Abstract

PT. XYZ is one of company manufacturing for product pipe gas with many size.Company has a problem in fulfill a demand of customer. Research at PT. XYZ have apurpose to increase the output production. Increasing output product with aimprovement at method and man. Those problems are long duration to exchange theroll, the emptiness of material and appear new production plan suddenly. The givenimprovement to company are make a work instruction for each machine and addedoperator. Estimation of the result improvement is reducing working time about 55-65%,the reduction of time make a good impact within increased the output of production.

Abstract

PT. XYZ is a company on manufacturing of Paper and Pulp. PT XYZ has not had a fully quality management system yet in accordance with the quality management system ISO 9001: 2008. The initial gap analysis showed that the company only fullfil 36.41% of requirements of quality management system ISO 9001:2008. The result of initial gap analysis used to make sure the document is suitable. This research aims to design quality management system accordance the requirements of quality management system. The increase was 51.63% after the quality management system was designed. All department in PT XYZ will implement a quality management system ISO 9001: 2008 except the Accounting and Transportation.

Abstract

Quality Document for PPIC Department (business process, standard operating procedure, work instruction and supporting document) was last updated in 2003, so it does not fit anymore. This condition causes high variation of work, obtructs the application of Toyota Production Sustem and ISO 9001. This problem can be solved by designing the new quality document. Results obtained 17 existing WI deleted, 6 existing  SOP revised, 31 existing WI revised, 2 proposed SOP and 130 proposed  WI added. Business process, supporting documents (form and standard) and tools are also improved in order to support the new quality document. Lastly, controlling system is designed to ensure that quality document always fits with actual condition.

Abstract

Metallurgical Laboratory of Petra Christian University is a laboratory that focuses on researching material properties. The problem faced by the Metallurgical Laboratory of Petra Christian University is that the 5S concept has not been applied in the laboratory area. Goods, tools, and machines in the laboratory area have not been neatly and cleanly arranged, resulting the operator having difficulty in carrying out activities in the laboratory area such as searching for items needed and dirty areas with remnants of work material. This thesis will be used to assist the design and application of the 5S concept in the area of Metallurgy Laboratory of Petra Christian University. The aim of the thesis is to create a work area that is neatly arranged, comfortable, and eliminates waste in the laboratory area so as to facilitate operators in their activities.

Abstract

Company X is a service company in plating. This aim of this research is designing a self assessment checklist associate with interest requirement of quality management of ISO 9001:2015 in Company X. Company X has implemented ISO 9001:2015, however this company never did the audit internal. To do the audit internal, company X need a self assessment checklist to help each department and to know the lack. This research was designed a self assessment checklist based on the requirement of clause 4 until clause 10 which exist on quality management system of ISO 9001:2015. On a self assessment checklist, the scoring category is using the modification from Baldrige scoring on process dimension and result dimension. Based on this application of a self assessment checklist, obtained the percentage from clause 4 until clause 10. This are the percentage 68,57%, 80%, 64,44%, 80%, 71,6%, 82,67%, 55,56%.

Abstract

The effort to reach zero accident in PT. Sari Mas Permai is by applying SMK3 (Occupational Safety and Health Management System). The first step was by making a HIRARC document (Hazard Identification, Risk Assessment, and Risk Control) based on OHSAS 18001:2007 standard. HIRARC document is useful for every worker to know more about dangers and risks that exist in the working area, so every worker will be able to realize the importance of working safety and health and apply the risks control regularly. Hence the zero accident target will be accomplished. We used proactive technique in which the dangers are knowing before happened. The result of the analysis shows that risk control in PT. Sari Mas Permai has been successful to decrease high hazard by 32%, but there are 25 working activities in the 3rd plant with high and the medium risks, so risk control should be applied continuously.

Abstract

PT XYZ is a manufacturer and produce a heat excahanger. The production process at PT XYZ takes over two production area that is creating component production and line production. The problems that happened PT XYZ is in the area of line production is about the many operator activities that not effective or as a category waste. Waste that happaned on the line production is waste transportation and waiting. An improvement that can be done to remove both waste is by using those method design facility layout and work time measurement. The facility layout design method used to decrease the movement of goods from 258 meters to 180 meters, decrease also happened to the cost of the materials handling from Rp 63,936 to Rp 41,688.

Abstract

PT Toyota Motor Manufacturing Indonesia (PT TMMIN) in 2014 is preparing the implementation of new engine type TR-Kai. Additional types of engine TR-Kai need additional type of new components too. Additional types of TR-Kai component means need additional space for placement, but the problem is unavailable space. The supply system design method includes layout change, equipment (material handling) and also system (works cycle) which is delivered well. The countermeasure idea is to change the material handling usage and also implementation direct supply system. Direct supply system in regular working time has reduced lead time almost 2 days of safety stock. Direct supply reduces 33.3% of inefficient handling, reduce hoist usage (safety matter) between 80,2% up to 93.4% each day, and also reduce 81.5% of forklift usage (safety matter). The evaluation result of this system is able to reduce 33.3% of manpower supply and monthly cost saving up to 46.2% with a Break Event Point (BEP) during 4 years 7.5 months.

Abstract

Scheduling is an important thing to determine accuracy on a project delivery to customers. Delays on a project delivery of PT. Aweco Indosteel Perkasa because of in­a­ccurate esti­mated calculation processing time which makes the difference from planned sche­dule with the plan conditions on production. To analyze the causes of delay, used the QCC (Quality Control Ci­rcle) method to get root cause is inaccuracy of estimated cal­c­ulation time jobs and uncontrolled material arrival process. Initial estimated schedule plan for polishing takes 8 shift, while based on the calculation time takes 11.24 shift and on­ the plan conditions takes 17 shift. This indi­cated that there were differences bet­ween plan time, the initial estimate, and the estimate time of calculation. Machine set up makes a difference estimated calculation, whereas the machine set up time very con­sidered in the estimated time calculation of polishing work. Efforts to catego­ri­zing tank types based on components of the tanks was also made. Beside that, calculating time esti­mation in polish machine done for categorizing type of tank.

Abstract

PT SMART, Tbk, is an agribusiness company that produce product from processed palm oil, the most well known product is cooking oil. Cooking oil product has quality specification to fulfill customer’s satisfaction, one of them is conformity of actual volume with volume that printed on the label. This quality specification affected by filling machine performance, therefore tool that can help create machine performance evaluation is needed. The purpose of this study is to design a dashboard  that can reduce time to process data into statistic parameters and to test the properness number of samples in current condition. Design of the dashboard will use Microsoft Excel program with Visual Basic for Application (VBA). The performance evaluation  is using statistic parameters (Cp, Cpk, and Sigma), while the properness of sample number is using two sample T-test. Dashboard reduce the processing time from 230% up to 763% and majority of the machine performance are lower than the standard. The result of number of samples’ test showed that total samples of 98 per hour has different significantly against total samples of 5 per hour, so that the sample number must be increased in order to represent the actual condition.
