Abstract

Malnutrition in Pregnancy (ICD 10-O25) is a malnutrition problem cause by the lack of food or nourishment intake in a long period of time, up to years. One of the potential problem in long term macro malnutrition, specifically in pregnant women with ICD 10-O25 is give birth to Low Weight Baby (LWB), anemia and decreased immunity. In Indonesia, there are 17.3% pregnant women with ICD 10-O25. Lampung in particular, there are 13.6% pregnant women with ICD 10-O25. Therefor, it is crucial to comprehensively manage nutrition status, especially in pregnant women. Identify internal and external risk factors and clinical problems present in the patient. Apply a holistic and comprehensive family doctor approach according to the problems found in the patient and carry out medicine-based management. This study is descriptive study about case report. The primary data obtained through anamnesis, physical examination and a visit to the home. Secondary data were obtained from medical records of patients. The patient is a pregnant women with ICD 10-O25. The internal risk factor aspect in the patient is the patient's unhealthy eating and drinking patterns. Aspects of the patient's external factors due to lack of support and information from the family and the environment. Patients are given information about CED and healthy eating patterns. In addition, patients are assisted in applying their knowledge. The diagnosis enforcement and management with this patient has been done with family medical approach, several theories and latest researches. After intervention to the patient, the patient has better understanding and attempted to apply the knowledge. Furthermore, the patient’s Mid Upper Arm Circumference (MUAC) has been increased in the last evaluation visit.

Abstract

Acquired Prothrombin Complex Deficiency (APCD) is a state when blood coagulation cascade is disrupted due to lack of vitamin K levels in the body that often occurs in infants and neonates. APCD often manifest as a spontaneous intracranial bleeding among neonates which has given numbers to child morbidity and mortality rate globally. The main aim of this case report is to talks about a case of a baby girl age 1 month and 8 days whose referred to RSUD dr. H. Abdul Moeloek, Lampung Provence, with history of seizure and loss of consciousness. Physical examinoation has shown the anterior fontanels was tense, pale conjunctivas, and anisochor pupils with minimal light reflex. Blood works have shown lower Hb, longer PT and aPTT, other aspects within normal limits. Ultrasound of the fontanel and brain CT without contrast have shown intracranial bleeding. History of prophylactic vitamin K is unknown. After craniotomy procedure and few pharmacological treatment has given this patient gained clinical improvement. Laboratory findings in PT and aPTT also improved after intravenous vitamin K was administered for 3 consecutive days .

Abstract

Twin pregnancies are high-risk conditions often accompanied by various complications, one of which is premature rupture of membranes (PROM). PROM is the rupture of the amniotic membrane before the onset of labor, occurring either before 37 weeks of gestation (preterm) or after (term). PROM significantly contributes to preterm births, a leading cause of global neonatal mortality. In Indonesia, the incidence of PROM increased from 13.1% in 2020 to 14.6% in 2021. This case study reports on a 36-week pregnant woman with twins who experienced PROM and successfully delivered vaginally with favorable outcomes for both the mother and neonates. The diagnosis of PROM was confirmed through anamnesis, physical examination, and supporting tests. Management included intensive observation, intravenous fluid administration, antibiotics, and antenatal steroids for fetal lung maturation. Previous research indicates that twin pregnancies significantly increase the risk of PROM. Key risk factors for PROM include a history of PROM, short cervical length, and low socio-economic status. Management of PROM in twin pregnancies involves the administration of antenatal steroids, latency antibiotics, magnesium sulfate for neuroprotection, and rigorous clinical and ultrasonographic monitoring. Diagnosis is performed via speculum examination and immunochromatographic tests. In conclusion, appropriate and effective management of PROM in twin pregnancies is crucial for improving pregnancy outcomes and reducing the risk of complications for both mother and infants.

Abstract

Abortion refers to the termination of pregnancy before 20 weeks of gestation or when the fetal weight is below 500 grams, typically due to fetal non-viability outside the uterus. Globally, there are over 2.3 million cases reported annually. Incomplete abortion is characterized by the presence of an open cervical ostium and bleeding, where all the products of conception are incompletely expelled from the uterus or do not correspond with the expected gestational age. This case study reports on incomplete abortion in a 26-year-old nulligravid woman presenting with vaginal bleeding. Clot discharge indicated expulsion of conception products, leading to a diagnosis of incomplete abortion at 12 weeks and 9 days of gestation (G1P0A0). Currently, misoprostol remains the preferred pharmacological intervention as Drug of Choice in cases of incomplete abortion. However, in practice, there are several ways to administer misoprostol, either sublingually, orally or vaginally. Several health organizations have their own protocols for administering misoprostol for cases of incomplete abortion. This case report will discuss misoprostol administration in patients with incomplete abortion.

Abstract

Diabetes mellitus (DM) is one of the highest causes of death in the world whose prevalence increases every year, DM is characterized by elevated blood glucose levels and damage of the pancreas. Streptozotocin is a drug used to induce diabetes mellitus in experimental animals, single high dose administration can cause type 1 DM. Garlic has various component that have the potential as antidiabetic, anti-inflammatory and antioxidant. This study aim to determine the effect of garlic extract (Allium sativum) on blood glucose levels and pancreas histopathological images in white rats (Rattus norvegicus) Sprague-Dawley strain induced by streptozotocin. This research is an experimental study with a post test only control group design. This study used 24 rats and divided into 4 group, K- (normal group), K+ (STZ 60 mg/kgBB), P1 (STZ + garlic extract 500 mg/kgbb), P2 (STZ + garlic extract 750mg/kgBB). Blood glucose levels were assessed with a glucometer and the histopathological images of the pancreas was assessed by counting the number and size of the langerhans islet. The mean blood glucose levels for each group were K- = 73.3mg/dL, K+ = 400 mg/dL, P1=203.8 mg/dL, and P2=132.3 mg/dL. One Way ANOVA test obtained p value = 0.000. The mean number of langerhans islet in 10 fields of view obtained K- = 26.1; K+ = 4.6; P1= 7.8; P2=15.1. One Way ANOVA test obtained p = 0.000. The mean area of ​​the langerhans islet in each group obtained K- = 19092.4 µm2; K+ = 4759.3 µm2; P1=8561,4 µm2; P2=9621.8 µm2. Kruskal-Wallis test obtained p = 0.000 (p <0.05). There is an effect of garlic extract (Allium sativum) on blood glucose levels and pancreas histopathological images in white rats (Rattus norvegicus) Sprague-Dawley strain induced by streptozotocin.

Abstract

Diabetes Mellitus is a degenerative disease with a high prevalence and increasing every year. There is a possibility that there will be an increase in the number of people with Diabetes Mellitus in the future, so that comprehensive management must be carried out which is expected to prevent complications and improve the quality of life of patients. Application of evidence-based medicine-based services to patients by identifying risk factors, clinical problems, and patient management based on a patient-centered problem-solving framework with a patient-centered, family-focused, and community-oriented approach. This study is a case report. Primary data were obtained through anamnesis, physical examination and home visits to assess the physical environment. The assessment is based on a holistic diagnosis of the initial, process, and final studies qualitatively and quantitatively. After the intervention, there was an increase in the knowledge of patients and their families regarding the importance of controlling health workers, taking medication regularly, eating patterns and physical activities that are suitable for people with Diabetes Mellitus. Holistic management has been carried out using the family doctor approach, Mrs. S, 52 years old with Type 2 Diabetes Mellitus adjusted based on initial holistic diagnostics. The interventions carried out have increased patient and family knowledge regarding the importance of control to health workers, regular drug consumption, dietary patterns and physical activity as indicated by improvements in the final holistic diagnostic.

Abstract

Non-communicable diseases (NCDs) and other degenerative diseases are still a major national health problem in Indonesia. One of the NCDs that continues to increase in Indonesia is Diabetes Mellitus cases. Morbidity and mortality rates due to type 2 diabetes and tuberkulosis (TB) are increasing among people with type 2 diabetes. According to WHO data, people with type 2 diabetes have a three times higher chance of contracting tuberkulosis (TB) than people without the disease. The aim of the family approach in the treatment of pulmonary tuberkulosis in diabetes mellitus sufferers is to determine the clinical, psychological and psychosocial determinants that influence the family. By implementing comprehensive family doctor services that are based on evidence-based medicine and a patient-centered approach. A 53 year old patient, who is the head of a family with teenage children and a school member whose only income is from the wife of a trader at school, has complaints of a cough that has not healed since the last month accompanied by intermittent fever, itchy throat, decreased appetite, body weight, and tingling in the hands and feet with a history of suffering from diabetes mellitus for 15 years. The patient was found to be suffering from diabetes and a new case of pulmonary tuberkulosis. The patient has difficulty carrying out activities and needs the role of family such as the patient's wife and children to help heal the patient's condition. After the intervention, the patient's fasting blood sugar decreased, clinical symptoms improved, and the level of knowledge increased. The patient's diagnosis of a new case of pulmonary tuberkulosis with type 2 diabetes was made according to recommendations. After implementing the patient-centered approach, family approach intervention, the patient's level of knowledge changed. As a result of implementing a comprehensive management strategy led by a family doctor, patient and family understanding of type 2 diabetes mellitus and pulmonary TB has increased. So that from the results of this research and intervention, it is hoped that patients and families will receive comprehensive services both in terms of physical, psychological health and level of knowledge with changes in behavior in managing the patient's illness.

Abstract

Prolonged labor or what is called dystocia is a condition where the labor process experiences difficulties with slow progress. Signs indicating dystocia include a low-lying fetal presentation that fails to enter the pelvis on Leopold's examination, slow cervical dilatation, and a loose presenting part into the cervix. There are several factors that can cause dystocia including the strength of uterine contractions (power), condition of the birth canal (passage), position and size of the fetus (passenger), number of previous pregnancies (parity), premature rupture of membranes, mother's age, distance between pregnancies, social conditions economy, and the level of response to stress. A gap between pregnancies that is too long (>10 years) can increase the risk of dystocia during delivery. This study is a case report. Data was obtained from a 35 year old woman who complained of being at term pregnant with heartburn since 1 day before entering the hospital (SMRS). The patient admitted that abdominal pain had spread to the waist since 1 day before entering the hospital (SMRS). The patient then had an examination with the midwife and it was said to be opening 2, then the patient was sent home. On June 4 2024 at 11 am the patient underwent an examination with a gynecologist and was given misoprostol 50 mcg and then the patient complained of increasingly frequent heartburn. At 15.30 the patient was checked again by the midwife and found that it was 4 cm dilated, it was observed until 22.30 and it remained 4 cm dilated, then the patient was referred to RSAM. The patient admitted that she was pregnant at term and still felt the baby's movements. Pregnancy spacing is the most important factor in these patients. This pregnancy is the patient's second pregnancy, the first pregnancy was in 2012 which was born at term, vaginally, male with a birth weight of 2800 grams. Several previous studies have shown that there is a relationship between pregnancy spacing and the incidence of prolonged labor, but there is no theory that is considered absolutely correct to date. Therefore, intervention is needed for early prevention and early management of prolonged labor, especially regarding prevention of too long a pregnancy distance.

Abstract

The prevalence of stunting in Indonesia was 37.2% in 2013, 30.8% in 2018, and 27.67% in 2019. The number of stunting was indeed decreasing, but based on World Health Organization standards, the prevalence of stunting in a country is classified as high if it exceeds 20%. This shows that the prevalence of stunting in Indonesia is still high. Inadequate breastfeeding practice in the first 1.000 days of life is one of the factors that play a role in the incidence of stunting. This research is an analytic study with a cross sectional approach. The sample used comes from secondary data from Riskesdas 2018 with a total samples of 36,259. The dependent variable is the incidence of stunting and the independent variable consists of colostrum administration, exclusive breastfeeding, and weaning age. Data analysis was performed by bivariate test using chi-square. The results showed that there was a significant relationship between colostrum administration (p-value = 0.009), exclusive breastfeeding (p-value = 0.001), and weaning age (p-value = 0.001) with the incidence of stunting in children under the age of 12- 23 months. There is a significant relationship between colostrum administration, exclusive breastfeeding, and weaning age with the incidence of stunting in children under two years old aged 12-23 months. The risk factors for exclusive breastfeeding and weaning age have the same influence on the incidence of stunting in children under two years old aged 12-23 months in Indonesia.

Abstract

Bell palsy is an acute paralysis of the peripheral facial nerve (N. Clinical manifestations caused by Bell's palsy can include impairment, disability, handicap, and entertainment, and can cause mental problems for sufferers. Applying the principles of family medicine services and carrying out holistic management based on a framework for solving patient problems with a patient-centered, family-focused, and community-oriented approach based on Evidence-Based Medicine. This study is a case report using primary data obtained through history taking, physical examination, and home visits to complement family data, and psychosocial and environmental data. Secondary data was obtained from patient medical records. Patient An. TM aged 18 years with complaints of stiffness on the right side of the face since two days ago. The patient has a habit of riding a two-wheeler without using head protection. The patient also had a history of varicella two years ago. Holistic management with a family doctor approach has been carried out for An.TM aged 18 years with Bell's palsy which was adjusted based on the initial holistic diagnosis. Before the intervention, the patient and family are given a pretest to measure the patient's level of knowledge regarding the disease they are suffering from. During the intervention, the patient is explained about the disease he is suffering from using media in the form of posters, and the patient is given education regarding the importance of completing treatment, how to prevent the disease, and doing facial exercises. At the last visit, a posttest was given to the patient and family with the results being a significant increase in knowledge about the disease they were suffering from and awareness of the importance of changing patient habits.
