Abstract

Garlic (Allium sativum) is known to have antioxidant activity that is beneficial to health. Antioxidant activity is expressed in IC₅₀ value, which is the concentration of the extract that can reduce 50% of free radicals. This study aims to test the antioxidant activity of a gel formulated from garlic extract using the DPPH (2,2-diphenyl-1-pyrrylhydrazine) method. The garlic extract and garlic gel extract were tested to evaluate the % inhibition and IC₅₀ values. Garlic ethanol extract showed an IC50 value of 89,290, and garlic ethanol extract gel showed the % inhibition value of 87,30. The decrease in the IC₅₀ value indicates that the gel formulation provides significant antioxidant activity against DPPH free radicals. In conclusion, the ethanol extract of Allium sativum formulated into a gel showed antioxidant activity based on IC₅₀ and percentage of inhibition values obtained from the DPPH assay.

Abstract

Houttuynia cordata Thunb. is a promising medicinal plant recognized for its rich diversity of secondary metabolites, particularly flavonoids and essential oils, that are differentially distributed across its plant organs. This narrative review aimed to critically synthesizes the reported pharmacological activities, specific phytochemical constituents of each organ, molecular mechanisms of isolated bioactive compounds, and contributions of the metabolites produced by associated endophytic microorganisms. Relevant literature was identified through electronic databases including PubMed, Scopus, ScienceDirect, and Google Scholar using the keywords “Houttuynia cordata”, “phytochemical”, “pharmacological activity”, and “secondary metabolites”, from 2015-to 2025.  A broad body of literature published was examined to map the diversity of bioactive compounds, experimental models, and mechanisms of action. The findings reveal that the leaves of H. cordata are rich in flavonoids such as quercetin and rutin, which act as potent antioxidants and immunoprotective agents. The stems and rhizomes are dominated by monoterpenes and volatile ketones exhibiting marked antimicrobial and anti‑inflammatory properties. Preclinical evidence consistently demonstrates antitumor activity via apoptosis induction, multitarget antiviral effects, antidiabetic, and antihyperlipidemic activities as well as anti‑inflammatory actions mediated through the inhibition of TNF‑α and PGE₂ pathways. Antibacterial and anticancer properties are also notably attributable to the secondary metabolites produced by the endophytic microbiota associated with this plant. The phytochemical diversity of H. cordata and its symbiotic endophytes offer promising prospects for the discovery of novel bioactive compounds from Indonesian biological resources. Collectively, H. cordata holds substantial potential as antioxidant, antimicrobial, antivirus, anticancer, antidiabetic, antihyperlipidemic, anti-inflammatory, immunomodulatory, and antiasthma, warranting further investigation as an innovative phytopharmaceutical candidate.

Abstract

Chemotherapy-induced nausea and vomiting (CINV) are common adverse effects experienced by patients undergoing chemotherapy. Antiemetic therapy is a key strategy for the prevention and management of CINV; therefore, appropriate antiemetic use is essential. This study aimed to determine the patterns of chemotherapy and antiemetic utilization and to evaluate the appropriateness of antiemetic therapy based on the emetogenic risk of chemotherapy agents and antiemetic dosing standards in breast cancer patients at Sultan Agung Islamic Hospital, Semarang, Indonesia, in accordance with the National Comprehensive Cancer Network (NCCN) Guidelines 2024. The study was conducted observationally using a cross-sectional design with retrospective data collection from patients’ medical records. A total of 84 breast cancer patients were included through purposive sampling, according to predefined inclusion and exclusion criteria. The most frequently used chemotherapy regimen was paclitaxel–cisplatin (26%). Antiemetics were administered both prophylactically and post-chemotherapy, with ondansetron combined with dexamethasone as the most commonly used regimen (99.4%), along with ranitidine and diphenhydramine as adjunctive therapy. Evaluation of antiemetic appropriateness showed that 98.8% of patients did not receive antiemetic therapy consistent with the emetogenic risk of the chemotherapy agents, particularly among those receiving high-emetogenic-risk regimens. In addition, dose evaluation revealed inappropriate dexamethasone dosing, primarily due to underdosing and non-standard dosing frequency compared with the NCCN Guidelines 2024. Overall, antiemetic use was largely inconsistent with NCCN Guidelines 2024 recommendations, potentially compromising optimal CINV prevention in highly emetogenic chemotherapy.

Abstract

Beliefs about medication influence treatment engagement and outcomes in patients with heart failure, yet evidence from low-resource settings such as Yemen remains limited due to under-supported health care systems and low monitoring in health services. To assess medication beliefs among heart failure patients in Yemen and examine factors associated with necessity and concern beliefs, including medication regimen complexity. A hospital-based cross-sectional study was conducted between December 2023 and February 2024 among heart failure patients attending cardiology outpatient clinics in Sana’a, Yemen after ethical approval from the hospital. Data were collected using structured questionnaires, including the Beliefs About Medication Questionnaire (BMQ-Specific) and the Medication Regimen Complexity Index (MRCI). Multivariable linear regression analyses were performed to identify predictors of medication beliefs. A total of 250 patients were included, with a mean age of 51.08±11.47 years, and 65.2% were male. The median total BMQ score was 38.0 (IQR:35–42). Higher necessity beliefs were independently associated with older age (B=0.046, p=0.029), higher educational status (B=−1.240, p=0.030), and a greater number of prescribed medications (B=0.802, p=0.001), whereas greater medication regimen complexity was negatively associated with necessity beliefs (B=−0.276, p=0.003). Regarding concern beliefs, employment status (B=2.605, p<0.001) and longer duration since diagnosis (>3 years) (B=−1.155, p=0.029) were significant predictors, indicating a mild but significant impact. No significant associations were observed between concern beliefs and age, gender, education, income, number of medications, or regimen complexity. Medication beliefs among heart failure patients in Yemen are shaped by both patient and treatment-related factors. The findings have important implications in clinical practice, both clinical pharmacists and cardiologist can consider patients’ perspective to guide intervention design especially simplifying medication regimens and addressing patient-specific concerns.
