Abstract

Objective: The aim of this study is to provide the characteristics, management, and outcome of bladder trauma at the center of trauma referral hospital in West Java, Indonesia. Material & Methods: This is a retrospective and descriptive study. The data of bladder trauma were collected from the medical records from January 2010 to December 2019. The data were presented using descriptive statistics in frequency and percentage. Results: Over a ten-year period, there were 1,174 cases of urogenital trauma, with 170 cases (14.48%) involving bladder injuries. The majority of the was 21-30 years old and were men (55.88%). The most common cause was blunt abdominal trauma (68.82%), which was caused by traffic accidents (91.45%). With 42.24% pelvic fracture as the most common associated injury. The most common type of bladder injury (45.88%) was bladder contusion. Iatrogenic trauma was the second leading causes (29.42%) with obstetrics and gynecology operations accounted for 84% of iatrogenic bladder trauma cases. The majority of bladder injuries were identified using a computed tomography (CT) scan. In 59.41% of cases, surgical intervention was required. The mortality rate was 2.94% during post-operative treatment due to coexisting or associated multiple injuries. Conclusion: We discovered that bladder trauma most commonly affects males of reproductive age with blunt abdominal trauma as the most common etiology and pelvic fractures as the most common associated injury. Abstract

Objective: We wish to update existing meta-analyses to assess the effectiveness of pharmacological agents against ureteral stent-related symptoms with better design of RCTs, as well as more valid and reliable measurement methods. Material & Methods: Relevant published articles up to February 2022 were obtained from PubMed, CENTRAL, Google Scholar, Science Direct, and DOAJ. All single-blind or double-blind RCTs with various comparisons using pharmacological agents and/or placebo in patients with ureteral stent-related symptoms, with outcome assessment using USSQ at week 1 and week 4 during stent placement was included in the study. Statistical parameter Mean Difference (MD) with 95% CI was used to measure effect size. The p value less than 0.05 indicates statistical significance. Results: Alpha blockers (2 studies, 85 samples) significantly reduced fourth week UIS compared to placebo (MD: – 5.46, 95% CI: – 7.47 to -3.46, I2 = 0%, P = < 0.001), and alpha blockers (2 studies, 42 samples) significantly reduced first week SIS compared to combination therapy (MD: – 0.82, 95% CI: – 1.56 to – 0.08, I2 = 0%, P = 0.03). The results of the meta-analysis of USSQ scores in other domains have no significant differences. Conclusion: Alpha blockers were better than placebo in reducing UIS due to ureteral stent in the fourth week of stent placement, and better than combination therapy in reducing SIS due to ureteral stent in the first-week post stenting. However, more RCTs are needed with larger samples and better designs to reach valid conclusions. Keywords: Stents, Randomized Controlled Trials as Topic, Single-Blind Method.

Abstract

Objective: Prostate cancer is the second most common cancer in men and the fifth leading cause of death worldwide. 10-20% of cases can progress to a more aggressive stage called castration-resistant prostate cancer (CRPC) with a higher death rate. Clinical guidelines recommend docetaxel as first-line chemotherapy and evaluated using the PSA test. This study aims to investigate the effect of docetaxel chemotherapy on PSA levels in CRPC. Material & Methods: A retrospective study was conducted from January 2016 until August 2022 according to the medical records database Saiful Anwar Hospital. There were 16 patients who met the inclusion criteria, including male patients with a diagnosis of CRPC who received castration therapy and found an increase in PSA 3 times of nadir or found 2 or more bone metastatic lesions or soft tissue lesions > 2 cm, received 6 series of docetaxel chemotherapy, and have a PSA data was performed before and after chemotherapy. Statistical analysis was performed using a differential T-test. Results: The results, obtained were significantly different (p<0.05) on the average PSA (log) pre-post chemotherapy, visual analog scale (VAS), and quality of life, and not significantly different on BMI pre-post chemotherapy. Conclusion: 6 cycles of docetaxel chemotherapy resulted in a decrease in PSA levels of CRPC significantly. It showed an excellent response to docetaxel chemotherapy in managing CRPC patients. Limitations of this study were specifically the retrospective cohort study model and the ability to reduce PSA levels in assessing the prognosis of CRPC. Abstract

Objective: To analyze the outcomes of single-session bilateral PCNL in a horseshoe kidney patient with bilateral staghorn stones in terms of safety and effectiveness. Case(s) Presentation: A 64-year-old woman with bilateral staghorn stones on HSK. She had an anesthesia status of ASA 2, with type 2 diabetes mellitus (uncontrolled with oral medication), 47% of ejection fraction from echocardiography and her body mass index was 21.7 kg/m2. A single session of bilateral PCNL was performed, with 150 minutes of surgery and total blood loss of ± 50 cc. A 4.7 Fr DJ stent was placed on the right kidney, a 5 Fr ureteral catheter was left in the left kidney, and a 16 Fr foley catheter was placed as a right and left nephrostomy. Control plain abdominal x-ray showed no residual stones. Postoperatively, the patient was hospitalized for 3 days, there were no significant complaints and discharged in good condition. Discussion: The procedure was performed in the prone position for two reasons. First, is to avoid intestinal injury during puncture and dilatation, the second is to simplify the positioning of the C arm, as well as the endourology monitor, and the other instruments. Conclusion: Single-session bilateral PCNL for HSK patients with bilateral staghorn stones, in our experience, is quite safe and effective. Abstract

Objective: This study aims to describe the characteristics of patients undergoing mini-PCNL at our center in Jambi, Indonesia. Material & Methods: This is a descriptive study involving patients with kidney stones treated with mini-PCNL during 2017 – 2021 in Jambi. The selection of the study subjects was using total sampling method. All patients had a medical history taken and a physical examination performed prior to surgery. The procedure was performed by three operators in one referral district hospital. Results: From 2017 to 2021, a total of 188 mini-PCNL procedures were performed at our center for the removal of kidney stones. However, only 186 cases were included in this study after excluding 2 due to incomplete data. The mean age of patients in this study was 49.3 years old (ranging from 12–87 years old) with the majority being male (101/165 [61.21%]). Of 186 cases, a total of 175 cases had no residual stone and only 11 had residual stone (stone-free rate [SFR] 94.08%). Most patients presented with hydronephrosis, and DJ stent placement was performed in 44 out of 186 cases. The SFR decreased as the stone size increased. Conclusion: Mini-PCNL proves to be a valuable choice for managing kidney stones, especially for medium-sized and hard stones, leading to enhanced patient results and reduced post-operative complications. Abstract

Objective: This study aims to present a case of accidental penile fracture without sexual activity. Case(s) Presentation: A 40-year-old guy was sent to our emergency center nine hours prior to admission with oedema and lack of tumescence on the penis. There were no records of sexual activity or masturbation. The patient was treated for a hematoma and a fracture. The patient raised no complaints during his one-week and two-month follow-up visits after the procedure. Discussion: A urological emergency involving penile fracture is uncommon. It is important to avoid unnecessary delays in obtaining medical help. In the current case study, with urgent surgical intervention, corporal tears were repaired. Recent research recommends prompt surgical treatment of penile fractures, as this appears to be associated with fewer complications and better results. Conclusion: A penile fracture that requires immediate surgical treatment can preserve erectile and voiding function. Keyword: Penile fracture, urological emergency, surgical treatment.

Abstract

Objective: In this paper we report our experience in the use of buccal mucosal grafts in the reconstruction of difficult upper ureteric and uretero-pelvic junction strictures. Material & Methods: We retrospectively looked at our hospital inpatient and outpatient records of all patients who underwent surgical treatment for benign upper ureteric strictures and ureteropelvic junction strictures. Results: During the study period a total of 22 patients with a mean age of 326.07 years underwent buccal mucosal graft repair. The mean length of the narrowing/stricture was 6.40 1.44 cm. Conclusion: Buccal mucosal onlay graft ureteroplasty is a suitable treatment option for long, complicated benign upper ureteric and select ureteropelvic junction stric­tures. It is technically simple and capable of providing optimum patency of ureter with good urinary drainage. It is safe and effective. Abstract

Objective: This study aims to know the surgical treatment of electrical injuries, perineum, and scrotum. Case(s) Presentation: A 55-year-old male sustained electric burns to the back, perineum and scrotum. The patient failed to void following removal of the catheter 6 weeks later, hence the patient was referred to the urology department for further management. was counselled for a two-phase urethral surgery. Discussion: In our case, the patient had sustained perineal and scrotal burns when he sat over a live wire. This led to the loss of skin and soft tissues, needing massive musculoskeletal and skin grafts. Our patient underwent a number of revision surgeries leading to repeated scarring of the perineal tissues and skin. Conclusion: Electrical injuries to the perinium and genitalia are rare. Hence the surgical strategy of electrical injuries requires a step-wise approach. Initially, conservative debridement needs to performed so as to preserve any tissue of questionable viability and ensure subsequent adequate soft-tissue coverage. The reconstructive surgical procedures bring about satisfactory outcomes. Keywords: Electrical injury, perineum, scrotum, surgical strategy.

Abstract

Objective: The report aims to present a rare bilateral UTUC focusing on diagnostic and surgical treatment to improve the survival rate of patients with bilateral UTUC. Case(s) Presentation: We present a case report of a 59-year-old male who came in with gross hematuria accompanied with clots. Laboratory reveals a decreased renal function, while CT shows a bilateral mass in both ureters and left renal pelvis without bladder involvement. The patient then underwent left nephroureterectomy and right ureterectomy. In Intraoperative we preserve the right kidney by evaluating the tumor margin using ultrasound and ureteroscopy and deciding to substitute a full-length ureter with ileum as a precaution for a safety margin. Later on the pathological examination shown high-grade invasive urothelial cell carcinoma on both sides with staging pT2N0M0. Six months after surgery patient is in an excellent condition; weight and karnofsky score improvement. The Baseline Creatinin level was decreased, whereas CT showed no residual tumor in the right urinary tract system. Discussion: This article presents a rare case of bilateral UTUC with staging pT2N0M0 that was successfully treated with multidisciplinary care of radical and reconstructive surgery without chemotherapy. Conclusion: Bilateral UTUC is a scarce case, even among all urologic malignancies. Preserving kidney function in these cases is a priority to reduce further morbidity for the patient. Abstract

Objective: This study aims to determine the survivability testicular cancer patients at a tertiary hospital in Malang, Indonesia. Material & Methods: This is study conducted from January 2021 to January 2022 at Saiful Anwar General Hospital in Malang, East Java. Involving 32 testicular cancer patients. Data was obtained through medical record January 2017 – January 2022. Analysis was carried out to determine the survivability of the patient. Results: The results showed that the proportion of 1 to 5 year survival rate in testicular cancer patients was 96% and 21%. Kaplan Meier test showed significant difference of patient survival in the radiation therapy exposure (p=0.021), Karnofsky score (p=0.010), tumour type (p=0.019), and response of chemotherapy (p=0.015). Conclusion: The survival rate of testicular cancer patients are strongly influenced by the karnofsky score, radiation therapy exposure, tumour type, and chemotherapy response. Therefore early diagnosis and treatment plays an important role in improving survival rate of the patients. Keywords: Testicular cancer, survival rate, seminoma, non seminoma.
