness of the skin.Keywords: salicylic acid, acne lesions, smoothness, moistness, lightness

Abstract

Graves'disease is the second common endocrine disease found in the clinic, characterized by hyperthyroidism, ophthalmopathy, diffuse struma with/without dermopathy.CoMmon treatments for hyperthyroidism of Graves' disease are anti thyroid drugs, subtotal thyroidectomyorradioiodine (ml). However, reports in the medical literature present widely varying and often conflicting with regard to cure, relapse, frequency aside effects, cost, and compliance. No single form of therapy is acceptable to or suitable for all patients.The choice of treatment should be based upon several factors, such as the age of the patients, the size of the thyroid gland, existence of complications, and other factors.Key Words: Graves' disease - hyperthyroidism - antithyroid drugs - thyroidectomy - radioiodine

Abstract

In traumatic cataract extraction it is often difficult to assess the result of the operation and the recovery of the visual acuity because there are many complications and recurrence of the previous uveitis.The purpose of this study is to evaluate the visual function improvement and many influencing factors after extraction of traumatic cataract with intraocular lens implantations in children.Eleven cases of posterior chamber intraocular lens implantations in cataract traumatic children were reported in Dr Sardjito General Hospital between 1990 and 1995.There were 10 boys and 1 girl. The mean age was 8 years ranging from 3 years to 14 years. The visual acuity improvement occurred in 10 cases and in 1 case the visual acuity remained stable. Five cases had post traumatic uveitis that reccurred after cataract extractions and IOL implantation and 1 patient showed retinal cicatrization identified after the operation.Key words: traumatic cataract - traumatic uveitis - intra ocular lens implantation - retinal cicatrization - visual acuity recovery
