Abstract

Tonsillitis is an inflammation of the palatine tonsils which are part of Waldeyer's ring. The spread of this infection can occur through the air and hands. It can occur at any age, especially in children. Tonsillitis has been one of the causes of decreased academic achievement and absenteeism from school. This study is a case report. Primary data were obtained through anamnesis (autoanamnesis and alloanamnesis from family and patients), physical examination, and home visits to assess the physical environment. The assessment is based on a holistic diagnosis of the initial, process, and final studies qualitatively and quantitatively. The result obtained is patient An. F, 5 years old with a diagnosis of chronic tonsillitis. Internal risk aspects include the age of the child, patient knowledge, clean and healthy lifestyle, dental and oral hygiene, and poor eating patterns. External risk aspects that affect the patient's condition include the lack of family supervision regarding the consumption of patient snacks, and the slow decision making of follow- up actions from patient complaints, the patient's family does not know about patient’s disease. Furthermore, holistic management of patients and families through the intervention of colored poster media in the form of information about the disease, triggers, and complications. Holistic and comprehensive management of chronic tonsillitis patients with patient centers and family focus methode according to EBM is needed so that they can be managed with therapy and change patient behavior so as to prevent complications.

Abstract

Low Back Pain (LBP) is one of the most common musculoskeletal disorders encountered in primary healthcare settings. The etiology of LBP is diverse but generally associated with poor posture, heavy or repetitive physical activity, degenerative processes, and psychosocial factors. In Indonesia, the prevalence of LBP is estimated to range from 7.6% to 37% of the general population, with a high proportion linked to physically demanding occupations. Diagnosis of LBP involves a comprehensive history-taking, systematic physical examination, and consideration of supporting investigations if red flags such as fractures, infections, or malignancies are suspected. Identifying the pain source, duration, characteristics, and distribution is crucial in establishing the diagnosis and determining the underlying cause. In clinical practice, a holistic and evidence-based approach by primary care or family physicians is highly recommended. This should include the assessment of internal risk factors such as advanced age and degenerative disease, as well as external factors like a history of strenuous work and physical inactivity. The case of Mr. S, a 73-year-old male, illustrates chronic LBP with worsening symptoms over the past month. The pain radiated to the right buttock and lower limb, leading to functional impairment and limitations in daily activities. A thorough evaluation using the biopsychosocial model and multidisciplinary management—including pharmacotherapy, patient and family education, targeted physical exercises, and lifestyle modifications—resulted in significant symptom improvement and increased patient awareness of the condition.

Abstract

Diseases in Indonesia can generally be divided into communicable diseases and non-communicable diseases. Based on Riskesdas data, the number of DM suffers in Indonesia is increasing every year. In Indonesia, the prevalence of dislipidemia increase to 15.5% in the 55-64 year age group. Of these, 80% of patients died suddenly due to a heart attack, and 20% showed no symptoms. Patients must know about their disease and have awareness and motivation to make lifestyle modifications so that treatment goals can be achieved, so management is needed through a family approach. Application of evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and patient management based on the patient's problem-solving framework with a patient centered approach and a family approach. This study is a case report. Primary data were obtained through anamnesis, physical examination and home visits to assess the physical environment. Secondary data were obtained from patient medical records. Assessments are based on a holistic diagnosis from the start, process, and end of the study in a qualitative and quantitative manner. Mrs. I, 54 years old with type 2 diabetes mellitus (DM) and dyslipidemia, was worried that his complaints would get worse and not improve. Limited knowledge about the patient's health condition in the form of causative factors, prevention of type 2 DM and dyslipidemia, foods that need to be avoided and the lack of motivation and encouragement from families to bring patients for routine health control are factors that cause problems that occur in patients and families. Holistic and comprehensive management of the problems patient named as Mrs. I  has been carried out by providing counseling to increase the knowledge of patients and their families.

Abstract

Otitis externa is an infectious or noninfectious inflammation that occurs in the outer ear.  Otitis externa can be acute, chronic, diffuse, and malignant caused by bacterial, fungal, or viral infections. The main predisposing factor is the swimming habit which can increase 5 times compared to people who do not swim. Factors that may increase the risk of infection, especially in humid climates or if there is narrowing of the ear canal. This article provides an in-depth review of the pathophysiology, diagnosis, management, and prevention strategies of otitis externa using the literature review method from reliable scientific sources. The data shows that the management of otitis externa involves the use of analgesics, topical antibiotics, oral antibiotics, and systemic antibiotics administered based on their classification, while prevention includes avoidance of precipitating factors and cerumen treatment. This conclusion emphasizes the importance of appropriate prevention strategies and accurate diagnosis to improve treatment outcomes and reduce the risk of recurrent infection.

Abstract

Hypertension in pregnancy is defined as an increase in blood pressure ≥140/90 mmHg that appears after 20 weeks of gestation without a history of previous hypertension. This case report discusses the case of a 39-year-old woman (G3P2A0) with a gestational age of 32 weeks who was diagnosed with gestational hypertension. The patient was referred from a health center with a history of high blood pressure during ANC (Antenatal Care) and recurrent headaches since 20 weeks of gestation. Examination showed blood pressure of 157/91 mmHg without signs of severe preeclampsia, while proteinuria +1 was detected. Risk factors include age >35 years and a history of hypertension in previous pregnancies. The patient was given pharmacological therapy, namely nifedipine 10 mg 1x1 orally with regular blood pressure monitoring. This study emphasizes the importance of early detection and management of gestational hypertension to prevent complications for the mother and fetus.

Abstract

Hemorrhoids are submucosal swellings in the anal canal, consisting of venous plexuses, small arteries, and dilated areolar tissue. in 2019, approximately 4.4% of the global population suffered from hemorrhoidal disease. Data from WHO in 2017, the number of hemorrhoids cases reached 230 million people. In 2017, the global incidence of hemorrhoids increased to 54%. In Indonesia, based on the Basic Health Research (Riskesdas) data, it is estimated 2030, the number of individuals suffering from hemorrhoids will reach 21.3 million. The objective of this study is to identify internal and external risk factors, as well as clinical problems in patients, and to apply Evidence-Based Medicine (EBM) in the family doctor services for these patients. The management of patients was conducted based on a patient-centered, family-focused, and community-oriented problem-solving framework. Primary data were obtained through autoanamnesis, physical examination, and home visits, while secondary data were gathered from patients' medical records. The evaluation was based on a holistic diagnosis from the beginning to the end of the study, both qualitatively and quantitatively. Patient Mr. T, 42 years old, had a main complaint of bulging protruding mass from his anus that is exacerbated with defecation and bowel movements. accompanied by bleeding for the past week. The patient was concerned about the worsening of the condition. Clinically, the patient was diagnosed with Grade II Internal Hemorrhoids. A holistic management approach was implemented, including an intervention using a poster as a media. The diagnosis and management of this patient were carried out holistically and comprehensively, based on several theories and current research. During the behavior change process, the patient has reached the trial stage.

Abstract

Spondylolisthesis is the displacement of a vertebra relative to the vertebra below it, causing pain. While various risk factors for spondylolisthesis exist, few studies have explored which risk factors are specific to one gender. Therefore, this study aims to find the relationship between gender and parameters that are commonly considered risk factors for spondylolisthesis. This cross-sectional study was conducted from May to June 2024. A total of 35 patients with spondylolisthesis who underwent surgery at RSUP Fatmawati Jakarta were selected. Patient data were collected from medical records and screened based on inclusion and exclusion criteria. Of the patients, 45.7% were male and 54.3% were female. The data were analyzed using SPSS 16.0. There was a significant difference in the average weight and height between male and female spondylolisthesis patients (weight p=0.017<∝, height p=0.000<∝). There was no significant difference in age. Women with multigravida had the highest frequency (19) compared to primigravida (5), indicating a potential risk factor. No differences were found in joint laxity (p=0.968>∝), BMI, heredity (p=0.922>∝), or physical activity (p=0.076>∝) between genders. There are various risk factors for spondylolisthesis. This study found significant differences in weight and height between genders in spondylolisthesis cases, but no significant differences in other parameters. The study also suggests that multiparity may be a significant risk factor, although further research is needed to confirm this

Abstract

Irritant contact dermatitis (ICD) is a non-immunologic inflammatory skin disorder resulting from direct damage to the epidermal barrier by chemical, physical, or biological agents. ICD is more common than allergic contact dermatitis since most individuals are at risk of repeated exposure to irritants in daily activities, such as detergents. We report the case of a 54-year-old woman presenting with peeling, erythema, pruritus, and stinging pain on both hands after washing clothes with powdered detergent without protective gloves. Dermatologic examination revealed erythematous plaques with thick scales, diffuse borders, and fissures on the dorsal and palmar regions bilaterally. The diagnosis of ICD was established based on a clear exposure history, characteristic clinical features, and the absence of allergic manifestations. Management included ceramide-based moisturizer to restore skin barrier function, high-potency topical corticosteroid (clobetasol propionate 0.05%) to reduce inflammation, and oral antihistamine (cetirizine) to relieve pruritus. Patient education emphasized adherence to therapy, skin hygiene, and strict avoidance of repeated detergent exposure. Marked clinical improvement was achieved within a short period following combination therapy. This case highlights detergent exposure as an important cause of ICD among housewives and underscores the significance of preventive strategies, including the use of personal protective equipment, safer cleaning alternatives, and continuous patient education. Early recognition and comprehensive management are essential to reduce recurrence and improve patient quality of life.

Abstract

Juvenile cataract is one of the leading causes of visual impairment in children and adolescents, either due to congenital abnormalities or acquired factors. This condition may significantly reduce quality of life and carries a risk of amblyopia if not managed promptly. We report a 12-year-old boy presenting with blurred vision in the right eye for the past seven months, worsening within the last month. The patient complained of cloudy vision, difficulty seeing both near and far objects, and frequent glare. His mother noticed a whitish spot on the right eye. Ophthalmological examination revealed visual acuity of 1/300 in the right eye, leukocoria, whitish lens opacity, and a positive shadow test, while the left eye was within normal limits. The patient was diagnosed with juvenile cataract in the right eye and scheduled for cataract extraction with intraocular lens (IOL) implantation.

Abstract

Mesenteric artery pseudoaneurysm is a rare condition that can occur due to blunt abdominal trauma or postoperative complications. This lesion is often difficult to recognize early due to nonspecific symptoms, leading to diagnosis typically being made through imaging or intraoperative findings. Delayed management can result in serious complications, including bleeding and bowel necrosis. A 59-year-old female patient was reported to have arrived at the Emergency Department of Abdul Moeloeki Hospital with complaints of central abdominal pain for the past week, accompanied by nausea and vomiting. History of abdominal trauma was obtained from a fall in the bathroom one week prior, as well as a traffic accident one year ago. Physical examination revealed abdominal tenderness with muscle rigidity, decreased bowel sounds, and vital signs indicating tachycardia. Laboratory tests were within normal limits. Abdominal ultrasound showed a paraaortic cystic lesion. The patient then underwent exploratory laparotomy, where a pseudoaneurysm of the ileocolic artery was found along with necrosis of the ileum and cecum. A right hemicolectomy was performed. Histopathological examination of the tissue supported the diagnosis of pseudoaneurysm with necrosis. The patient successfully underwent the right hemicolectomy procedure without major postoperative complications. The clinical condition of the patient improved with stable postoperative recovery. Pseudoaneurysm of the ileocolic artery is a rare complication of blunt abdominal trauma that can lead to intestinal necrosis. Early diagnosis requires a high clinical suspicion and confirmation through imaging. Exploratory laparotomy with right hemicolectomy is the definitive therapy for cases with intestinal necrosis.
