Abstract

The purpose of this paper is to review 309 patients of appendicitis operated in the Department of Surgery, Gadjah Mada University Hospital, Yogyakarta, between 1972 - 1976. Among the pa. tients 81 are males and 228 are females. The peak incidence of appendicitis is between 15 - 20 years of age. Results of the clinical diagnosis compared with the pathological diagnosis are as follows;81,8% is chronic appendicitis; 44,4% is acute appendicitis; 64,4% is chronic appendicitis with acute exacerbation; 60% is periappendicular infiltrate and 1,9% is normal appendix.Fecaliths within the lumen-of the appendix are found in 136 patients (44%); eggs of elscaris in 8 patiens (2,6%).Key Words: appendicitis - periappendicular infiltrate - Ascaris - normal appendix - fccaliih

Abstract

The presence of immunocompetent cells in the respiratory tract and their cellular interactions are undoubtedly crucial, leading to the development of bronchial asthma in hypersensitive individuals. The cells comprise various populations, such as mast cells, basophils, eosinophils, neutrophils, macrophages, dendritic cells and lymphocytes, in which the T-cells are believed to play a central role. It has been demonstrated that CD4+ T-cells consist of CD45RO reactive population and CD45RA, each secreting different cytokines. In terms of cytokine pattern production, these T-cell subpopulations are similar, to those found in the murine system, i.e. TH1 and TH2 cells. The CD45RO+ IL-4 and IL-2 cells produce directly or indirectly that govern the proliferation and differentiation of B-cells to IgE-secreting plasma cells, by which IgE antibodies are responsible for the development of allergic type I reaction. In addition, the number of T-cells expressing activation markers, like IL-2R, VLA-1 and HLA-DR increases in the serum of asthmatic patients, as well as in their bronchoalveolar lavage and bronchial biopsies. Moreover, the CD4+ T-cells produce IL-3, GM-CSF, IL-4, IL-5, IL-6 and IL-10 that induce mast cell differentiation and activation and recruitment of eosinophils. The function of dendritic cells and macrophages in antigen presentation to either naive or activated T-cells should be noted. Finally, immunologic-based therapy for the airways hyperreactivity may be proposed. This concept includes the regulation of certain cytokine production governing TH1 subpopulations, and by eliciting an optimal mucosal IgA response to down-regulate the production of IgE antibodies.Key Words: bronchial asthma - immunocompetent cell interactions - T-cell subpopulations - T-cell cytokines -                 mucosal IgA response

Abstract

Carcinoma of the male breast occurs so infrequently. Several articles reported that the relative frequency ranges between 0.46% —2% of all breast carcinoma. The histologic types of the male breast carcinoma were relatively the same as in the female, but in the male, lobular carcinoma and carcinoma with metaplasia were never found. The disease commonly occurs in elderly men, from the late fifth through the seventh decade, and in women it usually occurs before menopause. This situation suggests that estrogen has an important role in the pathogenesis of breast carcinoma. Two cases of male breast carcinoma were reported. One occurred in a man aged 28 years. The histologic type was papillary carcinoma with mucoid degeneration. The other occurred in an older man aged 70 years with infiltrating duct carcinoma. The latter case could support the important role of estrogen in causing carcinoma. But the other one, cannot support this. It must be thought that there are other factors, such as in genetic syndromes. Key Words: male breast carcinoma — estrogen — oncology — genetic syndromes — neoplasm

Abstract

Drug utilization has been defined as "the marketing, distribution, prescription and use of drugs in a society, with special emphasis on the resulting medical, social and economic consequences". The aim of drug utilization study is to achieve optimal use of drugs in a society, i. e. safe, efficacious, rational and cost-effective. Various benefits can be obtained from drug utilization study which by all means may increase the quality of patient care.Drug utilization study may be classified into two types based on its specific aim. First routine monitoring of drug use in a society which is aimed at examining the general pattern of drug use and predicting the forthcoming trends, and second special drug utilization study which is aimed at investigating specific study questions and hypothesis.Review of the pattern of drug utilization is likely becoming and effective feedback to the prescribers and drug policy makers towards more rational use of drugs.Key Words: drug utilization - patient care - drug policy - pharmacoepidemiology - drug abuse

Abstract

Background: Oxydative DNA damage, namely 8-OHdG, may occurred due to DNA oxydation by reactive oxygen species (ROS). Unrepaired 8-OHdG may initiate a carsinogenesis due to G to T transversion of gene point mutations. The 8-OHdG repair can be measured by determination of urinary 8-OHdG level. It is well known that ROS reaction with preexisting melanin may perform an immediate pigmentation darken-ing (IPD) and it can be measured by mexameter. The correlation between IPD and 8-OHdG repair capacity is still not known.Aim of Study: To know the correlation between IPD and 8-OHdG repair ability in order to predict the oxydative DNA repair capacity.Methods: Using of a quasi experimental design, 8-OHdG urine level alteration correlated with skin color alteration after oxydative stress among 30 healthy elementary-school children during sport activity pro-gram under sun exposure. 24 hours of skin color alterations as a IPD were measured by mexameter, and urinary-8- HdG- level were measured based on ELISA with 8-OHdG-Kit antibody. The correlation be-tween IPD and     8-OHdG urine level alteration was analyzed with Spearman test.Results: There was a very significant correlation between immediate pigmented darkening and urinary                      8- OHdG level alteration (r=0.8562; p<0.01).Conclusion: IPD represents 8-OHdG repair capacity after oxydative stressed under sun exposure Keyword: immediate pigmention darkening -  8-OHdG -mexameter - sun exposure

Abstract

A case of cerebrotendinous xanthomatosis in an Indonesian male, 25 years of age, is reported. The diagnosis was based on clinical features, laboratory and histopathologic examinations, without measurement of cholestanal concentration either in the plasma or in the xanthoma.Key Words: xanthoma - normolipidemia lipoidosis - cataracta juvenilis - neurologic disturb
