Abstract

Dry Eye Disease (DED) is a multifactorial ocular surface disorder characterized by tear film instability and chronic inflammation, resulting in discomfort and visual disturbance. Conventional treatments such as artificial tears and anti-inflammatory agents mainly provide symptomatic relief and show limited regenerative potential. Secretome-based therapy, derived from mesenchymal stem cells (MSCs), has recently emerged as a promising cell-free regenerative approach. This systematic review aims to evaluate the efficacy, safety, and mechanisms of topical secretome therapy for DED in animal models. A systematic literature search was conducted in PubMed, Google Scholar, and ScienceDirect for studies published between January 2004 and August 2025. Eligible studies included animal models of DED treated with MSC-derived secretome, exosomes, or conditioned medium administered as topical eye drops. Data extraction followed PRISMA guidelines, and study quality was assessed using the SYRCLE’s risk of bias tool for animal research. Eleven animal studies met the inclusion criteria. Topical secretome therapy significantly improved tear break-up time and tear production in most studies and reduced corneal fluorescein staining, indicating enhanced tear film stability and epithelial repair. Histological analyses revealed increased goblet cell density, reduced inflammatory cytokines, and decreased apoptosis markers. No ocular or systemic adverse effects were reported, confirming good tolerability across all animal models. Topical secretome therapy demonstrates strong anti-inflammatory, anti-apoptotic, and regenerative effects in animal models of DED, with a favorable safety profile. Future research should focus on standardizing secretome production and conducting clinical studies to confirm its translational potential as cell-free therapy for ocular surface regeneration.

Abstract

Platelet-rich plasma (PRP) is an autologous blood component that contains a high concentration of platelets. PRP is known contains growth factors that play roles in cell regeneration, proliferation, and differentiation. This study used a completely randomized design (CRD) consisting of 5 groups and 5 repetitions. The test animals used were 25 male Wistars rats aged 2 months. The animal model of glaucoma was given 250 mmHg pressure on the right eye for 60 minutes and PRP was given 7 days after the pressure was increased. Then the thickness of the eye retina was observed after 8 weeks. The eye of the test animals were collected and made into histology preparations using paraffin method and Hematoxylin Eosin (HE) staining. Data were analyzed using ANOVA test with a significant level of 5% and continued with Duncan’s multiple tests. The result showed that there was a significant effect of PRP injection on the retinal thickness in the inner plexiform layer. Intravitreal-intraperitoneal injection of PRP is an effective injection route because it is capable of repairing damage based on the results of the thickness of the inner plexiform layer.

Abstract

Background: Tuberculosis (TB) diagnosis is routinely established through sputum-based testing. However, obtaining high-quality sputum samples remains challenging, particularly for molecular diagnostic methods. Saliva has emerged as an attractive alternative specimen because it is safe, non-invasive, and easy to collect.Objective: This study aimed to evaluate the diagnostic accuracy of Xpert MTB/RIF Ultra in saliva compared with sputum as the reference standard for detecting Mycobacterium tuberculosis (MTB) and rifampicin resistance.Methods: This preliminary diagnostic accuracy study employed a cross-sectional design and included 22 patients with suspected TB attending a pulmonary outpatient clinic. Each participant underwent parallel Xpert MTB/RIF Ultra testing using paired sputum and saliva samples. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), likelihood ratios, and overall diagnostic accuracy were calculated using 2×2 contingency table analysis, with sputum Xpert MTB/RIF Ultra serving as the reference standard.Results: The results of this study showed that 22 subjects were included. MTB was detected in sputum samples from 16 subjects and in saliva samples from 13 subjects. Among respondents examined using both specimens, 11 had positive results, and 4 had negative results. Saliva examination produced five false-negative and two false-positive results. The sensitivity was 68.8%, the specificity was 66.7%, the PPV was 84.6%, and the NPV was 44.4%. At low bacterial concentrations, the results of this study were more likely to be unfavourable. The likelihood ratios were LR⁺ 2.06 and LR⁻ 0.47, indicating that sputum examination cannot yet be replaced. However, based on this study’s results, saliva has potential as a screening test, particularly in primary healthcare settings.Conclusion: Saliva represents a practical alternative specimen in cases where sputum collection is problematic and may support earlier detection of tuberculosis
