Abstract

Objective: This study aimed to determine the management of early-stage testicular cancer among urologists in Indonesia by using a survey. Material & Methods: This research was a descriptive-analytic study using a survey method through a Google-forms questionnaire distributed to urologists throughout Indonesia who were willing to participate. Data were collected from September 1 to 30, 2021. Results: Of the total respondents, 15 (14.2%) respondents were consultant urologic oncologists, and 90 (85.71%) respondents were non-consultant urologists or consultant urologists non-oncology. There was no difference in the treatment given by consultant uro-oncologists and non-consultant urologists/consultant urologists non-oncology in the management of surveillance of stage IA-IB seminoma (66.7% vs. 55.6%, p-value = 0.207), chemotherapy of stage IIA-IIB seminoma (100.0% vs. 90.0%, p-value = 0.080), surveillance of stage I non-seminoma cancers (46.7% vs. 41.1%, p-value = 0.304), and chemotherapy of stage IIA and IIB non-seminoma cancers (80% vs. 91.1%, p-value = 0.193). Conclusion: In general, as many as 84.4% of oncology consultant urologists and 83.7% of non-consultant urologists/consultant urologists non-oncology have performed initial diagnostic examinations, treatment, and follow-up following testicular cancer management guidelines according to the European Association of Urology 2021.

Abstract

Objective: This literature review aims to explain the role of nephrectomy in the treatment of localized RCC and its effects on postoperative renal function, as well as the role of the new baseline glomerular filtration rate (NBGFR) estimation method in the determination of the RCC method. Material & Methods: Articles were obtained through online searches using Pubmed, ScienceDirect, Web of Science, and Wiley Online Library using keywords NBGFR, renal function, nephrectomy, or localized renal cell carcinoma. Results: The development of several diagnostic factors as well as operative factors has made partial nephrectomy (PN) replace radical nephrectomy (RN) which for decades has been the main therapy in localized RCC. Postoperative determination of renal function is a crucial factor in the selection of therapies and several methods of determining NBGFR have been proposed in several previous studies. Conclusion: The NBGFR measurement method can be an accurate guide in the determination of nephrectomy methods in the treatment of localized RCC while use of computer software can help improve accuracy in the implementation of radical nephrectomy in RCC.
OUTCOMES ANALYSIS OF TRANSURETHRAL RESECTION OF THE PROSTATE (TURP) AND TRANSURETHRAL VAPORIZATION OF THE PROSTATE (TUVP) IN PATIENT WITH BENIGN PROSTATE HYPERPLASIA: SYSTEMATIC REVIEW

Abstract

Objective: This systematic review aims to compare IPSS score and Uroflowmetry (Qmax) results between Transurethral Resection of The Prostate (monopolar and bipolar TURP) and Transurethral Vaporization of The Prostate (PVP, ThuVARP, Biolitec, TUEVP, BPV). Material & Methods: Articles were obtained through online searches using Medline, Google Scholar, and Cochrane Library using keywords Transurethral Resection of The Prostate (TURP) dan Transurethral Vaporization of The Prostate (TUVP) and IPSS or Qmax. Results: A total of 6 RCTs and 3 Cohort consisted of 1212 BPH patients treated with TURP (B-TURP and B-TURP) or TUVP (TUVP, BPV, Biolitec, ThuVARP, GLVP(PVP)). There is a significant difference result of IPSS score and Qmax at the third month of follow-up after both therapies. Conclusion: The post-operative IPSS score and Qmax results at the third month of follow-up in BPH patients treated with TURP or TUVP presents varied results. TURP has a better post-operative IPSS score compared to Biolitec Laser, but not superior when compared with TUEVP, BPV, ThuVARP, and PVP. Notably, TURP has better post-operative Qmax results than BPV, TUEVP, and ThuVARP, but not when compared with PVP.
EXPERIENCE OF BILATERAL SYNCHRONOUS PERCUTANEOUS NEPHROLITHOLAPAXY (PCNL)

Abstract

Objective: This case report will present bilateral synchronous percutaneous nephrolitholapaxy (PCNL) that was performed in Saiful Anwar General Hospital Malang for the first time. Case(s) Presentation: 49 years old male with flank pain on both sides continuously. Non contrast abdominal Computed Tomography (CT) scan showed bilateral pyelum stone and bilateral hydronephrosis. From blood analysis, the creatinine serum was 1.5 mg/dL and ureum serum was level 69.8 mg/dL. Discussion: Bilateral synchronous PCNL was performed in prone position, and stone fragmentation was done with pneumatic lithotripter, the operation last for about 2 hours with minimal amount of bleeding. After 3 months of follow-up, urology ultrasound and abdominal X-ray shows no residual stone, and normal ureum-creatinine level (0.77mg/dL; 43.7mg/dL). Conclusion: Bilateral synchronous PCNL is a safe choice to be done in order to treat bilateral pyelum stone, it can decrease length of stay , total cost therapy, and morbidity of the patient.

Abstract

Objective: This study aims to analyze the factors associated such as hemodialysis periods, age, and smoking with ED severity in CKD patients undergoing routine hemodialysis. Material & Methods: A single-center, cross-sectional study was conducted involving a total 101 patients who were on routine hemodialysis who diagnosed with CKD and experienced complaint in sexual function were included. The severity of ED classified based on the International Index of Erectile Function (IIEF-5). The analysis was carried out descriptively and analytically using the ordinal regression method. Results: Hemodialysis duration (p = 0.044) were significantly related to severity of erectile dysfunction in patient with CKD and on routine hemodialysis. Meanwhile, although both older age (p =0.307) and smoking (p = 0.283) showed a positive estimate, they were not statistically significant. Conclusion: The study indicate that prevalence of ED observed in CKD patient with routine hemodialysis remain high. Several factors significantly contribute in ED severity such as shorter hemodialysis duration.

Abstract

Objective: This study aims to research the characteristic of patients with urinary tract stones and the profiles of uric acid and cholesterol at the Kustati Islamic General Hospital (RSUI) in the period from October 2020 to March 2022. Material & Methods: This research uses the cross-sectional retrospective approach with the descriptive method. The research subjects are the patients with urinary tract stones that have undergone operations in Kustati Islamic General Hospital in the period between October 2020 and March 2022 as well as patients that have had their level of uric acid and cholesterol examined in the laboratory of Kustati Islamic General Hospital in the period from October 2020 to March 2022. Results: Urinary tract stones occur the most in men (70.3%) and in the age group 46-59 years old with a percentage nearing 50%. Urinary tract stones occur the most in the upper section of the urinary tract, mainly in the kidneys (67.2%). Hyperuricemia occur mostly in men while hypercholesterolemia occur mostly in women. Conclusion: The location of stone is mostly found in the kidneys. Patients with hyperuricemia are mostly men, while patients with hypercholesterolemia are mostly women.

Abstract

ABSTRACT Objective: The purpose of this study is to find the relation between overweight and obesity with lower urinary tract symptoms due to benign prostatic hyperplasia in pre-elderly. Material & Methods: This analytical study was conducted in a cross sectional manner to 113 respondents. LUTS assessment uses the International Prostate Symptoms Score questionnaire. Measurements for overweight and obesity, this study uses the BMI and waist circumference methods. Fisher's exact test with SPSS v.25 was used for data analysis. Results: Results from data analysis showed that there was no significant relationship between overweight and obesity based on the BMI method on LUTS, p = 0.122. The results also showed a significant relationship between central obesity and LUTS, p = 0.003. Conclusion: Central obesity can be identified as a risk factor for LUTS while overweight and obesity based on the BMI method cannot be considered as a risk factor. These results can be used as education for pre-elderly to reduce the risk of LUTS due to BPH. Keywords: Overweight, obesity, lower urinary tract symptoms, benign prostatic hyperplasia, pre-elderly.

Abstract

ABSTRACT Objective: Hypertension is one of BPH (Benign Prostate Hyperplasia) risk factors. This study was to compare the prostate volume of hypertensive-BPH patient and normotensive-BPH patient at Soebandi General Hospital Jember. Material & Methods: This is an observational analytic study with cross-sectional design conducted at Soebandi General Hospital, Jember. The data (blood pressure and prostate volume) were obtained from the medical records of BPH patients visiting the Urology Department during December 2021 until January 2022. They were analyzed with Independent T-test. Result: The results showed that there was a significant difference in prostate volume between hypertensive-BPH patients and normotensive BPH patients (p-value: 0.013). Among the BPH patients, the hypertensive groups had a bigger prostate volume than the normotensive groups. Conclusion: This study proves that hypertension is associated with bigger prostate volume among BPH patients at Soebandi General Hospital, Jember. Abstract

ABSTRACT Objective: The purpose of this study was to determine the effect of a woman’s age during pregnancy on the incidence of hypospadias in children. Material & Methods: This research is an analytic study with a case control design. The sampling technique is a non-probabilistic technique with consecutive sampling. This study was analyzed by univariate and bivariate. Results: From 62 samples, the number of hypospadias cases was 28 people (90.3%) with woman's age at pregnancy <35 years and 3 people (9.7%) with woman's age at pregnancy ≥ 35 years. While the number of controls was 29 people (93.5%) with woman's age at pregnancy <35 years and 2 people (6.5%) with woman's age at pregnancy ≥ 35 years. Conclusion: There is no significant relationship found in this research between a woman's age at pregnancy and the incidence of hypospadias in children. Keywords: Age, hypospadias, pregnancy.

Abstract

ABSTRACT Objective: To present a case of urethral injury treated in a male adult. Case(s) Presentation: A 48 year old Asian male patient previously involved in a motor vehicle accident presented to the hospital with a chief complaint of pain and bleeding from the genitalia. On examination, it is revealed bleeding from the urethral meatus, pain in touch, palpable bladder, high riding prostate, and a butterfly-shaped bruising on the perineum. X-ray examination demonstrated soft tissue mass in the right scrotal region. The patient is diagnosed with urethral rupture with urinary retention and then underwent ultrasonography-guided cystostomy and perineal hematoma evacuation. The patient is followed-up 1 month later with a complaint of difficulty in voiding and erectile dysfunction. The patient is then diagnosed with urethral stricture and underwent direct vision urethrotomy. Discussion: Urethral injury is a rare type of injury of genitourinary trauma that may cause long term morbidity such as strictures, incontinence, impotence, and infertility. Urethral injury can be classified into posterior urethral injury which is associated with pelvic fracture and anterior urethral injury which is associated with blunt trauma. The gold standard of radiographic examination in urethral injury is dynamic retrograde urethrography. Prompt diagnosis, staging, and selecting intervention must be done for minimalizing complications. Sequelae may also develop which also affects quality of life. The usage of universal health coverage such as BPJS in In Indonesia can increase the accessibility of medical treatment. Conclusion: Urethral injury is a rare injury that requires prompt and careful management to minimize complications and increase the patient’s quality of life.
