Abstract

Androblastoma is a rare semimalignant ovarian tumor which occurs most frequently during the reproductive years and is assumed to arise from sexually ambivalent cells noted in the ovary of the 6-7 weeks embryo or to be of teratoid origin. The excessive secretion of testosteron will occur and lead to masculinism, while androgens will suppress normal ovarian function and cause the sign of virilization. A 3-years subferdIc case was a 27 year-old-woman with typical syndrome of androblastoma manifested by virilization and defeminization. The testosteron concentration was 700 ng/dl, prolactin: 8,4 p.g/1, Barr body drumstick (+), while the chromosomal analysis indicated female genotype (46 XX). Simple oophorcctomy was done, the size of the left ovarian tumor was 9 x 7 x 8 cm, encapsulated and there was no adhesion. The uterus and adnexa were normal. The pathological result was malignant. Alkeran cytostatic treatment was given in 5 series of 12,5 mg orally. However, the pregnancy occurred during the treatment. The patient was then managed carefully as a routine procedure without and-cancer drugs anymore. The baby was born normally without any congenital anomaly. The signs of either virilism or masculinism decreased. Hormonal examination showed a normal level of testosteron.Key Words: infertility - androblastoma - cytostatics - ovarian tumor - virilization

Abstract

This article argues for the existence of racial or even subracial differences in growth rhythm, so that more than one growth standard has to be used for world populations.For Indonesia, the application of a certain percentile of the Harvard—Iowa standard as norm is not recommended because of the different timing of growth spurts and allometry. Before an propriate norm is established, it is suggested to utilize the Chiang Mai growth diagram which in the authors opinion fits the growth pattern in Indonesian children very well.Key Words. growth norm - body height - allometry growth spurt - auxology

Abstract

In most patients with epilepsy the prognosis for seizure control is very good. However, about 10-20% of patients develop chronic or refractory epilepsy despite the advent of new antiepileptic drugs and intensive monitoring. Meanwhile, the definition of refractory or intractable epilepsy may vary, and a correct diagnostic work-up is of the utmost importance for the choice of treatment. Refractory epilepsy represents a challenge for both experimental and clinical research to obtain knowledge about why and how it develops and whether the condition can be prevented by early and adequate treatment. Definitions and epidemiology, experimental models, diagnostic evaluation, consequences, risk and costs, medical management, neurosurgery, and vagus stimulation are all important elements in managing refractory epilepsy. When dealing with therapy-resistant patients, a systemic step-by-step evaluation is essential. This includes: diagnosis, seizure/syndrome classification, aetiological considerations, review of drug treatment hitherto given, and evaluation of significant learning and psychosocial problems.Key words : refractory epilepsy - seizure - diagnostic evaluation - medical management - aetiological consideration

Abstract

Terapi embolisasi intraarterie1 melalui kate ter (transcatheier arterial emboliralion) merupakan terapi alternatif pada hepatoma stadium lanjut. Tindakan ini dapat dilakukan di rumah sakit dengan fasilitas peralatan ang-iografi. Teknik embolisasi intraarteriel tidaic rerlalu sukar, tetapi diperlukan pengalaman dalam bidang angiografi untuk jangka waktu tertentu.Indikasi yang tepatsangatpen Li ng diperhatikan uniuk rnelakukan re rapi embolisasi in iraarreriel, oleh karena beberapa komplikasi dan efek samping dapat terjadi setelah tindakan. Abstract

There is a tendency of primary liver cancer between 1970-1973 corresponding to the introduction of liver biopsy in Yogyakarta. Liver cancer had previously been found to be the most frequent tumour in necropsy material in Java. In the series, composed of biopsies only, it accounted for 36.8% of all abdominal cancers biopsied in Yogyakarta in 1973 and is thus more frequent than indicated by the total material covering a 4-year period (1970-1973).Based on the data of the Department of Pathology and population in 1976 by utilizing the formulathe primary liver cancer incidence in 1974 through 1978 is provided.It seems that the minimum incidence of primary liver cancer in Yogyakarta is lower than the incidence in other countries in Asia and AfricaKey Words: liver cancer - cancer registration -incidence of hepatoma - racial predisposition for cancer - viral hepatitis

Abstract

This study was aimed at evaluating the T cell and B cell reactivity against Mycobacterium leprae antigens or epitopes among leprosy patients and their household contacts in the Indonesian population. Through this study, M. leprae epitopes (either protective or suppresive) will be identified and hopefully proved useful for the development of an effective leprosy vaccine in the future.Fifty-nine leprosy patients consisting of 34 tuberculoid type (TT/BT) and 24 lepromatous type (LUBL) at Dr. Sardjito General Hospital Yogyakarta and 50 household contacts were recruited for this study. After the informed consent was given, 20 ml venous blood was drawn from each subject for assays of the T cell and B cell reactivities. The T cell reactivity was tested by lymphocyte transformation (LTT) and the B cell reactivity was tested serologically by EUSA. M. leprae antigen, PGL-1 and some recombinant proteins (65 kD, 30 kD, 45 kD and 43 kD) were used as antigens in both assays. In addition, Phytohemagglutinin (PHA) and lnterleukin-2 (IL-2) were used as mitogens in the LTT. Statistical analysis was done by using One way ANOVA and Chi-square tests.The results showed that cellular immune deficiency in LUBL patients was found to be specific to the M. leprae antigen, but not to mitogens and other antigens. The T-lymphocyte of the patients (either TT/BT or LUBL) and healthy contacts demonstrated very low reactivities againts all recombinant antigens. On the other hand, the sera of LL/BL leprosy patients reacted significantly against all antigens, most strikingly against PGL-I and 43 recombinant protein of M. leprae. Whereas, the sera of TT/BT patients and healthy contacts showed no or least reactivity against those antigens. These data indicate that although in a small proportion M. leprae recombinant proteins of 65 kD, 30 kD, 45 kD and 43 kD are recognized by T-cell of leprosy patients and healthy contacts. These antigens contain more B-cell epitopes rather than T-cell epitopes. So, these antigens should be eliminated. as soon as a possible candidate in the development of any leprosy vaccine.Key words: leprosy - T-cell and B-cell - Mycobacterium leprae antigen - ELISA - tuberculoid and lepromatouse types

Abstract

The paper describes the Quality Assurance Program (QAP), including utilization review and patient care audit. A survey was carried out in Puri Nirmala.Hospital, a private mental hospital in Yogyakarta, during 3 months period between January - April 1984. The aim of the survey is, to evaluate schizophrenic in-patients based on 8 criteria: diagnostic procedures, reasons of admission, mental status after admission, length of stay, mortality, complications, therapy and laboratory examinations. 56 schizophrenic in-patients during the surveys period were evaluated. The result chows some deficiencies and shortcomings in the application of QAP. Finally, some recommendations are offered. Abstract

Latar Belakang: Usaha penanggulangan tuberkulosis sering terhmbat oleh penyebaran strain Mycobacterium tuberculosa yang resisten multi obat. Salah satu upaya untuk mengatasi masalah tersebut adalah dengan penyampaian informasi mengenal data resistensi kuman secara berkala.Tujuan penelitian: Penelitian dilakukan dengan tujuan mengetahui pola resistensi Mycobacterium tuberculosa terhadap beberapa obat antituberkulosis, dalam rangka pemantauan strain bakteri resisten obat, terutama di Yogyakarta.Bahan dan care: Penelitian dilaksanakan sejak Januari 2000 sampai Desember 2004 dengan menggunakan 99 isolat Mycobacterium tuberculosa di Laboratorium Mikrobiologi, Fakultas Kedokteran UGM. Resistensi isolat-isolat tersebut terhadap obat pilihan pertama dan pilihan kedua dievaluasi. Suspensi bakteri dengan kepadatan setara dengan standar Mac Farland 1 (108 cfu/ml) ditanam pada medium Lowenstein Jensen yang mengandung masing-masing obat antituberkulosis (OAT) pilihan pertama yaitu: lsoniazid/INH (1,Oug/ ml), Sreptomisin (2,Oug/mI), Rifampisin (1,Oug/m1), Ethambutol (2,Oug/m1), dan obat pilihan kedua, yaitu: Kanamisin (lug/mi), Siprofloksasin (1ug/m1), Ofloksasin (5ug/mI). Sebagai kontrol suspensi bakteri ditanam di medium yang sama tanpa obat.Hasil: Pengamatan terhadap resistensi Mycobacterium tuberculosa menunjukkan bahwa sejumlah isolat telah resisten terhadap obat pilihan pertama yang diujikan, dengan kisaran 24,24% sampai 43,43%. Resistensi terendah adalah terhadap INH (24,24%) dan tertinggi Rifampisin (43,43%), sedangkan terhadap Streptomisin terdapat resistensi sebesar 33,33% dan terhadap Ethambutol 26,26%. Resistensi terhadap OAT pilihan kedua berkisar antara 14,29% sampai 49,50%. Resistensi tertinggi terhadap Kanamisin dan terendah terhadap Ofloksasin.Simpulan: Terhadap obat pilihan pertama, 74,75% dari isolat uji telah resisten terhadap satu OAT atau Iebih, 15,15% telah resisten terhadap semua OAT, dan hanya 25,25% isolat masih peka terhadap obat yang diujikan. Sementara itu 4.04% isolat merupakan strain resisten multi-obat (MDR-TB) karena resistan terhadap INH dan Rifampisin.
