Abstract

Telah diketahui infeksi Mycobacterium leprae dapat menimbulkan gejala neuropati. Kecepatan Hantar Saraf Tepi (KHST) dapat menunjukkan level neuropati. Pada penelitian ini pemeriksaan KHST motorik dikerjakan pada sejumlah penderita lepra yang berobat jalan di Rumah Sakit Umum Pusat Sardjito, Yogyakarta. Gambaran KHST motoriknya menunjukkan terdapatnya gejala neuropad. Saraf yang terkena lehih berm nampaknya adalah n. radialis, n. peroneus dan n. tibialis. Di camping cara-cara lain, pemeriksaan elektromiografi kiranya dapat bermanfaat untuk memantau penyembuhan gejala neuropati. Abstract

Penyakit infeksi cacing usus terutama yang rermasuk infeksi cacing usus yang ditularkan melalui perantaraan tanah merupakan masalah yang umum di negara yang sedang berkembang, yang masih kurang menyadari arti kebersihan perseorangan dan sanitasi.Juga cara-cara kehidupan dan tradisi yang turun temurun yang kurang balk dapat mempermudah berlangsungnya penularan infeksi cacing tersebut.Program pengobatan penyakit cacing secara periodik dapat diharapkan akan memperbaiki status gizi anak-anak, sambil menunggu membudayanya cara-cara hidup yang lebih baik.Key Words: soil transmitted helminths - Kam thick smear method - Taenia solium - periodic deworming - nutritional status

Abstract

Background: Iron absorption is determined by iron status, heme- and nonheme-iron contents and amounts of various dietary factors that influence iron absorption. Few studies have examined the influence of food to iron absorption but only limited information is available about the net effect of these factors.Objectives: Objective of this study was to know the influence of Ferrous Sulphate @ 300 mg ingestion shortly after and 2 hours after meal on pharmacokinetic profiles of iron in serum sample in 12 women with Hb >_ 12 g/dLMethods: The study was carried out in a cross over design in which each of participants underwent 2pharmacokinetic studies. One tablet of Ferrous Sulphate @ 300 mg was taken by each subject shortly after and 2 hours after standard meal at the first and second study. Series of serum samples were taken at0, 0.25, 0.5, 1, 1.5, 2, 2.5, 3, 4, 6, 8, 10, and 12 hours after iron administration. The concentrations of ferric iron in serum were measured using Vitors Fe Slides System, while pharmacokinetics parameters were calculated using a non-compartmental method.Results: Pharmacokinetic parameters obtained from the first and second study were compared using t test with the following results (meant SEM): C,,, 275.92 ± 28.97 & 284.58 ± 30.44 ug/dL, T.3.50 ± 0.29& 3.50 ± 0.38 hours, K. 0.3235 t 0.0011 & 0.3816 ± 0.0010 hours-', Ka, 0.1138 ± 1.5696 & 0.1152 ±1.3331 hours' T,201 7.80 ± 1.45 & 7.03 ± 1.02 hours and AUCo-12 2493.43 ± 397.54 & 2331.15 ± 283.64 ug/dL.hours. No difference of all pharmacokinetic parameters were detected by intervention typeConclusions: It can be concluded that after ingestion of Ferrous Sulphate @ 300 mg in women with Hb 12 ug/dL shortly after and 2 hours after meal did not show significant diferrences on pharmacokinetic profiles of iron in serum sampleKey words: iron absorption - pharmacokinetic - ferrous sulphate - absorption rate - Cmax

Abstract

Accurate gestational age assessment is important in newborn examination to determine further management. Oneof methods to estimate newborn gestation is New Ballard Score (NBS). With limited number of pediatrician especiallyin remote areas, it is expected trained paramedic will be able to perform NBS examination properly. The aim of thisstudy was to assess whether there is agreement between pediatricians and trained paramedics in determiningnewborn gestational age using NBS. This was a cross sectional study. Ballard scoring was performed by pediatriciansand trained paramedics in Dentatama Mother and Child Hospital, Sragen Distric, Central Java, Indonesia fromFebruary-May 2008. Statistical analysis was performed using intraclass correlation. This study was performed in175 newborns those consisted of 55.4% males and 44.6% females. From those newborns, 30.3% were deliveredby caesarian section, and 10.3% were premature baby. Majority of Ballard score examination (69.7%) was performedin 2nd day. According to intraclass correlation, there was a very strong correlation agreement between pediatriciansand trained paramedics (r=0.925 and p<0.05). The result of Ballard score examination that close to gold standardthose which were performed at 48-96 hours of age (r=0.993 and p<0.05). In conclusion, there was a goodagreement between pediatricians and trained paramedics in assessing newborn gestational age using NBS.Key words: Ballard score - trained paramedic - newborn – pedriatician - hospital

Abstract

Psoriasis is a chronic, systemic, and immune-mediated skin disease that can affect physical,psychological and social functions of patients leading to a significant impact on sexualdysfunction. The aim of study was to evaluate the relationship between disease severity anddegree of stress with sexual dysfunction in psoriasis patients. This was an observational studyusing case control design. Subjects were psoriasis vulgaris patients who visited SquamousDivision, Polyclinic of Department of Dermato-Venereology, Dr. Sardjito General Hospital/Facultyof Medicine Universitas Gadjah Mada, Yogyakarta. Disease severity was assessed using psoriasisarea severity index (PASI), whereas sexual dysfunction was evaluated using arizona sexualexperience scale (ASEX) and Psoriasis-related psychological stress was assessed using psoriasislife stress inventory (PLSI). Among 93 patients of psoriasis, 38 patients (40.8%) suffered fromsexual dysfunction. Factors that influence the occurrence of sexual dysfunction in psoriasispatients were disease severity (p=0.00), psoriasis-related stress (p=0.00), disease onset (p=0.03),age (p=0.04) and joint pain (p=0.04). In conclusion, disease severity and degree of stress areassociated with sexual dysfunction on psoriasis patients.

Abstract

In the clinical study researchers want to answer the most important question whether a new therapy is better than the old one. Many researchers continue to frame the question in terms of null hypothesis and answer the question in terms of P value. The null hypothesis itself is typically not plausible, and even in a study where the null hypothesis was plausible, our concern is typically not only limited to the issue of whether or not the treatment has any effect but we also want to know how much the impact. In the study which the researcher pressed the P value into service as an indicator of effect size, it, lends itself to misinterpretation because it combines information about the magnitude of an effect with information about the precision with which that effect is estimated. By contrast, confidence intervals (Cis) focus one's attention on an estimate of a more meaningful parameter (e.g. the rate difference) and, as a separate matter, on the precision of the estimate. The CI is a range of values that is likely to cover the true but unknown value (the extremely low up to the extremely high value of e.g. rate difference, mean difference, and Odds Ratio) if we measure the value many times on samples using the same method. Cis in a clinical trial where the result is statistically significant we might find that it is of no clinically Importance (in a very large sample). On the other hand, the effect of a treatment might be statistically not significant but In fact it is of clinical importance (in a small sample). Cl Is affected by sample size: the larger the sample size the narrower the interval of Cl. The interval Is also affected by the standard error and hence by standard deviation and the confidence level we claim. The higher the confidence level (90%, 95% or 99%, arbitrarily 95% Cl is commonly used) the wider the confidence interval. In case that the 95% Cl does not include zero value (in mean or proportion difference) or one (in Odds ratio or relative risk) it also reflects statistical significance (p<0.05) with a = 5%. A sample size that is enough in terms of power might not be enough in terms of precision due to the confidence Interval level we choose.Key Words : P value - confidence interval - clinical importance - sample size - statistical significant

Abstract

Background: One of the factors that determine the success of efficient woul')d healing is wound healing rate, that can be achieved by increasingcell proliferation, angiogenesisor neovascularisation,and extracellular matrix production. Dermalfibroblast is a cell that plays an important role in wound healing. Fibroblast proliferation and neovascularisation are critical elements in granular tissue formation. Local hypoxia causes fibroblasts to express HIF-1a that will induce fibroblast VEGFexpression. The nature of vitamin C makes it easily oxidized. The addition of vitamin Con fibroblast culture medium is expected to produce local hypoxia condition that will induce fibroblast expression of HIF-1a, so that the expression of fibroblast VEGFwill be increased. Vitamin C may modulate the growth of various types of cells. The effect of vitamin Con normal fibroblast proliferation and fibroblast VEGFexpression is still unknown. Objective: This study was aimed to know whether vitamin Ccan increase normal human fibroblast proliferation and expression of VEGF. Method: A simple experimental study was conducted by using preputial skin fibroblast culture from 10-year-old donor, subculture passage 3. Fibroblast culture was divided into 6 groups, each group received vitamin C treatment with the dose of 50pg/mL, 100pg/mL, 150pg/mL, 200pg/mL, and 300pg/mL, and one group without treatment acting as control. Measurement of fibroblast proliferation was conducted by spectrophotometer using MTT, and fibroblast expression of VEGFwas measured using ELISA. The average of difference in fibroblast proliferation and VEGFexpression was analyzed with one-way analysis of variance. Result: There was a significant increase in fibroblast proliferation rate in the groups receiving vitamin C with the dose of 200 mg/mL (p = 0.016) and 300 mg/mL (p = 0.005), whereas in the group with the dose of 50 mg/mL, 100 mg/mL and 150 mg/mL there was no significant difference compared to the control (p = 0.933, p = 0.961, P = 0.301, respectively). Average fibroblast VEGFexpression between various concentrations of vitamin C compared to the control showed no significant difference (p > 0.05). Conclusion: Vitamin Ccould be considered to be used as an agent to accelerate wounds healing. Keywords: vitamin C, skin fibroblast culture, fibroblast proliferation, fibroblast VEGFexpression

Abstract

G. spinigerum adalah sejenis Nematoda yang hidup sebagai parasit pada Carnivore seperti kucing, anjing, harimau dsb. Infeksi pada manusia banyak terjadi di kalangan masyarakat penggemar makan ikan air tawar mentah atau setengah matang, yang mengandung larva infektif G. spinigerumtingkat rig-a. Manusia merupakan incidental host, sehingga tidak memungkinkan perkembangan larva cacing menjadi bentuk dewasa, tetapi larva ini di dalam tubuh manusia dapat bermigrasi ke seluruh organ dan menimbulkan gejala-gejala penyakit yang berbeda-beda, tergantung pada organ apa yang terserang. lnfeksi G. spinigerum pada sistem saraf pusat umumnya bersifat fatal, dengan gejala khas myeloensefalitis eosinofilik. Sam ini belum ada obat yang bersifat efektif terhadap cacing Mi. Usaha menanggulangi penyakit ini hanyalah mencegah penyebaran dari second intermediate host ke manusia.
