Abstract

Objective: Excessive use of MSG in everyday life can cause infertility to men. Exposure to MSG can cause cells to experience oxidative stress which subsequently triggers the generation of free radicals. Free radicals can be resisted with antioxidants. Material & Methods: This research was a laboratory experimental test with a post-test only control group design using male mice as the experimental object. The statistical test used the One Way ANOVA test. Results: In this research, 5 male mice were assigned into 4 groups and 1 control group. Using the One Way ANOVA test, it was found that there were significant differences in ≥ 2 treatment groups in the motility group of spermatozoa with active movement (p < 0.001) and the motility group of spermatozoa with weak movement (p = 0.036). The two groups showed significant differences. The Post hoc LSD test in the motility group with active movement showed significant differences between the control group with MSG treatment (p < 0.001) and the treatment group given Moringa leaves extract of 600 mg/kgW (p < 0.001) and 1200 mg/kgW (p < 0.001). Conclusion: Moringa leaves extract can effect the improvement of the motility of spermatozoa exposed to monosodium glutamate at a given dose of 300 mg/kgW and 600 mg/kgW.

Abstract

Objective: This study aims to show our first case of female with periurethral condyloma and how we managed it. Case(s) Presentation: A 40-year-old female came with asymptomatic lesion in the genital area for 4 months, without any history of promiscuity. The physical examination showed a mass of 5 x 5 cm located in the periurethral area. The patient underwent mass excisions without any complication, further pathological examination confirmed the diagnosis of CA. Discussion: After operation and discharge there are no sign of recurrence after 6 months follow up. Conclusion: Despite the rare occurrence in periurethral region, clinicians should be aware of CA case in female and for our first case we used mass excision with satisfactory result.

Abstract

Objective: To evaluate the usage of MRI in prostate cancer staging, especially in nodal involvement (N-staging) and metastasis (M-staging) of prostate cancer. Methods: This is a systematic review and meta-analysis assessing role of MRI in nodal and metastasis staging of prostate cancer. Search of studies were done through search engine using Pubmed, Cochrane, and EBSCO Host and manual searching. Quality of eligible studies were assessed using a revised version of Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) and analyzed in pooled analysis according to nodal involvement or metastasis staging, modality of diagnosis used as the index test and gold standard used using STATA version 13. Results: Total 26 studies corresponding with study’s eligibility criteria were found. Overall, usage of MRI has a sensitivity of 47% (95% CI 35% - 60%; I2 83.08%) and a specificity of 93% (95% CI 89% - 96%, I2 82.21%) in nodal involvement staging of prostate cancer, while using of MRI in M-staging of prostate cancer shows a sensitivity of 94% (95% CI 86% - 97%) and a specificity of 99% (95% CI 97% - 99%). Using lymphotrophic superparamagnetic nanoparticle (LSN) - enhanced MRI gives higher sensitivity than using MRI without LSN for N-staging of prostate cancer. Conclusion: The usage of MRI in prostate cancer staging has a moderate sensitivity and relatively high specificity in detecting lymph node. Moreover, it plays an important role and even can be used as a modality of choice in assisting bone metastatic prostate cancer detection.

Abstract

Objective: To report our experience on management of testicular teratoma in pediatric patient. Case(s) presentation: A 2-years-old boy presented with progressive mass in his left testis. The mass was found 3 months ago but became larger in a few days. The patient had no other genitourinary complaint. Vital signs were within normal limits. A hard and tender mass in the left scrotum sized 5x4x2.5 cm was palpated from the physical examination. An imaging study with Computed Tomography (CT) Scan revealed an enhancement in the left scrotum mass area. There was no ring enhancement in pelvic and paraaortic lymph nodes. The laboratory examination within normal limit. Inguinal radical orchiectomy was performed, and histopathological examination revealed a mature testicular teratoma of the left testis. Discussion: Testicular teratoma in children is usually benign. Testicular germ cell tumors generally have a good prognosis with current therapy. Post-orchiectomy management depends on the histology type, staging, and tumor markers. Conclusion: Testicular teratoma is a rare case and can cause minimal symptoms until it grows significantly. Testicular teratoma should be considered in the differential diagnosis of non-traumatic painless progressive scrotal mass. Inguinal radical orchiectomy may be considered as the primary management.

Abstract

Objective: This study aimed to determine the functional role of the urine leukocyte count on type of UTI. Material & Methods: This is a systematic review study with searches for articles from 2010-2020 through the PubMed, ProQuest, and EBSCO databases. The exclusion criteria in this study are patients using catheters (CAUTI), UTI patients in the Intensive Care Unit (ICU), and pediatric or infant patients. There were 1158 studies found and 3 studies synthesized. Results: There are 1130 patients studied. Age range from 18 to 89.1 years. Three studies showed that there was an association between the number of urinary leukocytes and the type of UTI (Tommaso Cai et al., p < 0.0001; and Alexander R. Levine et al., p < 0.001) and one study had no relationship. Conclusion: There is a relationship between the number of urinary leukocytes with the type of UTI. An increase in the number of urinary leukocytes above the normal limit is the sign of body’s immune response to eliminate uropathogens. The innate immune characteristic factors and the body’s immune response have an important role in causing symptoms in UTI patients.

Abstract

Objective: To obtain the association between the erythrocyte indices and the stages of CKD. Material & Methods: This is a systematic review study conducted by searching for articles through PubMed, Proquest, and Google Scholar. Publication year are from 2010 to 2020. Results: There are 10 articles analyzed. There were 3 articles that showed a significant association and 2 articles that showed no significant association between erythrocyte indices and stages of CKD. Other article that used anemia morphology to represent the erythrocyte indices showed the normocytic normochromic as the largest percentage. 4 out of 5 articles with CKD patients who were known to be undergoing hemodialysis showed a significant difference in erythrocyte indices. Conclusion: There is association between the erythrocyte indices and stages of CKD.

Abstract

Objective: This study compared several factors that are thought to contribute to the incidence of complications of urethroplasty fistula after urethroplasty, including meatus location, degree of chordee, surgical technique, and postoperative stent type. Material & Methods: This study is an analytical study with retrospective cohort design. After adjusting to the inclusion and exclusion criteria, 89 subjects with proximal hypospadias were subjected to urethroplasty at Fatmawati General Hospital, Jakarta from 2016 to 2019. An analysis of the relationship between several variables and the incidence of uretrocutaneous fistula was analyzed. Results: There was no significant difference between the one-stage and two-stage urethroplasty technique on the incidence of urethrocutaneous fistula (p = 0.063). There was no significant difference between the degree of chordee and complications of urethrocutaneous fistula after urethroplasty (p = 0.677). The relationship between the use of silastic stent, catheter, or cystostomy type was also not significant in complications of urethrocutaneous fistula (p = 0.576). The location of the urethral meatus also did not have a significant role in predicting the incidence of post-urethroplasty urethrocutaneous fistula (p = 0.169). Conclusion: Surgical technique (one stage and two stages), type of stent, degree of chordee, and location of the urethral meatus, did not have a significant correlation with the incidence of urethrocutaneous fistula in hypospadias patients after urethroplasty.

Abstract

Objective: This study aimed to analyze the profile of bladder disease in Soetomo General Hospital based on histopathological examination. Material & Methods: This study was using secondary data. Histopathological examination data of patients with bladder disease were evaluated based on age, gender, and histopathological types. Results: There were 419 patients with bladder abnormalities. Non-neoplasm bladder diseases were found in 62 patients consisted of 56 patients (90.32%) with cystitis and 6 patients (9.67%) with glandular cystitis. Neoplastic bladder diseases were found in 357 patients and were divided into benign and malignant neoplasm. Benign neoplasms were found in 11 patients consisted of 5 patients (45.45%) with UP and 6 patients (54.54%) with IUP. Malignant neoplasms were found in 340 patients consisted of 300 patients (88.23%) with urothelial carcinoma, 26 patients (7.64%) with adenocarcinoma, 14 patients (4.12%) with SCC. Moreover, six male patients with PUNLMP were also reported. Conclusion: Histopathological examination in patients with bladder abnormalities shows that neoplastic bladder diseases (357/419, 85.20%) were more common than non-neoplastic bladder disease (62/419, 14.80%).
THE EFFECT OF SUKUN LEAF (ARTOCAPTUS ALTILIS) EXTRACT ON RENAL ISCHEMIA/REPERFUSION INJURY IN WISTAR RATS WITH PARAMETER OF KIDNEY HISTOPATHOLOGICAL FINDINGS SERUM CYSTATIN-C AND SERUM CYSTATIN-C

Abstract

Objective: To determine the effects breadfruit leaf extract (Artocaptus altilis)in Wistar rats, which had induced renal ischemic-reperfusion injury, with evaluation of serum cystatin C level and kidney histopatologic findings. Material & Methods: A total of 30 Wistar rats, three groups of ten, were included in the study and were performed in Bandung Eijkman Animal Laboratory. In group I (sham group), a right nephrectomy was done. In group II (control), a right nephrectomy was also done and the left renal pedicles were occluded for 60 minutes and reperfused for 60 minutes. In group III (treatment), the treatment was equal with group II, and breadfruit leaf extract (4.75%) was administered intraperitoneally. At the end of the study, a left nephrectomy was performed on all groups. Afterward, serum cystatin C levels and kidney histopathological findings of all rats were examined. Data were analyzed by an unpaired numerically comparative analysis with SPSS 20.0 software. Results: The mean of serum Cystatin C in group I was 3183.2 ± 680.9 pg/dL, group II was 3776.4 ± 326.7pg/dL, and group III was 3492.7 ± 1.326.7pg/dL. The mean of histopathology renal score was also assessed, group I was 4.4 ± 0.52, group II was 8.6 ± 0.84, and group III was6.3 ± 1.70. Comparative test of serum cystatin C level, using ANOVA test and Benferroni post hoc test, in the treatment group has a p-value of > 0.05, it showed that there were no significant differences on the value of cystatin C level on the treatment group. In correlation test of renal histopathology score, suing Kruskal-Wallis test and Mann-Whitney post hoc test has a p-value of < 0.05, therefore there were significant differences on the mean value of renal histopathology score in the treatment group, with Eta correlation coefficient 0.847. Conclusion: The beneficial effects of breadfruit leaf extract can be attributed to its protective effects on renal cells damage. This treatment has an important role in reducing renal injury resulting from renal ischemia-reperfusion. However, breadfruit leaf extract did not significantly improve renal function based on serum cystatin C level.

Abstract

Objective: This research aims to determine the correlation between maternal age during pregnancy, low birth weight (LBW), prematurity, and history of hormonal drugs used in the first trimester of pregnancy with the occurrence of hypospadias. Material & Methods: This research is an observational analytic research with case control design. The sample in this study were 50 samples that consisting of 25 patients with hypospadias and 25 patients without hypospadias at Bina Sehat Jember Hospital, Paru Jember Hospital, and Bhayangkara Bondowoso Hospital. After determined every case and control group, data acquired retrospectively by traced the hypospadias risk factors (maternal age at pregnancy, LBW, prematurity, and history of hormonal drugs using in the first trimester of pregnancy) by interviewing the patient's mother. The data of this research were analyzed by bivariate and multivariate using SPSS 16.0. Bivariate analysis in this study used chi-square Yates correction (chi-square with continuity correction) and fisher's exact test, whereas multivariate analysis used logistic regression techniques with significance p-value<0.05. Results: The result of bivariate analysis in this study showed that variable maternal age during pregnancy more than 35 years (p = 0.667) has no correlation with occurrence of hypospadias, while variable of LBW (p = 0.027), prematurity (p = 0.041), and the history of hormonal drug use in the first trimester of pregnancy (p = 0.049) have a correlation with the occurrence of hypospadias. Conclusion: In this study, there was a significant correlation between LBW, prematurity, and history of hormonal drugs using in the first trimester of pregnancy with the occurrence of hypospadias. This study did not show a significant correlation between maternal age during pregnancy with the occurrence of hypospadias.
