Abstract

Dental caries is a prevalent oral health issue in Indonesia, largely caused by Streptococcus mutans. While Piper betle L. essential oil possesses known antibacterial properties, its utilization in a stable mouthwash formulation requires scientific validation. Objective: This study aimed to formulate Piper betle L. essential oil into a mouthwash emulsion and evaluate its antibacterial activity against S. mutans. Essential oil was obtained via steam distillation and formulated into mouthwashes at concentrations of 1%, 2%, and 4% (v/v) using Tween 80 as an emulsifier. Antibacterial activity was assessed using the well-diffusion method against S. mutans (0.5 McFarland standard) in independent triplicate wells (n=3). Vehicle control (without essential oil) and Povidone-Iodine 10% (positive control) were used for comparison. Data were analyzed using One-Way ANOVA followed by Tukey HSD post-hoc test (p<0.05). The formulation containing 4% essential oil exhibited the strongest antibacterial activity among the test groups, with an inhibition zone of 12.66 ± 1.76 mm, which was significantly higher than the vehicle control (9.29 ± 2.67 mm; p < 0.05). The positive control showed the highest inhibition (18.03 + 1.16 mm). Physical evaluation confirmed that all formulations remained stable during 7 days of storage at room temperature (27 ± 2°C), with no specific control of relative humidity, with pH values (6.46–6.82) and specific gravity (1.03–1.04 g/mL) within physiological limits. Piper betle L. essential oil mouthwash is physically stable and effective against S. mutans in a concentration-dependent manner, suggesting its promising potential as a natural alternative for oral hygiene maintenance

Abstract

Liver cancer is one of the leading causes of cancer death worldwide. Genetic factors play a role in determining a person's susceptibility to this disease. Genome-Wide Association Studies (GWAS) have identified several genetic variants associated with liver cancer, but their functional mechanisms still need to be further explored. Therefore, this study aims to identify genetic variants that contribute to liver cancer, evaluate their functional effects on proteins, analyze allele frequencies across global populations, and examine gene expression in various human tissues. This study used a bioinformatics approach to identify genetic variations associated with liver cancer from the GWAS Catalog. Five selected missense variants were analyzed using SIFT and PolyPhen-2 to assess their functional impact. Allele distributions in the global population were analyzed using 1000 Genomes Project data, and gene expression was analyzed using the GTEx Portal. The analysis identified 77 candidate genes with significant associations with liver cancer, based on p-values meeting the threshold (p < 5 × 10⁻⁸). Five genes in the Missense Variant category showed a strong association with liver cancer: IFNL3, SLC30A10, PNPLA3, OSMR, and CMTR2. In the analysis using SIFT and PolyPhen-2, the rs3096380 variant (CMTR2) was deleterious, and rs738409 (PNPLA3) and rs188273166 (SLC30A10) were deleterious and probably damaging, with the potential to disrupt protein function and contribute to the pathogenesis of liver cancer. Conclusion: Genetic variations rs738409 (PNPLA3) and rs188273166 (SLC30A10) are deleterious and probably damaging, potentially disrupting protein function and contributing to liver cancer pathogenesis.

Abstract

Chronic kidney failure is a clinical syndrome caused by various chronic kidney diseases, characterized by imbalanced water, electrolyte and acid-base metabolism. Hemodialysis separates uremic substances from blood through a semipermeable membrane in an artificial kidney called a dialyzer. Adherence refers to an individual's behavior in following treatment rules, guidelines, and discipline, including medication compliance, dietary recommendations, and lifestyle changes as recommended by treatment and health advice. This study analyzes adherence levels among adult patients with chronic kidney failure undergoing hemodialysis therapy at Dr. Moewardi General Hospital, Surakarta. This analytic-observational quantitative study used cross-sectional methods with prospective purposive sampling. Data collection used closed questionnaires from February to March 2025. Results demonstrated that the majority of patients were 40-60 years old, male, with a high school education, with a hemodialysis duration of 1-5 years, and who relied on hospital-set schedules. Analysis revealed relationships between adherence and patient profiles, including age, educational level, and duration of hemodialysis

Abstract

Topical drug delivery systems are rapidly evolving to enhance therapeutic efficacy and patient compliance. Among these, in-situ gelling systems that transition from a sol to a gel state in response to physiological stimuli have emerged as a highly promising platform. This review provides a comprehensive analysis of chitosan, a natural polysaccharide, as a cornerstone polymer for the development of advanced in-situ gels. This paper, based on a systematic literature review of 63 articles published between 2015 and 2025, explores the formulation, characterization, and application of these intelligent delivery systems. Key findings highlight chitosan's exceptional properties, including biocompatibility, biodegradability, and mucoadhesion, which are crucial for effective topical therapy. The review details the primary gelation mechanisms, such as pH, temperature, and ion sensitivity that enable controlled, localized drug release. Furthermore, it summarizes extensive preclinical evidence demonstrating the versatility of chitosan-based gels in various applications, including ocular, nasal, wound healing, and even nose-to-brain drug delivery, where they significantly improve bioavailability and prolong residence time. While challenges like poor solubility and weak mechanical strength persist, innovative solutions involving polymer blending and chemical modification are effectively expanding their functional capabilities. This review concludes that chitosan-based in-situ gels represent a sophisticated and adaptable platform poised to advance next-generation, non-invasive therapeutics.

Abstract

Staphylococcus aureus is a major cause of skin infections and remains a public health concern due to its high prevalence and the increasing emergence of antibiotic-resistant strains such as methicillin-resistant S. aureus (MRSA). These conditions highlight the importance of preventive approaches using non-antibiotic topical products. This study aimed to evaluate the antibacterial effectiveness of solid soap formulated with green mussel shell powder (Perna viridis), as well as its physical quality and consumer acceptance. A true experimental posttest-only control group design was applied using five formulations: F I (2%), F II (4%), F III (6%), F IV (0%, negative control), and F V (benzalkonium chloride as positive control). Antibacterial activity was assessed using the disc diffusion method, and the data were analyzed using one-way ANOVA. The results showed a concentration-dependent antibacterial effect, with higher concentrations of green mussel shell powder producing greater inhibition against S. aureus. All formulations met physical quality standards and demonstrated good stability and homogeneity. Sensory evaluation indicated high consumer acceptance across all formulations. Overall, these findings suggest that green mussel shell powder has strong potential as a sustainable and effective antibacterial ingredient for solid soap formulations intended for preventive skin hygiene

Abstract

Jasmine leaves (Jasminum sambac L.) are known to contain phenolic and flavonoid compounds with antioxidant potential. This study evaluated the antioxidant activities of the ethanol extract, solvent fractions, and subfractions of jasmine leaves. It characterized the chemical profile of the most active subfraction using gas chromatography–mass spectrometry (GC–MS). Jasmine leaves were extracted by maceration using 96% ethanol and subsequently fractionated with n-hexane, ethyl acetate, and water. The ethyl acetate fraction, which exhibited the strongest antioxidant activity, was further separated by vacuum liquid chromatography to obtain subfractions. Antioxidant activity was determined using the DPPH method and expressed as IC₅₀ values. The most active subfraction was analyzed by GC–MS. The results showed that the ethyl acetate fraction had the lowest IC₅₀ among the solvent fractions, while subfraction 1 (SF1) exhibited the strongest antioxidant activity, with an IC₅₀ of 49.78 ppm, indicating strong activity. GC–MS analysis of the most active subfraction suggested the presence of antioxidant-related compound classes with tentative identification. These findings demonstrate that subfractionation enhances antioxidant activity and enriches antioxidant-related constituents in jasmine leaves, while highlighting the need for advanced analytical techniques to confirm compound identities.

Abstract

Chronic kidney disease (CKD) is a progressive condition characterized by a gradual decline in glomerular filtration rate, often accompanied by hypertension, necessitating antihypertensive therapy. This study aimed to evaluate the effectiveness of antihypertensive drugs in achieving blood pressure targets (<140/90 mmHg or a mean arterial pressure reduction of ≥20 mmHg) and to identify the incidence of adverse effects in CKD patients at Menggala Regional General Hospital during 2024. A retrospective observational study was conducted using medical records from 199 CKD inpatients selected via purposive sampling. Data analysis employed descriptive statistics, paired t-tests for effectiveness, and chi-square tests for associations between drug types. Results indicated that combination therapy with furosemide IV, candesartan, and amlodipine was the most common (77.4%) and demonstrated effectiveness in 50.8% of cases (p=0.01). Adverse effects occurred in only 1.0% of patients, primarily peripheral edema. The findings underscore the efficacy of combination therapies involving diuretics, ARBs, and CCBs in managing hypertension in CKD, with minimal adverse events. This research provides valuable insights for optimizing antihypertensive regimens in similar settings.
