Abstract

Medicinal plants are plants that have medicinal properties or uses and are used to cure and prevent various diseases. Medicinal plants, both leaves, stems and roots, are partly used by the community for traditional or alternative medicine. The use of medicinal plants has evolved from generation to generation, with knowledge being passed down from generation to generation. In its utilization, medicinal plants have good opportunities, such as they are often sold as food supplements, use by the public is increasingly popular and promises minimal side effects and has challenges that must be faced, namely the need for scientific evaluation, a long time with large capital, there is a distance between preclinical models and its applications used in clinical trials as well as the lack of clinical trial data. Many drugs developed from natural sources have had revolutionary impact in medicine, including antibiotics, antiparasitic, antimalarial, lipid control agents, immunosuppressants for organ transplantation, and anticancer drugs. Medicinal plants that have properties or uses as medicinal ingredients which are currently being researched include miswak plants, centella asiatica plants, and mangrove plants.

Abstract

Retirement is a crucial phase and must be well-prepared. Retirement is the end of regular work and entering a period of rest. Someone approaching retirement age should be prepared with knowledge about healthy lifestyle and various health issues and their management. This will increase individual preventive efforts to maintain their health. Healthy lifestyle can be achieved by consuming a balanced nutritious diet, engaging in proper and regular physical activities or exercises, managing stress well, avoiding smoking, and maintaining personal hygiene. In addition, health problems can occur in the elderly because as they age, the physiological abilities of the body decline, increasing the risk of degenerative diseases such as hypertension, musculoskeletal problems, diabetes, heart attacks, strokes, chronic kidney disease, and cancer. These diseases can be managed by recognizing symptoms, seeking early treatment, and preventing them before they occur. Knowledge about retirement preparation about healthy lifestyle and various health issues and their management is essential to improve the quality of life during retirement.

Abstract

Tuberculosis (TB) is an infectious disease that is still a problem in developing countries. Worldwide. Although the government has implemented many programs for controlling the TB, the role of family physician is very important in treating TB. In dealing with these problems, the need for cooperation from various parties in a comprehensive and holistic manner. Implementing comprehensive and holistic family doctor services by identifying risk factors, clinical problems, and patient management based on evidence based medicine with strategies that are patient centered, family oriented and community oriented.The analysis of this study is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from aupporting investigation.The assessment is carried out based on a holistic diagnosis of the initial, process, and final study quantitatively and qualitatively. Patient Nn.N, 19 years old, came to the clinic with cough , easy fatigue, weight loss and night sweats since 2 months ago. The patient was diagnosed with clinically confirmed pulmonary tuberculosis. After the intervention, pulmonary TB complaints improved. There is a change in behavior towards the patient's disease where the quantitative assessment found an increase in all aspects assessed, namely knowledge, diet, and physical activity.

Abstract

Diabetes mellitus (DM) is a heterogeneous disease, characterized by chronic hyperglycemia caused by defects in insulin secretion, insulin action, or both. However, in diabetes mellitus therapy, oral hypoglycemia drugs cause side effects in form of digestive tract complaints in patients, hypoglycemia,and weight loss. Black cumin seed extract as a herbal therapy can reduce blood glucose levels. This study aims to determine effect of administering Black Cumin (Nigella sativa) extract in reducing blood glucose levels in white rats induced by streptozotocin. This study used a pre and post test control group design with a sample size of 32 white rats divided into 4 groups with treatment for 21 days. The groupK- was given 2 ml of distilled water. The group K+ was given an injection of 20 mg/kgBW streptozotocin. Thegroup P1 was given an injection of streptozotocin 20 mg/kgBW and metformin 40 mg/kgBW. Thegroup P2 was given an injection of 20 mg/kgBWstreptozotocin and 200 mg/kgBW black cuminseed extract. Assessment of blood glucose levels is carried out using a glucometer. The results of the difference in the mean pre and post test blood glucose levels at K1, K2, P1, and P2 respectively were 2,34 mg/dl, 157,4 mg/dl, 251,33 mg/dl, and 185,33 mg/dl. This study used a one-way ANOVA parametric test (p<0.05) and continued with the post-hoc Bonferroni test (p<0.05). Administration of black cumin seed extract (Nigella sativa) can have effect of reducing blood glucose levels in white rats (Rattus norvegicus) induced by streptozotocin.

Abstract

Pendahuluan: Menurut United Nation Office Drug and Crime Indonesia termasuk ke dalam 10 negara dengan tingkat peredaran narkoba tertinggi, terutama ekstasi dan ganja. Berdasarkan Indonesia Drug Report provinsi dengan kasus tindak pidana narkoba tertinggi ialah Sumatera Utara sebanyak 7.353 kasus, Jawa Timur 6.193 kasus, DKI Jakarta 4.749 kasus, Jawa Barat 2.494 kasus, Sulawesi Selatan 2.120 kasus. Jenis NAPZA yang banyak disalahgunakan adalah sabu, ganja dan ekstasi dengan pelaku didominasi berjenis kelamin laki-laki. terpapar penyalahgunaan NAPZA. Metamfetamin atau sering dikenal di Indonesia dengan sebutan sabu. Sabu dapat menyebabkan ketergantungan, masalah fisik (meliputi malnutrisi, denyut jantung meningkat, gangguan ginjal, hepatitis, HIV-AIDS bagi mereka yang menggunakan suntikan amfetamin), masalah psikiatri (perilaku agresif, kondisi putus obat yang menyebabkan lelah, gangguan tidur, serangan panik, confusional state, psikosis dan skizofrenia bahkan gangguan kepribadian. Kesimpulan: Pasien dalam laporan kasus ini mengalami gangguan kepribadian antisosial dikarenakan adanya konsumsi metamfetamin. Pemicu yang mendasari gangguan kepribadian antisosial ini dikarenkan adanya konsumsi pemakaian zat adiktif yang berulang. Faktor lain terkait perkembangan psikososial sejak perinatal atau di dalam kandungan hingga masa dewasa awal sehingga mempengaruhi kondisi pasien saat ini. Abstract

Carpal tunnel syndrome (CTS) is a syndrome that arises due to neuropathic pressure on the median nerve in the carpal tunnel at the wrist which is precisely under the flexor retinaculum. Risk factors for CTS include obesity, sedentary wrist activity, pregnancy, genetic heredity, and rheumatic inflammation. So that proper treatment is needed and carried out holistically with a family medicine approach to achieve therapeutic success. Application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered and family problem-solving framework. Primary data were obtained through anamnesis, physical examination, supporting examinations and home visits to assess the physical environment. Patient Mrs. S, 49 years old, came with complaints that the thumb, index finger and middle finger of the right hand felt numb and stiff since one month ago. The patient is worried that the complaint will worsen and interfere with activities. The patient was clinically diagnosed with CTS (ICD-10 G56.0). Furthermore, management, intervention and evaluation are carried out. On evaluation, there were reduced complaints, increased patient knowledge about the disease and changes in behavior in the position of doing daily homework. After carrying out holistic and comprehensive management, patients experience an increase in knowledge about their disease. Complaints of numbness and stiffness in the patient's right hand are reduced, the patient and his family do homework in the right way and position.

Abstract

Herpes zoster (HZ) is an acute vesicular eruption caused by reactivation of latent varicella-zoster virus (VVZ) in sensory ganglia previously exposed to primary varicella infection. The incidence of HZ increases with age but many case reports show that herpes zoster can also occur in adolescents and even in children. The purpose of this case report is to implement a holistic and comprehensive family doctor service by identifying risk factors, clinical problems, and patient management based on evidence-based medicine with a patient-centered and family approach. This study is a case report study. Primary data were obtained through history taking (autoanamnesis from the patient and alloanamnesis from the patient's family), physical examination and home visits. Secondary data were obtained from the patient's medical record. Assessment is based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. Patient Ms. K, 7 years old, has been diagnosed with uncomplicated herpes zoster, has concerns about his health condition, the patient feels inferior and is afraid that the disease cannot be cured and is afraid of transmitting it to other family members. The patient hopes that the complaints disappear completely and do not recur, so they can be comfortable with their activities as usual. The patient's limited knowledge of the illness and an unfavorable lifestyle are factors that cause problems that occur in patients and their families. The diagnosis of herpes zoster, in this case, was in accordance with several theories and a critical review of recent studies. Management has been given in accordance with the management guidelines. There is good progress in patients starting from decreased clinical symptoms to changes in behavior after an intervention based on evidence-based medicine that is family-approach and patient-centered.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) and Hypertension is a non-communicable disease which become a health problem globally and in Indonesia. COPD and Hypertension can be controlled through a healthy lifestyle which support from family and community is needed, therefore it is important to carry out holistic management through a family approach to patients. This study is a case report. Primary data were obtained through anamnesis from the patient's family, physical examination, and home visits to assess the physical environment. The assessment is based on a holistic diagnosis from the beginning, process, and end of the study. The results, Mr. E is 64 years old, with COPD and history of uncontrolled hypertension in the past 10 years. Internal risk factors include a history of smoking in the past 30 years and lack of knowledge about the disease. External risk factors are the lack of knowledge of the patient's family about the patient's disease, smoking behavior in the home by other family members and the patient's workplace with a lot of exposure to smoke. Interventions were carried out in the form of education. After the intervention, changes in patient knowledge, blood pressure, CAT Score and lifestyle were obtained. The diagnosis in this patient complies with the national guidelines. The management given is in accordance with the guidelines, there is a change in knowledge of the patient and the patient's family after the intervention is carried out by providing counseling based on evidence-based medicine which is patient centered and a family approach.

Abstract

Gout arthritis is a progressive disease due to the deposition of monosodium urate crystals in joints, kidneys, and other connective tissues due to chronic hyperuricemia. This disease needs to be managed comprehensively so that patients must know about their disease and have awareness and can make lifestyle modifications so that treatment goals can be achieved, prevent complications that may occur, and improve the patient's quality of life. This can be realized with family medicine-based management. The aim of this study is to identify internal, external risk factors and clinical problems found in patients, apply a holistic and comprehensive family doctor approach according to the problems found in patients, and carry out evidence-based medicine management that is family-approach, patient-centered and community oriented. The study is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record. A 50-year-old female patient came with complaints of pain in the both knees. The patient has been diagnosed with uncontrolled gout since 2 months ago. In the knowledge assessment, the initial score before the intervention was 5 (low) and after the intervention it was 9 (high). In the aspect of uric acid levels there is a decrease in uric acid levels from 9,3 mg/dL to 5,6 mg/dL. In the aspect of diet there is a change in the type of food that is appropriate. In the aspect of physical activity there is a change in physical activity of 30 minutes of exercise per day. Enforcement of diagnosis and management of these patients has been done holistically, patient centered, family approach and community oriented based on several theories and the latest research. In the process of behavior change, the patient has reached the trial stage.

Abstract

Hypertensive heart disease (HHD) is a term applied to refer to heart disease as a whole, starting from left ventricular hypertrophy (LVH), cardiac arrhythmias, coronary heart disease, and chronic heart disease, which are caused by increased blood pressure, either directly or indirectly. The number of patients with Hypertensive Heart Disease (HHD) is not known with certainty, but in several studies it is stated that people with hypertension will develop heart disease. Often patients fail in treatment because of their inability to modify their lifestyle, so doctors must treat patients holistically from various aspects. Application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered and family problem-solving framework. The study conducted was a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from patient medical records. Patient Mrs. S, 65 years old, came with complaints that discomfort when breathing and frequent palpitations. Known to suffer from hypertension since 18 years ago and a history of hospitalization due to a previous heart disease. The patient had been taking antihypertensive medication but stopped it because he felt he was fine. The patient is worried that the complaint will worsen and interfere with activities. The patient was clinically diagnosed with HHD. Furthermore, management, intervention and evaluation are carried out. The diagnosis and management of these patients has been carried out holistically, patient centered, family approach and community oriented based on several recent theories and research. In the process of behavior change, the patient has reached the trial stage.
