Abstract

This research’s purpose is to discuss the result in achieving quality goal from every department in PT. X. Process of monitoring for guality goal can be assisted by an application like Excel in the form of Dashboard  This design of monitoring system use dashboard, that could show progress or movement in achieving goal, so it enable the top management to detect any possible sign that the quality goal not achieved, and could take an immediate action to solve the problem, in order to ensure the progess ini achieving goal stay on the right track. Dashboard’s purpose is to showcase the final result in achieving goal that is displayed in the form of picture that is more attractive than only form of number. This program needs data to be inputted regularly, so it can be for this monitoring system use dashboard that needs data to be input regularly so that the data can be processed automatically because it is designed with a formula. Result of monitoring and evaluation for quality goals, nine of total eleven quality goal were achieved.

Abstract

PT. X is a multinational company that produces consumer goods. This research is aimed to get the design of quality management system needed for ISO 9001: 2015 certification in Printing Development department, PT. X. This research was conducted by interview and direct field observation in PT. X. The scope of ISO 9001: 2015 quality management system design is the Printing Development department which includes three functions inside. There are Printing Project Management function, RND and Productivity Printing Project function, and Mock up and Prototyping. The design is done through the making and renewal of SOP, WI, support form, quality plan, and RACI matrix. Risk analysis using the Failure Mode and Effect Analysis (FMEA) method is also applied to three processes in the Printing Development department. Implementation is carried out on one of the processes considered to have a high risk of failure. The preliminary compliance quality score indicates the suitability of 82.21% against the requirements of ISO 9001: 2015, and the final compliance of 93.95%.

Abstract

PT. Schneider Electric Indonesia is one of the electrical manufacturer in Indonesia. The company has a HIRARC document which is based on OHSAS 18001:2007 standard. The last updated of HIRARC document was in 2013. The production proses activities have been changed in 2014 and required adjustment, including the changes of HIRARC document. This research was conducted to make HIRARC document fit with the actual conditions. After the identification of hazards, there is a change in the amount of activities that are at risk condition. HIRARA 2014 document showed 2 activities at high level of risk, 69 activities at moderate level of risk, and 436 activities at low level of risk at LV (Low Voltage) production line and FQC (Final Quality Control) LV production line.

Abstract

PT. X is a company that deals in ship voyage at Surabaya, Jawa Timur. PT. X has problems to fix a schedule for their ships going to East Indonesia. This difficulty is caused by longer travel time compared to West Indonesia. There are policies that the company use that affects in modelling the system in Vehicle Routing Problem with Time Windows (VRPTW). First is the call time, call time defines how many times a month a port has to be visited. This call time splits the initial 15 ports in East Indonesia to 62 locations. Second is the minimal frequency between each visit of the same port. Third is the maximum limit of ports visited by a ship, which is five ports. Ship schedule is then searched using the Insertion Heuristic Method, which is modified according to the case’s condition. Solution generated has a total travel time of 264 days, which is 11,11% more efficient compared to the schedule used by PT. X which has a total travel time of 297 days. Number of ships used is also reduced, which is reduced from 21 ships to 19 ships.

Abstract

PT. X merupakan perusahaan dengan produk yang dihasilkan adalah circuit breaker. Tahun ini PT. X berencana untuk memproduksi produk baru, dimana produk baru ini merupakan produk yang akan digunakan pada otomotif. Produk perusahaan digunakan pada otomotif harus memiliki sertifikasi ISO/TS 16949:2009 untuk menjamin kualitas dari produk. Akhir Tahun 2012, PT. X akan melakukan sertifikasi ISO/TS 16949:2009.Pemenuhan persyaratan Juli 2012 yang telah dilakukan oleh PT. X sebesar 74%. Persyaratan pada klausul 7 dan klausul 8 mengharuskan perusahaan untuk membuat MSA (Measurement System Analysis) sebagai record untuk kehandalan alat ukur dan operator dan SPC (Statistical Process Control) untuk mengendalikan proses produksi. Dilakukan validasi alat ukur dan operator, serta mendesain sistem pengendalian kualitas untuk produk baru di PT. X.  Hasil dari MSA menunjukkan bahwa alat ukur yang digunakan adalah reliable. Hasil dari SPC juga menunjukkan bahwa proses terkendali dan memiliki indeks kapabilitas mesin dan proses yang memenuhi standar industri.Hasil analisa akhir pada Desember 2012 menunjukkan bahwa PT. X telah 100% memenuhi persyaratan pada setiap klausul dan juga telah melakukan audit internal, sehingga sudah siap dilakukan sertifikasi ISO/TS 16949:2009.

Abstract

PT. X is engaged in cigarette manufacturing and is one of the leading tobacco companies in Indonesia and even the world. Planning to supply materials is a very important part of the production process. This research focuses on efficiency that can be done to simplify and accelerate the process of conducting various material supply planning activities. The two activities that will be discussed are PR review and material readiness. The efficiency process carried out is by making a proposal template that will facilitate the user in carrying out his activities. PR review templates that have been made are able to shorten the required activity time by 69.68%. While the material readiness template that was made was able to shorten the time of activities by 81.13%.

Abstract

Inventory is the thing to consider in ordering. Inventories have a variety of properties, one of which is a level of decline. Many researchers have done research on multi-item inventory but not with inventory are deteriorating and assume constant demand. The real condition that occurs today is that the company sells multi-item inventory in the presence of stochastic deterioration and demand. The situation is approached by building a model of inventory simulation using real data in Pin'kan Bakery. The simulation model is done with the help of monte carlod simulation and evolutionary algorithm. This study will compare two ordering modes, namely continuous review and periodic review. The results show that the continuous review method is an appropriate ordering method for multi-item inventory ordering with stochastic degrees and demand.

Abstract

PT. Teno Tract Indonesia, a foundation piling company, had dealt with many type of building construction for private sector, and now wants to get government projects. However, the government requires that PT Teno Tract Indonesia has ISO certificate. Hence, PT. Teno Tract Indonesia planned and prepared to implement quality management system based on ISO 9001:2008. PT. Teno Tract Indonesia also want to increase customer satisfication and improve management system. Three stages to get ISO 9001: 2008 certificate are planning stage, implementation stage and also scoring stage. Planning stage consists of mapping, identification, and documentation. The second stage is quality management system implementation. The last stage is scoring via internal audit. Audit has to be done to know compability of system that has been run based on standard requirements of ISO 9001 : 2008.  Based on internal audit simulation, PT Teno Tract Indonesia has been ready to submit the papers to get ISO certification.

Abstract

PT. X is a company engaged in the field of animal feed. The huge demand of animal feed causes machines work continuously and rarely stops. This condition can cause large machines downtime, especially for the pellet line. Downtime can be reduced by identifying the root cause by using Fault Tree Analysis. The cause of the problem will be given a value using Failure Mode Effect Analysis. FTA and FMEA are part of Reliability Centered Maintenance. Improvements were made by recapitulating failure data of the components to find out the effective days for conducting machines maintenance. The components maintenance day will be used to create a new maintenance schedule, which is expected to reduce the downtime. Downtime of main machine can be reduced by 31.94%.

Abstract

PT. XYZ is a global leader in the shoe industry that produces innovative shoes for men, women and children with the best quality and quality and become one of the world's lead­ing shoe manufacturers. The purpose of this study is to determine the conditions of occupational ­in­jury risks and determine the priority of handling accidents and provide benefits in the form of pr­oposals that need to be done company to minimize or prevent accidents at PT. XYZ. Risk Anal­y­­­sis is done by use Failure Mode and Effect Analysis (FMEA) method dan Risk Assesment st­ep on Hazard Identification, Risk Assessment and Risk Control (HIRARC) method. At PT. XYZ can still be said to have a large risk of accidents so that should be given improvements in order to mi­n­imize and prevent the risk of work accidents. Low risk of occupational injury by 20%, moderate ac­­c­ident risk by 42%, and high occupational accident risk by 38%. Priority of work handling that has high accident risk there are 13 categories of accidents from total 69 categories of work acci­dent­. Priority of work handling that has medium accident risk there are 29 categories of accide­nts from total 69 categories of work accident. The risk of accidents in a high company need to be co­n­s­idered and followed up to be reduced even to avoid the accident.
