Abstract

Bruxism merupakan suatu kebiasaan parafungsional berupa gerakan menggertakan dan menggerus gigi. Tidak jarang pasien yang mempunyai kebiasaan bruxism memerlukan perawatan ortodontik. Perawatan bruxism dapat dilakukan bersamaan dengan perawatan ortodontik cekat. Studi kasus ini bertujuan untuk menganalisa efek Trainer for Braces (T4B) pada pasien bruxism yang memerlukan perawatan ortodontik cekat. Pasien perempuan usia 21 tahun, mengeluhkan gigi berjejal dan tidak rapi. Pemeriksaan objektif menunjukkan bidental protrusi, crowding rahang atas dan rahang bawah, deep overbite, konstriksi berat pada regio gigi premolar dan molar rahang atas dan rahang bawah, 47 linguoversi dan disertai bruxism. Maloklusi Angle Kelas I dengan hubungan skeletal Kelas I, bidental protrusi, overjet 3,7 mm, overbite 4 mm, crowding, edge to edge bite, cross bite dan bruxism. Pasien dirawat menggunakan alat ortodontik cekat teknik Edgewise dengan alat tambahan Lingual Arch Bar untuk ekspansi rahang dan koreksi 47 yang linguoversi dan Trainer for Braces (T4B) untuk bruxism. Setelah 8 bulan perawatan, crowding rahang atas dan rahang bawah terkoreksi, ekspansi rahang dapat tercapai, 47 yang linguoversi terkoreksi, overjet dan overbite berkurang menjadi 3,5 mm, perawatan pada pasien masih berlangsung hingga saat ini. Kombinasi perawatan ortodontik cekat dengan penggunaan alat tambahan seperti Trainer for Braces (T4B) efektif untuk membantu koreksi maloklusi pada pasien bruxism. ABSTRACT: Edgewise Technique Combined with Trainer for Braces for Bruxism Patient. Bruxism is a parafunctional habit of grinding and clenching the teeth. It is common for patients with fixed orthodontic treatment to experience bruxism. When dealing with these patients, clinicians could initiate the bruxism treatment in conjunction with the orthodontic treatment. This case report will analyze the effects of Trainer for Braces (T4B) in a patient with malocclusion and bruxism habit. A 21 year old female patient complained of her crowding in upper and lower anterior teeth. The objective examination shows protrusion and crowding in upper and lower teeth, deep overbite, severe maxillary and mandibulary constriction, 47 linguoversion and bruxism habit. Angle Class I with skeletal Class I malocclusion, bidental protrusion, overjet 3.7 mm, overbite 4 mm, crowding, edge to edge bite, crossbite and bruxism habit. The orthodontic treatment used fixed preadjusted Edgewise appliance with Lingual Arch Bar for expansion and lingoversion molar correction. Trainer for Braces (T4B) was also prescribed to treat her bruxism. After 8 months of treatment, the crowding in upper and lower teeth was corrected, dental arch expansion was achieved, linguoversion molar was corrected, and overjet and overbite became 3.5 mm and the treatment was still on going. The combination of fixed preadjusted Edgewise appliance with Trainer for Braces (T4B) can be considered as an effective therapy for correcting malocclusion in bruxism patient.

Abstract

Estetika memegang peranan penting untuk meningkatkan kepercayaan diri, terutama gigi anterior. Masalah estetika pada gigi anterior yang sering dijumpai adalah lesi karies yang dapat menyebabkan hilangnya mahkota gigi dan perubahan warna gigi. Makalah ini bertujuan untuk melaporkan restorasi mahkota porselen fusi metal pada gigi insisivus sentralis kiri rahang atas pasca perawatan saluran akar (PSA). Seorang pasien laki-laki berusia 25 tahun ingin merestorasi gigi insisivus lateralis maksila yang berubah warna. Diagnosis dalam kasus ini adalah gigi nekrosis disertai diskolorasi. Perawatan untuk kasus ini adalah PSA satu kunjungan dilanjutkan restorasi mahkota jaket porselen fusi metal dengan custom dowel. Tiga bulan kemudian pada saat kontrol tidak terdapat keluhan saat mengunyah, tidak terdapat traumatik oklusi, dan keadaan jaringan pendukung gigi baik. Kesimpulan dari laporan kasus ini adalah restorasi gigi anterior dan PSA dapat mengembalikan fungsi gigi terutama fungsi estetik yang berpengaruh pada tingkat kepercayaan diri. ABSTRACT: Porcelain fuse to metal restoration and customed dowel post one visit endodontic treatment. Common esthetic problem on anterior teeth is caries, causing damage to the crown and discoloration. The aim of the case report is to report a porcelain fuse to metal restoration and custom dowel post one visit endodontic treatment in the left central maxillary incisor. A 25-years-old male patient came to seek care of his discolorized left central maxillary incisor. The diagnosis has been established as followed, pulp necrosis with discoloration on left central maxillary incisor. Abstract

Inter occlusal record serves as a guide to copy the upper and lower jaw relationship of the oral cavity to the articulator. Proper inter occlusal record minimizes the adjustment time of fixed denture in the patient’s mouth, thus saves the working time and cost. The making of fixed unilateral dentures involving the posterior teeth as abutments is often difficult in term of placing the working model of the upper jaw and lower jaw in articulator with proper occlusion (last tooth in the arch syndrome), therefore it is necessary to use inter occlusal record. One method to produce precise and simple inter occlusal record is a triple tray technique. This report provided information about how to use triple tray technique to produce inter occlusalrecord in the preparation of fixed denture of a patient with last tooth in the arch syndrome. A 54 years old male patient came to Prof. Soedomo Dental Hospital, Yogyakarta, Indonesia for a treatment using a denture due to the extraction of 16, 14, 24, 25, 26, and 46. The management of this case was comprised of anamneses, clinical and radiographic examination, first stage preparation of abutment tooth on 45 and 47, triple tray try in on prepared teeth, adjustment of the size of gauze, materials bite registration positioning on the surface of the upper and lower triple tray, inter occlusal record with a triple tray technique on maximum inter cusp position, the excess of bite registration materials trimming, articulator mounting, laboratory processing, and the fixed bridge insertion. Furthermore, stage II and III tooth preparation were performed using the same procedure. Based on our work, the triple tray technique is a simple method to produce an accurate inter occlusal record on a patient with a last tooth in the arch syndrome. ABSTRAKInter occlusal record berfungsi sebagai catatan untuk memindahkan hubungan rahang atas dan bawah dari rongga mulut ke artikulator. Inter occlusal record yang tepat meminimalkan penyesuain gigi tiruan cekat dalam rongga mulut sehingga menghemat waktu perawatan dan biaya. Pembuatan gigi tiruan cekat unilateral yang melibatkan gigi paling posteriorsebagai gigi penyangga seringkali terhambat karena kesulitan penempatan model kerja rahang atas dan rahang bawah pada artikulator dengan oklusi yang tepat (last tooth in the arch syndrome) sehingga diperlukan inter occlusal record. Salah satu teknik inter occlusal record yang sederhana dan akurat adalah menggunakan triple tray technique. Laporankasus ini memberikan informasi mengenai penggunaan triple tray technique untuk menghasilkan inter occlusal record dalam pembuatan gigi tiruan cekat pada kasus last tooth in the arch syndrome. Pasien laki-laki 54 tahun datang ke RSGM Prof. Soedomo Yogyakarta, Indonesia untuk dibuatkan gigi tiruan karena gigi 16, 14, 24, 25, 26, dan 46 telahdiekstraksi. Penatalaksanaan yang dilakukan meliputi anamnesa, pemeriksaan klinis dan radiografi, preparasi gigi penyangga tahap pertama pada elemen 45 dan 47, try in triple tray pada gigi yang dipreparasi, penyesuaian ukuran kasa, peletakkan bahan bite registration di permukaan atas dan bawah triple tray, inter occlusal record dengan triple tray technique pada maximum inter cusp position, pengurangan kelebihan bahan bite registration, penanaman padaartikulator, pemrosesan di laboratorium, dan insersi gigi tiruan cekat. Selanjutnya, dilakukan preparasi gigi penyangga tahap II dan III dengan prosedur yang sama. Berdasarkan laporan kasus ini, triple tray technique merupakan teknik inter occlusal record yang sederhana dan akurat pada kasus last tooth in the arch syndrome.

Abstract

An open apex on necrotic pulp is a common clinical cases. This condition complicates the root canal treatment by the risk of extrusion and the risk of fracture due to the thin canal walls. The aim of this case report was to describe the procedure of calcium hydroxide and bioceramic sealer use in the treatment of an open apex. A 24 years old male patient came to Oral and Dental Hospital Prof. Soedomo, Yogyakarta, Indonesia to have a treatment on his left maxillary central incisor that was injured 10 years ago. There was no pain history on the related tooth. An examination on element 21 showed an Ellis class IV fracture, fistula, discoloration, negative thermal test Chlor Ethyl (CE) and a radiolucent area around an open apex. Treatments were including apexification, bleaching, and direct composite veneer with Fiber-Reinforced-Composite post. Apexification was performed using calcium hydroxide for 3 + 3 months. After 6 months, a rather dense calcified material that was slightly shorter than the apex was observed. A checking procedure by inserting a file into the root canal showed that a hard tissue barrier had been formed. Obturation was then performed using bioceramic sealer. Bioceramic sealer was expected to strengthen the root against the risk of fracture, to continue the stimulation of tissue formation and to fill the remaining void left by apexification. Although Mineral Trioxide Aggregate (MTA) is widely use to replace calcium hydroxide, the classic material was proven to be suitable for the treatment of open apex. This is particularly useful because MTA is not always easy to obtain. ABSTRAKApeks terbuka pada nekrosis pulpa merupakan kasus klinis yang sering dijumpai. Keadaan ini menyulitkan perawatan saluran akar karena risiko ekstrusi serta risiko fraktur pada dinding saluran akar yang tipis. Tujuan laporan kasus ini adalah menunjukkan penggunaan kalsium hidroksida dan siler biokeramik untuk perawatan kasus apeks terbuka. Pasien laki-laki 24 tahun datang ke RSGM Prof. Soedomo, Yogyakarta, Indonesia untuk merawat gigi insisivus sentralis kiri maksila yang terkena benturan karena jatuh 10 tahun yang lalu. Gigi tidak pernah dirasakan sakit. Pemeriksaan pada gigi 21 menunjukkan fraktur Ellis kelas IV, fistula, diskolorasi, tes termal dengan Chlor Ethyl (CE) negatif dan area radiolusen di sekitar apeks yang terbuka. Perawatan yang dilakukan meliputi apeksifikasi, bleaching, restorasi vinir direkkomposit dengan pasak Fiber Reinforced Composite. Apeksifikasi dilakukan menggunakan kalsium hidroksida selama 3 + 3 bulan. Setelah 6 bulan gambaran radiograf menunjukkan adanya bahan terkalsifikasi agak padat yang terletak sedikit lebih pendek dari apeks. Pengecekan dengan file dalam saluran akar menunjukkan sudah adanya barrier jaringankeras. Selanjutnya obturasi dilakukan menggunakan siler biokeramik. Siler biokeramik diharapkan dapat memperkuat akar dari risiko fraktur, melanjutkan stimulasi pembentukan jaringan, serta mengisi rongga yang tidak tertutupi dengan apeksifikasi. Walaupun Mineral Trioksid Agregat (MTA) telah banyak menggantikan kalsium hidroksida, namun bahanklasik ini terbukti dapat digunakan untuk merawat apeks terbuka. Hal ini terutama bermanfaat karena MTA tidak selalu mudah didapat.

Abstract

Bare root overdenture is a denture supported by a dome-shaped endodontic treated teeth with height 2-3 mm above the gingival surface. All prepared area of 45 was filled with composite material. The abutment teeth resisted the alveolar bone resorption process and maintains the height of alveolar crest, in order to support the retention and stability of removable partial denture (RPD). The aim of this case study was to investigate the treatment of acrylic resin RPD with bare root overdenture on extrusion tooth 45. A 54 years old male patient with loss of teeth 18, 17, 15, 14, 22, 23, 24, 26, 27, 28, 38, 37, 36, 35, 32, 31, 44, 46 was presented in Prof. Soedomo Dental Hospital Faculty of Dentistry, Universitas Gadjah Mada, Yogyakarta.Tooth 45 was extruded thus promoted locked occlusion. This case management was comprised of anamnesis, clinical examination, study model impression, design of RPD with bare root overdenture on tooth 45, endodontic treatment, dome-shaped preparation, filling with composites material cover the entire surface of prepared tooth 45, working model impression, determination of maxilla-mandibular relation, artificial teeth arrangement, try-in denture, insertion, and control. Evaluation after insertion showed good retention and stability, established good occlusion and satisfied aesthetic by using the RPD with bare root overdenture on tooth 45. At control appointment, patients described comfortable feeling because the use of bare root overdenture could maintain the denture stability and retention, and there was no locked occlusion when RPD was used. In this reported case, the RPD with bare root overdenture on extruded 45 repaired occlusion and aesthetic of patient and did not lock the occlusion. ABSTRAKBare root overdenture adalah gigi tiruan yang didukung oleh gigi yang telah dirawat endodontik dan dipreparasi dengan bentuk dome-shaped setinggi 2-3 mm di atas permukaan gingival serta dilakukan penambalan komposit menutupi seluruh area yang dipreparasi. Adanya gigi pendukung dapat menghambat proses resorpsi tulang alveolar dan tinggi processus alveolaris dapat dipertahankan dalam menunjang retensi dan stabilitas gigi tiruan lepasan. Laporan kasus ini bertujuan untuk mengkaji perawatan gigi tiruan sebagian lepasan (GTSL) resin akrilik dengan penyangga bare root pada gigi 45 ekstrusi. Pasien laki-laki berusia 54 tahun dengan kehilangan gigi 18, 17, 15, 14, 22, 23, 24, 26, 27, 28, 38, 37, 36, 35, 32, 31, 44, 46 datang ke Rumah Sakit Gigi dan Mulut Prof. Soedomo Fakultas Kedokteran Gigi, Universitas Gadjah Mada, Yogyakarta. Gigi 45 ekstrusi, sehingga mengakibatkan oklusi terkunci. Penatalaksanaanpasien dilakukan sebagai berikut: anamnesa, pemeriksaan klinis, perawatan endodontik dan preparasi serta penambalan dengan komposit pada gigi 45, pembuatan desain GTSL resin akrilik dengan penyangga bare root gigi 45, pencetakan model kerja, menentukan relasi maksila – mandibula, penyusunan gigi, percobaan protesa malam, insersi, dan kontrol. Insersi menunjukkan retensi, stabilisasi, oklusi, dan estetis baik pada GTSL dengan bare root gigi 45.Hasil kontrol menunjukkan pasien merasa nyaman kerena penggunaan bare root overdenture dapat mempertahankan stabilitas gigi tiruan dan oklusi tidak terkunci saat GTSL berfungsi. Dari kasus yang dilaporkan dapat disimpulkan GTSL dengan penyangga bare root pada gigi 45 ekstrusi dapat memperbaiki fungsi oklusi dan estetis yang baik, serta tidak mengakibatkan oklusi terkunci.

Abstract

The accuracy of biopsy on oral malignant lesion examination depends on the adequacy of clinical information obtained, biopsy type and procedure, determination of biopsy site, and integrity of biopsy tissue. Fine needle aspiration biopsy (FNAB) is one of among cytology examinationthat can be used for oral lesion. However, the results are variative, either false positive or false negative. The adequate management of a malignant lesion started with an accurate diagnosis. Histopathology and tissue biopsy examination is a gold standard in diagnosis determination of suspected malignant lesion. The aim of this case study was to report a refferal case of misdiagnosed lower lip squamous cell carcinoma using FNAB examination. Patient reffered from local hospital with squamous cell carcinoma lesion misdiagnosed as a benign cystic lesion prone to mucocele through FNAB finding. From this case study, it was concluded that FNAB examination is not reliable to diagnose superficial lesions, such as cystic lesions and SCC in the lower lip. ABSTRAKKetepatan diagnosis dari pemeriksaan lesi ganas rongga mulut tergantung pada adekuatnya informasi klinis yang didapatkan, jenis dan prosedur biopsi yang tepat, pemilihan tempat biopsi dan pemenuhan syarat dari jaringan biopsi. Aspirasi jarum halus (AJH) merupakan salah satu pemeriksaan sitologi yang bisa dilakukan untuk lesi di mulut, namun hasil pemeriksaan AJH masih ada yang dilaporkan false negative maupun false positive dengan hasil yang bervariasi.Penanganan yang tepat pada lesi ganas dimulai dengan melakukan diagnosis yang akurat. Standar baku dalam mendiagnosa terletak pada pemeriksaan histopatologi dari biopsi jaringan yang dicurigai mengalami lesi ganas. Tujuan studi kasus ini adalah melaporkan suatu kasus rujukan kesalahan diagnosis squamous cell carcinoma bibir bawah dengan pemeriksaan AJH. Seorang pasien rujukan dari salah satu RSUD dengan lesi squamous cell carcinoma yangdidiagnosis benigna cystic lesion yang condong ke mucocele melalui pemeriksaan AJH. Kesimpulan studi kasus ini adalah pemeriksaan AJH tidak tepat digunakan untuk kasus lesi superfisial seperti lesi kistik dan SCC di bibir bawah.

Abstract

Endodontically treated teeth were more susceptible to fractures due to loss of coronal sections caused by caries, trauma, and root canal treatment itself. Posts are used to increase the retention of the restoration on the tooth with extensive defect. Radix Anchor system is one of metal post that has a high elastic modulus and with the excessive mastication forces, it can lead to fractures. This article reported the result of surgical endodontic treatment followed by porcelain fused to metal crown on endodontically treated tooth which fracture after being restored with Radix Anchor post. A 46 years old male patient came with complaints of pain in the lower right molar after eating a solid food. Root canal treatment was conducted onthe same tooth 2 years ago. According to subjective, objective and radiograph examination, diagnosis of this case is non-vital tooth after root canal treatment with class VI Ellis fracture and grade 2 bifurcation lesion. The treatment for this case is bicuspidization and crown lengthening of the tooth 47. The tooth was restored with full porcelain fused to metal crown and fiber post on mesial root. One month after treatment, the symptoms diminished and the tooth was normallyfunctioned. Bicuspidization was an alternative treatment to avoid extraction and preserve remaining tooth strucure to build a functional restoration. ABSTRAKGigi pasca perawatan saluran akar rentan mengalami fraktur karena kehilangan bagian koronal yang cukup banyak akibat karies, trauma, serta perawatan saluran akar itu sendiri. Pasak digunakan untuk meningkatkan retensi restorasi pada gigi dengan kerusakan yang luas. Radix anchor adalah salah satu pasak metal yang memiliki modulus elastisitas yang tinggi sehingga tekanan pengunyahan yang berlebihan dapat menyebabkan terjadinya fraktur. Studi kasus inimelaporkan hasil perawatan bikuspidisasi dan crown lengthening yang diikuti restorasi mahkota porselin fusi metal pada gigi pasca perawatan saluran akar yang mengalami fraktur setelah direstorasi dengan pasak Radix Anchor. Seorang pasien laki-laki usia 46 tahun datang dengan keluhan gigi geraham kanan bawah sakit saat mengunyah setelah makan makanan keras. Gigi tersebut pernah dilakukan perawatan saluran akar 2 tahun yang lalu. Berdasarkan pemeriksaan subjektif, objektif dan radiografis, diagnosis dari kasus ini adalah gigi nonvital pasca perawatan saluran akar disertai fraktur Ellis kelas VI dan lesi pada bifurkasi derajat 2. Perawatan yang dilakukan adalah bikuspidisasi gigi 47 dan crown lengthening, kemudian gigi direstorasi dengan mahkota penuh porselin fusi metal dengan pasak fiber pada akar mesial. Satu bulan pasca perawatan menunjukkan bahwa gejala sakit saat mengunyah sudah tidak ada dan gigi dapat berfungsi dengan normal. Bikuspidisasi merupakan perawatan alternatif untuk menghindari pencabutan gigi dan menjaga jaringan keras yang tersisa untuk membangun restorasi yang fungsional.

Abstract

Cerebral abscess is a localized suppurated infection in between of a brain tissue caused by a wide variety of bacteria, fungi, and protozoa. The etiology of cerebral abscess includes chronic suppurated disease, congenital cardiomyopathy, or a trauma from a neurosurgical procedure. Sinusitis, otitis, and untreated odontogenic infections promote the formation of a brain abscess. The purpose of this case report was to present the management of an odontogenic focal infection in a case of cerebral abscess. Thirty six years old male referred to the Policlinic of Oral Surgery of Dr. Sarjito Central General Hospital, Yogyakarta by the Neurology Department to investigate the focal infection in the oral cavity of the patient with a cerebral abscess. The patient was treated on the Neurology Department for six month with initial complaint of headache, seizures, nausea, vomiting, and unable to move his right extremity. After a drug therapy, complaints were gradually receded. Based on the anamneses and clinical examination, an abnormality on the other organ was not suspected, but two impacted teeth and several radices were found in the oral cavity. Focal infection elimination was performed twice under a local anesthesia. Four weeks after the treatment, the neurological complaint of patient was decreased. The management of odontogenic cerebral abscess required a collaboration of the neurologist, radiologist, neurosurgeon, and oral surgeon. The treatment was aimed to eliminate the focal infection and reduce the effects caused by the mass of necrotic tissue, suppurated inflammatory response and edema on the surrounding cerebral tissue

Abstract

One visit root canal treatment is non-surgical conservative treatment of irreversible pulp disease or pulp necrosis. Treatment includes cleaning and shaping, sterilization, and obturation of root canal performed in one visit. The aim of this case report is to evaluate the success of one visit root canal treatment using rotary instrument followed by semi-direct composite restoration. A 23 year-old patient presented into dental hospital Faculty of Dentistry UGM to obtain treatment of leaking composite filling on his mandibular right molar that previously had been treated 1 year ago. The tooth was asymptomatic. Radiograph showed a cavity reaches the pulp and there was no periapical lesions, clinical examination showed necrosis. Patient was treated with one visit root canal treatment using rotary instrument followed by semi-direct technique of composite restoration. Five months after treatment the patient had no complaints. Radiographic examination showed no periapical lesions. One visit root canal treatment using rotary instrument followed by semiindirect technique composite restoration may be an effective treatment option when performed with case selection, proper diagnosis, aseptic work, as well as the appropriate operator’s knowledge and skills
