Abstract

Background: Callibration of  CT number and uniformity is very important to the CT QC program particularly when the program aims to ensure that the CT Scanner equipment operates within the manufacturer's specifications. Since the commercial CT QC phantom is not always available or affordable in the department, a simple, easy and cheap modified CT water phantom design could be useful to support further regular departmental CT QC program.Purpose: (1) to design , a simple and an easy to handle of the modified CT water phantom,  and (2) to verify its callibration and measurement functions based on to the gold standard of a commercial CT water phantom.Methods: This study is an experimental research laboratories, where a modified design of  CT water phantom is then compared to the comercial gold standard CT water  phantom. The commercial CT water phantom uses a cylindrical phantom that contains water with various diameter in sizes. In the modified CT phantom design, it filled with water as well, where the three different size in phantom diameters (15 cm, 20 cm and 32 cm recpectively) created for specific measurement purpose that of the CT Number and uniformity CT Number.Results: Calliberating the function of the modified CT phantom design resulted in the three observasional data. For all different phantom diameters (15, 20 and 32 cm) showed some variation about their CT number and homogenity. Most of the callibration and homogenity tests of the CT number in water deemed significant (cal. CT number for the 20 cm and 32 diameter phantoms, 0,001 0,05 and sig. 0,04 0,05 respectively), instead of that the uniformity test  of the phantom sized with 20 cm in diameter (0,432 0,05).Conclussion: A modified water CT phantom designed with 20 cm in diameter could be used for measurement of the CT number uniformity. Yet, measurements of the CT number callibration and uniformity needed to be consider againts from a failed measurement due to the phantom problem design with sizes at 20 and 32 cm in diameters.

Abstract

Background: Quality control is part of the quality assurance program related to the techniques used in the monitoring the maintenance of the technical elements of the system that affect picture quality. KVp accuracy and linearity (test mAs) is one of the components which are tested on the routine test performance evaluations. The X-ray Biomedical International unit brands BRG 100 RF of Laboratory Diploma in Mechanical Radiodiagnostic and Radiotherapy Purwokerto have not been tested yet. The objectives of this study are to determine suitability output kVp accuracy and linearity that have been beeing setting at control panel X-ray unit.Methods: This research belongs to a quantitative research which was conducted in April 2016 at the Laboratory of Radiodiagnostic Diploma Engineering and Radiotherapy Purwokerto on X-ray Biomedical International unit brands BRG 100 RF. The tests carried out two stages of testing. They were the accuracy of kVp with MPD (Multi-purpose detector) tool at kVp 50, 60, 70, 80, 9 and testing linearity method using Stepwedge and Reciprocity mAs Densitometer at 55 kVp, 10 mAs with a variation of 200 mA and 100. The results was analysed based on the quality control examining contained in KMK RI No.1250, the value of deviation accuracy of kVp 6% and for linearity deviation value (mAs) ≤ ± 0.1.Result: The test results showed that there are some deviations occur repeatedly on the accuracy of kVp on the value of eksposi 50, 60, 70, 80, 90, are 0.78%, 1.31%, 1.32%, 1.63%, 2.5%, but the deviation is still within the allowable limits that does not exceed 10%. The linearity testing showes that there are differences in optical density values on the radiograph with variations mA and s which are still within the allowable limit that is ≤ ± 0.1.Conclusion: Based on the result of kVp accuracy and linearity test, the deviation value is still within the tolerance limit or still in accordance with the permissible tolerance according to KMK RI   1250 about quality control testing parameter

Abstract

Backgroud: Examination of the abdomen CT scan is often done by using standard protocol, meanwhile the actual parameter can be modified according to local needs considering image quality and radiation dose based on Karabulut and Ariyuek (2016). Abdomen CT Scan by GE 16 slices unit in Radiology Instaallation of Dr. Saiful Anwar Malang Hospital, using exposure factor of 120 kV, 234 mAs and the value of the CTDI dose was 53.04 mGy. While the BAPETEN’s reference of CTDI value, a CT scan of abdomen was 25 mGy.  This study aims to determine the changes of the value of kV and mAs to the image quality and the radiaton dose in the abdomen MSCT examination on abdominal tumor case in Radiology Installation of Dr. Saiful Anwar Malang Hospital.Methods: This research was a quantitative research with an experimental approach. The data were collected from three variations of tube voltage (kV) ie 100 kV, 120 kV and 140 kV and three variations of tube current value and time (mAs) ie, 180 mAs, 195 mAs, and 210 mAs. Radiographs was evaluated by three radiology physicians. Data were analyzed by scoring method of respondent’s assessment to assess MSCT image quality of abdominal tumor, while the radiation dose was obtained by CTDI recording.Results: The results showed that there was influence of tube voltage variation to image quality of abdominal tumor using MSCT unit. The higher kV used, the higher image quality resulted. From the calculation of the percentage from the assessment, the highest score of variation was at 140 kV, while the lowest score of variation was at 100 kV. Based on the recording CTDI radiation dose on the monitor, found that the higher value of kV, the higher radiation resulted. The mAs variations influenced the image quality of abdominal tumor using MSCT unit. Acoording to the percentage of the assessment, the highest score of variation found in 210 mAs, and the lowest score of variation found in 180 mAs. Based on the recording of radiation dose on the monitor, the higher mAs value, the higher radiation dose using MSCT unit. It was recommended to use 195 kV and 120 mAs for MSCT examination of abdominal tumor in Radiology Installation of Dr. Saiful Anwar Malang Hospital.Conclusion: There was influence of kV and mAs variation to anatomical image clarity and radiation dose of abdomen MSCT examination

Abstract

Background : The data overlap technique is the distance between the reconstruction images on the Z axis. This technique is useful for reducing the partial effect of volume, where images will be better when post processing is 2D or 3D. The purpose of this research is to know the effect of data overlap technique  variation  on  axial anatomical  image of CT-Scan Thorax  of lung tumor case   and what is  the variation of data overlap technique can yield anatomical image with the best quality.Methods : This research is an quantitative study. Subjects in this study were patients with CT Scan Thorax examination in  cases of lung tumors amounted to 7 people. The results of the scanning that has been done and then recons of data overlap technique using the value of 2 mm, 4 mm, 6 mm and 8 mm then the reconstruction results are assessed by 2 radiologists.Results : it is known that the variation of data overlap significantly affect the quality of the resulting image. For the most optimal data overlap value on CT thorax CT scan with lung tumor case, 2 mm data overlap value has the best result compared to other values with mean rank is 3.99.Conclusion : Based on these results indicate that the variation of data overlap significantly affect the quality of the resulting image. Optimal value of reconstruction increment to see a metastatic pathology is 2 mm.

Abstract

Background: Have been done research on inspection procedures examination in the case of abdominal CT Scan cholecystitis at Radiology installationProf. Dr. Margono Soekarjo Purwokerto Hospital. This study aims to determine the inspection procedure examination in the case of abdominal CT Scan cholecystitis not using the biphase technique diagnosis and to find out information on examination in the case of abdominal CT Scan cholecystitis with monophase technique at Radiology installationProf. Dr. Margono Soekarjo Purwokerto Hospital.Methods:This type of research is aqualitaive research case study approach. The data collection methods with unstructured observation , depth interview with 3  radiographers, 1 radiologist, a sending doctor and a patient, and the documentation of the results of reading radiographs and photographs. The data obtained  then reduced, classified, dried and then interpreted in the form   then  concluded kuotasi and suggestions.Results: The result showed that the procedure examination abdomen ct scan in case of colecystitis at radiology installation Prof. Dr. Margono Soekarjo Purwokerto Hospital before the hearing preparation charge informed consent of patients, perform laboratory checks urium creatinin, fasting from the night and drank appoxcimately 1000 ml of Water the morning before the examination. While the position of the patient supine feet first and the administration of contrast media intra vena with a volume of 80 ml plus 20 ml Na Cl and arrangements flow rate 2,0 ml/sec with a scan delay of 40 seconds. Reason not to do engenering biphase technique examination in the case of abdominal CT Scan cholecystitis because only monophase technique, arterial phase alone can establish the diagnosis of cholecystitis.Conclusion: Information on CT Scan diagnosis in Radiology Installation  Prof. Dr. Margono Soekarjo Purwokerto Hospital was able to show in indication of cholecystitis but have not been able to show the mass and metastase to the surrounding organs lake the liver gallbladder and pancreas, intrahhepatic and extrahepatic tract.

Abstract

Background: In brain MR angiography examination, there are many clinical situations where the radiologist want to optimal suppression of stasionary nuclei. So stationary nuclei and flowing nuclei will produce a good contrast. One of the most commonly used method brain MR angiography o is a 3D TOF because it can show  the blood vessels by hyperintens with more hypointens of background. Parameters TR and flip angle is one of the parameters that will determine a stationary nuclei experiencing well suppression and the flowing nuclei will still generate a signal that more hyperintens. The purpose of this study is  to determine the influence of time repetition and flip angle of the anatomical information of 3D TOF brain  MRA examination, and also to determine value the time repetition and flip angle best to display anatomical information of 3D TOF brain MRA examination.Methods: This research is an experimental approach to the pre-experimental research design and post-test only research design. This research was conducted in Bintaro Premier Hospital. The data in the form of 54 3D TOF brain MRA images of 6 volunteers with nine variations time  repetitions and flip angle. Assessment  anatomical information performed by the two respondents. The data analysis  by  testing the influence is multiple linear regression to determine the value of time repetition and flip angle optimal use Friedman mean rank test.Result: The result showed that there is the influence of time repetition and flip angle of the anatomical information on 3D TOF brain MRA examination with a significance value  p 0.001 (p 0.05). The time repetition 30 ms and flip angle 30o is and optimal value for anatomical information showing 3D TOF brain MRA examination. Application of time repetition 30 ms and flip angle 30o can be properly saturate or suppression the stationary nuclei so that is displays excellent contrast between the nuclei. Conclution: There are influence of  time repetition and flip angle to   anatomical information  of  3D TOF  brain MRA examination with a significance value of  p 0.001 (p 0.05). The time repetition 30 ms and a flip angle of 30o is an optimal value for anatomical information showing 3D TOF brain MRA examination.

Abstract

Background : Examination of Calcium scoring in Radiology Departement Telogorejo Hospital Semarang using CINE scan type and  recon phase images using a slice thickness of 2.5mm, this affect will be plaque with small size cann’t to calculate. Aim : The Purpose of this study was to determine the variation of parameters of calcium scoring MSCT scan examination right in calculating optimal plaque.Methods : The study was preceded by Pre experiments using a thorax phantom with an added 3-calcium artificial heart is likened to the RCA, LCX, LAD and have determined the weight and volume. Experiments with 10 patient samples by applying the best parameters from the pre experiment. Statistical analysis of the results of research conducted by Paired T-Test, while the assessment of the quality radiographs evaluated by an expert in this specialist radiology by providing anatomical criteria scores with 3 choices of items (not clear = 1, is quite clear = 2  and obvious = 3)Result : The volume of plaque is embedded in the thorax Panthom 900mm3. Results of pre experiments with hellical scan type and pitch of 0.5, 0.9, 1.3 in a score of plaque volume is 104mm3, 67mm3, 60mm3. While the scan type CINE with slice thickness 0.625mm, 1.25mm, 2.5mm produces a score of volume plaque is 880mm3, 878mm3, 867mm3. Further experiments were performed by applying parameters with the best result is CINE scan with variations slice thickness 0,625mm, 1.25mm, 2.5mm. To determine the best parameters assessment of image quality with the results on slice thickness 0.625mm was 2.2 (quite obvious), slice thickness 1,25mm scoring average of 2.7 (quite obvious), and slice thickness 2.5 mm average scoring 2.9 (obviously). While image noise on the slice thickness 0.625mm, 1.25mm, 2.5mm, namely 2.7(very high), 1.6 (low), 1.1 (low).Conclusions : Selected parameters that calculate optimal scoring calcium and low noise image for MSCT scan calcium scoring in Telogorejo Hospital Semarang is Scan type CINE with slice thickness variation 1,25mm

Abstract

Background: Brain Magnetic Resonance Venography (MRV) is a MRI examination of veins without contrast media. Phase Contrast (PC) MRV sequences uses phase shift protons that flow along the magnetic gradient into needed area (Field Of View).  PC MRV sequences use bipolar gradient to compensate the proton spin speed by adjusting the amplitude. The study aims to determine the Velocity encoding Value (VENC) to MRV image information on brain tumor patient and to determine the best image information through a technic to visualize the veins in brain tumor patient.Methods: This study was a quantitative study with quasi experimental approach. Data were collected in Radiology Department of Dr. Moewardi Surakarta Hospital on May to June 2016. Data was collected using questionnaire filled by three respondents. Data were analyzed statiscally using Friedman test and Wilcoxon test to determine the difference of value of Velocity Encoding (VENC) between 10 cm/sec, 15 cm/sec, 20 cm/sec, as well the mean rank test used to find out the best image information.Results: The statistical test showed that there were significant differences in image information of MRV, between VENC 10 cm/sec, 15 cm/sec and 20 cm/sec with p value was 0.032 (p˂0,05). The best result of MRV image information was when VENC value of 20 cm/sec used, indicated by the mean rank which was 2.83.Conclusion: There were significant differences of MRV image information between VENC 10 cm/sec, 15 cm/sec and 20 cm/sec. The best result of MRV image information was when VENC value of 20 cm/sec used, to visualize better anatomy of the veins, so that the presence of tumor thrombus in the veins can be ensured.

Abstract

Background: TSE factor is parameters that affect Signal to Noise Ratio (SNR) and Contrast to Noise Ratio (CNR). TSE factor for brain MRI examination is a long TSE factor. There are differences when using TSE factor. At the theory, the brain MRI examination is using TSE factor ≥16 while at Siloam  Surabaya  Hospital was using TSE factor 14. The writer ever seen some noises at brain MRI image therefore the radiographer doing modification of TSE factor. The purpose of this research are to determine the influence of modification in the TSE factor value against SNR and CNR and to define the SNR and CNR optimum from that.Methods: This research is a quantitative study with an experimental approach. This research was done by MRI Philips Achieva 1,5 T with 10 modification TSE factor (8, 10, 12, 14, 16, 18, 20, 22, 24 and 26). SNR and CNR obtained by measurement of ROI in the grey matter, white matter and CSF with the result an average signal and compared with the average standard deviation of the background image. Data was analyzed by linear regression test to know the influence of TSE factor against SNR and CNR and data was analyzed by descriptive test mean rank to obtain the optimum TSE factor value.Result: The result showed that there was the inluence of TSE factor to SNR and CNR at T2W TSE axial brain. There was a significant correlation between TSE factor with all of area SNR and CNR with coefficient correlation of SNR grey matter r=0,591, with coefficient correlation of SNR white matter r=0,604, with coefficient correlation of SNR CSF r=0,687, with coefficient correlation of CNR CSF–grey matter r=0,690, with coefficient correlation of CNR CSF-white matter r=0,658. The significant value of linear regression test is (0,000*) p value (0,05). TSE factor optimum value at T2W TSE axial brain was TSE factor value 10 for SNR with mean rank SNR 45,05 and TSE factor value 8 for CNR with mean rank CNR 35,43.Conclusion: There was the influence of TSE factor to SNR and CNR at T2W TSE axial brain. TSE factor optimum value in brain MRI T2W TSE axial is 10 to SNR and TSE factor 8 to CNR.

Abstract

Backgroud: Have been done research about making of the design tools fixation on radiographic examination shoulder against the backdrop of the difficulty in positioning the patient to commit abduction. The purpose of the design is to replace the role of the patient's family or resident physician in charge of holding the cassette and the patient's arm in abduction during the examination takes place.Methods: This research is a quantitative with explorative experimental approach. The method used in data collection that is designing the tools, use tools, and test tools function. Test function tools made directly to the patient in the Radiology department of DR.Sardjito Yogyakarta. The results of test functions are analyzed based check list of respondents about the value of work tools.Results: The results of design tools fixation on radiographic examination shoulder joint inferosuperior axial projections in the form of iron under the buffer, the buffer on iron, steel main buffer, where handrail 1, where handrail 2 and the cassette place. The workings of the device is placed beside the examination table, abduction of the patient's arm and place it on a hand grip and place cassette on a the cassette place. The function test results on the assessment tool's ability to adjust to the state of the patient's arm, the ability of the tool settings when adjusting the tool height to the height of the examination table or brangkar, the ability of the tool settings when adjusting the length of the tool with the patient's arm length of, the ability of the tool setting in place the cassette on tools, the ability of setting tool in positioning the tape on the object of examination, the ability of the tool to produce radiographs that do not cause artifacts..Conclusion: the ability to show the joints between the glenoid cavity of the humeral head to 2.37. It can be concluded that the tools fixation radiographic examination of shoulder joint inferosuperior axial projection has a pretty good performance.
