Abstract

Cystoid macular edema after cataract extraction with implantation of an intraocular lens is considered to be the most frequent and troublesome complication. The advantage of topical indomethacin had been previously proven, but the influence of oral administration had not been investigated. The purpose of this study is to Investigate the effect of the using of indomethacin on the incidence of cystoid macular edema after cataract surgery with an intraocular lens implantation. Two groups of patients with matured and immatured senile cataract undergone extracapsular cataract extraction with implantation of an intra ocular lens had been reported. Each group consisted of 40 patients. Indomethacin group received 25 mg indomethacin orally 2 hours preoperativelly, as well as at 2, 6, and 10 hours postoperativelly then continued for 5 days postoperativelly. Ocular examinations were performed on postoperative days 1, 3, 7, 14, 28, 60, and 90. No Incidence of cystoid macular edema in group indomethacin was found and in placebo group was 2,5% (x2 = 0,001; p> 0,05). After seven days observation on the effect of indomethacin to the postoperative inflammatory reaction demonstrated that there was insignificantly difference between indomethacin and placebo group (x2 = 2,15; p >0,05). Observation on the therapeutic side effect phenomenon indicated that Indomethacin was relativelly safe even if used in the aged patients.Key Words : oral indomethacin - cystoid macular edema - cataract operation - intra-ocular lens -inflammatory reaction

Abstract

Background: The major problem in the treatment of pterygium is to prevent recurrences after surgical excision. To prevent the neovascularization and recurrence, variable doses and administration of mitomycin C installation had been used. However no single treatment has been universally effective yet.Objective: The purpose of this studies is to evaluate the safety and efficacy of two different methods of 0.04% Mitomycin - C application as adjunctive chemotherapy to the recurrence of pterygium. Methods: The research had been conducted at the Dr. Sardjito Central Hospital, Wates Central Hospital Gombong PKU Hospital, and Purworejo Aisyiah Hospital from January 1995 to December 1998. Eighty eight patients underwent pterygium excision were involved in this study, and divided into two groups. Group I or intraoperative 0.04% MMC group of 44 cases receiving 0.04% MMC drops applied to the bare sclera for 4 minutes intraoperatively. Group II or post operative group of 44 cases receiving 0.04% MMC eye drops one day after excision once daily during the first postoperatively week and continued with a dosage of one drop 3 times a day during the second postoperative week. Observations were done daily during the first postoperative week, weekly during the first postoperative month, and monthly up to 6 months. Observations were done including of the symptoms of recurrence and side effects of the treatment. The data were analyzed using the Students test and chi square technique.Results: In the intraoperative group, 6 cases (13.67%) recurred, 2 cases of superficial punctate keratitis. In the postoperative group,. 7 cases (15.95%), and 3 cases superficial punctate keratitis. There was no statistical difference in the rates of recurrence (x2 ---- 0,0902, p>0,05). Most of the cases of pain, tearing and lid edema occurred in the group intra-operative. No other complication was observed in the course of the study.Conclussion: A single dose intraoperative application of MMC is a simple, economic, affective alternative adjunctive treatment for pterygium.Key words : Intraoperative mitomycin-C - pterygium excision - recurrence

Abstract

The Vogt-Koyanagl-Harada Syndrome is characterized by bilateral panuveitis and exudative retinal detachments, in association with cutaneous and neurosensory manifestations. This syndrome is seen most commonly in darker pigmented races, such as Orientals, Hispanics, American Indians, and blacks. It affects both sexes, although it appears to be a distinct female predominance. Most patients are between 20 to 50 years of age, but children and older adults also may be affected. A patient with some clinical symptoms of Vogt-Koyanagi-Harada (VKH) syndrome was reported. This patient had chronic bilateral iridocyclitis, posterior uveitis, exudative retinal detachment, cataracts related to cutaneous manifestations. This patient well responded on ophthalmologic manifestations by corticosteroid treatment but the other signs of VKH syndrome were still developing after 4 months period. The therapy for VKH syndrome was the high doses of systemic corticosterolds, and, with the severity of the anterior uveitis, topical corticosteroid was administrated frequently. A short-acting corticosteroid such as prednisone, in the range of 100-120 mg/day was given, based on the severity of the Inflammation. Some patients might require the addition of cytotoxic agents or cyclosporine.Key words : Vogt-Kayanagi-Harada syndrome - bilateral iridocyclitis - exudative retinal detachment - cutaneous manifestations.

Abstract

Background: Human amniotic membrane has anti-adhesive properties and is felt to promote epithelization and decrease inflammation. Fresh human amniotic membrane transplantation has been shown to be effective in the reconstruction of the corneal surface in the setting of persistent epithelial defects, sterile corneal ulcerations, and partial limbal stem cell deficiency.Objectives: The purpose of this study is to report two cases of perforated bacterial corneal ulcer and one case persistent epithelial defect treated by preserved human amniotic membrane transplantation and to determine whether preserved human amniotic membrane transplantation can be used as an alternative method for treating perforated bacterial corneal ulcer.Methods: Observed human amniotic membrane transplantation was performed on 2 eyes of 2 patients with perforated bacterial corneal ulcer and one case with persistent epithelial defect of different causes. The amniotic membrane was placed on the surface of the cornea in overlay and secured by interrupted 10.0 nylon sutures to the surrounding conjunctiva.Results: There was significant improvement of the visual acuity in case 3 with persistent epithelial defect. Despite absence of improvement of the visual acuity in cases 1 and 2, the epithelization occurred in all patients and the perforated ulcers healed. As a wound dressing, human amniotic membrane has a number of benefits: in both patients the decreased inflammatory reaction, diminishes pain and increases the ocular mobility, and may help in prediction of readiness for grafting.Conclusion: Preserved human amniotic membrane transplantation may be considered as an alternative method for treating perforated bacterial corneal ulcer which is refractory to conventional treatment.Key words: amniotic membrane transplantation - decrease inflammation - perforated corneal ulcer -persistent epithelial defect - readiness for grafting.

Abstract

Background: The combined cataract surgery was a procedure by the creation of a new channel for aqueous outflow between the anterior chamber and sub-Tenons space and followed by extracapsular cataract extraction and posterior chamber intra ocular lens implantation. This procedure is relatively new, that the result need to be reported.Objectives: To report the result of combined cataract surgery with intra ocular implantation in advanced primary glaucoma.Methods: A 68-year-old man suffering from senilis cataract and advances glaucoma had combined surgery on the right eye. The combined surgery was effective to control intra ocular pressure, so the glaucaomatous defect was not worsening and visual acuity would be good. The target 10P was reduction of 20-30% 10P or closer to 15 mmHg.Results: The mean post operative intra ocular pressure was normal, but still needed antiglaucoma drugs. Visual acuity was 6/30 cc S + 1.50 cc S +1.50 / C-1.25 with axis 90 to 6/7.5 three months post operative and decreased to 6/15 with the same glasses three years later. With addition of S +3.00 the patient could read a newspaper, however the visual field was slightly worsened.Conclusions: Extra capsular cataract extraction and posterior chamber lens implantation combined with trabeculectomy can improve visual acuity and decrease intra ocular pressure.Key words : advanced glaucoma - combined surgery - target pressure - visual acuity - visual field - anti glaucoma drug.

Abstract

Principally, complications of post operative cataract surgery with pseudophakos implantation cover those complications due to the operation, and those due to the existence of pseudophakos within the eye ball. In Dr. Sardjito hospital, this operation has been done since 1988. There are many kinds of complication of this operation. This retrospective study is limited to those complications in the form of anterior uveitis, endophthalmitis, secondary glaucoma, and endothelial decompensation which occurred within 1992-1994 period.A study has been performed on 405 cases, consisting of 397 cases with posterior chamber pseudophakos implantation and 8 cases with posterior chamber pseudophakos implantation. In the same period, a study on 432 cases with extra capsular cataract extraction without pseudophakos implantation was performed. The complication in the group with pseudophakos implantation were: anterior uveitis (2.96%), secondary glaucoma (1,48%), endhopthalmitis (0,99%), endothelial decompensation (0.74%) and other complications such as iris prolapse and subluxation of pseudophakos. The complication in the group without pseudophakos implantation were anterior uveitis (0.23%), endothelial decompensation (0.23%), and prolapse of iris 0.46%. In this study, the complication in the form of endophthalmitis occurred 1 week after surgery in 3 cases, and 2 weeks after surgery in one case. One case was caused by fungi infection. The complication in the form of secondary glaucoma mostly related with the occurence of post anterior uveitis surgery. The anterior uveitis in post cataract surgery with pseudophakos implantation was considered to be related with the kind of lens implanted.Key words : post operative complications -- cataract surgery -- pseudophakos implantation -- anterior uveitis -- kind bf lens implanted

Abstract

Background: Astigmatism change after cataract surgery is common, but if the astigmatism is too high it can affect the visual acuity. The astigmatism change depends on the incision methods, sclera! rigidity, and age.Objectives: To compare the astigmatism change between corneoscieral and sclera! incision methods after cataract surgery and intraocular lens implantation.Methods: Eighty patients after cataract surgery with IOL implantation were evaluated. Keratometry examination was performed pre operative, 2 weeks, 4 weeks, and 8 weeks post operative. All of the patients underwent cataract surgery by fornix base flap conjungtiva, 40 patients were performed corneoscieral incision, and 40 patients were performed sclera! incision. All of them was sutured with ethilon 10.0, consist of 6 stichesResults: The corneoscleral incision induced astigmatisms were 0-8.6 D(dioptri), while the scleral incision was 0-8.50 D. The mean astigmatism of the corneoscieral incision 2 weeks post operative was 3.08 D, 4 weeks was 2.72 D, 8 weeks was 2.13 D, while the astigmatism of the sclera! incision 2 weeks post operative was 3.35 D, 4 weeks was 2.32 D, and 8 weeks post op was 1.75 D. There is no statistical significant difference between the corneoscieral and sclera! incision.Conclusion: The mean astigmatism 2 weeks, 4 weeks, and 8 weeks after operation was decreased in both methods of incisions. Despite no statistical significant difference, the mean astigmatism was lower in scleral incision method.Keywords : astigmatism change - keratometry - cataract surgery - corneoscieral incision - sclera! incision

Abstract

The effectiveness of trabeculectomy has been studied in 65 primary glaucomatous patients consisting of 30 men and 35 women. The trabeculectomy was able to control the intraocular pressure in 89% of cases and it improved visual acuity in 67.69% of cases. Based on the iridocorneal angle appearances, the cases of primary closed angle glaucoma were more frequent than those of primary open angle glaucoma. The most frequent complication found was hyphaema which occured in 23.08% of cases, but it I usually disappeared in 7 days post-operatively. Anterior uveitis was the second complication and this happened in 6.15% of cases. Other complications such as flat anterior chamber and cataract were very rare. Conjunctival fistula, bleb rupture and intraocular infection were not found here. Key Words: trabeculectomy - glaucoma - control of intraocular pressure - visual acuity improvement - hyphaemia

Abstract

Background: The frequency of meibomian gland dysfunction (MGD) patients is increasing in elderly and especially in female. Meanwhile the relationship of dry eye with meibomian gland dysfunction is close. Dry condition of eye will result in epitheliopathy, susceptibility to superinfection, scarring sequelae, and neovascularization in ocular surface. Vitamin A can prevent epitheliopathy and disturbance of tear film stability. Oral doxycycline inhibits lipase activity of any attendant microorganism.Objective: To evaluate the effect of systemic combined therapy vitamin A and doxycycline with vitamin A eye drops versus vitamin A eye drop and oral doxycycline in the management of meibomian gland dysfunction.Methods: The design of study was double blind randomized clinical trial. Setting was at Dr Sardjito Eye Clinics. The subjects were 60 MGD patients aged 50-79 years old, male and female were enrolled in this study and divided into two groups. The group I received vitamin A eye drops and oral doxycycline of 100 mg for 4 weeks. The group II was treated with oral vitamin Aof 6000 IU two times per day for 3 weeks, vitamin A eye drops, and oral doxycycline of 100 mg for 12th weeks. Both groups were followed at 4, 8, and 12 weeks for clinical signs, tear production (Schirmer test), quality of mucine (ferning test), and quality of ocular surface (rose bengal test).Results: Improvement was obtained in both group, in clinical signs of MGD as well as in tear production, quality of mucine, and ocular surface condition. There was no significant difference between both groups (p=0.58, RR =0.86, 95%, CI 0.21-3.50). The combined treatment of oral vitamin A, vitamin A eye drops and oral doxycycline of 100 mg in 12 weeks results in similar improvement with treatment by vitamin A eye drops and oral doxycycline in 4 weeks in MGD patients. The side effect of doxycycline was gastrointestinal problem in 2 patients.Conclusion: The result of this study supports that four weeks treatment is sufficient in achieving improvement of meibomian gland dysfunction patients.Key words: Vitamin A - doxycycline - meibomian gland dysfunction - four weeks

Abstract

Background: Phacolytic glaucoma is a secondary glaucoma which occurs in open angle hypermature cataract and caused by trabecular obstruction. This abstruction occurs due to the leaked of lens protein through an intact capsule. Hyphaema in phacolytic glaucoma is an unusual case.Objective : To evaluate the result of conservative therapy and extra capsular cataract extraction on phacolytic glaucoma associated with hyphaema.Methods :A case of hyphaema in phacolytic glaucoma has been reported in a 62 year old man. Symptoms of decreased visual acuity, pain, headache were found in this case. Ophthalmology examination found right visual acuity 1/", light projection and color perception good, conjunctival hyperaemia, ciliary injection, corneal edema, hyphaema, opacity of anterior chamber, iris atrophy, posterior synechiae, and increased intraocular pressure. This case was treated conservatively and cataract surgery was also done accordingly.Result: The result of treatment was loss of vitreous resulted from rupture of posterior capsule, decreased of intraocular pressure.Conclusion : Conservative therapy and extra capsular cataract extraction can improve visual acuity in phacolytic glaucoma associated with hyphaema.Key words: hyphaema - phacolytic glaucoma - conservative and cataract surgery - vitreous loss.
