Abstract

Indonesia is the 7th country with the highest prevalence of diabetes,90% of patients who suffer from diabetes mellitus (DM) throughout the world are DM type 2. Preparation of Medical Base Evidence-based tasks in male patients with risks and problems as well as patient management based on patient responsibility and centralized and family patient approach. This study is a case report. Primary data were obtained through anamnesis (autoanamnesis), physical examination, and home visits. Secondary data obtained from the patient's medical records. Assessment is based on diagnosis from the beginning, process, and end of qualitative and quantitative studies. Mrs. W 50 years old patient was diagnosed with Diabetes Mellitus Type 2 accompanied by hypertension. Patients have aspects of internal risk in the form of patients having a poor diet, patients having poor lifestyle, lack of knowledge of patients about controlling and monitoring DM, family history factors of hypertension, and patients' curative treatment. External aspects of the family psychosocial risk in the form of a lack of understanding of the patient's disease, as well as patient family members less motivating patients to maintain a regular diet according to the DM diet, controlling blood sugar and blood pressure, and regular treatment. Holistic management with a family doctor approach can improve knowledge, attitudes, and behavior in patients. Also, the role of the family is very important in the care and treatment of sick family members.

Abstract

Anemia is one of the health problems in the whole world, especially in developing countries where an estimated 30% of the world's population suffer from anemia. The application of Evidence Base Medicine based services on adult male patients by identifying risk factors and clinical problems and management of patients based on the framework of problem solving and the patient centered approach and family approach. Assessment is based on a holistic diagnosis from the beginning, process, and end of study in a qualitative and quantitative manner. Men aged 46 years, who had been diagnosed with Iron deficiency Anemia had a fear that their complaints would get worse and not improve because after a laboratory examination of Hb patients was 9.8 gr / dl. Limited knowledge about the patient's health conditions in the form of factors that cause anemia, complications from anemia, prevention of anemia, foods that need to be avoided as well as lack of motivation and family encouragement to bring control patients to the nearest health care facility.After a family physician approach is made to anemic patients, there is a change in knowledge about anemia.

Abstract

The prevalence of hypercholesterolemia in Indonesia at the age of 25-34 years is 9.3% and increases with age up to 15.5% at the age of 55-64 years. Hypercholesterolemia is generally found in women (14.5%) than men (8.6%). This case report aims to identify risk factors and patient clinical problems to improve the patient's quality of life and carry out holistic and comprehensive management based on Evident Based Medicine which is family-approach and patient-centered. Primary data were obtained through anamnesis, physical examination and home visits. Secondary data were obtained from patient medical records. The assessment was carried out based on a qualitative and quantitative initial to final holistic diagnosis so that data were obtained in the form of a female patient aged 47 years, diagnosed with hypercholesterolemia and grade II obesity. Limited knowledge regarding the prevention of hypercholesterolemia and grade II obesity, foods that need to be avoided and the lack of motivation and family encouragement to bring patients to control their health regularly are factors that cause problems that occur in patients. Management in the form of simvastatin 1x10 mg and counseling about the patient's health condition. Then the evaluation was carried out by checking total cholesterol, the results of the pre-test 40 and the post-t test 80. There was an increase in the knowledge score including how to calculate ideal body weight, normal total cholesterol value in the body, normal value of BMI, and factors that cause increased cholesterol in the body. Management with a family medicine approach is proven to increase patient knowledge about their health condition and change their life behavior.

Abstract

Erysipelas is a relatively common infectious disease of the skin form of erythema which is clearly demarcated with elevated margins with signs of acute inflammation. The incidence of erysipelas in 2012 in Surabaya was 14 cases with the most cases in women and in the age group 45-64 years. Erysipelas is a disease that can be treated in primary health care facilities or general practice. But there are risk factors for this disease that cause frequent cases of recurrent erysipelas so that proper management is needed. This study is a case report which aims to identify internal and external risk factors and clinical problems in patients, as well as holistically manage patients by applying family doctors according to evidence based medicine through a patient centered approach and family approach. Primary data were obtained through history taking (autoanamnesis from patient and alloanamnesis from patient's family) physical examination and home visits. Secondary data were obtained from the patient's medical record. Assessment is based on a holistic diagnosis from the beginning, process, and end of study in a qualitative and quantitative manner. Patient Mr. W age 42 years old has been diagnosed with erysipelas having concerns about health conditions that are considered to interfere with his work and daily activities. Limitations on the patient's knowledge of the disease (cause, mode of transmission, risk factors, treatment, and complications). In addition, patients pay little attention to personal hygiene, especially related to the risk of hygiene in their work. Holistic and comprehensive management of patients' problems Mr. W has been done by providing counseling to improve patient and family knowledge of erysipelas.

Abstract

Rheumatoid Arthritis (RA) is an autoimmune disease characterized by erosive, symmetrical synovitis and requires long-term treatment so family support is needed in its management. This study is a case report. Primary data obtained through history taking, physical examination, and home visits. Secondary data were obtained from the patient's medical record. At the first visit, the patient's complaint was pain in the proximal interphalangeal joint (PIP) digital I-V dextra et sinistiti. Internal risk factors namely lack of knowledge about the causes of RA, precipitating factors, how to deal with acute symptoms, diet and exercise are recommended. External risk factors in patients are families who pay less attention to diet and exercise recommended to patients and do not use shoes and waterproof gloves and do repetitive movements without resting when farming. The second visit was to be intervened with the family conference method using poster media. The third visit found complaints of reduced joint pain because the patient took piroxicam 10 mg and applied liniment to the painful location. Patients have been consuming fruit and milk regularly and exercising muscle stretches three times a week for 30 minutes. Families remind diet and exercise accordingly. When farming use shoes and waterproof gloves and rest between repetitive movements. There is an increase in patient knowledge about the causes, precipitating factors, how to deal with acute symptoms, diet, and exercise that is appropriate for RA, characterized by an increase in post-test scores of 40.

Abstract

Urticaria is skin disease characterized by the sudden appearance of wheals and/or angioedema. Chronic urticarial is characterised by the recurrence of hives/angioedema for >6 weeks. The disease is a common and important clinical problem in care and treatment so that it becomes a diagnostic and therapeutic challenge. This study is a case report. Primary data obtained through history taking, physical examination, and home visits. Secondary data were obtained from the patient's medical record. At the first visit, the patient’s complaints were obtained which appeared reddish bumps on the neck, chest, back, both arms and thighs. Internal risk factors in patients are the lack of patient knowledge about the disease they are experiencing, as well as the causes/factorsat influence it and how to prevent the disease, curative treatment patterns, lifestyle: frequent swimming in the river during the day, bathing habits that are less clean and irregular . External risk factors for patients are families seeking medical treatment if complaints are already disturbing (hygiene around the outside of the house is lacking), neighborhoods and playgrounds in dense residential areas surrounded by rice fields. The second visit was an intervention with a family conference method using pictorial leaflet media. The third visit showed complaints of redness and red itching due to the patient taking loratadine 10 mg and dexamethasone 0.5 mg. Patients already have better lifestyle patterns such as maintaining personal hygiene and the environment. There is an increase in patient knowledge about the disease experienced, as well as the causes / factors that influence it and how to prevent the disease experienced, characterized by an increase in post-test scores by 6 points.

Abstract

The study aims were to (1) assess the nutritional status, adequacy level, and physical fitness of the nonpregnant reproductive female workers, and (2) analyze the effect of micronutrients supplementation on their iron status and physical fitness. This research employed experimental design of double blind, completely randomized design by involving 338 households of female workers. The experimental unit consists of 39 female workers whose hemoglobin level were 80-125 g/L. However, five of them were drop out due to pregnancy or sickness. The treatments applied in the study were three capsules containing: (1) iron and folic acid (BF), (2) multi-vitamin and mineral (MVM), which contains of 15 different vitamins and minerals, and (3) placebo (P). The body mass index of the female workers were thin 12 percent, normal 70 percent and overweight 18 percent. Most of their nutritional intake of vitamin C, calcium and iron were deficient but they had a good to superior of physical fitness. Abstract

The number of TB cases in Indonesia in 2019 reached 543,874 cases, of which 11.92% of the total cases in Indonesia were aged 0-14 years. In the management of TB patients, especially TB in children, it really needs cooperation from various parties, one of which is the role of the family doctor. The study is Case Report. Primary data was obtained through history taking and physical examination, filling out family folders, and filling in patient files. Assessment was carried out based on the initial holistic diagnosis and end of the visit qualitatively and quantitatively using foodrecall. Based on a holistic diagnosis, patients complain of coughing that does not improve for 1 month and has weight loss. The patient is concerned that the illness cannot be cured and can spread to other people. Patient perceives that TB is dangerous diseases. Patient hopes the child can get the treatment completely so that he can recover again. Patients diagnosed with new case of pulmonary tuberculosis in child that diagnosed clinically based on the results of history, physical examination, and rontgen. The patient has a history of contact with child TB. The diagnosis of pediatric pulmonary TB in this case is in accordance with the theory and critical analysis of current research, using TB scoring system. The management given is in accordance with the guidelines, there are changes in behavior and knowledge of healthy life and dietary habit in patients and  families of patients after an intervention based on evidence based medicine that is patient centered, family approach.

Abstract

Dermatophytosis is a disease cause by the colonization of dermatophyte fungi that attack  keratin-containing tissues such as the stratum corneum of the skin, hair and nails in humans. The prevalence of dermatophytosis in Asia reaches 35,6%.  In Indonesia, dermatophytosis is 52% of all dermatomycoses, which is most are tinea cruris and tinea corporis.  The study was Case Report study.  Primary data is obtained through history taking  and physical examination by making home visit, filling out family folders, and filling in patient files.  Assesment is carried based on the initial holistic diagnosis, process, and end of the visit quantitatively and qualitatively.  Lack of patient knowledge about the disease and poor personal hygiene of patient are the main internal risk factors causing patient to have gray patch type of Tinea capitis.  The intervention that was given is education and counseling  of the patient and patient family about matters that must be modified and the prevention about the disease.  The intervention divided into patient center, family focus and community oriented.  After holistic intervention with educational and counseling methods, there are decrease in clinical symptoms and behavioral changes was obtained by maintaining personal hygiene and the environment

Abstract

Tuberculosis (TB) is a disease caused by infection with the bacterium Mycobacterium tuberculosis in the lungs. This disease usually attacks the lungs (pulmonary TB) but can also affect other organs (extrapulmonary TB). This study is a case report with analysis of primary data obtained through history taking, physical examination and home visits to complement family data, psychosocial and environmental data. Secondary data were obtained from the patient's medical record at the puskesmas. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. Patient An. N, 2 years old with TB and poor nutritional status, so there needs to be an intervention to improve the patient's nutritional status to support the success of TB treatment. In this case, a diagnosis of tuberculosis was made in accordance with the theory and technical guidelines for the management and management of TB in children, the Ministry of Health of the Republic of Indonesia. The management of category 1 pulmonary tuberculosis given was in accordance with evidence based medicine. After the intervention, there was a decrease in clinical symptoms and behavioral changes in the patient's family, seen after the family was given the intervention and finally understood better to motivate the patient and always remind and accompany the patient when taking medication, as well as providing food with balanced nutrition to the patient. The management given to the patient was in accordance with the technical instructions for the management and management of TB in children, the Ministry of Health of the Republic of Indonesia.
